1.Treatment of dentine hypersensitivity with self-made herbal desensitizer

Objective:To investigate the effect of self-made herbal desensitizer in the treatment of dentine hypersensitivity. Methods:Herbal desensitizer was made of corydalis tuber, asarum herb, long pepper, galangal rhizom and prepared wild aconite root into three different forms:gargle(GG), embrocation(EC) and cataplasm(CP). 750 g/L sodium fluoride paste(SFP) was used as the control. 765 teeth with dentine hypersensitivity in 216 patients were treated with GG,CP,EC and SFP respectively at random. In group CP dental tray was used to carry the desensitizer.The clinical effects were observed in the following 6 months. The effects of the drugs on the obstruction of detinal tubules were observed with scanning electron micoscope(SEM) in the in vitro experiment on 12 extracted teeth.Results:The clinical immediate effective rate(%)of GG,CP,EC and SFP was 57.3(110/192),91.8(179/195),86.9(166/191) and 63.6(119/187) (CP vs GG,EC or SFP P
2.Effect of minimally invasive percutaneous nephrolithotomy in treatment of recurrence of kidney stones after open operation

Objective To explore the curative effect of minimally invasive percutaneous nephrolithotomy in treatment of recurrence of kidney stones after open operation.Methods 56 patients with postoperative recurrence of renalcalculi were retrospectively reviewed.They were randomly divided into the control group and the observation group.The observation group was treated by minimally invasive percutaneous nephrolithotomy,while the control group received the open operation.The stone clearance rate,operation time and hospitalization time of the two groups were compared.Results The stone clearance rate in the observation group was 92.86%,which was significantly higher than 64.29% of the control group (x2 =11.37,P < 0.05) ; The average operation time in the control group was (131.8 ± 8.9)min,which was more than (92.5 ± 5.4)min of the observation group ;The average hospital stay in the control group was (19.7 ± 3.2) days,which was more than (10.4 ± 2.6)days of the observation group,the differences were statistically significant (t =16.38,17.25,all P <0.05).Conclusion Minimally invasive percutaneous nephrolithotomy can improve the stone clearance rate in treatment of recurrent renal calculi after open operation,which can shorten the operation time and hospital stay,it is worth the clinical promotion.
3.Comparison of the effect of nifedipine controlled-release tablet and nifedipine sustained-release tablet in the treatment of hypertension

Objective To investigate the clinical efficacy of nifedipine sustained-release formulation(SR)and nifedipine controlled-release(GITS)in the treatment of essential hypertension.Methods 160 patients with essential hypertension were randomly divided into two groups.The SR group was treated with SR 20 mg twice a day;GITS group was treated with GITS 30 mg once a day.All patients were given ambulatory blood pressure monitoring(ABPM)and observed the 24-hour average blood pressure,heart rate,changes in the trough/peak(T/P)values,and adverse reactions before and after treatment in two groups.Results The T/P value of systolic blood pressure(SBP)and diastolic(DBP)in the GITS group was 68.5%,74.6%,respectively,which were higher than those of SR group(57.2%,61.1%)(x2=6.32,7.05,all P<0.05);GITS group was significantly lower than the SR group in heart rate(t=4.17,P<0.05);Adverse reactions between two groups had no significant difference(x2=0.16,P>0.05).Conclusion SR and GITS can effectively lower blood pressure,but GITS is more efficient,stable,safe,small impact on the heart rate.
4.Association of rs2282679 A/C polymorphism in vitamin D binding protein gene with vitamin D deficiency

[Summary] To investigate the association of rs2282679 A/C polymorphism in vitamin D binding protein gene with vitamin D deficiency in 1 216 participants.A standardized questionnaire was applied to collect information on alcohol consumption,smoking habits,and use of medications.Serum 25-hydroxyvitamin D [25 (OH) D] was determined by radioimmunoassay.Vitamin D binding protein genotypes were determined by SNaPshot method.1216participants included 457 women(37.6%),478 (39.3%)with vitamin D deficiency [25 (OH) D<50 nmol/L],and 386 (31.7%) overweight/obese participants.The frequencies of rs2282679 CC,AC,and AA genotypes were 8.7%,41.0%,and 50.3%,respectively.The distributions of genotypes and alleles differed significantly between participants with sufficient vitamin D and those with vitamin D deficiency (P < 0.05).There were significant differences in the serum levels of 25 (OH) D between three genotypes before and after being adjusted for covariates (P<0.01).AA had a 22% (OR =0.78,95% CI 0.65-0.93,P =0.006) lowered risk of vitamin D deficiency compared with the whole studied population.Compared with CC genotype,AA had a 36% lowered risk of vitamin D deficiency(OR =0.64,95 % CI 0.42-0.98,P =0.006),while there was no significant difference between AC and CC genotypes(OR=0.87,95% CI0.57-1.34,P=0.53).In conclusion,the vitamin D binding protein rs2282679A/C polymorphism was significantly associated with serum level of 25 (OH) D and risk of vitamin D deficiency.
5.Study on Percutaneous Absorption in vitro of 0.03%Tacrolimus Ointment

OBJECTIVE:To study the percutaneous absorption of 0.03% Tacrolimus ointment,and to compare the difference of domestic test preparation and imported reference preparation. METHODS:Modified Franz diffusion cells were adopted in trans-dermal test in vitro;HPLC-MS method was used to determine permeation amount and rate in vitro,delay time of domestic test preparation and imported reference preparation 0.03%Tacrolimus ointment. RESULTS:24 h in vitro permeation amount of test and reference preparations were(3 907±1 191)and(3 896±1 064)ng/cm2;permeation rates were 186.7 and 182.9 ng/(cm2·h);de-lay time were 1.95 and 2.00 h,respectively(P>0.05). CONCLUSIONS:Test preparation shows good percutaneous property,and is similar to reference preparation in penetration absorbency through nude mice skin.
6.Study on the clinical value of combined detection of HE4,CA199 and CA125 in diagnosis of ovarian cancer and benign ovarian cysts

Objective To explore the clinical value of combined detection of human epididymis protein 4(HE4) ,carbohydrate an‐tigen 199(CA199) and carbohydrate antigen 125(CA125) in diagnosis of ovarian cancer and benign ovarian cysts .Methods The se‐rum levels of HE4 ,CA125 and CA199 were determined by using chemiluminescent immunoassay in 45 cases of patients with ovarian cancer(ovarian cancer group) ,60 cases of patients with benign ovarian cyst(benign disease group) ,and 30 cases of healthy individu‐al(control group) .Results The serum levels of HE4 ,CA199 and CA125 in the ovarian cancer group were higher than those in the ovarian cancer group and control group ,there were statistically significant differences(P<0 .05) .Compared with the control group , the serum level of CA125 in benign disease group was significantly higher(P<0 .05) ,while no statistically significant differences of serum levels of HE4 and CA199 were observed between the two groups(P>0 .05) .The sensitivity of combined detection of the three indicators increased to 82 .2% ,and the specificity remained at 90 .0% .Conclusion Combined detection of HE4 ,CA125 and CA199 might have clinical significance in early diagnosis and identification of ovarian cancer .
7.Predictive value of lipid accumulation product and visceral fat index for metabolic syndrome among middle-aged and elderly populations

Objective:
To investigate the value of lipid accumulation product (LAP) and visceral fat index (VAI) for prediction of metabolic syndrome (MS).
Methods:
Based on the 2018 Survey on Chronic Diseases and Risk Factors in Yantai City of Shandong Province, residents at ages of 45 years and older were sampled, and subjects' age, disease history, waist circumstance (WC), body mass index (BMI), blood pressure and blood lipid were collected to calculate LAP and VAI. MS was diagnosed with the a joint interim statement of the International Diabetes Federation Task Force on Epidemiology and Prevention; National Heart, Lung, and Blood Institute; American Heart Association; World Heart Federation; International Atherosclerosis Society; and International Association for the Study of Obesity (JIS definition) and the recommended criteria proposed by the Chinese Diabetes Society (CDS) of Chinese Medical Association (CDS criteria), and the values of LAP and VAI for MS screening were evaluated using the receiver operating characteristic (ROC) curve analysis.
Results:
Totally 9 366 subjects were enrolled, including 4 340 men (46.34%) and 5 026 women (53.66%), and had a mean age of (54.49±9.73) years. According to the CDS criteria, the prevalence of MS was 24.58%, and LAP and VAI showed areas under the ROC curve (AUC) of 0.837 (95%CI: 0.828-0.846) and 0.751 (95%CI: 0.739-0.762), sensitivities of 78.82% and 63.31% and optimal cut-off values of 44.64 and 1.86 for screening of MS. According to the JIS definition, the prevalence of MS was 35.26%, and LAP and VAI showed AUC values of 0.842 (95%CI: 0.834-0.850) and 0.790 (95%CI: 0.780-0.800), sensitivities of 75.73% and 68.42% and optimal cut-off values of 42.01 and 1.67 for screening of MS.
Conclusions
Both LAP and VAI are effective for screening MS among middle-aged and elderly residents, and LAP presents a higher accuracy than VAI.
8.The Investigation on Relationship Between the CT Value and Injury of Ultrastructure in Posttraumatic Acute Diffuse Brain Swelling

Objective To study the relationship between the CT value and injury of ultrastructure in posttraumatic acute diffuse brain swelling(PADBS). Methods The change of CT value of brain tissue was analyzed at posttrauma and preoperation in 9 patients, in combination with the ultrastructure in brain parenchyma in 36 specimen taken from operations. The relationship between the descend of CT value and ultrastructure injury was analysed.Results The CT value of brain in preoperation was lower than it in posttrauma first scanning(2.5~4.3 HU).The capillary distention and stenosis and the diffuse edema in pericapillary and intercellular were observed under transmission electron microscopy(TEM). The nucleolus of neuronal cells displaced to membrane or disappeard. Chromation agglutionation, nuclear membrane circuity, perinuclear diffuse lipid drops and blankspace were detected. The mitochondrion swelling, mitochondrial crest blurring or effacement, rough endoplasmic reticulum distension and its’ granules detachmen were also seen under TEM. Axolemma edema, microfilaments and microtubules derangement in axis-cylinder were found too. The similar phenomena existed in astrocyte.Conclusion The descent of CT value in PADBS was relevant to the aggravation of vasogenic cerebral edema, cytotoxic cerebral edema and ultrastructure injury in brain parenchyma.
9.Regulatory effect of Astragalus Polysaccharides on immune function of mice infected by Brucella suis S2

To study the regulating effect of Astragalus Polysaccharides ( APS) to the mice infected by Brucella suis S2.Methods:120 BALB/c mice were randomly divided into 4 groups:experimental mice were injected APS 1 ml ( 0.4,1.2,3 mg/ml) via peritoneal cavity respectively once a day and the control group was injected with the same volume of saline for 3 days,then infected with Brucella suis S2 1 ml (1×107 L-1 ) by ip.Five mice of each group were killed through eye bloodletting at 1,6,12,24,48, 72 h respectively post-infection with Brucella suis S 2 and the peritoneal macrophage were obtained respectively to make smear.Phagocytic rate and phagocytic index were calculated by the Wright Giemsa staining after infected 1 h.TNF-α,IL-12 and IFN-γlevels of serum at different time points were measured by ELISA.The bacterial load of MΦand spleen were measured by coating method.Results:The phagocytic rate and phagocytic index of MΦin APS 3 dose groups were higher than those of the control group ( P<0.05 ).The microbial load of MΦin APS 3 dose groups at 1 h infected by Brucella suis S 2 were significantly higher than those of control,but significantly lower than those of control at 6,12,24,48,72 h after infected by Brucella suis S2.The microbial load of spleen in APS 3 dose groups at 6 h infected by Brucella suis S 2 were significantly higher than those of control ,but significantly lower than those of control at 12,24,48,72h after infected by Brucella suis S2.The concentrations of TNF-α,IL-12 and IFN-γin the serum of APS groups had significantly been improved ( P<0.05 ).Conclusion: APS can promote the activation of MΦin vivo and strengthen the activity of phagocytosis and killing to Brucella suis S 2.APS can promote the secretion of TNF-α,IL-12 and IFN-γof mice,strengthen the cellular immune response of mice to Brucella suis S 2.
10.Mitral, aortic and tricuspid valve replacement for severe rheumatic disease

Objective: To evaluate the mitral, aortic and tricuspid valve replacement for rheumatic heart disease. Methods: From June 1990 to June 2001, 941 patients underwent valve replacement. Among them, 24 patients underwent simultaneously mitral, aortic and tricuspid valve replacement. There were 17 females and 7 males, with mean age of 36 years (ranged from 18 to 59 years). Eight patients had previous closed mitral valvotomy. Eleven patients complicated with left atrium thrombus, 16 with hapotomeglia, and 8 with ascites. The X ray results showed that the C/T ratio was 0.66 to 0.91. The heart function (NYHA) was class III in 9, and class IV in 15. All patients had combined mitral, aortic valve lesions associated with severe tricuspid valve regurgitation. Results: One patient died postoperatively with an operative mortality of 4.2%. 23 patients followed up from 20 to 36 months (mean, 26.4 months). There was no thrombolism and anticoagulant related hemorrhage. The echocardiography demonstrated the diameter of left atrium right ventricle, and LVDd significantly decreased after operation. The heart function was also significantly improved. Conclusion: The combination of mitral, aortic and tricuspid valve replacement could achieve a satisfactory result with low mortality and better recovery of heart function.
