ObjectiveTo observe the effect of Orem self-care theory on the patients after renal transplantation.Methods102 patients after renal transplantation into were randomly divided into the observation group(52 cases),which received Orem self-care,and comparison group(50 cases),which received general care.Self-care ability,rate of infection and mental status of the 2 groups patients were observed and recorded.ResultsFor patients of the observation group,self-care ability of was obviously better(P<0.01),the rate of infection was lower(P<0.05),and mental status was improved(P<0.01) compared with patients of the observation group.ConclusionThe application of Orem self-care on patients after renal transplantation can promote self-care ability and decrease post-operation complications.

Using Kato's Self-abrasive Device, mass-screening for uterine cervical cancer has been carried out for 5, 983 married women at Chicheng country of a mountainous farm village and Shijiazhuang city in Hebei province, China, and the following result has obtained:
The women who diagnosed as class III and over were 42 persons (0.70%). Cancer cases which were confirmed by the biopsy were 10 (3 at Shijiazhuang city and 7 at Chicheng country); the occurrence rate of the cervical cancer was 0.17% and no false positive was observed. The mass -screening of the uterine cervical cancer has been carried out simultaneously by both way of the self-abrasive method and the scraper method for one third of persons who will take medical advice atboth districts. The result of comparative analysis for the quality of smear and the Papanicolaouclassification by the two method was;
1. As for the self-abrasive method, mixing with erythrocytes and overlapping of cells were few compared with the scraper method, and the diagnosis was more easily made in cytological examination.
2. The rate of coincidence by both methods were 78.85% in class I, 94.14% in class II, and 100.00% in class III-V.

OBJECTIVETo explore the effects of warm needling combined with Zhangmo decoction (see text) on endometrial receptivity in patients with clomiphene (CC)-induced ovulation.

METHODSOne hundred and sixty cases were randomly divided into a CC group (group A), a CC+ progynova group (group B), a CC+ Zhangmo decoction group (group C) and a CC+ Zhangmo decoction + warm needling group (group D), 40 cases in each one. In the Group A, CC alone was applied. In the group B, progynova was jointly used on the 8th day of menstrual cycle. In the Group C, Zhangmo decoction was jointly used on the 5th day of menstrual cycle. In the group D, based on treatment of the Zhangmo decoction, warm needling was applied at Guanyuan (CV 4), Zhongji (CV 3) and Zigong (EX-CA 1) etc. The endometrial thickness and type, pulsatility index (PI), resistance index (RI), ratio of S/D on day of human chorionic gonadotropin (HCG) and pregnancy rate were observed in fou groups.

RESULTSThe PI, RI and S/D in the group C and D were obviously lower than those in group A and B (all P < 0.01). The endometrial thickness was (7.7 +/- 1.49) mm in group B, (8.2 +/- 1.54) mm in group C and (8.9 +/- 1.51) mm in group D, which were significantly different from (6.4 5 +/- 1.26) mm in the group A (all P < 0.01) also there was a significant difference between group C and D (P < 0.05). The rate of endometrial type A was 65.0% in the group D, which was significantly higer than 27.5% in the group A, 32.5% in the group B and 35.0% in the group C (all P < 0.01). The pregnancy rate was 30.0% in the group D, which was obviously higher than 12.5% in the group A, 15.0% in the group B and 17.5% in the group C (P < 0.05). The endometrial thickness and rate of endometrial type A in the pregnant were obviously higher than those in the non-pregnant (both P < 0.01) while PI, RI and S/D was lower than those in the non-pregnant (P < 0.01, P < 0.05).

CONCLUSIONWarm needing combined with Zhangmo decoction could improve endometrial thickness, morphology and uterine spiral artery to improve pregnancy rate, which has superior effect to clomiphene, clomiphene combined with progynova and clomiphene combined with Zhangmo decoction.

Objective To study and evaluate the therapeutic efficacy of CT-guided radioactive 125Iseeds implantation combined with percutaneous vertebroplasty for spinal vertebra metastases.Methods Fifteen patients and 21 lesions were enrolled in this study.Based on the CT images,a computer-based treatment planning system (TPS) was used to determine the optimal seeds distribution.Under CT-guided,radioactive 125I seeds were implanted into the lesions.Based on the X-ray and CT image after the implantation,quality check was carried out with TPS.The ostalgia-relieving degree and the image alterations of the spinal vertebra metastases lesions were observed.Two months later,15 patients underwent percutaneous vertebroplasty with DSA guidance,bone cement was made according to the ratio methyl acrylic resin polymer/liquid methyl acrylic resin monomer/contrast as 3 ∶ 2 ∶ 1 in injecting paste form.The puncturation through pedicle of vertebral arch and lateral posterior body of vertebra was both adopted in thoracic and lumbosacral vertebrae site.After making sure that the puncturation was completed and there was no leakage in vertebral body,the bone cement was injected into vertebral body quickly.Results Accepted radioactive 125I seeds plantation,relief of pain was obtained.Among 15 patients,8 patients were powerful,6 patients were effective and 1 patient was inefficiency.After treatment,the pain grading significantly decreased.After treatment for 2 months,CT was used to recheck,7 lesions were local controlled,12 lesions had no changes,2 lesions were progress.The responsive rate was 90.5％ (19/21).No serious side-effect happened.Conclusions Radioactive 125I seeds implantation can be a safe and effective method in treating spinal vertebra metastases and obtaining good clinical effects with minimal damage and few complications.Percutaneous vertebroplasty can strengthen the target and relief pain quickly.CT-guided radioactive 125I seeds implantation combined with percutaneous vertebroplasty for the treatment of spinal vertebra metastases has complementary advantages.

The levels of triglyceride(TG)and free fatty acid(FFA)in serum,liver,skeletal muscle,and pancreas of lipoprotein lipase gene knockout heterozygous(LPL+/-)mice and C57 mice were determined.Intraperitoneal glucose tolerance test(IPGTT)was performed to evaluate insulin sensitivity and β-cell function.The results showed that the Iipid content in 16 weeks LPL+/- group did not increase significantly.The TG and FFA contents in 28 weeks LPL+/- group were significantly higher than those in control and 16 weeks LPL+/-group(all P<0.05).In 50 weeks LPL+/- group,FFA levels in serum and pancreas,and TG content in pancreas increased significantly compared with other three groups(all P<0.05).The IPGTT result showed that the blood glucose levels increased from 15 to 120 min,not at 0 and 5 min.The blood glucose levels during 30-120 min increased significantly in 50 weeks LPL+/- group compared with other three groups(P<0.05).Fasting insulin(FINS),homoestasis assessment of insulin resistance(HOMA-IR)and pancreatic β cell function also increased gradually with age.FINS and HOMA-IR in 28 weeks LPL+/- group were higher than those in control and 16 weeks LPL+/- group.These results suggest that LPL is a key enzyme in lipid metabolism and plays a crucial role in the development of insulin resistance and diabetes.

Objective To discuss the common causes for postoperative complications after treatment of spinal fractures and their preventive measures.Methods To analyze the 32 failed eases who received internal fixation for their thoracolumbar fractures between July 1998 and April 2005 in our department.Their preoperative and postoperative results of X-ray,CT and MIR examinations were reviewed to find out the causes for failure.Three of them were treated through anterior approach and 29 through posterior approach.Eighteen screws broke in eight eases, the rods got loosened in eight cases and broken in four cases,15 pedicte screws were mis-located in eight cases and the implants got loosened in three cases.Results Thirty patients had to get a reoperation.Two were cured through non-surgical treatment.The causes for failure were found to be as follows:1,incorrect operation approach;2,in- stability of the front-middle colunm;3,unskillful operation;4,ignorance or the lesion to the adjacent vertebral discs; 5,flaws in pedicle screw design.Conclusion To prevent postoperative complications after internal fixation for spinal fractures,surgeons should strictly stick to the indications before operation,choose a proper approach and method for internal fixation,try to stabilize the front-middle column by the first intention,and have a good command of anatomy and operation skills.
