Chinese medicine (CM) develops with the survival, reproduction, growth, and progressing of the Chinese nation. Scientific technologies not only promote continual progressing of human societies, but also provide new ideas and methods for the development of CM. In recent years, great changes have taken place in CM complying with developing modern scientific technologies, mainly manifested in the depth of CM theories at molecular levels, the combination of syndrome differentiation and disease identification, continuous innovation and development of clinical diagnosis and treatment techniques, diversified dosages of Chinese materia medica, the academic tendency of education patterns, occupational refinement, diversified medical practice modes, and so on.

3.Suboptimal Therapy for Dyslipidaemia in Coronary Bypass Surgical Patients with Premature Ischaemic Heart Disease

The incidence of premature multi-vessel coronary artery
disease (CAD) is on the rise in Malaysia. The pathogenesis
of coronary atherosclerosis is multi-factorial with
dyslipidaemia being one such risk factor. Elevated total
cholesterol (TC), low density lipoprotein cholesterol (LDL-C)
and triglycerides (TG) levels are primarily responsible. We
analysed the fasting pre-operative lipid profiles of coronary
artery bypass graft (CABG) patients with symptomatic
severe premature CAD. A majority of patients had an
elevated LDL cholesterol level despite being on a statin.
Similarly, no patient with an elevated TG level was
prescribed a fibrate.
Pre-operative control of known dyslipidaemia was
suboptimal in young adults with angiographially proven
severe symptomatic CAD. This is either due to
subtherapeutic dose prescribing or failure to commence
appropriate anti-lipid drugs. Collectively, general
practitioners, cardiologists and cardiac surgeons must be
more diligent in monitoring lipid profiles in such patients
and be more meticulous in prescribing therapeutic doses to
achieve target control.

4.Risk factors of severe intrahepatic cholestasis during early period after liver transplantation

Objective To investigate the risk factors of severe intrahepatic cholestasis during early period after liver transplantation.Methods The clinical data of 225 patients who received orthotopic liver transplantation at the Nanfang Hospital of Southern Medical University from August 2004 to February 2011 were retrospectively analyzed.All patients were divided into positive group (60 patients with intrahepatic cholestasis) and negative group (165 patients without intrahepatic cholestasis).Preoperative,intraoperative and postoperative factors of the 2 groups were compared via t test,chi-square test,Wilcoxon test or Logistic regression analysis.Results The proportion of patients with hepatic cirrhosis,hepatic encephalopathy integral,ascites integral,international normalized ratio,and the levels of prothrombin time (PT),total bilirubin (TBil),aspartate aminotransferase of the positive group before operation were significantly higher than those in the negative group (x2 =6.09,Z =2.22,2.60,2.46,2.84,4.81,3.42,P < 0.05),while the levels of albumin,Na +,K +,hemoglobin,platelet (PLT) of the positive group in the operation were significantly higher than those in the negative group (t =2.10,4.97,Z =2.49,t =3.51,Z =3.66,P < 0.05).The ratio of compatible blood type of the donors and recipients,ratio of fatty liver graft,cold ischemia time,relative warm ischemia time,intraoperative blood loss,intraoperative transfusion of red blood cells,PLT,and cryoprecipitate of the positive group after the operation were significantly higher than those in the negative group (x2 =4.29,13.11,Z =2.45,2.61,3.75,3.20,2.89,3.95,P <0.05).The incidences of acute rejection,hepatic artery embolism,pulmonary infection,bacteraemia,fungal infection and cytomegalovirus (CMV) infection were significantly higher than those in the negative group (x2 =9.87,4.91,8.21,6.29,3.92,9.26,P <0.05).The results of multivariate analysis revealed that preoperative level of TBil > 51.3 μmol/L,fatty of the liver graft,intraoperative transfusion of cryoprecipitate,postoperative acute rejection,hepatic artery embolism,postoperative pulmonary infection,bacteraemia,CMV infection were independent risk factors of severe inrahepatic cholestasis (OR =15.82,7.99,2.88,3.03,53.20,3.34,4.11,3.22,P < 0.05).The incidence of severe intrahepatic cholestasis was significantly lower in patients with higher level of PLT and longer PT (OR =0.33,0.25,P < 0.05).The mortality rates of the positive group and negative group at 6 months after the operation were 41.7% (25/60) and 19.4% (32/165),and the mortality rate of the positive group was significantly higher (x2 =11.54,P < 0.05).Conclusion Correction of poor clinical status before liver transplantation,reinforcement of infection control and anti-rejection may reduce the incidences of complications and decrease the associated early mortality.

Melatonin holds a wide range of biological effects, its multi-channel and multi-target impacts on reproductive sys-tem are the basis of pregnancy related rhythm, including conception, the duration of pregnancy and delivery.This paper describes the synthesis of melatonin, the characteristics of melatonin receptors distribution and melatonin′s biological property.It also illuminates the influence of melatonin rhythm on the above-mentioned courses, hoping to provide theory basis for clinical suggestions to modulate rhythm around pregnancy.

7.Experimental study on the toxicity from PMNLs during ischemia reperfusion cerebral injury

Objective To study the change of activity and toxicity of superoxide (O - 2 ) produced in polymorphonuclear leukocytes(PMNLs) during ischemia reperfusion cerebral injury.Methods The rats were administrated by both PMA (an activator of single transduction of O - 2 produced in alkaline phosphatase(ALPase) positive granules of PMNLs) and the inhibitor BCA respectively; the model of middle cerebral artery (MCA) occlusion was made by suture cleat mothod,the activity changes of both myeloperoxidease (MPO) and O - 2 were measured at 6, 12, 24, 48, 72 and 168h reperfusion following ischemia 1h, and the pathological ultrastructural changes were observed. Results The MPO activity of both PAM group and BCA group reached the peaks at 24h after reperfusion; however,there were no remarkable differences in MPO activity between these two groups in the same time point. The O - 2 activity in the PAM group was significantly higher than those in the BCA group. The O - 2 activity reached the peak at 72h of ischemia reperfusion. In the same experimental time point, the pathological changes of the ultrastructure in ischemic reperfusive injury brain of the PAM group were much more serious than the those of the BCA group, which showed obviously the neurons edema, the abnormal structures of nerve felt and synapse in the ischemia reperfusion injured brain.Conclusion The increase of brain O - 2 activity from PMNLs during cerebral ischemia reperfusion injury was direct ratio to the degree of cerebral injury. BCA might depress the activity and the toxicity of the O - 2.
8.Dynamic changes of Tlymphocyte subpopulation in the peripheral blood and its clinical significance in patients with acute cerebral infarction

Objective To explore the dynamic changes and the meaning of T lymphocyte subpopulation in peripheral blood and its clinical significance in patients with acute cerebral infarction.Methods The numbers of CD3 +,CD4 + and CD8 + T lymphocyte in peripheral blood were examined with flow cytometry in 20 patients with acute cerebral infarction(CI) at the 3rd,5th,7th and 14th day after the onset.Results The numbers of CD3 + and CD4 + T lymphocytes and the ratio of CD4 +/CD8 + in the patients with acute CI were lower than those in normal group at the 3rd and 5th day after the onset (P0.05).Conclusion The immunological function of the cells in the patients with acute CI decreased obviously at the 3th and 5th day, and then restored partly at the 7th day, at last restored completely at the 14th day after the onset. It suggests that the focus time for the prevention of infective complications is in 10 days after the acute CI.
9.The biomechanical analysis of the etiologies of spontaneous pneumothorax in flat-chest teenagers

Objective To investigate the etiologies of spontaneous pneumothorax in flat-chest teenagers. Methods 139 teenagers with flat chest and spontaneous pneumothorax were designed as the study group, while 152 teenagers without spontaneous pneumothorax during the same period were taken as control. The ratio of thoracic transverse diameters to anterior-posterior diameters, and the ratio of tracheal length to transverse diameters were measured respectively by CT scan. The resected pulmonary tissues in 47 cases with spontaneous pneumonthorax and in 47 cases without spontaneous pneumonthorax underwent various examinations, including tension, compression, and bending tests. Student t-test was employed to perform the statistical analysis. Results The ratios of thoracic transverse diameters to anterior-posterior diameters and the ratios of tracheal length to transverse diameters in the study group were remarkably higher than those were in the control group, with significant statistical difference (P

Objective: To raise the all-round informational level through analyzing the necessity of constructing compound medical library.Methods: Through aiming at the fundamental characters of compound medical library,so as to carry out the practices of theoretical researches,the development and optimization of network environments,the integration of informational resources,the enhancement and innovation of information service quality.Results: To provide some helpful references from the traditional to the compound state.Conclusion: Constructing compound medical library by combining the traditional with the digital base is the only way to accelerate the development of medical library.
