1.Shan Guangzhi's Experience for the Treatment of Bone Metastasis of Prostate Cancer

Objective] To sum up professor Shan Guangzhi's clinical experience for the treatment of bone metastasis of prostate cancer. [Method] From following professor Shan Guangzhi in the clinical practice and further studying his medical cases,summarizes his experience in the treatment of prostate cancer bone metastases,and sums up his academic thoughts for the treatment of the disease.[Result] Professor Shan Guangzhi thinks that deficiency of the kidney is the basic pathogenesis of bone metastases of prostate cancer,at the same time accompanied by spleen deficiency and qi-movement disturbance. To prostate cancer patients with bone metastases, pain is the most common symptom,the reason for it is that stagnation leading to pain and poor nutrition lead to pain. The treatment should focus on two aspects,both the inner treatment and the external treatment; the internal treatment regards invigorating Qi and tonifying the kidney as the basis,also pays attention to regulate spleen and stomach,dredges the body down;the external treatment regards promoting blood circulation to remove meridian obstruction as the rule,it can effectively relieve patients' pain,and cooperates with diet,as a whole to improve the quality of survival in patients. [Conclusion]Professor Shan Guangzhi's experience in the treatment of bone metastasis of prostate cancer has exact curative effect,worth learning and promotion.
2.Treatment of osteoporotic spinal compression fractures by percutaneous vertebroplasty and kyphoplasty

[Objective]To study the effect and complications of percutaneous vertebroplasty and kyphoplasty in treating osteoporotic spinal compression fractures.[Method]Eighteen patients with twenty-seven vertebras in group,PVP(percutaneous vertebroplasty)were performed in fourteen patients(twenty-three vertebras),PKP(percutaneous kyphoplasty)were performed in four patients(four vertebras),VAS(Visual analogue scale) and mobility were evaluated 3 days before and after operation.Relief of back pain、recovery of living ability and complications were observed in the follow-up.[Result]All patients were operated on successfully and their back pain relieved immediately after operation,VAS was distinctly decreased from(7.5?0.7)pre-operation to(2.5?0.6)at 3 day after operation with a significant difference(P
3.Association of serum thioredoxin and 3-nitrotyrosine levels with diabetic retinopathy in the elderly

Objective To investigate the association of serum thioredoxin (Trx) and 3-nitrotyrosine (3 NT) levels with diabetic retinopathy (DR) in the elderly,in order to provide new evidence for clinical prevention and treatment of DR.Methods A total of 156 patients diagnosed with type 2 diabetes mellitus (T2DM) at the Department of Endocrinology were selected.Based on of fundus examination results,the patients were divided into the non-diabetic retinopathy group (NDR) (n=52),the nonproliferative diabetic retinopathy group (NPDR) (n =52) and the proliferative diabetic retinopathy group (PDR) (n=52),with 52 healthy people enrolled in the control group (CON).Solid phase enzyme linked immunosorbent assays (ELISA) were used to measure serum levels of Trx and 3-NT in all subjects.The results were statistically analyzed.Results The expression of Trx in the serum was the lowest in CON,followed by NDR,NPDR and PDR [[(324.1± 69.3) pg/L,(429.5±36.9) pg/L,(515.2±43.0) pg/L and (588.8±112.5) pg/L,respectively],and there were statistically significant differences between the groups (F=19.00,P=0.000).Similarly,the expression of 3 NT in the serum was the lowest in CON,followed by NDR,NPDR and PDR [(0.16±0.07) ng/L,(0.22±0.05) ng/L,(0.28±0.06) ng/L and (0.38±0.09) ng/L,respectively],and there were statistically significant differences between the groups (F=28.78,P=0.000).The expression levels of Trx and 3 NT in the serum were positively correlated with the severity of DR (r=0.64 and 0.66,respectively,P 0.001 for both).There was a correlation between serum Trx and 3-NT (r 0.71,P=0.001).Conclusions Serum levels of Trx and 3 NT are increased with the severity of DR,and they are positively correlated with the severity of DR.

5.Serum Lipid Profile in Chinese Premenopausal Women with Chronic Paraplegia

Objective To investigate the characteristics of serum lipids levels in young women with paraplegia in sedantary lifestyles.Methods 183 female paraplegics who admitted in China Rehabilitation Research Centre (CRRC) from Jan 2004 to Dec 2009 were investigated.Lipid profiles included triglyceride (TG), total cholesterol (TC), low density lipoprotein (LDL), and high-density lipoprotein (HDL).Results Female paraplegics showed higher mean TG and TC levels than mean levels of Chinese females (TG 1.44 mmol/L vs. 1.24 mmol/L; TC 4.45 mmol/L vs. 4.30 mmol/L). But no difference was found in HDL and LDL levels. TG, TC and LDL levels were correlated positively to age (r=0.158, P<0.05; r=0.271, P<0.01; r=0.207, P<0.01, respectively), but HDL showed no correlation to age. Prevalence of dislipidemia was 60%, including low HDL(33%), high TC(25%), high TG(24%), and high LDL(21%). Conclusion Lack of exercise may result in some dyslipidemia and increase cardiovascular risks in women who are under estrogen protection.
6.Effects of electroacupuncture at Chize (LU 5) versus Shangjuxu (ST 37) in rats with ulcerative colitis

Objective:To compare the effects between electroacupuncture (EA) at Chize (LU 5, the He-Sea point of the Lung Meridian) and Shangjuxu (ST 37, the lower He-Sea point of the large intestine) in rats with ulcerative colitis (UC) on the variations of mesenteric microcirculation and vasoactive intestinal peptide (VIP) in the colon, lung, and hypothalamus. The relative specificity of acupoints was also explored.
Methods: A total of 28 male Wistar rats were randomized into a normal group, a model group, a Chize (LU 5) group and a Shangjuxu (ST 37) group, 7 rats in each group. The UC model was established by enema with acetic acid. Since the third day after modeling, rats in the Chize (LU 5) group and Shangjuxu (ST 37) group respectively received EA at Chize (LU 5) and Shangjuxu (ST 37), 15 min each time for successive 7 d. The variations of mesenteric microvascular calibers and blood flow status were observed by a microcirculation microscopic tester; VIP in the colon, lung and hypothalamus was measured by radioimmunoassay.
Results:Compared with the normal group, the mesenteric microvascular calibers were significantly expanded in the model group (P<0.05); there was no significant difference between the model group and Chize (LU 5) group (P>0.05); compared with the model group and Chize (LU 5) group, the calibers were obviously shrunk in Shangjuxu (ST 37) group (P<0.05). The four groups showed no significant inter-group differences in comparing blood flow status (P>0.05). The colonic VIP levels in the model group and Chize (LU 5) group were significantly higher than that in the normal group (P<0.01,P<0.05); the VIP level in Shangjuxu (ST 37) group was markedly lower than that in the model group (P<0.01). There were no significant differences among the four groups in comparing VIP level in lung and hypothalamus (P>0.05).
Conclusion:The effects of Chize (LU 5) and Shangjuxu (ST 37) were different in treating UC. Shangjuxu (ST 37) showed a more significant efficacy in down-regulating VIP in the colon and regulating mesenteric microcirculation, while the effects of Chize (LU 5) were not obvious.
7.Biomechanical characteristics of posterior transpedicular screw fixation combined with calcium sulfate vertebroplasty in treatment of thoracolumbar compression fracture

BACKGROUND: The biomechanical studies about calcium sulfate cement vertebroplasty are only limited to the single fracture vertebra, not performed in spinal compression fracture unit with posterior transpedicular screw fixation. Furthermore, performing experimental study in the whole function spine unit (FSU) conforms to actual clinical situation. OBJECTIVE: To assess the biomechanical properties of calcium sulfate vertebroplasty combined with posterior transpedicular screw fixation in treatment of thoracolumbar compression fractures. DESIGN, TIME AND SETTING: A controlled experiment was performed at the Biomechanical Laboratory of Shanghai University in March 2009. MATERIALS: Fifteen fresh thoracolumbar spines were harvested from male calves and made into T_(11)-L_1 FSU, then divided into 3 groups randomly: normal control group, posterior transpedicular screw fixation group and transpedicular screw fixation plus vertebroplasty group. METHODS: T_(12) flexion-compression fracture models were made in all specimens of posterior transpedicular screw fixation group and transpedicular screw fixation plus vertebroplasty group, undergoing reduction and posterior transpedicular screw fixation, and calcium sulfate vertebroplasty combined with posterior transpedicular screw fixation respectively. MAIN OUTCOME MEASURES: All specimens were placed on the WE-10A universal testing machine for mechanical test. Load-straining, load-displacing, rigidity, strength and torsion of the FSU were performed in axial compression, flexion, extension and lateral bending states. The experimental outcomes were collected and compared by statistic analysis. RESULTS: The load-strain and loed-displacement showed a linear relationship. Straining values in vertebral body and intervertebral disc of calcium sulfate vertebroplasty plus posterior transpedicular screw fixation group were 14% and 12% less than that of posterior transpedicular screw fixation group, 21% and 13% less than that of normal control group. The thoracolumbar displacement in calcium sulfate vertebroplasty Plus posterior transpedicular screw fixation group decreased 25% and 37% as compared with other 2 groups respectively. Compared with normal control and posterior transpedicular screw fixation group, the thoracolumbar stiffness in calcium sulfate vertebroplasty plus posterior transpedicular screw fixation group increased 53% and 44% respectively. The strength in vertebral body and intervertebral disc of calcium sulfate vertebroplasty plus posterior transpedicular screw fixation group were 14% and 24% higher than that of posterior transpedicular screw fixation group, 13% and 20% higher than that of normal control group. The maximal twisting strength of FSU in calcium sulfate vertebroplasty plus posterior transpedicular screw fixation group were 18% and 30% higher than that of other 2 groups, the twisting stiffness were 30% and 40% higher than that of other 2 groups. The data above were significant differences statistically (P < 0.05). CONCLUSION: Posterior transpedicular screw fixation combined with calcium sulfate vertebroplasty show superior biomechanical properties for treatment of thoracolumbar compression fractures, which exhibits not only strong strength and stiffness, but also stable FSU, thus could decrease the stress loading of the internal fixation, the incidences of screw breakage and avoid the altitude loss of vertebral body.

9.Curative effects of percutaneous vertebroplasty with poly(methyl methacrylate) versus conservative treatment on fresh osteoporotic thoracolumbar vertebral compression fractures

BACKGROUND:Percutaneous vertebroplasty for vertebral fractures can effectively relieve acute pain and has the advantages of smal trauma, good curative effect and less complications, but for patients with osteoporotic compression fractures, there were varying degrees of osteoporosis after surgery, which have a longer course of disease and cannot be easy to cure. So the effectiveness of percutaneous vertebroplasty cannot be ful y evaluated based on the pain relief. OBJECTIVE:To study the curative effect of percutaneous vertebroplasty for patients with fresh osteoporotic thoracolumbar vertebral compression fractures. METHODS:We selected 24 patients undergoing percutaneous vertebroplasty and 24 patients receiving conservative treatment at the same time who had fresh osteoporotic compression fractures as research objects;and compared pain degree, vertebral body height and the kyphosis Cobb angle, function activity of the lower lumbar before and after treatment, the quality of life and clinical incidence of complications within 6 months after treatment in the two groups. RESULTS AND CONCLUSION:The degree of pain, the vertebral body height, kyphosis Cobb angle, function activity of the lower lumbar were al improved in the two groups after treatment (P<0.05), and these indexes in the percutaneous vertebroplasty group were better than those in the conservative treatment group (P<0.05). The quality of life and incidence of complications within 6 months after treatment were improved better in the percutaneous vertebroplasty group than the conservative treatment group (both P<0.05). These results suggest that the percutaneous vertebroplasty for fresh osteoporotic thoracolumbar vertebral compression fractures can effectively reduce the pain of patients, improve vertebral deformity and activities of the lower lumbar, and has obvious role in promoting the postoperative quality of life of patients.
10.Effects of shenmai injection on postoperative delirium in patients undergoing off-pump coronary artery bypass graft

Objective:To study the effects of shenmai injection on postoperative delirium (POD) in patients undergoing off-pump coronary artery bypass graft (OPCABG).Methods:40 patients undergoing OPCABG from September 2019 to March 2020 in Linyi People's Hospital were randomly divided into (group S) and control group (group C), 20 cases in each group. Shenmai injection (0.6 ml/kg)was diluted to 150 ml with 5% glucose in group S, and pumped through the central vein at the beginning of the operation. In the group C, at the beginning of the operation, 5% glucose injection (150 ml) was pumped through central vein. The values of ejection fraction (EF), cardiac output (CO), stroke volume (SV), stroke volume variation (SVV), bispectral index (BIS), oxygen partial pressure (PO 2), central venous oxygen saturation (ScVO 2) and lactic acid (LAC) at anesthesia induction (T 0), at the beginning of operation (T 1), at the beginning of coronary artery bypass graft (T 2), at the end of coronary artery bypass graft (CABG) (T 3), and at the end of operation (T 4) were recorded. Patients were evaluated for delirium on the 3rd and 7th day after surgery. Results:The EF, CO in the patients of group C and group S at T 2 were found to be significantly lower than those at T 0 ( P<0.05). The ScVO 2, PO 2 in the patients of group C and group S at T 4 were found to be significantly lower than those at T 0 ( P<0.05). The EF, CO and SV in the patients of group S at T 3 to T 4 were found to be significantly higher than those in group C ( P<0.05). The incidence of postoperative delirium on the third day in the group S was significantly lower than that in the group C ( P<0.05). Conclusions:Shenmai injection can improve the cardiac function and reduce the incidence of POD in patients with OPCABG.
