1.Operative treatment of Pipkin fracture
Journal of Chongqing Medical University 2007;0(07):-
Objective: To evaluate the result of operative treatment of hip dislocation combined with fractures of the femoral head. (Pipkin fracture). Methods: 32 cases were classified according to Pipkin classification, including 8 cases of type Ⅰ,15 cases of type Ⅱ,3 cases of type Ⅲ and 6 cases of type Ⅳ. Various surgical procedures were taken according to the classifications, ages, preoperative images and intraoperative check of the patients. Open reduction and fixation were taken in 30 patients and total hip replacement were performed in 2 patients. Results: Follow-up averaged 28 months, the post-operative function in 27 cases of internal fixation were: excellent in 11 cases (40.7%),good in 11 cases(40.7%), fair in 3 cases(11.1%),poor in 2 case (7.5%). The excellent rate was 81.4%. Conclusion: The key to good result for treating Pipkin fracture is early diagnosis and early operation. Operative approach should be chosen with various situations. Internal fixation with absorbable screw is an ideal method.

Objective To study the influence of early dynamization of interlocking intramedullary nail on the treatment effect of the patients with lower limb fracture .Methods 62 patients with lower limb fracture treated with interlocking intramedullary nail in our hospital from June 2010 to March 2012 were selected as the research subjects and divided into the control group and the obser-vation group with 31 cases in each group according to random number table .The control group was treated with routine treatment method ,while the observation group was treated with early dynamization .Then the Johner-Wruch score ,healing time ,rate of ad-verse fracture healing ,incidence rate of complications and fracture healing related factors before and after the treatment were ana-lyzed and compared .Results The Johner-Wruch score excellent and good rate of the observation group was higher than that of the control group ,the healing time was shorter than that of the control group ,the rate of adverse fracture healing and the incidence rate of complications were lower than those of the control group ,the fracture healing related factors after treatment were all better than those of the control group ,the differences had statistical significance (all P<0 .05) .Conclusion The early dynamization of interloc-king intramedullary nail has the better effect for treating lower limb fracture and its influence on the fracture healing related factors is better too .

OBJECTIVE To establish the project approval evaluation system for traditional Chinese medicine (TCM) preparations in medical institutions guided by new drug conversion, to improve the success rate of approval for TCM preparations in medical institutions and lay the foundation for the later drug conversion. METHODS Research and development team used the literature research method and brainstorming method to list and organize relevant elements of project evaluation and determine the initial indicator system. Experts were consulted using the Delphi method to confirm the evaluation index. The weights were calculated based on the proportion of importance scores for each indicator and assigned specific scores to each item. The indicator system was used to evaluate 31 TCM preparations applied for filing by various departments of our hospital from April to July 2023. RESULTS After two rounds of 17 experts’ consultation, the final TCM preparation system included five primary indicators, i.e. theoretical basis, clinical research foundation, pharmaceutical foundation, prescription, and clinical value, as well as 17 secondary indicators including prescription source, traditional Chinese medicine theory, clinical positioning and so on. Human experience was considered as the item which would be rejected as one vote. Based on the above indicator system, our hospital further improved the filing and project approval process for TCM preparations in medical institutions. Among the 31 TCM preparations applied for filing by various departments from April to July 2023, 8 TCM preparations with a score ≥65 were selected for development. CONCLUSIONS The evaluation system is objective, comprehensive, and highly operable. It is suitable for the selection of TCM preparations in medical institutions before research and development.
