Obstructive sleep apnea (OSA),the most common type of sleep disordered breathing was closely associated with the occurrence of ischemic stroke.It has been demonstrated that OSA is an independent risk factor for ischemic stroke.Its pathogenic mechanism includes atherosclerosis,coagulation abnormalities and cerebral hemodynamic changes caused by long-term periodic hypoxia hypoxemia and hypercapnia.The prognosis of ischemic stroke patients with OSA is poor.Therefore,it needs to be treated early to avoid further damage to brain tissue and reduce the mortality.

Objective To study the effects of nitric oxide (NO) on the growth and metastasis of tumor.Methods The literatures of recent years were reviewed.Results NO had double effects on the growth and metastasis of tumor. NO promoted the growth and metastasis by regulating the expression of tumor proliferation gene and inducing tumor angiogenesis. On the other hand, NO had antitumor effects by interfering with the metabolism of tumor cells, inducing the damage of DNA, forming high toxic free radical, inducing apoptosis of tumor cells and mediating the antitumor action of endothelial cells and macrophages.Conclusion Selective blockage or induction of synthesis of NO may be a new way for tumor therapy.

Objective:To investigate the expression of focal adhesion kinase(FAK) and Ki67 in the invasive breast ductal carcinoma tissues, seeking their relationship with tumor invasion and metastasis and to study the mechanisms for cell signaling transduction mediated by FAK in the carcinogenesis and progress of breast carcinoma.Methods:Tissues were obtained from 88 patients with invasive breast ductal carcinoma and 25 with breast ductal epithelial benign proliferative disease. The expression of FAK and Ki67 was examined by immunohistochemical technique(streptavidin peroxidase conjugate method, SP).Results:In 88 cases of invasive breast ductal carcinoma, the positive incidences of FAK was 68.2%(60/88), and the Ki67 proliferation index was 29.6%?22.9%, which distinguished apparently to those of the control group(P0.05). Objective To observe the effect of external fixator combined with limited internal fixation in the treatment of complex ankle fractures.Methods According to the digital table,65 patients with complex ankle fracture were randomly divided into treatment group(33 cases) and control group(32 cases).The treatment group was treated with external fixator combined with limited internal fixation,the control group was treated with internal fixation.All patients were followed up for 5 to 20 months.The curative effect and incidence rate of complication of the two groups were observed.Results The excellent rate of the treatment group was 84.85%,which was higher than 62.50% of the control group,the difference was statistically significant(x2=4.20,P<0.05).The incidence rate of complications in the treatment group was 3.03%,which was lower than 18.75% of the control group,the difference was statistically significant(x2=4.18,P<0.05).Conclusion External fixator combined with limited internal fixation can effectively treat complex ankle fractures,the method can not only make patients satisfied with the reduction effect,but also the internal fixation complications such as infection are minimized,the clinical value is extremely high,it is worthy of wide application.

Objective To observe the clinical efficacy of scalp acupuncture plus body acupuncture in treating diarrhea-predominant irritable bowel syndrome (D-IBS). Method Totally 122 D-IBS patients were randomized into a scalp acupuncture group (41 cases), a body acupuncture group (41 cases), and a combined group (40 cases). The scalp acupuncture group was intervened by acupuncture at the Stomach and Intestine Zones on the head; the body acupuncture group was by ordinary body acupuncture; the combined group was by both scalp acupuncture and body acupuncture. The symptom score of traditional Chinese medicine (TCM) was evaluated prior to the treatment, at the end of the first and second treatment courses, and after the whole intervention; the clinical efficacies were also compared among the three groups. Result The symptom scores of the combined group were significantly lower than that of the scalp acupuncture group and body acupuncture group respectively at the end of the second treatment course and after the whole intervention (P<0.05); after the whole intervention, the cured and markedly-effective rate was 62.5% in the combined group, significantly higher than 24.4% in the scalp acupuncture group and 29.3% in the body acupuncture group (P<0.05). Conclusion Scalp acupuncture plus body acupuncture can produce a satisfactory clinical efficacy in treating D-IBS, better than body acupuncture or scalp acupuncture alone; its advantages include rapid onset of action, significant effect, and reducing intestinal inflammation, etc.

Objective:To observe the effectiveness and security of piperacillin sodium/sulbactam sodium in the treatment of bacte-rial infections and perform the pharmacoeconomics evaluation. Methods: Totally 126 patients with hand trauma bacterial infections were divided into the control group and the treatment group. The control group was given piperacillin sodium/tazobactam sodium 2. 5g, ivd, bid, while the treatment group was treated with piperacillin sodium/sulbactam sodium 1. 25g, ivd, bid. After 14-day treatment, the clinical therapeutic effect and adverse reactions of the two groups were observed, and the cost-minimization analysis was carried out. Results:The effective rate of the treatment group was 93. 65%while that of the control group was 92. 06%(P>0. 05). The bac-terial clearance rate of the treatment group was 96. 83%while that of the control group was 95. 24% (P>0. 05). The incidence of ad-verse reactions of the treatment group was 4. 76% (3/63) while that of the control group was 9. 52%(P>0. 05). The cost-effective-ness of piperacillin/sulbactam was significantly lower than that of the control group (P<0. 05). Conclusion: The effectiveness and safety of the two groups show no obvious differences in the treatment of hand trauma bacterial infections, and both have good antibacte-rial effect with low incidence of adverse reactions. For the better economic benefits, piperacillin /sulbactam is worthy of promoted ap-plication.
