1.Analysis of monitoring results of coal-burning borne endemic fluorosis in Fuyuan county Qujing city in 2009

ObjectiveTo explore the prevalence state of children in coal-burning borne endemic fluorosis in Fuyuan county Qujing city,to know the implementation of preventing measures,and to provide a scientific basis for prevention of endemic fluorosis.MethodsIn 2009,105 families were sampled to check the service condition of defluoridation stove and the effect of health education (the awareness of use defluoridation stove reasons) in Xiaoyuwang,Tuoga and Shaliang villages,Fucun town of Fuyuan county.Food fluorine of indoor stored peppers and corns was monitored of 20 families in Xiaoyuwang village,5 household were randomly selected from east,west,south,north and middle places of the village.Air fluorine was monitored from indoor air samples of 5families,dental and urinary fluorine was monitored of the native-born children aged 8 to 12,and dental fluorosis of these children was checked.Clinical endemic fluorosis and X-ray endemic fluorosis was monitored of local adult residents over the age of 30 that had been lived in the area for more than 5 years.ResultsThe rate of correct use of the defluoridation stove was 100%,and adult awareness rate of use defluoridation stove reasons was 82.86%(87/105).Of the 19 pepper samples,fluorine content were 9.98 - 538.40 mg/kg and median was 32.38 mg/kg,all exceeded the standard(1.5 mg/kg).Of the 25 corn samples,fluorine content were 0.42 - 101.54 mg/kg and median was 6.32 mg/kg,with 18 samples exceeded the standard(1.0 mg/kg).Of the 6 air samples,fluorine content were 0.000 98 - 0.003 33 mg/kg and median was 0.001 45 mg/kg.Detection rate of dental fluorosis of children aged 8 - 12 was 42.3%,urinary fluorine content were 0.13 - 1.80 mg/L,geometric mean was 0.58 mg/L,4 adults were diagnosed clinical skeletal fluorosis,detection rate was 2.46%(4/162),5 adults were found by X-ray with endemic fluorosis,detection rate was 9.61%(5/52).ConclusionPeppers and cons fluorine pollution is serious,general public should be guided to change their lifestyles to minimize the fluorine pollution.
2.Protective effect of vitamin C on endothelium-dependent arterial dilation in patients with impaired glucose tolerance during oral glucose loading

During oral glucose tolerance test(OGTT),endothelium-dependent vasodilation(EDD)at different time points in impaired glucose tolerance(IGT)group was lower than that in normal control group.EDD at 60 and 120 min in IGT + vitamin C group was higher than that in IGT group(all P<0.05).There was a negative relationship between blood glucose level and EDD during OGTT in IGT patients.
3.Clinical effects of sub-maximum ergometry exercise rehabilitation in patients with chronic obstructive pulmonary disease.

OBJECTIVETo study the rehabilitation effects ergometry on COPD patients.
METHODSThirty COPD out-patients in our Hospital were randomly divided into 2 groups. Rehabilitation group, 15 patients, performed leg ergometry exercise of 80% peak Watt x 30min/d x 3d/w x 12w. Another 15 patients were control group without exercise. All patients received conventional therapy. Pulmonary function testing (PFT), cardiopulmonary exercise testing (CPET), arterial blood gas analysis (ABG), Borg and CAT sores were done at both baseline and 12 w.
RESULTSThere was no statistically difference in lung function testing, blood gas analysis and cardiopulmonary exercise test when pre- exercises between 2 sub-groups. The IC, peak VO2 and peak, W of rehabilitation group significantly increased (P < 0.05); and Borg and CAT.scores significantly decreased (P < 0.05) from baseline; and other PFT and ABG did not change (P > 0.05). While there was no difference in control group (P > 0.05).
CONCLUSIONLeg submaximal ergometry rehabilitation improves health condition and ameliorate dyspnea symptoms in COPD patients.

4.Meatoplasty with pedicle flap for meatal stenosis secondary to chronic balanitis.

OBJECTIVETo evaluate the effect of meatoplasty with the pedicle flap in the treatment of meatal stenosis secondary to chronic balanitis.
METHODSWe retrospectively analyzed 32 cases of meatal stenosis secondary to chronic balanitis treated by meato- plasty with the pedicle flap. All the patients had a history of chronic balanitis and had received meatal dilatation or simple ventral mea- totomy without significant effect. Their mean maximum urinary flow rate (Qmax) was (4.3 ± 2.4) ml/s. During the operation, A "/\"-shaped incision was made in the healthy epidermis and a flap was harvested from the frenulum. After complete removal of the scar, the flap was placed into the urethral wall, followed by reconstruction of the external urethral orifice.
RESULTSThe patients were fol- lowed up for 6 to 30 months, which revealed smooth urination in all the patients with Qmax of (26.7 ± 4.5) ml/s and normal erectile function and uresiesthesis.
CONCLUSIONWith little invasiveness and few complications, meatoplasty with the pedicle flap is an ideal surgical method for the treatment of meatal stenosis secondary to chronic balanitis. However, there might be some change in the normal appearance of the balanus postoperatively, and its long-term effect needs further observation.

5.Application of a self-made steel wire guide in the treatment of avulsion fractures of tibial posterior cruciate ligament.

OBJECTIVETo explore the effect of a self-made guiding needle of steel wire in guiding the wire through the tibial tunnel for the treatment of avulsion fractures of tibial posterior cruciate ligament with open reduction and wire fixation.
METHODSFrom February 2011 to June 2014, a total of 22 patients with avulsion fractures of tibial posterior cruciate ligament underwent surgical treatments were analyzed, including 14 males and 8 females with an average age of 35.6 years old (ranged, 17 to 63 years old). According to Meyers classification, 9 patients were classified as type II, 13 patients were classified as type III. All the patients underwent open reduction and wire fixation with medial knee "L" shape approach. A wire guiding needle was used to guide the wire through the tibial tunnel during operation.
RESULTSWith the assistance of wire guidance needles, wires passed through the tibial tunnel rapidly during the operation in all the 22 patients. All the patients were followed up, X-ray imagings 6 months after operation showed the fractures healed well. The average follow-up time in all patients was 6 months (ranged, 6 to 12 months). The averaged Lysholm knee score in 22 knee was 92.7 +/- 3.4. All patients' posterior drawer test were negative.
CONCLUSIONSelf-made wire guiding needle can simplify the operation procedures in which the wires pass through the tibial tunnel, shorten the operation time, reduce the surgical trauma and complications, and be worthy of clinical application.

6.18F-FLT PET/CT imaging for detecting and staging patients with nasopharyngeal carcinoma

Objective To evaluate the usefulness of 18F-FLT PET/CT imaging in detecting and staging nasopharyngeal carcinoma (NPC) patients.Methods Thirteen patients with NPC underwent wholebody 18F-FLT PET/CT imaging,one of which underwent whole-body 18F-FDG PET/CT imaging one day earlier.SUVmax and SUVmean from 18F-FLT and18F-FDG imaging were obtained using circular ROI in primary and metastasis lesions and were compared with the results of histopathology.The staging results by 18 F-FLT PET was compared with those by CT.Results The SUVmax and SUVmean obtained from 18F-FLT imaging in 22nasopharyngeal sites in 13 patients were 6.04 ±3.61 and 5.09 ±2.89,and the SUVmax and SUVmean in 26 lymphadenopathy were 5.56 ± 3.11 and 4.65 ± 2.79.18 F-FDG SUVmax were higher than 18 F-FLT SUVmax in one primary lesion ( 8.32 vs 4.38 ) and two lymph nodes (3.30 ± 0.07 vs 1.48 ± 0.06) in the patient who underwent the two imagings.Compared with CT staging results,the TNM stage in 3 patients had been changed based on 18 F-FLT PET/CT imaging.Conclusions High radioactivity of primary and second lesions can be detected on 18F-FLT imaging in patients with NPC and 18F-FLT PET/CT imaging may be useful in staging for NPC.

8.Effect of androgen deprivation therapy on bone mineral density in prostate cancer patients.

AIMTo evaluate the effect of androgen deprivation therapy (ADT) on bone mineral density (BMD) in prostate cancer patients.
METHODSForty-nine prostate cancer patients with their BMD determined were divided into two groups: the non-treated group included 21 patients before the commencement of ADT and the treated group, 28 patients, who had received ADT for more than 1 year. BMD was measured by dual energy X-ray absorptiometry (DEXA) in the lumbar spine (L2-4) and femoral neck.
RESULTSThirteen (62 %) non-treated and 23 (82 %) treated patients fulfilled the BMD criteria for osteopenia or osteoporosis. Z scores for age-matched control in lumbar spine and femoral neck were -0.9 +/- 0.7 and -0.6 +/- 0.5, respectively, in the treated group, and -1.8 +/- 1.1 and -1.6 +/- 1.0, respectively, in the non-treated group, the differences between the two groups were highly significant (P<0.01).
CONCLUSIONProstate cancer patients who received ADT for more than 1 year had a significantly lower BMD in the lumbar spine and femoral neck than those before the beginning of ADT.

9.Effect of oleum fructus brucease injection via bronchial arterial infusion in treating advanced lung cancer.

OBJECTIVETo explore the clinical effects of bronchial arterial infusion (BAI) with oleum fructus bruceae (OFB) Injection and chemotherapeutic agents (CTA) in treating advanced non-small cell lung cancer (NSCLC).
METHODSOne hundred and three patients with advanced NSCLC were randomized into 2 groups, the 98 patients in the treatment group treated by BAI with OFB + CTA and the 50 in the control group by BAI with CTA alone. The incidence of adverse reaction, change of tumor size and patients' quality of life (QOL) in the two groups were observed and compared.
RESULTSThe objective effective rate (CR + PR) was 63.3% in the treatment group and 46.0% in the control group (P < 0.01); the median survival duration in them was 363 days and 305 days; the 1-year cumulative survival rate was 70.4% and 44.0%, and the QOF improving rate was 83.7% and 62.0% respectively, the difference between groups were all statistically significant (P < 0.01). In addition, the incidence of adverse reactions of digestive symptoms, bone marrow suppression and the hepato-, renal and cardiac toxicities were lower in the treatment group than those in the control group (P < 0.01).
CONCLUSIONBAI with OFB + CTA in treating NSCLC could enhance the objective therapeutic effect of simple chemotherapy, as well as raise the QOL and protect immune and medulla function in patients.

10.Application of continuous renal replacement therapy in the rescue of MODS patients.

OBJECTIVE:
To investigate the effect of continuous renal replacement therapy (CRRT) on patients with emergency and serious diseases.
METHODS:
The patients were divided into 2 groups: Group A [71 patients with multiple organ dysfunction syndrome (MODS)] were treated by CRRT for 175 times. Group B (30 patients) were treated with common hematodialysis (HD). Blood urea nitrogen (BUN), serum creatinine (Scr), natrem (Na(+)), kalium (K(+)), chlorine (Clj), power of hydrogen (pH), bicarbonate (HCO3-), carbon dioxide partial pressure (PCO(2)), and oxygen pressure (PO(2)) were measured before and after the treatment.
RESULTS:
After the treatment, all patients' general state of health, water-electrolyte and acid base disorder were improved. The levels of BUN, Scr, K(+) (All P(s)<0.01), and Na(+) (P<0.05 in Group A, P<0.01 in Group B) were all lower and the levels of CO(2)CP (P<0.01 in Group A, P<0.05 in Group B) were higher than that before the pretherapy in both Group A and Group B. There was significant difference on the levels of BUN, Scr, K(+)(All P(s)<0.01), Cl(-) and CO(2)CP (All P(s)<0.05) between Group A and Group B, which indicated the therapeutic effect in Group A was better than that in Group B. Furthermore, the levels of pH, PO(2) and HCO(3)(-) were higher (P<0.01) and PCO(2) was lower (P<0.05) obviously than that before the CRRT in 30 patients with acute respiratory distress syndrome (ARDS) in Group A. Among the 9 patients with poisoning, 7 were alleviated completely and 2 died after the CRRT plus hemoperfusion (HP). The survival time of patients was lengthened in Group A. No blood or bleed aggravation occurred in patients with bleeding inclination or 48 h before the operation after using the method of heparin soaking the blood filter.
CONCLUSION
CRRT has better curative effect on removing the toxin effectively, rapidly correcting the turbulence of water electrolyte and sour-alkali balance for patients compare with HD. At the same time, it can increase the level of PO(2) and reduce CO(2) retention, improve the lung function and brain edema, and increase the patient's survival rate. Using CRRT as early as possible is very important and it can improve the prognosis of the patients who had MODS or other dangers because the death rate of MODS patients is very high.
