Objective: To study the relationship between blood pressure (BP) rhythm and cognitive function in elder hypertension patients. Methods: Our research included in 2 groups: Hypertension group:n=74 patients≥80 years and Control group, n=36 elder subjects without hypertension. 24-hour ambulatory blood pressure monitoring (ABPM) was conducted in both groups. According to nocturnal BP reduction rate, Hypertension group was further divided into 4 subgroups as Dipper type BP subgroup, Non-dipper type BP subgroup, Anti-dipper type BP subgroup and Super-dipper type BP subgroup. Cognition function was evaluated by Mini-menta state examination (MMSE) form, MMSE score was compared among different groups and subgroups. Results: By MMSE score, the abilities of orientation, memory, attention, calculation and recall were different between Hypertension group and Control group,P<0.05. By MMSE score, the abilities of orientation and memory were different between Anti-dipper type BP subgroup and Dipper type BP subgroup,P<0.05; the abilities of orientation, memory, attention, calculation and recall were different between Non-dipper type BP subgroup and Dipper type BP subgroup,P<0.05; the memory was different between Super-dipper type BP subgroup and Dipper type BP subgroup,P<0.05. Conclusion: In elderly hypertension patients, compared to those with normal BP rhythm, the abnormal BP rhythm patients had the higher probability to suffer from cognitive dysfunction which was mainly as decreased abilities of orientation, memory, attention, calculation and recall.

We carried out the multiplicity teaching by remodeling experiment process, designing integral test, and resolving practical issue and setting up comprehensive trail. It was proved by fact that the multiplicity teaching increased the study interesting of students, inspired their activity, initiative and creativity.

Objective Speckle is the inherent problem exists in B-Mode ultrasound image,and effective speckle noise reduction will improve the image quality and contribute greatly to clinical doctors to diagnose,especially in fine ophthalmic examination with high frequency ultrasound.Methods This paper proposed a new speckle reduction method based on the Laplacian pyramid and multiscale analysis.In the Laplacian pyramid,true clinical features were separated from noise,according to the different bandpass ultrasound image characteristics in each layer,and the advanced eight directions speckle reduction anisotropic diffusion (SRAD)was adapted to suppressed the speckle noise,and the identified noise and the extracted features were selectively emphasized by suitable edge,coherence and contrast enhancement filtering from fine to coarse scales.The performance of the proposed method was compared to the traditional SRAD method and coherence-enhancing diffusion method by measuring the equivalent number of looks (ENL) and Time cost.Results The ENL and Time cost value of the proposed method were higher compared to the SRAD and the cedif method,i.e 1.172 3 vs 1.122 3,0.929 3 and 0.864 0 vs 1.396 0,1.468 3.Conclusions In summary,the proposed method can more effectively remove the speckle noise while preserving the edge and details of the images.

To study maternal and perinatal outcomes after cervical cerclage in both singleton and twin pregnancies,we retrospectively reviewed women undergoing cervical cerclage for cervical insufficiency at Tongji Hospital,Wuhan,China from January 1,2010 to July 31,2015 to evaluate primary and secondary outcomes for subgroups with cervical length (CL) ≤15,＞15 to ＜25,and ≥25 mm.Of 166 patients who underwent cervical cerclage,after exclusion of patients with missed abortion and continuing pregnancy,141 patients (121 singleton and 20 twin pregnancies) were included in the analysis.Mean gestational age at birth was 34.22 and 28.27 weeks for singleton and twin pregnancies,respectively.There were 17 (14.05％) and 13 (33.33％) neonatal deaths in singleton and twin pregnancies,respectively.Mean age (31.60-±4.62 vs.31.22±4.63 years,P=0.39) and gestational weeks at cerclage (18.50-±4.62 vs.19.31±4.99,P=0.47) were similar for both groups.Mean gestational weeks at delivery (34.22±5.77 vs.28.27±6.17,P＜0.001) and the suture to delivery interval (15.72±7.15 vs.8.96±6.70,P＜0.001) were significantly longer in the singleton group.These variables indicate a linear negative correlation with the degree of CL shortening,with better outcomes in patients with CL ≥25 mm who underwent cerclage,both in singleton and twin pregnancies.No difference in mode of delivery existed between the singleton group and twin group.Our results indicate a high risk of preterm delivery in both groups,especially in the twin group.Patients with a history of preterm labor and CL ＞25 mm in the current pregnancy,possibly in a twin pregnancy,could benefit from elective cervical cerclage;however,cervical cerclage was inadvisable for twin pregnancies with a CL ＞15 and ＜25 mm.Our data emphasize the importance of re-evaluating the efficacy of cervical cerclage for twin pregnancies in well-designed clinical trials.

Objective To assess the right ventricular (RV) performance in patients with type 2 diabetes mellitus (2-DM) by two-dimensional speckle tracking imaging (2D-STI),and to explore the clinical value of RV longitudinal strain and strain rate.Methods Thirty-seven patients with 2-DM only and thirtyone patients coexisting diabetes and hypertension (DM + HTN) were studied.Thirty-nine healthy age matched persons served as control subjects.In each patient a conventional two-dimensional echocardiography was performed and also an echocardiographic study with strain/strain rate imaging was studied.Analysis of RV longitudinal systolic strain were obtained in the apical four-chamber view of the RV for the assessment of the RV free wall(F-PLSS),interventricular septum (S-PLSS) and the global RV wall (G-PLSS).The entire RV longitudinal peak systolic strain rate (G-SRs),peak early-diastolic strain rate (G-SRe) and peak late-diastolic strain rate (G-SRa) were performed in the apical four-chamber view.Results Compared with controls,F-PLSS,S-PLSS,G-PLSS,G-SRs and G-SRe were decreased in 2-DM and DM + HTN (all P ＜0.01),with lower values in DM + HTN (P ＜0.05).In the patients coexisting diabetes and hypertension,G-SRa was significantly lower than those in the control group (P ＜ 0.05).Compared with 2-DM,this difference was not significant in controls and DM + HTN (P ＞0.05).Conclusions No matte the patients of 2-DM with or without hypertension,the early changes of RV dysfunction can be found by 2D-STI.Patients coexisting diabetes and hypertension may have worse RV dysfunction.
