2.Relationship between ways of nutritional support and immune function in patients with malignant obstructive jaundice after PTCD

Objective To investigate the clinical effect of different nutritional therapies on the immune function of patients with malignant obstructive jaundice after percutaneous transhepatic cholangiodrainage (PTCD).Methods A total of 50 patients with malignant obstructive jaundice who were admitted to our hospital from January 2009 to March 2013 were randomly divided into two groups according to the admis-sion order.The patients in group A (n=25 )received enteral nutritional support after PTCD,and those in group B (n=25 )received total parenteral nutritional support after PTCD.Intra-group and inter-group comparisons were made in terms of jaundice clearance,nutritional indices,and body’s immune function on preoperative day 1 and postoperative day 7;comparison between the two groups was made by t test. Results Among the 50 patients who underwent PTCD,39 (78%)had good drainage,while 1 1 (22%)did not reach the expectation,of which,5 (10%)were in group A and 6 (12%)in group B.In both groups,the nutritional indices on postoperative day 7 were significantly higher than those on preoperative day 1(P<0.05),but no significant difference in these indices was observed between group A and group B on postoperative day 7 (P>0.05).The immune function of patients in both groups was significantly improved following PTCD and nutrition-al support (P<0.05),but there was no significant difference between the two groups (P>0.05).Although the same scheme of nutritional support was used,there were 1 1 patients who did not achieve the expected jaundice clearance after PTCD and had limited improvement in immune function compared with those who had complete jaundice clearance (all P<0.05).Conclusion Jaundice clearance is closely re-lated to PTCD in patients with malignant obstructive jaundice,but not markedly associated with the ways of nutritional support.

5.The Effect of Anxiety and Depression on Cell Adhesion Molecules and Fibrolytic Activity in Cardiac-pacing Patients

Objective To observe the effect of anxiety and depression on cell adhesion molecules and fibrolytic activity in cardiac-pacing patients. Methods The levels of plasma P-selectin, E-selectin, sICAM-1, vWF and PAI-1 were measured in 39 cardiac-pacing patients with anxiety or depression as rersearch group, 32 cases cardiac-pacing patients without any psychological problems as observation control group and 20 healthy subjects as normal control group. Result The levels of plasma P-selectin, E-selectin, sICAM-1, vWF and PAI-1 in cardiac-pacing patients were significantly higher than those in normal control group, and their plasma levels in the patients with anxiety or depression were significantly higher than those in the patients without psychological problems. Conclusion There were significantly elevated cell adhesion molecules level and decreased fibrolytic activity in cardiac-pacing patients. The anxety and depression status could aggregate this condition.
6.Effects of High Density Lipoprotein on Expression of Alpha-Granulemembrane Protein (CD62P) and Lysosome Intact Membrane Protein (CD63) on Platelet

Objective To investigate the effects of high density lipoprotein (HDL) on thrombin-activated platelet al-pha-granulemembrane protein (CD62P) and lysosome intact membrane protein (CD63) expressions in vitro. Methods The equivalent volume of washed platelets prepared by hand was preincubated with HDL (1 g/L) in 37℃water for 15 minutes, which was then stimulated with different concentrations of thrombin (0.5 U/mL, 1 U/mL and 10 U/mL) for 10 minutes in wa-ter of 37℃. Meanwhile another three groups of washed platelets were incubated with thrombin (0.5 U/mL, 1 U/mL and 10 U/mL) for 10 minutes, respectively. The CD62P and CD63 from each sample were analyzed by flow cytometry (FCM). Results The CD62P positive rates of HDL-preincubated groups were significantly lower than those of different concentrations of thrombin groups (0.5 U/mL,1 U/mL and 10 U/mL) in the absence of HDL (11.55%± 1.34% vs 18.14%± 1.50%, 17.19%± 0.17% vs 26.24%± 0.77% and 19.79%± 0.32% vs 80.38%± 5.66%,P < 0.01). Meanwhile, The CD63 positive rates of HDL-preincubated groups were also significantly lower than those of thrombin-treated (0.5 U/mL, 1 U/mL and 10 U/mL) groups without HDL, namely,2.92%±0.22%vs 8.09%±0.48%(P<0.001), 4.20%±0.98%vs 14.15%±1.39%(P<0.001) and 5.12%± 0.09% vs 24.48%± 1.71%(P < 0.01). Conclusion HDL inhibits the expression of CD62P and CD63 on throm-bin-stimulated platelets in vitro.
7.Fatty liver development and changes of metabolic parameters

Objective To investigate the relationship between fatty liver progress and metabolic changes.Methods A total of 414 patients who had B-mode ultrasonography confirmed fatty liver development during 2 health check-ups within 2 years were enrolled in this study.Paired t and χ2 tests were used to compare body mass index (BMI),alanine aminotransferase (ALT),aspartate aminotransferase (AST),fasting plasma glucose (FPG),serum uric acid (UA),triglycerides (TG),and high-density lipoprotein cholesterol (HDL-C) among the participants.Results BMI,TG,HDL-C,ALT,AST,and UA were significantly increased after fatty liver progressed,although HDL-C was largely decreased.ConclusionsPrevention and treatment of MS as well as early intervention for fatty liver should be important for successful control of fatty liver.
8.Preventive effects of punicalagin against ultraviolet B-induced damage to human HaCaT keratinocytes

Objective To investigate the mechanisms underlying the protection by punicalagin against ultraviolet B (UVB)-induced damage to keratinocytes.Methods Cultured human HaCaT keratinocytes were divided into several groups:blank control group receiving no treatment,punicalagin groups treated with various concentrations of punicalagin,UVB group irradiated with UVB at 30 mJ/cm2,combination groups pretreated with different concentrations of punicalagin followed by UVB radiation at 30 mJ/cm2.The concentrations of punicalagin were 5,10,20,40 and 80 μmol/L in the cell proliferation assay,10,20 and 40 μmol/L in the other assays.After additional culture for different durations,methyl thiazolyl tetrazolium (MTT) assay was performed to evaluate the proliferation of HaCaT cells,Hoechst and propidium iodide (PI) staining as well as flow cytometry to detect the apoptosis in cells,reverse transcription-PCR to quantify the mRNA expressions of matrix metalloproteinase-1 (MMP1) and tissue inhibitor of metalloproteinase-1 (TIMP1) in HaCaT cells,Western blot to determine the phosphorylation levels of the mitogen-activated protein kinase (MAPK) pathway-related proteins including P38,JNK and ERK.Statistical analysis was carried out by t test,one-way analysis of variance,and Dunnett's t-test.Results As the MTT assay showed,punicalagin at 10-40 μmol/L showed stronger pre-protective effects against UVB-induced damage to HaCaT cells compared with punicalagin at the other concentrations.The number of cells highly positive for both Hoechst and PI staining was larger in the UVB group than that in the blank control group,but smaller in the combination groups than in the UVB group.The percentage of apoptotic cells increased significantly in the UVB group compared with the blank control group (9.82% ± 0.11% vs.1.24% ± 0.91%,P < 0.01),but decreased significantly in the three combination groups (punicalagin (10,20 and 40 μmol/L) + UVB) compared with the UVB group (6.38% ± 0.14%,5.24% ± 0.17% and 3.77% ± 0.11% vs.9.82% ± 0.11%,all P< 0.01).Theexpression of MMP1 mRNA was significantly higher,but that of TIMP1 mRNA was significantly lower in the UVB group than in the blank control group (both P < 0.01),whereas no statistically significant difference was observed in the expression of MMP1 or TIMP1 mRNA between the punicalagin groups and blank control group (all P > 0.05).The pretreatment with punicalagin significantly reduced the expression level of MMP1 mRNA (P < 0.01),but elevated that of TIMP1 mRNA (P < 0.01) in the combination groups compared with the UVB group.As Western blot showed,the phosphorylation levels of P38,JNK and ERK were markedly increased in the UVB group (all P <0.01),but experienced no significant changes in the punicalagin groups (all P > 0.05) compared with the blank control group,and decreased to different degrees in the combination groups compared with the UVB group (all P <0.01).Conclusion Punicalagin has a pre-protective effect on UVB-induced damage to HaCaT cells.
9.Application and Prognosis of Intra-Aortic Balloon Pump for Patients with Acute Myocardial Infarction

Objective To investigate the prognosis of the intra-aortic balloon pump (IABP) used for patients with acute myocardial infarction and to the risk factors. Methods 88 patients accepted IABP were analyzed retrospectively. Results In spite of overall survival rate (54.5%), under circumstances of percutaneous coronary intervention (PCI), survival rate could reached to 59.9%, with 31.6% and 72.5% in cardiogenic shock group and high-risk group respectively. The time of IABP application increased in drug group. Multivariate Logistic Regression Analysis demonstrated independent-predictor effects of C-reactive protein (CRP), pulmonary infection, renal dysfunction, and wall motion disorder. Moreover, type of coronary artery disease (CAD), renal function, ejection fraction and pre-IABP mean arterial blood pressure could influence the time of IABP application. Conclusion In view of safety, IABP could be used to provide circulatory support for patients with very high risk as well as with cardiogenic shock. Application of IABP could hardly improve in-hospital mortality, and the time of IABP-using could be influenced by many clinical factors, such as type of CAD, renal dysfunction, etc.
10.Intermittent porta hepatis occlusion is a risk factor of postoperative intra-hepatic recurrence of hepatocellular carcinoma

Objective To investigate the impact of intermittent porta hepatis occlusion on postoperative intra-hepatic recurrence of hepatocellular carcinoma.Methods We retrospectively reviewed the records of 335 patients who underwent partial hepatectomy.The patients were classified into 2 groups:(1) the study group (n=97):porta hepatis was occluded with intermittent Pringle maneuver with 2-3 cycles of clamp/unclamp time of 15 min/5 min,repeated 2-3cycles; (2) the control group (n=238):including using Pringle maneuver,preconditioning occlusion of porta hepatis and selective occlusion of portal blood inflow.Patients were followed-up in the Outpatient Department once every 2-3 weeks in the 1st year,and once every 3-6 weeks in the 2nd year with US/CT/MRI and serum AFP test.The mean duration of follow-up was 26.5 months.Results The perioperative mortality was 1.8% (6/335).Tumour recurrence in the study group was 31.6% and 48.4% in the 1st and 2nd year,respectively.The recurrence rates were significantly higher,than the 21.4% and 38.0% in the control group (P<0.05).To exclude the miscellaneous factors which were involved in intra-hepatic recurrence of HCC,we set up 3 criteria to include patients for subgroup analysis:tumor ≥5 cm; serum AFP decreased to normal level within 4 weeks; negative intra-operative US scan.The number of patients included were 79 and 155 in the study and the control groups,respectively.There were significant differences in recurrence rate between the study and the control groups in the 1st and 2nd year (29.1% vs 18.7%,46.8% vs 35.5%,P<0.05).There were no significant differences in overall survival rate between the two groups.Conclusions Intermittent porta hepatis occlusion is a risk factor of postoperative intra hepatic recurrence of hepatocellular carcinoma.
