1.Analysis of monitoring data on congenital heart disease in Shenzhen city from 2009 to 2013

Objective To analyze monitoring data on congenital heart disease (CHD) in Shenzhen city from 2009 to 2013.Methods This study described epidemiological characteristics of perinatal CHD.Patients with CHD were divided into isolated CHD group and complex CHD group (the patients were found with malformations of other systems).The prognosis,social and demographic characteristics,and complications of pregnancy were analyzed.Results The average incidence rate of CHD was 7.25‰,of which 94.36% were diagnosed by ultrasound,of which 33.20% were diagnosed before delivery.The top three cardiovascular abnormalities were patent ductus arteriosus,patent foramen ovale,and ventricular septal defect.There were about 14.11% of CHD with other system malformations,mainly including musculoskeletal system,facial features,and nervous system.Compared isolated cardiac malformations and complex cardiacmalformations,significant differences were found in the age stratification,culture degree,prognosis,diagnosis time,incidence rate of complication of pregnancy,and childbirth complications rate (P <0.01).Conclusions Most CHDs were diagnosed by ultrasound.Simple cardiovascular malformation was common,especially CHD with two or more types of cardiovascular malformations.The incidence rate of CHD in 30 to 40-year-old women,stillbirth rate,incidence of prenatal diagnostic rate were significantly higher than the simple CHD group.
2.Prenatal ultrasonographic screening for fetal facial anomalies using cross-sectional view of the fetal lateral ventricle with probe tilting at the11th-13th+6 gestational week

ObjectiveTo analyze the prenatal sonographic features of facial anomalies and evaluate the relationship between facial anomalies and increased nuchal translucency.MethodsPregnant women coming to Shenzhen Maternity and Child Healthcare Hospital afifliated to Nanfang Medical University between October 2008 and December 2012 for the 11th-13th+6 gestational week scanning were included in this study. Fetal facial region was evaluated by the technique of cross-sectional view of the fetal lateral ventricle with probe tilting. Comparative analysis was performed on autopsy and prenatal ultrasonograpgy. ResultsThe prenatal ultrasonic results of 10 518 fetus: 10 343 fetus (98.3%, 10 343/10 518) were conifrmed as facial normal with the technique of cross-sectional view of the fetal lateral ventricle with probe tilting at the ifrst trimester. Twenty-eight cases of facial malformation were conifrmed at the ifrst trimester (8 cases of cleft lip/plate, 14 cases of absence of nasal bone, 5 cases of holoprosencephaly facial feature and 1 cases of irregular facial cleft). Forty-nine cases of facial malformation were conifrmed by postnatal evaluation or autopsy (the incidence was 0.47%, 49/10 518 ). Twenty-one cases were misdiagnosed (11 cases of cleft lip/plate, 7 cases of ear deformity, 2 cases of micrognathia and 1 cases of irregular facial cleft). Ultrasonic soft marker: 34 cases were combined with abnormal soft marker (69.4%, 34/49), 22 cases of various kinds of facial malformation were combined with other structural deformity. Twenty-three cases were combined with increased nuchal translucency. Fourteen cases of absence of nasal bone were detected in the ifrst trimester, including twelve cases of increased nuchal translucency, six cases of structural deformity, two cases of chromosomal abnormalities (21-trisomy). Five cases of holoprosencephaly facial feature were detected in the first trimester, including three cases of increased nuchal translucency and structural deformity. Eight cases of cleft lip/plate were detected in the ifrst trimester, including ifve cases of increased nuchal translucency. One cases of irregular facial cleft were detected in the ifrst trimester, who was combined with structural deformity and increased nuchal translucency. Postpartum examination showed: there were 25 cases of simple facial anomaly, 22 cases were combined with structural deformity (9 cases of cardiac defect and 8 cases of holoprosencephaly), there were 2 cases of chromsomal abnormalities. The results showed that increased nuchal translucency had certain relationship with facial anomaly. The value of nuchal translucency of facial anomalies fetus was apparently higher than that of normal fetus and fetus with simply facial anomalies. There was no signiifcant difference between normal fetus and fetus with simply facial anomalies.ConclusionsFindings of holoprosencephaly and cardiac defects is a signiifcant clue for diagnosing facial anomalies. Fetuses with cleft palate/lip have an abnormal conifguration of the retronasal triangle. Cross-sectional view of the fetal lateral ventricle with probe tilting caudal slightly technique is an important method to screen fetal facial malformation in the ifrst trimester.
3.Metorchis orientalis founded in basin of Wuhu City,Anhui Province

Objective To investigate the epidemic situation of Metorchis orientalis in Wuhu City Anhui Province. Meth?ods Three lakes Fengming Longwo and Kui lakes were selected in Wuhu City and the poultries around the lakes and fresh?water fishes in the lakes were captured to test the infection of M. orientalis and the infection rates were calculated. Results The adult worms of M. orientalis were found in gall bladder and hepatic duct of domestic ducks with infection rate of 16.7%. The metacercariae of M. orientalis were discovered in the muscle of Pseudorasbora parva and Pseudogobio rivularis with infection rates of 7.13%and 3.38%respectively. Conclusion M. orientalis is endemic in Wuhu City which should be paid enough at?tention to the animal husbandry aquaculture and medicine.
4.Population dynamics and spatial distribution pattern of Aleuroglyphus ova-tus

Objective To characterize the population dynamics and spatial distribution of Aleuroglyphus ovatus in the flour warehouse,so as to provide the basic evidence for improving the sampling guidelines that are essential for effective pest monitor?ing and management. Methods The samples from flour warehouses of four localities were collected,examined and counted for A. ovatus in every month in Wuhu City. The dispersion pattern target,Iwao m*/-x regression analysis and Taylor power method were used for analyzing the spatial distribution pattern of A. ovatus in the flour warehouses. Results The peaks of population dynamics of A. ovatus were discovered in July and September,respectively. The indexes of dispersion were as follows:I>0, CA>0,m*/-x >1. At the same time,the parameters in the equation of Iwao:m*=5.471+1.022-x (r=0.999)and Taylor:lgS2=0.697+1.111 lg-x (r=0.987)showed that the spatial distribution pattern of A. ovatus was assembled. Conclusion The peaks of population dynamics of A. ovatus in the flour warehouse are bimodal pattern ,and the spatial distribution pattern of A. ovatus is assembled.
5.The influence of blood pressure on prognosis during speech therapy after stroke

ObjectiveTo investigate the improvement of speech in patients with increased blood pressure(BP) and stable blood pressure during speech therapy. MethodsAfter monitoring blood pressure with dynamic blood-pressure meter during speech therapy, patients were divided into increased BP group and stable BP group. Patients received two-month speech therapy, then their score changes in ABC examination of pre-treatment and post-treatment were compared. ResultsIn oral expression, score changes in increased BP group were significantly different from those in stable BP group (P<0.05). In listening comprehension, score changes had no significant difference between two groups(P>0.05). Conclusions Patients in increased blood pressure group progressed obviously in oral expression.
6.Logistic analysis of impact factors of treatment decision: a study on 147 patients with traumatic ruptured spleen

Objective To investigate the current status of treatment on traumatic ruptured spleen in a regional medical center in Tianjin,China.Method Logistic analysis of 16 impact ffactors of treatment decision on 147 patients with traumatic ruptured spleen.Results The type of trauma,pulse rate at admission,result of abdominal puncture and CT grade were related to treatment decision making.Splenectomy was carried out in 89 patients (60.54%) which accounted for 89.90% of all surgical procedures.Splenectomy was carried out in 50.85% of patients with CT grade 1 or 2 (or 85.71% of grade 1 and 2 surgery),and 100% of CT grade 3 or 4.Spleen preservation surgery was carried out in 7 patients with CT grade 1 or 2.Conclusion Splenectomy was carried out in most cases in this study.It is necessary to investigate the treatment of splenic trauma in regional medical centers,and to develop more reliable surgical techniques to carry out more spleen-preservation surgery ffor CT grade 1 or 2 ruptured spleen.
7.Effectiveness and safety of ultrasound-enhanced thrombolysis for acute ischemic stroke

Objective To evaluate the effectiveness and safety of ultrasound-enhanced thrombolysis for acute ischemic stroke.Methods Fifty stroke patients with acute middle cerebral artery occlusion were randomly divided into either a ultrasound-enhanced thrombolysis group (recombinant tissue-plasminogen activator [rtPA] +2 MHz ultrasound monitoring for 2 h) or a standard thrombolysis group (rtPA alone).The demographic characteristics,vascular risk factors,blood pressure before treatment,thrombolysis in brain ischemia (TIBI) grade before thrombosis,and vascular occlusion site of the patients were collected.The primary outcome endpoint was the good outcome rate (defined as the modified Rankin Scale score 0-1) at 3 months.The secondary outcome endpoints were complete recanalization at 2 h after thrombolysis,sustained complete recanalization,symptomatic intracerebral hemorrhage,and mortalitY.Results The good outcome rate of the ultrasound-enhanced thrombolysis group at 3 months after treatment was significantly higher than that of the standard thrombolysis group (64% vs.36%;P=0.011).The sustained complete recanalization rate (40% vs.8%;P =0.018) and complete recanalization rate (48% vs.12%;P =0.012) of the ultrasound-enhanced thrombolysis group were significantly higher than those of the standard thrombolysis group,but there were no significant differences in the reocclusion rate (8% vs.12%;P =0.637),incidence of symptomatic intracerebral hemorrhage (4% vs.4%;P=1.000),and mortality (4% vs.4%;P=1.000) compared with the standard thrombolysis group.Conclusions Ultrasoundenhanced thrombolysis can improve the sustained complete recanalization rate,complete recanalization rate,and good outcome rate after using rtPA within 2 h,and it does not increase the risks of symptomatic cerebral hemorrhage and death.It is a safe and effective adjunctive thrombolytic therapy.
8. The two-stage urethroplasty with distal urethral preset for proximal pediatric hypospadias

Objective:
To investigate retrospectively a modified procedure of two-stage urethroplasty with distal urethral preset for proximal pediatric hypospadias and evaluate the feasibility and advantages of the method.
Methods:
From January 2013 to January 2018, two-stage urethroplasty with distal urethral preset was performed in 36 boys from 19 months to 15 years old (median age 36 months) with proximal hypospadias. The penile straightening and distal urethroplasty were performed at the first stage, meanwhile, the original urethral opening was preserved. After the distal reconstructed urethra performed in 6 months, the neourethra was connected with proximal meatus to re-establish the continuous urethra. The wound infection, urinary leakage, urethral stricture, appearance of penis and urination were observed after the second stage operation.
Results:
One case had skin infection around the wound after first stage operation and healed after wound management. Another one case had the distal urethra dehiscence but the wound has no signs of infection. The child had moderate anemia, and the hemoglobin is 80 g/L.It was therefore considered that anemia was the cause. The hemoglobin level was normal during the second stage operation, and repaired by Snodgrass method in the second stage. Five cases had the balanue dehiscence.Thirty case had being succeeded after the first stage, included a case of wound infection 5 days after operation. Three cases developed urethral fistula after the second stage repair, which were mended in the third stage. At a mean follow-up of 20 months postoperatively (14 to 48 months), all patients had satisfied appearance of penis, unobstructed urination and continuous urine line, without urinary fistula, urethral stricture and urethrocele at the second stage repair.
Conclusions
The distal urethral preset and staged procedure is a feasible operation that corrects proximal hypospadias with few complications and with good cosmetic and functional outcomes.
9.Relevance exploration of blood free fatty acid, uric acid and Testosterone with C peptide in male patients with incipient type 2 diabetes mellitus

Objective To conduct a relevance analysis of blood free fatty acid (FFA),uric acid (UA)and testosterone (TTE)with C peptide in male patients with incipient type 2 diabetes mellitus (T2DM).Methods A total of 41 patients with incipient T2DM in outpatient or hospital-ized in our hospital were selected to detect the relevance between relevant indexes and C peptide. Results Serum C peptide was in positive relation with ages,body mass index (BMI),UA level, fasting insulin,FFA,homeostasis model assessment of Insulin Resistance (HOMA-IR)and home-ostasis model assessment-B (HOMA-B)and in reverse connection with glycated hemoglobin,but in no relationship with TTE.Additionally,the multiple linear regression analysis showed that UA was an independent risk factor for C peptide.Conclusion Serum C peptide is in close association with ages,obesity,UA,FFA and glycated hemoglobin and its independent risk factor is Hyperuricemia, which has no correlation with TTE in male patients with incipient T2DM.
