1.Safety and feasibility of Szabo technique in percutaneous coronary intervention of coronary artery ostial lesions

Background and Object: Percutaneous coronary intervention (PCI) on ostial lesions in coronary arteries associated with higher procedural difficulty and complication morbility. It has been technically difficult because it should be done with precise stent placement in ostium and absence of side branch compromise. The Szabo technique consists of side branch wiring through most proximal stent strut as well as main branch wiring through stent lumen. The side branch wire or anchor wire prevents stent advancement beyond ostial segment and makes possible the accurate stent implantation in ostium. The purpose of this study is to evaluate the safety, feasibility and success rate of Szabo technique by analysing technical, angiographic and IVUS (Intravascular Ultrasonography) findings. Methods: The data of 39 PCIs in 39 patients with a significant lesion at a coronary artery ostium which was treated percutaneously using Szabo technique were retrospectively analysed. The technically successful procedure was defined as there was neither stent loss nor second guide wire pull back during stent advancement. A successful procedure from angiographic point of view was defined as a precise stent implantation at ostium without side branch compromise. Successful procedure from IVUS point of view was defined as accurate stent placement in ostium without proximal protrusion and without any stent uncovered area. A total of 39 patients was with 28 males and 11 females. They aged from 43 to 79 years with a mean age of (65±12)years. The 6F and 7F guiding catheter were used in 35 (89.7%) and 4 (10.3%) patients separately. The access was radial in 31 (79.5%) and femoral in 8 (20.5%) patients. The culprit vessel was left anterior descending (LAD) in 26(66.7%), right coronary artery (RCA) 5 (12.8%), circumflex-obtuse marginal (LCX-OM) 3(7.7%), and posterior descending (PDA) 5(12.8%). IVUS was performed through culprit vessel in 30 (76.9%) and was also done in side branch in 9 (23.1%) patients after stent implantation. Results The procedure was technically successful in 36 (92.3%) patients. All technically successful patients had angiographic success (100%). IVUS examination of culprit vessel showed accurate stent placement in ostium 29 (96.7%) and slight stent proximal protrusion in 1(3.3%) patients. No serious complication occured. Conclusions This study shows that Szabo technique is safe and feasible for PCI in ostial coronary artery lesions with a high angiographic success rate,its deserved to spread.

Heme oxygenase-1(HO-1)is a type of stress related gene,which harbors the ability of anti-inflammatory,anti-apoptosis,anti-oxidation and drug resistance.In physiological status,the expression of HO-1 gene in human normal tissues is extremely low.When subjected to a constant stimulation of alcohol,spicy and hot,over expression of HO-1 gene in the related tissues is often observed.In tumor tissues,the abnormal expres-sion of HO-1 is closely related to their biological behaviors.In recent years,there is a trend to believe that the gene polymorphism and its metabolic products( bilirubin,Fe2+,CO) involve in the decomposition of heme partici-pate in the activation of many signaling pathways,and affect the initiation and development of tumor diseases.In addition,the expression level of HO-1 gene is up-regulated in many tumor diseases,which can not only affect the growth and proliferation of tumor cells by regulation of the oxidative stress levels of tumor microenvironments, but also induce the tumor cells to produce drug resistance.Therefore,the abnormal expression of HO-1 may be involved in the initiation and development processes of tumors.
3.Mechanisms and Performance of Voice Disorders in Aphasia (review)

This paper discussed the performance and cerebral localization of tone error occurred in patients with aphasia. The definition and characteristics of tone and aphasia were introduced briefly, while focusing on the brain lateralization of the tone. Tone is a language signal, but also has characteristics of melodic acoustic. The dual nature affects the cognitive process of tone languages. Therefore studies at home and abroad about tone showed that the conclusion of advantage of lateralization was inconsistent. In this paper, by summarizing the advantage of lateralization of listening perception with different languages, and generalizing the brain damage localization in aphasia patients with voice disorders, the dominant side of the tone in the brain was discussed. This paper also summarized and discussed the characteristics of voice disorder in aphasia.
4.Estradiol and prolactin levels in young females with cerebral trauma combined with extremity long tubular bone fracture

Objective To investigate the estradiol (E2) prolactin (PRL) levels in young females with cerebral trauma combined with extremity long tubular bone fracture. Methods E2 and PRL were detected in 39 young females with cerebral trauma combined with extremity long tubular bone fracture (experimental group) and 42 young females with just extremity long tubular bone fracture (control group) at 1 to 3, 5 to 7, 10 to 14, 28 to 30, and 56 to 60 days after injury. Results E2 levels in the experimental group were significantly higher than in the control group at 1 to 3, 5 to 7, 10 to 14, and 28 to 30 days after injury (P < 0.05), but there was no significant difference in the E2 level between the 2 groups at 56 to 60 days after injury (P > 0.05) . Compared with 1 to 3 days, the E2 level was significantly decreased in the experimental group at other time points (P < 0. 05), while there was no significant difference in the E2 level in the control group among each time point (P > 0. 05). PRL level in the experimental group was significantly higher than in the control group at all time points. In particular, the PRL level in the experimental group was significantly increased at 5 to 7, 10 to 14 and 28 to 30 days after injury, compared with that at 1 to 3 days after injury (P<0.05), but there was no significant difference in the PRL level between 56 to 60 days and 1 to 3 days (P > 0. 05). Additionally, the PRL level did not change in the control group at all time points (P > 0.05). Time of fracture healing in the experimental group was significantly shorter than in the control group, while the time of hospital stay in the experimental group was significantly longer than in the control group (P<0.05). Conclusion E2 and PRL levels increase remarkably in the young females with cerebral trauma combined with extremity long tubular bone fracture at an early onset stage.
5.Expression of transforming growth factor-β in local bony callus in traumatic brain injury combined with extremity fracture in rats

Objective To investigate gene expression of transforming growth factor-β(TGF-β) in local bony callus in traumatic brain injury combined with extremity long bone fracture in rats.Methods Eighty male SD rats were randomized into 2 even groups: the experimental group were models of traumatic brain injury combined with extremity bone fracture and the control group were models of simple extremity bone fracture. Samples of bony callus were harvested at 1, 2, 3, and 4 weeks in both groups, each time from 10rats respectively, for detection of gene expressions of TGF-β by reverse transcription PCR techniques.Results Levels of gene expression of TGF-3 in local bony callus in the experimental group were significantly higher than in the control group at one week ( P < 0. 05); but significantly lower at 3 weeks( P < 0. 05) .Peak values occurred at 2 week in both groups, though, significantly higher than at other times, there was no significant difference between the 2 groups( P > 0. 05) .Conclusion Changes in gene expression of TGF-β in local bony callus in traumatic brain injury combined with extremity long bone fracture indicate that TGF-β may play a role in the process of increased fracture healing.

Objective:To construct a fusion DNA vaccine pEGFP-C3-B7.2-hTERT and to observe the capability of pEGFP-C3-B7.2-hTERT DNA vaccine to induce specific anti-tumor immune responses and to inhibit growth of the hepatoma transplanted from H22 cells. Methods: The B7.2 and hTERT cDNA (amplified by RT-PCR), together with pEGFP-C3 as the vector were used to construct fusion gene plasmid pEGFP-C3-B7.2-hTERT, which was then used to vaccinated C57BL/6 mice for 3 times at a 7 d interval. Animals vaccinated with pEGFP-C3-B7.2, pEGFP-C3-hTERT, pEGFP-C3 and PBS were taken as controls. Splenocytes CTLs of immunized mice, the levels of IL-2, IFN-? in the culture supernatant, the levels of antibodies against hTERT, and the changes of the T lymphocyte subsets in the peripheral blood were all examined. The mice harboring hepatocarcinoma H22 cells were challenged with pEGFP-C3-B7.2-hTERT and the tumor forming time and the survival periods of mice were observed. Results: Agarose gel electrophoresis, nuclease digestion and sequencing confirmed the successful construction of pEGFP-C3-B7.2-hTERT. 7.The Establishment of the Apoptotic Model of Cultured Human Adult Cardiomyocytes

Objective To establish a model for hypoxia-induced apoptosis of human adult cardiomyocytes in vitro. Methods The human adult heart cells of ventricular muscle were isolated by the digestion of trypsin and collagenase, cultured in Iscove's modified Dulbecco's medium (IMDM), and purified by means of differential attachment technique, adding Brdu and slicing fibroblasts by sterile needle. After 4 to 6 days, the cardiomyocytes were cultured in an incubator containing 3% O 2, 92% N 2 and 5% CO 2 for 6, 12, 24 and 48h to form hypoxia injury for cells. Morphological changes of apoptotic cardiomyocytes were measured by invertion microscopy and electronic microscopy. Apoptotic rate was measured by TUNEL staining. Results The cardiomyocytes occurred opoptosis after hypoxia injury, apoptotic rate of which at 6, 12, 24 and 48h after hypoxia was (3 3?0 9)%, (8 3?1 8)%, (16 1?2 6)% and (19 4?2 3)% respectively. Conclusion The apoptotic rate of cardiomyocytes increased with time of hypoxia lengthened. The model for hypoxia-induced apoptosis of human adult cardiomyocytes was successful and reliable.

Objective To evaluate the clinical significance of prostatic hypoechoic lesions on transrectal ultrasound(TRUS) for prostate cancer detection Methods 438 patients referred to our hospital between August 1999 and August 2006 for biopsy of the prostate.Every patient received 6~13 biopsy cores of the prostate.If a hypoechoic was identified,the biopsy was taken from this lesion.Correlation between hypoechoic lesions,isoechoic areas and cancer detection for each core was performed.Results Among 438 patients,cancer was detected in 112(25.6%) patients.Hypoechoic lesions were seen in 75(67%)patients with prostate cancer,the isoechoic areas in 37(33%).A total of 3504 biopsy cores were obtained from 438 patients.Among them,636 biopsy cores were taken from hypoechoic lesions.163(25.6%)biopsy cores taken from hypoechoic lesions harbored cancer and 473(74.4%) had no cancer.The cancer detection rate was similar in patients with hypoechoic lesions on TRUS or not,25.4%(75/298)and 26.4%(37/140)respectively.The difference was not statistically significant(P

Objective To investigate the effects of submucosal resection by guidance under esophageal endoscopic ultrasound. Methods 79 cases ofsuspected upper gastrointestinal submucosal tumors were found by endoscopic ultrasonography,transesophageal endoscopic ultra-sound showed lesions under muscularis mucosa,after informed consent,they were underwent endoscopic mucosal resection. Pathological diag-nose of the lesions were found out after resection. Results Compared with pathological results showed that endoscopic ultrasound accurately determine tumor location in the hierarchy;through endoscopic mucosal resection to remove all lesions,5 patients were found bleeding postop-erative,6 patients were found esophageal stricture,the prognosis was good after treatment. Conclusion Endoscopic ultrasonography can accu-rately determine submucosal myoma,it can be used to guide resection of submucosal myoma.
10.Endoscopic ultrasonography features of gastric ectopic pancreas

Objective To describe the characteristic endoscopic ultrasonography (EUS) features of gastric ectopic pancreas.Methods Totally 23 patients were diagnosed pathologically as having gastric ectopic pancreas.The EUS images of the lesions were retrospectively reviewed regarding layer of origin,size,growth pattern,margin,and internal echo pattern.Results The coincidence of EUS and pathologic diagnosis was 91%(21/23).EUS revealed that the lesions originated from the second,third,and/or fourth layers of the gastric wall.Most lesions were heterogenous,mainly isoechoic.The borders of the lesions were indistinct in 70%(16/23) patients. Anechoic cystic or tubular structures within the lesions appeared in 10 of the 23 lesions(43%).The mean longest/shortest diameter ratio of ectopic pancreas was 1.7.Conclusions Characteristic EUS features of gastric ectopic pancreases include:mural lateral growth pattern,third layer (submucosa) origin,indistinct margin,intermediate echogenecity,anechoic areas,and fourth-layer thickening.Careful assessment of the EUS findings may be a useful aid in the diagnosis of gastric ectopic pancreas.
