1.Study on a Pre-enrichment Medium for the Simultaneous Recovery of Salmonella spp., Escherichia coli and Staphylococcus aureus

This text involved with a pre-enrichment medium for the simultaneous recovery of Salmonella spp.,Escherichia coli and Staphylococcus aureus.This medium is named buffered saline broth(BSB),which contains peptone 10g,beef exact 3g,disodium hydrogen phosphate 9g,potassium dihydrogen phosphate 1.5g,additive 50g,deionized water 1 000mL,pH7.2.1cfu per one milliliter of Salmonella spp.,Escherichia coli and Staphylococcus aureus in saline were stimutaneously added to 97 mL of BSB,buffered peptone water,lactose broth,nutrient broth,Escherichia coli broth,rappaport-vassiliadis enrichment broth,7.5% sodium chloride enrichment medium,respectively,and incubated for 18h at 37℃.The results showed BSB was the best enrichment medium,in which Salmonella spp.,Escherichia coli and Staphylococcus aureus multiplied at nearly same speed,and reached at 10~6 、10~6 、10~7 cfu/mL,respectively.Multiplex PCR produced specific amplicons of expected sizes,284bp for Salmonella spp.invA gene,622bp for Escherichia coli phoA gene,484bp for Staphylococcus aureus nuc gene.In contrast,the three bacteria couldn't multiply harmoniously in the other six media.So BSB might be considered as the medium,which could enrich above mentioned three bacteria.
2.Surgical treatment for chronic instability of the elbow

Objective To study the cause and treatment of chronic instability of the elbow.Methods Between August 1998 and August 2002,12 patients with chronic instability of the elbow were treated in our de- partment:They underwent complete medical check-ups and imaging examinations.Surgical exploration was done to determine the causes of their conditions.Their radial and(or)ulnar collateral ligaments were repaired respectively or simultaneously through the medial and lateral approaches for all the patients.All the patients were followed up for two to six years,with an average of three years.Results The follow-ups revealed no recurrent dislocation of elbow in this group.The stress valgus test and lateral pivot shift test(PST)were negative for all the patients.No elbow pain occurred.The mean flexion and extension of elbow ranged from 0 to 135?.The mean pronation was 85?and mean supination 80?.According the evaluation system of Chinese Medical Association,eight cases were rated as excellent and four as fine,with the excellent and fine rate of 100%.Conclusions Lesions of radial and(or) ulnar collateral ligaments are the major cause for chronic instability of the elbow.Surgical reconstruction of the collateral ligaments can obtain good effects in treatment of recurrent dislocation of the elbow.
3.Clinical effect of orthokeratology for juvenile with myopia astigmatism and its effects on corneal endothelial cells

Abstract?AIM:To investigate the clinical effect of orthokeratology for 400 juvenile with myopia astigmatism and its effects on corneal endothelial cells.?METHODS:Four hundred patients(800 eyes), of whom the average age was 11.5 ±2.3 years old, 239 male, 161 female, were divided into two groups: orthokeratology group and spectacles group. Parameters including efficacy data ( uncorrected visual acuity, corneal curvature, axial length and diopter ) and corneal endothelial cell data ( count of endothelial cell, endothelial cell density, fluorescein staining and central corneal thickness) were observed at 1d, 1, 6, 12 and 24mo after wearing.? RESULTS: The visual acuity of spectacles group recovered to normal after wearing, that of orthokeratology group recovered to normal at 1mo after wearing.At 2a after wearing, the corneal curvature, diopter of orthokeratology group decreased significantly (40.09 ±0.31D, 0.23 ±0.06D respectively); while those of spectacles group increased, the differences between the two groups were significant (P<0.05).The axial length of the two groups increased slightly at 1mo after wearing ( P>0.05 ) compared to those before wearing. At 2a after wearing, the axial length of the two groups were 23.96 ± 0.38mm, 26.49±0.88mm respectively (P<0.05).At 2a after wearing, central corneal thickness was 527.33 ± 27.69mm, 526.98±26.89μm(P>0.05).The count of endothelial cell and endothelial cell density both decreased after wearing without significant differences (P>0.05).?CONCLUSION: Orthokeratology has less effect on the corneal endothelial cells, no obvious adverse reactions and can control the prognosis of myopia.

Objective To study the anti-tumor effect of 5-aza-2′-deoxycytidine(5-aza-dC) on breast carcinoma xenografted in nude mice. Methods The model of breast carcinoma xenografted in nude mice was established.Ten mice were randomized into the treatment group(treated with 5-aza-dC) and control group(treated with PBS).The mass of the tumors before and after treatment were measured in the two groups,and the inhibition rate of the tumor was calculated and the growth curve was drawn.Immunohistochemistry and Western blotting were employed to detect the expression of normal epithelial cell specific-1(NES1)gene. 5.Segmental anterior cervical decompression with fusion for the treatment of multilevel cervical myelopathy.

OBJECTIVETo explore effectiveness and safety of segmental anterior cervical decompression in treating multi-level cervical myelopathy.
METHODSTwenty-four patients with four levels of cervical myelopathy were treated with segmental anterior cervical decompression (reservation of middle vertebrae, bone graft and plate-screws fixation). Among patients, there were 15 males and 9 females aged from 47 to 75 (averaged 57.9) years old. Preoperative, postoperative at 1 week and the latest following-up AP and lateral X-rays were used to observe bone union, displacement of implant, adjacent segment degeneration, changes of Cobb angle of fusion segment. JOA scoring were applied for evaluate recovery of nerve function.
RESULTSAll operations were completed successfully, 2 cases ocurred hoarseness, and improved after treated symptomatically. Nineteen patients were followed up from 3.1 to 5.3 years with an average of 3.9 years. Bone union time ranged from 3 to 7 (averaged 4.5) months. No screw loosening and displacement occurred. Nine patients occurred titanium mesh subsidence in different degrees, and 4 of them subside >3 mm; four patients ocurred adjacent segment degeneration. Postoperative Cobb angle of fusion segment at 1 week (10.40±2.94)° was improved from preoperative (5.76±4.16)°, but decreased at the latest follow-up (8.57±2.82)°, and had significant meaning compared with preoperative (P<0.01). JOA score at the latest follow-up (14.6±1.1) was higher than that of before operation (8.2±1.9), and had siginificant differences (P<0.01).
CONCLUSIONSegmental anterior cervical decompression for the treatment of multilevel cervical myelopathy has a high clinical operability, and plays an important role in recovering cervical curvature and nerve function based on completely decompression.

6.Surgical treatment for Forestier disease: a report of 8 cases.

OBJECTIVETo investigate the clinical effects and operative options for the treatment of Forestier disease.
METHODSFrom June 2005 to May 2012, 8 patients with progressive dysphagia due to Forestier disease were treated through anterior approach, their clinical data were retrospective analyzed. There were 6 males and 2 females, aged from 65 to 83 years old with an average of 73 years. Among the patients, osteophytes removal was performed in 3 cases, osteophytes removal with discectomy and fusion was performed in 2 cases, osteophytes removal with corpectomy and fusion was performed in 3 cases. According to Bazaz dysphagia score to assess the improvement of the patients' symptoms before and after operation.
RESULTSAll patients were followed up from 12 to 40 months with the mean of 18.5 months. Seven cases were asymptomatic and 1 case had mild symptom in the last follow-up. Radiographs showed the space enlargement between vertebral body and trachea.
CONCLUSIONIt is effective to treat patients with progressive dysphagia due to Forestier disease through surgical method. And the operative options depend on the stability of cervical spine and the neurological symptoms of the patients.

7.Acetylcholine and A23187 stimulate epinephrine secretion of isolated rat adrenal medulla cells in vitro

Acetylcholine and A23187, two acetylcholinergic agonists, were used to induce the stimulus-secretion coupling of the isolated rat adrenal medulla cells. Morphometry of numerical density and electron probe X-ray quantitative microanalysis were adopted to measure the alterations in the number and calcium content of chromaffin granules in adrenal medulla cells during the agonist treatment. Secretion of epinephrine was detected by the high performance liquid chromatography (HPLC). It was found that the Ca content of the isolated rat adrenal medulla chromaffin granules dropped significantly after a 10-min incubation with the agonists, whereas the number of chromaffin granule decreased slowly during the agonist treatment and the epinephrine concentration raised significantly after a 20-min incubation with the agonists. The decrease of granule Ca content occurred earlier than the increase of epinephrine concentration, suggesting that the calcium released from chromaffin granule may in part account for the induced cell secretion.
8.Mobilization and collection of autologous peripheral blood stem cells by CIE or IEV protocol in children with malignant solid tumors.

OBJECTIVEAutologous peripheral blood stem cell transplantation (APBSCT) is an important method for treatment of malignant solid tumors in children. The mobilization and collection of blood stem cells is crucial for APBSCT. This study aimed to evaluate the clinical efficacy of mobilization and collection of blood stem cells by CIE or IEV chemotherapy protocol in APBSCT in children with neuroblastoma (NB) or rhabdomyosarcoma.
METHODSThe protocols of CIE (cisplatin, etoposide) and IEV (vincristine, dosfamide, etoposide) were used as mobilization chemotherapy in 8 cases of NB with stage IV and 3 cases of rhabdomysacoma with stage III, respectively. The results of the mobilization of blood stem cells were observed.
RESULTSOf the 11 cases, mononuclear cells (MNC) and CD34+ cells were successfully collected and the volume of MNC and CD34 averaged (5.55 ± 1.43)× 10(8)/kg and (4.88 ± 2.48) × 10(6)/kg, respectively. No severe complications were observed during the mobilization and collection. A rapid hemopoietic reconstitution was observed in 10 children after APBSCT. One with NB out of the 10 children died of left heart failure 32 days after APBSCT. Others (9 cases) showed a nearly normal result of routine peripheral blood test 60 days after APBSCT.
CONCLUSIONSCIE or IEV protocol is effective and safe for the mobilization and collection of peripheral blood stem cells in children with NB or rhabdomysacoma.

9.Alternatives of anterior and posterior approaches for cervical spondylotic myelopathy.

OBJECTIVETo discuss on the effect of cervical spondylotic myelopathy before and after surgery and assess its indications and efficacy.
METHODSFrom June 2002 to June 2006, 125 patients with cervical spondylotic myelopathy were analyzed retrospectively involving 58 anterior routine and 67 posterior routine, of which 71 cases of males, 54 cases of females, aged from 28 to 69 years (average 53.4 years). The course was 0.5 to 48 months (means 14 months). According to JOA score system preoperative and postoperative nerve function were analyzed, summarized anterior and posterior cervical spine surgery.
RESULTSAll patients were followed up for from 6 to 30 months (average of 18 months). According to JOA score criteria: anterior cervical surgery preoperative JOA score was (8.78 +/- 2.43) points, postoperative JOA score was (14.68 +/- 2.37) points, the results were excellent in 40 cases, good in 10 cases, effective in 6. Posterior surgical group preoperative JOA score was (8.49 +/- 2.58) points, postoperative JOA score was (14.26 +/- 2.83) points, the results were excellent in 42 cases, good in 12 cases, effective in 8. Invalid operation occurrenced in 6 cases included 2 of anterior, 4 of posterior. The postoperative symptoms had worsed in 1 case of posterior operation. The two groups had no statistical difference in efficacy, but there were differences statistically in the same approach comparing preoperative and postoperative.
CONCLUSIONBoth anterior and posterior approaches have fine effect to the treatment of cervical spondylotic myelopathy (CSM). But the selection of the indication is very important, the patients with the pressure from anterior, the short-level changes, and the main symptoms of pyramid trac compression, adopt anterior approach on principle. While the patients with the pressure of spinal cord from posterior, multilevel changes, main symptoms of sensory disturbances, and accompanied by cervical spine canal stenosis mainly is introduced posterior approach.

10.Case-control study on therapeutic effects between arthroscopic medial retinaculum plication and plaster external fixation for the treatment of acute patella dislocation.

OBJECTIVETo compare therapeutic effects between arthroscopic medial retinaculum plication and plaster external fixation for the treatment of acute patellar dislocation.
METHODSFrom February 2006 to October 2012,29 patients with acute patellar dislocation were divided into two groups: operation group and non-operation group. The patellar dislocation duration was 2 weeks. In operation group, there were 7 males and 10 females, with an average age of (16.2 ± 6.2) years old, and the patients were treated with arthroscopic medial retinaculum plication. In non-operation group, there were 5 males and 7 females,with an average age of (16.3 ± 5.0) years old,and the patients were treated with plaster external fixation. The Kujala scores, patellar tilt angle measured on CT film, apprehension test and recurrence rate of patellar instability were observed before and 1 year after treatment.
RESULTSIn operation group, the pre-treatment and post-treatment patellar tilt angles had no statistical difference, but the post-treatment Kujala score was lower than that of pre-treatment; while in non-operation group, the post-treatment patellar tilt angle was larger than that of pre-treatment, and the post-treatment Kujala score was lower than that of pre-treatment. At 1 year after treatment, the patellar tilt angle (21.2 ± 5.3) of patients in non-operation group was larger than (13.5 ± 3.5) of operation group, and the Kujala score 73.3 ± 10.5 of patient in non-operation group was lower than 84.1 ± 5.6 of operation group.
CONCLUSIONDuring 1 year after operation, arthroscopic medical retinaculum plication is a more effective treatment for acute patellar dislocation compared with plaster external fixation.
