Journal of China Pharmaceutical University 2022;53(5):577-590
This study was performed to determine the metabolic profile of four amide synthetic cannabinoids that recently abused, i.e., ADB-4en-PINACA, 4CN-CUMYL-BUTINACA, 5F-EMB-PICA and 4F-MDMB-BUTICA, in human liver microsomes (HLMs). The four amide synthetic cannabinoids were added to the microsomal incubation model, being incubated for 10 min, 60 min or 3 h to simulate human hepatic metabolism.Liquid chromatography quadrupole time-of-flight mass spectrometry (LC-QTOF-MS) analytical instrument was employed to determine and speculate the structure of phase I metabolites and their possible metabolic pathways. The results showed that there were 27 phase I metabolic pathways for the four amide synthetic cannabinoids, including hydroxylation, carboxylation, N-dealkylation and ester hydrolysis, with the main phase I metabolic pathways of ester hydrolysis, dihydrodiol (pentenyl tail), oxidative defluorination to carboxylic acid, monohydroxylation (alkyl side chain or indole/indazole ring) and N-dealkylation.The results of this study may provide potential detection markers for forensic identification and sewage abuse assessment of the four amide synthetic cannabinoids.

Objective: To observe the effects of electroacupuncture (EA) at Neiguan (PC6) on arrhythmia during acute myocardial ischemia-reperfusion and the expression of connexin 43 (Cx43) in rats. Methods: A total of 40 Sprague-Dawley male rats were used. Ten rats were randomly selected as the blank group, and the remaining 30 rats were randomly divided into a model group and an EA group, with 15 rats in each group. Before modeling, rats in the EA group received one session of EA intervention at bilateral Neiguan (PC6) for 30 min; the other groups were treated with the same grasping and anesthesia for 30 min without intervention. PowerLab physiological recorder was used to record electrocardiograph within 30 min of infarction. After the experiment, cardiac tissue and serum were collected from rats. Hematoxylin-eosin (HE) staining was used to observe the morphological changes of myocardial tissue in the ventricular infarction area of rats in each group. The expression of Cx43 protein in the myocardium of each group was detected by Western blotting (WB). Enzyme-linked immunosorbent assay (ELISA) was used to determine the activity of Na+-K+-ATPase in myocardial tissue and the serum content of endogenous digitalis-like factor (EDLF) in rats. Results: There was no statistical difference in arrhythmia score between the EA group and the model group, but the total duration and average duration of arrhythmia in the EA group were decreased (P<0.01). HE staining showed that compared with the blank group, myocardial cells in the model group were disorganized and seriously damaged. The pathological changes in the EA group were similar to those in the model group, but the damage was relatively minor. The results of WB showed that compared with the blank group, the Cx43 expression in myocardial tissue of the model group was decreased (P<0.01); compared with the model group, the Cx43 expression in the EA group was increased (P<0.01); compared with the blank group, the Na+-K+-ATPase activity in myocardial tissue of the model group was significantly decreased (P<0.01); compared with the model group, the Na+-K+-ATPase activity in the EA group was increased (P<0.01). ELISA results showed that compared with the blank group, the serum EDLF content in the model group was significantly increased (P<0.01); compared with the model group, the EDLF content in the EA group was decreased (P<0.01). Conclusion: EA at Neiguan (PC6) can delay and reduce the onset of arrhythmia during myocardial infarction in the rat model of myocardial ischemia-reperfusion. Its mechanism of action may be related to the regulation of the Cx43 expression in myocardial tissue, improvement of the activity of Na+-K+-ATPase in myocardial tissue, and increase in the content of serum EDLF.

Objective To evaluate the short-term impact of comprehensive community-based intervention programs on smoking cessation counseling services provided by community medical staff and related influencing factors in three districts of Hangzhou city. Methods Within the framework of Community Interventions for Health (CIH) Program,a community trial was conducted in two districts (Xiacheng and Gongshu) and a district (Xihu) as control,by a parallel comparison and random grouping based quasi-experimental design. Two independent questionnaire-based surveys of cross-sectional samples in the intervention and comparison areas were used to assess the impact of intervention. Results There were 299 and 141 medical staff in the areas of intervention and ‘ control',respectively. For the intervention area, the quantity of available resources increased from 2to 3 and the quantity of used resources increased from 1 to 3 (both P<0.001),while the area of control had a downward trend for both quantities. For the ratios of well-readiness for smoking counseling,in the intervention area were 35.5%,52.0% before and after intervention (P<0.001),while the ‘controlled' area they were 28.1%,39.3%,respectively (P=0.162).A slight increase was seen in the intervention area for the proportion of the medical staff who had been provided smoking cessation counseling,as ≥90% patients (including asking smoking status, announcing risks related to the risks of smoking and advising smoking cessation programs etc.),while the proportion decreased in the area of ‘control'.The improvement of preparedness would promote medical staff to ask their patients about smoking status (OR=1.43,P=0.007),while all factors as a whole would not influence the medical staff to inform patients about the danger of smoking and advice patients to quit smoking. Conclusion Comprehensive community-based interventions could increase the opportunities for medical staff to acquire and utilize smoking cessation resources to some extent,as well as promote those staff' s preparedness. However, intervention itself might fail to improve the behavior of providing such services.Support that came from the policy or from the institutions also need to be strengthened.

Acute pancreatitis ( AP ) is one of the common disorders of gastrointestinal system in clinical practice characterized by sudden onset, long course of disease, complex complications, severe conditions, heavy social and economic burden and the like. Recent years has seen a decreasing mortality due to the update and promotion of a series of therapeutic concepts. However, as the imbalance of comprehensive healthcare levels among different regions in China, substandard diagnosis and treatment of AP still exist, lowering the overall success rate of AP treatment. Moreover, the waste and miss of AP patients' data during the treatment period affect our original innovation in the field of AP research. Thus, after the accumulation of long-term exploration and practice, we integrated diverse data resources and established a single-disease database of AP by cooperating with computer and artificial intelligence ( AI) professionals, finished the deployment of hardware and network arrangement, generated a logical system of "business database→scientific database→sharing database", and built AP data sharing platform (www. ap-database. com), providing the fundamental functions including browse, search, upload and download ( with permissions) . We hope to share the abundant and free medical date with clinicians, computer or AI practitioners and attract communications and cooperation from different fields, which may deepen new values from current data mining and advance the research on AP.
