1.Treating atrial fibrillation in beating heart during surgery using bipolar radiofrequency ablation system

Objective To investigate the feasibility and efficacy of treating atrial fibrillation(AF)in beating heart surgery using bipolar Radiofrequency Ablation(RFA)system. Methods Treating AF in beating heart under normal temperature CBP surgery with bipolar RFA following the circuit of maze operation,2 - 6 times each place,until sinus rhythm appeared,then perform AVR or MVR in the stopped beating heart. Results All 18 cases survived the surgery. Sinus rhythm appeared in all cases. The average treating time was 28 minutes. Conclusions Application of bipolar RFA in the treatment of AF in beating heart is a safe and efficacy method. It is recommended to be applied widely.

5.To investigate the overexpression of Stat3 and Cyclin D1 in laryngeal neoplasm by immunohistochemistry.

OBJECTIVE:
To investigate the overexpression of Stat3 and Cyclin D1 in laryngeal neoplasm with immunohistochemistry method.
METHOD:
With immunohistochemistry method, we investigated the expression of Stat3 and Cyclin D1 in laryngeal neoplasm and analysis the relationship between Stat3 and clinical pathological factor.
RESULT:
Stat3 and Cyclin D1 overexpressed in laryngeal neoplasm tissue and they have positive relationship.
CONCLUSION
Active Stat3 may promote the transcription of target gene Cyclin D1, which could accelerate carcinomatous change.

6.The effect of heart valve replacement in the same period of radiofrequency ablation on atrial fibrillation

Objective To investigate the effectiveness of heart valve replacement in the same period of radiofrequency ablation in the treatment of atrial fibrillation.Methods Eighty-six patients with atrial fibrillation underwent heart valve replacement in the same period of bipolar radiofrequency ablation system according to the maze heart valve replacement operation principle.And optimized the operation method.Results No death occurred during operation.The average ablation time was (23 ± 8) minutes with a range of 16-56 minutes.Sinus rhythm of the heart was restored after surgery in all patients (100%).The long-term success rate was 98.8% (85/86).Conclusion Our optimization of heart valve replacement in the same period of radiofrequency ablation enhanced the success rate of the operation,simplified the procedures,meanwhile it might need widely application clinically.
7.Study of heart and lung protection in thoracoscopic mitral valve replacement operation on beating-heart

Objective To investigate the clinical significance of thoracoscopy combined with beating-heart technique to provide a certain reference for selecting clinical operation mode.Methods Forty patients with chronic rheumatic mitral stenosis receiving thoracoscopic mitral valve replacement surgery in the cardiothoracic surgery department of our hospital from September 2012 to September 2015 were included in this study and divided into the experimental group(beating-heart group,20 cases) and control group(arrested heart group,20 cases).The operative time,CPB time,enzymology indicators,RI and operative complications were analyzed and compared between the two groups.Results One case in each group died of postoperative low cardiac output syndrome.Other cases successfully completed the operation without serious operative complications such as low cardiac output syndrome,serious cardiac arrhythmia and air embolism.Two groups had no statistically significant difference in the operation time and time of CPB (P>0.05),but the ICU staying duration had statistical difference between the two groups(P<0.05).The levels of CK-MB,cTnI,LDH,TNF-α,IL-6,IL-8 and RI were significantly increased in two groups(P<0.05),while the levels in the experimental group were significantly lower than those in the control group(P<0.05).Conclusion Thoracoscopic mitral valve replacement operation with beating-heart may have better protection effect on heart and lung than thoracoscopic arrested-heart mitral valve replacement operation.
8. Blood purification therapy in children with acute liver failure

Acute liver failure(ALF)is a life-threatening clinical syndrome in children,which has a high mortality. Extracorporeal liver support systems include bioartificial liver,nonbioartificial liver and hybrid artificial liver,with the aim of supporting children with ALF,as a bridge to recovery or liver transplantation. The theoretical basis of non-bioartificial liver support system is blood purification. Although biochemical efficacy has been demonstrated by these devices,the effect on the final prognosis has been unclear. Compared with adults,there are less data on the treatment for children with ALF,and some new blood purification modes and combined blood purification modes have shown potential in the treatment of ALF in recent years. The aim of this review is to introduce the practice of blood purification and to provide references for the treatment of children with ALF.
9.Prevalence and risk factors of hyperuricemia in physical examination population of Shenyang City in 2013-2020

Objective To analyze the prevalence and risk factors of hyperuricemia (HUA) in Shenyang City, and to provide evidence for the prevention and treatment of HUA in this region. Methods From January 2013 to December 2020, 98,327 subjects who underwent physical examination in Shengjing Hospital of China Medical University in Shenyang were selected. The detection rate of HUA was calculated, and the risk factors of HUA were analyzed by multivariate logistic regression analysis. Results From 2013 to 2020, the overall prevalence of HUA was 22.48%, 30.75% in males and 9.13% in females. The prevalence rate in the male was significantly higher than that in the female (P<0.05). Except for a slight decrease in 2015 and 2020, the total prevalence rate showed an increasing trend year by year. With the increase of age, the prevalence of HUA in males decreased, while in females, it decreased slightly from 40 to 59 years old and increased significantly after 60 years old. With the increase of BMI (Body Mass Index), the prevalence of HUA also increased, and the difference was statistically significant (P<0.05). Logistic regression analysis showed that male, body mass index, physical examination year, hypertension, hyperlipidemia, hyperglycemia, triglyceride, LDL-C, abnormal liver and renal function were positively correlated with HUA, while age and HDL-C were negatively correlated with HUA. Conclusion HUA occurs mainly in male people among physical examination population in Shenyang. With the increase of BMI, HUA increases.
10.Clinical observation of traumatic granuloma after CO₂ laser cordectomy and laryngopharyngeal reflux.

OBJECTIVEThrough clinical observation of granuloma after CO₂ laser cordectomy under suspensive laryngoscope to discuss the effects of laryngopharyngeal reflux(LPR) to traumatic granuloma.
METHODSAccording to the classification of depth and range of CO₂ laser cordectomy, 111 cases were divided into 5 groups, and the incidence of postoperative granuloma of each group was observed. The largest number of the 5 groups was 49 cases of type III CO₂ laser cordectomy which was subdivided into 4 groups according to whether or not laryngopharyngeal reflux and whether or not proton pump inhibitor (PPI) treatment.
RESULTSIn 111 cases, 56 cases (50.5%) developed granuloma after CO₂ laser cordectomy. The incidence of traumatic granuloma after surgery was 10% (1/10), 26.1% (6/23), 53.1% (26/49), 78.6% (11/14), 80.0% (12/15) in I, II, III, IV, V type, respectively. It had statistical significance in chi-square test between 5 kinds of operative classification and the incidences of postoperative granuloma (χ² = 20.32, P < 0.01) and Spearman correlation analysis showed positive correlation between classification of operation and incidences of granuloma (r = 0.44, P < 0.01). According to LPR (-), LPR (+)and PPI (+), PPI (-), the incidence of granuloma had statistical significance in these 4 group patients of 49 type III cases (χ²= 5.83, P < 0.05). The incidence of granuloma after surgery was the lowest in LPR (-) PPI (+) group (30%) and the highest in LPR (+) PPI (-) group (80%), and it showed significant difference (χ² = 6.25, P < 0.05).
CONCLUSIONSWith the increase of removal depth and the range after CO₂ laser cordectomy, it appears rising trend in incidence of granuloma. Laryngopharyngeal reflux and PPI therapy on the incidence of traumatic granuloma has certain influence.
