1.Efficacy of dexmedetomidine versus remifentanil for awake nasotracheal intubation assisted by fiberoptic bronchoscope

ObjectiveTo compare the efficacy of dexmedetomidine and remifentanil for awake nasotracheal intubation assisted by fiberoptic bronchoscope.MethodsForty ASA Ⅰ -Ⅲ aged 18-73 patients with difficult airways undergoing awake nasotracheal intubation assisted by fiberoptic bronchoscope were randomly divided into 2 groups ( n =20 each): dexmedetomidine group (group D) and remifentanil group (group R).A loading dose of dexmedetomidine 1.0 μg/kg was injected iv and then infused at a rate of 0.5 μg·kg-1 ·h-1 in group D.Remifentanil was target controlled-infused with target plasma concentration of 3.2 ng/ml in group R.Awake nasotracheal intubation was performed after dexmedetomidine loading dose was injected in group D and when the target plasma concentration of remifentanil was reached.Ramsay score was recorded during intubation,and intubation conditions (vocal cord movement,coughing,limb movement) and tolerance (during and after intubation) were evaluated.The intubation time,rate of successful intubation,side effects during intubation and 24 h after surgery,satisfactory score of patient with intubation and memory of intubation were recorded.ResultsRamsay score,the first attempt success rates of intubation and satisfactory score of patients with intubation were significantly higher,intubation conditions and tolerance were better,intubation time was shorter,incidence of side effects and memory score of intubation were lower in group D than in group R ( P < 0.05).ConclusionDexmedetomidine provides better intubation conditions,less side effects and awareness of intubation than remifentanil for awake nasctracheal intubation assisted by fiberoptic bronchoscope.
2.Protection of polysaccharide GEP-2 from gastrodia elata on the immunity-mediated liver injury induced by BCG+LPS in mice

Objective:To observe the protection of polysaccharide GEP-2 from Gastrodia elata on the immunity-mediated liver injury induced by BCG+LPS in mice.Methods:BCG and LPS were adopted to establish BCG+LPS liver injury model in mice. The contents of serum alanine aminotransferase (ALT), aspartate aminotransferase (AST), the activity of superoxide dismutase (SOD),glutathion reductase (GSH-Px) and malondiadehyde (MDA) consistent of liver homogenate in mice were measured by colorimetric method. The content of tumor necrosis factor-?(TNF-?)and interleukin-1( IL-1)in serum were measured by enzyme linked immunosorbent assay (ELISA), and T-and B-lymphocyte proliferation were measured by MTT.Results:Polysaccharide GEP-2 from Gastrodia elata (25,50,100 mg/kg) could obviously reduce the high level of ALT and AST,and serum TNF-? and IL-1 either.It inhibited high level of MDA, increased the low activity of SOD and GSH-Px in liver homogenate,and enhanced T-and B-lymphocyte proliferation.Conclusion:Polysaccharide GEP-2 from Gastrodia elata had apparently protective effects on the immunity liver injury in mice induced by BCG+LPS.
3.Research and Design of Medical Data Integration Platform

In this paper, through in-depth study on characteristics of hospital information system (HIS) construction and requirements of medical data integration, and combined with the medical data straits of heterogeneity, multi-source, complexity, dynamic growth, etc., the problem of information exchange in different systems was aimed to solved, in order to realize medical data sharing among internal systems in hospital.Based on Extensible Markup Language (XML), public data model was described by XML Schema technology. Global and local medical data were queried using XQuery technology. The results showed that a medical data integration platform based on HL7 standard and the middleware technologies were proposed. It was concluded that the platform shielded the heterogeneity of underlying medical data by middleware technology. It finally realized the global query and transparent access of medical data in HIS for users.
4.Study on in Vitro Dissolution Rate of Niaosaitong Sugar-coated Tablets and Film-coated Tablets

Objective:To investigate the quality of Naosaitong Tablets by determing of the dissolution rates of different batches of tablets. Methods: The rotatory-basket method was used. The cumulative dissolution percentage was determined by UV. The dissolution parameters were got and their correlation was studied. Results: Statistics indicated there was a significant difference in dissolution parameter (T 50) between film-coated tablets and sugar-coated tablets. The dissolution percentage of film-coated tablets in 45 minutes was over 90%, whereas sugar-coated tablets was only 49-78%. Conclusion: Niaosaitong Film-coated Tablets in superior to its sugar-coated tablets in dissolution rate. It is necessary to improve the quality of Niaosationg Sugar-coated Tablets.
5.Update of the relationship between the insulin-like growth factor system and primary adrenocortical tumor

THe insulin-like growth factor(IGF) system,a very important polypeptide growth hormone,has been found to have some relation with primary adrenocortical tumorigenesis and modulates its growth.This article reviews the general biological characteristics of IGF and the relations of the IGF system with the normal adrenal cortex and primary adrenocortical tumor.It also and discuss the clinical meaning of its possible presence in the primary adrenocortical tumor.
6.Comparison of MELD score and Child-Pugh grading in patients with liver Cirrhosis

Objective To compare the differences between MELD and Child-Pugh grading in assessing of hepatic functional reserve in patients with cirrhosis,and analyse the correlation of two systems.Methods Retrospective analysis was made on 92 cases of hepatitis cirrhosis hospitalized with the MELD and Child-Pugh score by ROC curve,the difference and relation between two systems were compared.Results MELD and Child-Pugh system were significantly correlated(r=0.669,P < 0.000),3-month survival in patients with two systems ROC curve areas was significantly different(P < 0.05),while the MELD in patients survived more than 6 months was not significantly different compared with Child-Pugh(P>0.05).ConclusionMELD and Child-Pugh can accurately assess the prognosis of survival for patients with hepatic cirrhosis,and MELD is suitable for assessment of risk,heavy patients,not for those survived more than 6 months.
8.Protection of Polysaccharide GEP-2 from Gastrodia elata on Carbon Tetrachloride and Alcoholic Liver Injury in Mice

Objective To observe the protection effect of Polysaccharide GEP-2 from Gastrodia elata on carbon tetrachloride(CCl4)and alcoholic liver injury in mice.Methods CCl4 and alcohol were adopted to establish CCl4 and alcoholic liver injury model in mice respectively.Colorimetric method was used to assay the content of serum alanine aminotransferase(ALT)and aspartate aminotransferase(AST),and the content of triglyceride(TG)in alcoholic liver injury,the activity of superoxide dismutase(SOD)and malondiadehyde(MDA)consistent of liver homogenate in mice,and observe liver tissue pathological section.Results Polysaccharide GEP-2 from Gastrodia elata(25,50,100 mg/kg)administration obviously reduced the high level of ALT and AST in serum and the content of TG in alcoholic liver injury,inhibit the high level of MDA and increase the low activity of SOD in liver homogenate.Patholory inspection result indicated that Polysaccharide GEP-2 from Gastrodia elata exert notable influence on CCl4 and alcoholic liver injury in mice.Conclusion Polysaccharide GEP-2 from Gastrodia elata has apparently protective effects on CCl4 and alcoholic induced liver injury in mice.
9.Effect of mannitol combined with psychological intervention on post-operative swelling of extremities fractures

Objective To observe the influence mannitol combined with psychological intervention on post-operative swelling of extremities fractures.Methods96 patients from January 2013 to March 2015, were randomly divided into observation group and control group of 48 cases.In the control group were treated with mannitol treatment, observation group were treated with mannitol combined with psychological intervention, the two groups were recorded cases of fracture after swelling sweling.Results①After treatment, patients swelling time was (7.02+1.02) d, than the control group (9.87±1.23) d, and the difference was statistically significant (P<0.05).②The observation group were 6 days and 9 days after treatment forearm limb circumference, respectively (25.76±1.06) cm, (23.15±0.76) cm, were lower than in the control group (27.12±1.10) cm, (24.20±0.79) cm, and the difference was significant sex (P<0.05).The observation group were 6 days and 9 days after treatment were leg limb circumference (35.18±0.74) cm, (32.04±0.31) cm, were lower than in the control group (36.76±0.78) cm, (33.17±0.37) cm, and the difference was significant sex (P<0.05).③ Observation group patients after the treatment of anxiety and depression were on average lower than the control group, and the difference was statistically significant (P<0.05).ConclusionMannitol combined with psychological intervention effect fracture surgery swelling of extremities significant, can significantly reduce the postoperative pain is worthy of further research and application.
