1.Chemical constituents in stem of Acanthus ilicifolius

Objective To study the chemical constituents in the stem of Acanthus ilicifolius.Methods Some compounds were repeatedly isolated and purified on silica gel column chromatography and vacuum liquid chromatography,their chemical structures were elucidated by their physicochemical properties and spectral data,such as IR,(()~1HNMR),(()~(13)C-NMR),and ESI-MS.Results Eight compounds were obtained from the stem of A.ilicifolius and identified as hexacosanoic acid(Ⅰ),stigmasterol(Ⅱ),tetratriacontanol(Ⅲ), 2-benzoxazolinone(Ⅳ),stigmasterol-3-O-?-D-glucopyranoiside(Ⅴ),vanillic acid(Ⅵ),4-hydroxy-2-benzoxazolone(Ⅶ),quercetin(Ⅷ).Conclusion Compound Ⅶ is first reported as a natural product.Compounds Ⅰ,Ⅲ,and Ⅵ-Ⅶ are reported from this plant for the first time.
4.The influence of Herba Artemisiae capillaris on the UDPGT activity in rat's hepat

Objective:In order to investigate the influence of Herba Artiemisiae Capilloais on the UDPGT activity in rat's hepat.Methods: The UDPGT activities induced by Herba Artemisae capillaris were determined by the Black's assay.Results: Herba Artemisiae capillaris increased the activity of UDPGT in rats' hepat and also increased the ratio of liver weight/body weight. Conclusions: Herba Artemisiae capillaris was able to induce the acitvity of UDPGT in rats' hepat.
5.Study on the antioxidation activity and resistance of lipid peroxidation of panax notoginseng flower total saponins

Objective To investigate the antioxidation activity and resistance of lipid peroxidation of panax notoginseng flower total saponins.Methods The panax notoginseng flower buds were extracted with ethanol. The hydroxyl free radical(?OH), 1,1-diphenyl-2-picrythydrazyl radical(DPPH)clearing rate and resistance of lipid peroxidation of rat liver induced by Fe2+-cysteine were determined by spectrophotometry. Results Half clearance of hydroxyl free radical and DPPH. by panax notoginseng flower total saponin was 0.035 mg/ml and 0.094 mg/ml, the maximum inhibition rate of lipid peroxidation of rat liver induced by Fe2+-cysteine was 89.31%, therefore moderate concentration of extracts had a strong inhibitory effect on lipid peroxidation. Conclusions Panax notoginseng flower total saponins have antioxidant activity and resistance of lipid peroxidation.
6.The effect of heart valve replacement in the same period of radiofrequency ablation on atrial fibrillation

Objective To investigate the effectiveness of heart valve replacement in the same period of radiofrequency ablation in the treatment of atrial fibrillation.Methods Eighty-six patients with atrial fibrillation underwent heart valve replacement in the same period of bipolar radiofrequency ablation system according to the maze heart valve replacement operation principle.And optimized the operation method.Results No death occurred during operation.The average ablation time was (23 ± 8) minutes with a range of 16-56 minutes.Sinus rhythm of the heart was restored after surgery in all patients (100%).The long-term success rate was 98.8% (85/86).Conclusion Our optimization of heart valve replacement in the same period of radiofrequency ablation enhanced the success rate of the operation,simplified the procedures,meanwhile it might need widely application clinically.
7.Protective effects of nitidine chloride on rats during myocardial ischemia/reperfusion

BACKGROUND: Studies indicated that lipid peroxidation due to increase of free radical is the key factor of ischemia/reperfusion injury.Shinyleaf pricklyash root extracts, rutaceae plant, is bitter in taste, no stimulation, which has the effects of promoting qi, relieving pain, promoting blood circulation by removing blood stasis, dispelling wind and dredging collaterals and antioxidation.OBJECTIVE: To observe the influence of nitidine chloride on myocardial ischemia/reperfusion injury in rats and analyze its mechanism.DESIGN: Randomized controlled study.SETTING: Departmentof Pharmacology and Department of Chemistry,Guangxi Medical University.MATERIALS: A total of 60 healthy Wistar rats were selected, half male and half female, with the body mass of 250-300 g. Nitidine chloride was provided by Department of Chemistry, Guangxi Medical University, batch number 20050609. MS4000U biological signal quantitative record analysis system, 722N evident spectrophotometer, hydrochloric acid verapamil (batch number 020701, 2 mL in each), malondialdehyde (MDA) and superoxide dismutase (SOD) kit were purchased from Guangzhou Longfeida Technology Co., Ltd., Shanghai Precision Scientific Instruments Corporation, Shanghai Harvest Pharmaceutical Co, Ltd. and Nanjing Jiancheng Bioengineering Institute, respectively. Hitachi 7170A full automatic biochemistry analyzer was also applied.METHODS: The experiment was performed at the Department of Pharmacology and Department of Chemistry, Guangxi Medical University between June 2004 and May 2006. ①Totally 60 healthy Wistar rats with normal ECG (half male and half female) were randomly divided into 6 groups:sham operation group, model group, 2, 1, 0.5 mg/kg nitidine chloride groups, positive control group with 10 rats in each group. The rats in the sham operation group received threading without deligation, and 90 minutes later the experiment was accomplished. Other 50 rats received left anterior descending branch of coronary artery deligation, ischemia for 30 minutes reperfusion for 60 minutes. 2 mg/kg verapamil, 2,1,0.5 mg/kg, 5 mL/kgnitidine chloride, saline of the same volume were injected into femoral vein in rats of the positive control group, different doses nitidine chloride groups and model group, respectively 10 minutes before deligating left anterior descending branch of coronary artery. ②Monitoring was conducted successively with standard limb Ⅱ lead ECG when performing reperfusion. Type,incidence rate and duration of cardiac arrhythmia were recorded within 60minutes. Change of ST segment was also recorded after reperfusion for 15minutes and 60 minutes. ③At the end of experiment, serum myocardial enzymology indexes were measured wi th full automatic biochemistry analyzer.MDA content and SOD activity in myocardial tissues were examined with thiobarbituric acid(TBA) method and xanthine oxidase (XOD) method, respectively. ④Measurement data and enumeration data between two groups were compared with t test and x2 test. MAIN OUTCOME MEASURES: Occurrence of cardiac arrhythmia, ECG ST segment elevation, change of serum myocardial enzymology indexes, MDA content and SOD activity of myocardial tissues in rats of each group.RESULTS: A total of 60 rats were involved in the result analysis. ①Degree of cardiac arrhythmia and ECG ST segment elevation of rats: The emergency time of cardiac arrhythmia in 1 and 2 mg/kg nitidine chloride groups was significantly later than that in the model group (P < 0.05,0.01). The duration of cardiac arrhythmia in the 1 and 2 mg/kg nitidine chloride groups and positive control group was obviously shorter than that in the model group (P < 0.05-0.01). The incidence rates of various kinds of cardiac arrhythmia were markedly less than those in the model group (P < 0.01). The degree of ST segment elevation at reperfusion for 15 and 60 minutes was remarkably lower than that in the model group (P < 0.05-0.01). ②Serum myocardial enzyme level: It was significantly higher in the model than the sham operation group after 60-minute myocardial ischemia and reperfusion (P?.01). Activity of myocardial enzyme in the 1 and 2 mg/kg nitidine chloride groups was remarkably lower than that in the model group (P < 0.01,P < 0.05). The level of myocardial enzyme decreased with the increase of nitidine chloride. It was lower significantly in the positive control group than the model group (P < 0.05-0.01 ). ③SOD activity of myocardial tissues: It was markedly lower in the model group than the sham operation group after 60-minute myocardialischemia and reperfusion (P < 0.01); It was dramatically higher than in the 1 and 2 mg/kg nitidine chloride groups than the model group (P < 0.01). The activity also increased with the increase of nitidine chloride. ④MDA content of myocardial tissues: It was distinctly higher in the model group than the sham operation group after myocardial ischemia reperftsion for 60 minutes (P < 0.01). It was remarkably lower in the 1 and 2 mg/kg nitidine chloride groups than the model group (P < 0.01). The content decreased with the increase of nitidine chloride. It was obviously lower in the positive control group than the model group (P < 0.05 ).CONCLUSION: ①1 and 2 mg/kg nitidine chloride can reduce the incidence rate of cardiac arrhythmia in rats with myocardial ischemia and reperfusion, postpone the emergence time of cardiac arrhythmia and shorten its duration, decrease the degree of ST segment elevation after reperfusion for 15 minutes and 60 minutes, which have similar effect with verapamil.② 1 and 2 mg/kg nitidine chloride can reduce the release of myocardial enzyme, relieve the severity of oxygen-derived free radicals injury, and has the effect of protecting myocardial injury during ischemia-reperfusion, in which represents a dose-dependent effect.
8. Influence of dexmedetomidine combined with ropivacaine in brachial plexus block: a Meta analysis

Objective:
To evaluated the efficacy of dexmedetomidine (DEX) as a local anesthetic adjuvant for brachial plexus block (BPB).
Methods:
The PubMed, Embase and Cochrane Library was searched on a computer to find randomized controlled trial (RCTs) of the effect of ropivacaine combined with DEX in brachial plexus block. The time limit for searching was from the establishment of the library to December 30, 2017. At the same time retrospective references were included in the literature. Two reviewers independently screened literatureaccording to the inclusion and exclusion criteria, extracted the data, and assessed the methodological quality of the included studies. Then Meta analysis was performed using RevMan 5.3.
Results:

9.Application of debridement and bone autografting combined with proximal femoral anatomical plates for benign tumor in proximal femur.

OBJECTIVETo explore the clinical effects of debridement and bone autografting combined with proximal femoral anatomical plate in treating benign tumor in proximal femur.
METHODSFrom January 2010 to October 2014, 30 patients with benign tumor in proximal femur were treated with debridement, autogenic ilium, autogenic ilium and allogeneic bone implant, and anatomic plate fixation. Among them, there were 13 males and 17 females aged from 12 to 68 years old with an average of 42 years old. The courses ranged from 1 month to 2 years with an average of 9 months. MSTS scoring were observed and compared before and after operation, and also applied to evaluate lower-extremity function. X-ray was examined to evaluate healing of focus. Postoperative complications were observed.
RESULTSAll patients were followed up from 12 to 48 months with an average of 29 months. MSTS score at the final following-up (27.06±2.59) was higher than preoperative (16.44±1.35), and there was significant difference(<0.05). X-ray at the final following-up showed bone graft fusion, pathological fracture were recover consciously, internal fixation was well, no loosening, deformation and displacement occurred. One case occurred incision fat liquefaction and 1 patient with giant cell tumor of bone relapsed at 13 months after operation.
CONCLUSIONSDebridement and bone autografting combined with proximal femoral anatomical plate is an effective method in treating benign tumor in proximal femur. It could control tumor, relieve pain, promote function and prevent occurrence of pathologic fractures.
10.Isolated pancreatic tuberculosis in non-immunocompromised patient treated by Whipple's procedure: a case report.

PANCREATIC tuberculosis (TB) is a rare disease and its diagnosis is difficult because of the lack of specific clinical manifestations. Computed tomography (CT) and magnetic resonance imaging (MRI) have some diagnostic values in this disease, but it is easy to misdiagnose pancreatic TB as a pancreatic tumor.1 In this article, we present a case of non-immunocompromised patient developing an isolated pancreatic TB, report the CT and MRI findings, and the surgical procedure for it.
