1.Effect of homecare on health of diabetic patients in China:a Meta-analysis

Objective To study the effect of homecare on the health of diabetic patients in China. Methods A retrieval was done all on CNKI, Wanfang database, VIP database, China Biological Medicine. After excluding the literature out of the criteria, the meta-analysis was performed to compare the intervention group and control one in terms of the fasting plasma glucose , 2-hour postprandial blood glucose , quality of life , treatment compliance , standardized mean difference ( SMD ) and risk ratio ( RR ) . Results 12 RCTs, which involved 1,193 participants including 596 patients in the intervention group and 597 patients in the control group, were included in the final analysis. The fasting plasma glucose, 2-hour postprandial blood glucose, the quality of life, treatment compliance of the intervention group were significantly better than those of the control group , with effect size and 95% CI: SMD =-1.77, 95%CI (-2.28,-1.26);SMD=-1.62, 95%CI (-2.28,-0.95);SMD=1.31, 95%CI (0.50, 2.13);RR=1.27, 95%CI (1.13, 1.43) (P<0.01). Conclusion Homecare can improve the plasma glucose and quality of life of patients with diabetes and can help them to develop a healthy lifestyle.
2.Effect ofDanhuai-Yinxiecondensed pill on the expression of VEGF and its receptors in the model rabbits with psoriasis

Objective To observe the effect ofDanhuai-Yinxie condensed pill on the expression of VEGF and its receptors in the model rabbits with psoriasis.Methods Fourty-two healthy Japanese white rabbits were randomly divided into the control group, the model group, the Acitretin group, and the low-, medium- and high-doses ofDanhuai-Yinxie condensed pill groups. The backs of rabbits’ ears were wiped with 5% propranolol for 4 weeks in the latter four groups, while with saline for the same period in the control group. The rabbits in the Acitretin group were intragastric administrated with 1% Acitretin one day since the models of psoriasis were successed, while the rabbits in the low-, medium- and high-doses ofDanhuai-Yinxie condensed pill groups were intragastric administrated withDanhuai-Yinxie suspension 20, 40, 80 mg/kg daily, respectively. The rabbits in the control and model groups were intragastric administrated with equal-volume saline daily. The expression of VEGF, its receptor 1 (FLT-1) and receptor 2 (KDR) were detected by ELISA method, and the expression of VEGF mRNA was detected by PCR method, dermal papillary vascular density (MVD) and the T lymphocytes proliferation from peripheral blood were detected.Results The expression of VEGF (114.43 ± 952 ng/ml, 109.65 ± 9.15 ng/mlvs. 179.90 ± 11.30 ng/ml), VEGF mRNA (0.83 ± 0.07, 0.80 ± 0.06vs. 2.74 ± 0.15), FLT-1 (4.54 ± 0.26 ng/ml, 4.16 ± 0.24 ng/mlvs. 6.92 ± 0.38 ng/ml), KDR (2.48 ± 0.16 ng/ml, 2.51 ± 0.19 ng/mlvs. 6.79 ± 0.27 ng/ml), MVD (10.12 ± 1.97, 9.83 ± 1.12vs. 16.45 ± 2.05) and T lymphocytes proliferation (87.35% ± 6.29%, 86.21% ± 6.42%vs. 123.74% ± 9.18%) in the medium- and the high-dose groups significant decreased than those in the model group (P<0.05).ConclusionDanhuai-Yinxiecondensed pill could reduce the expression of VEGF, block the binding of VEGF to its receptors, inhibit angiogenesis, inhibit the reconstruction of subcutaneous tissues, reduce the dysplasia of dermal microvessels, and inhibit the proliferation of T cells.
3.Effect of tanshinone IIA on the calmodulin and heart function of the rats with acute ;myocardial infarction

Objective To observe the effect of tanshinone IIA on the myocardial calmodulin (CaM) and calmodulin dependent protein kinase II (CaMK) mRNA expression and heart function of acute myocardial infarction rats. Methods A total of 100 rats were randomly divided into the sham operation group, the model group, and the tanshinone ⅡA group. The model of acute myocardial infarction was established by the ligation of left anterior descending coronary artery ligation with 45 min, and reperfusion with 45 min, and the above process was repeated 3 times. Tanshinone IIA group was treated with intravenous injection of 10 mg/kg sodium tanshinone ⅡA, while the sham operation group and the model group with 10 mg/kg saline, and all treatment last 7 days. The mRNA expressions of CaM and CaMKII were determined. TTC staining was used to observe areas of myocardial infarction. The left ventricular maximum rise/fall rate (±LVdp/dtmax), myocardial contraction tension, heart rate and heart function index were detected pre-and post-treatment by the Langendorff device perfusion of isolated heart. Results Compared with the model group, the mRNA expression of and CaM mRNA (1.29 ± 0.19 vs. 2.31 ± 0.21) and CaMK II (1.10 ± 0.07 vs. 2.13 ± 0.18) in the tanshinone IIA group were significantly lower (P<0.05), and the areas of myocardial infarction (25.12%± 0.43%vs. 35.15%± 0.64%) significantly decreased (P<0.05), and the IT (2.03 ± 0.14 g vs. 1.06 ± 0.12 g), the ±LV dp/dtmax (4 701.2 ± 135.3 mmHg/s vs. 3 214.7 ± 110.2 mmHg/s, 2 518.7 ± 65.4 mmHg/s vs.1 960.3 ± 62.5 mmHg/s) significantly increased (P<0.05). Conclusion Tanshinone IIA may protect myocardial ischemia by regulating the mRNA expression of CaM and CaMKⅡ.
4.The relationship between hope level and quality of life of psoriasis vulgaris patients

Objective To explore the relationship between hope level and quality of life with psoria-sis vulgaris patients, and to make a reference for improving quality life of with psoriasis vulgaris patients. Methods 126 psoriasis vulgaris patients were investigated using the questionnaires of Herth Hope Inedx ( HHI) and Dermatology Life Quality Index( DLQI) .Results The average scores of HHI and DLQI of pso-riasis vulgaris patients were(33.37 ±5.15),(11.18 ±4.83)respectively,and the HHI score of psoriasis vul-garis patients was negatively correlated with the DLQI score ( r=-0.433, P<0.01) .The stepwise multivari-able regression showed that interconnectedness( R 2=0.177),education level( R 2=0.054) and years of psoriasis vulgaris( R 2=0.037) were the main influential factors on life quality of psoriasis vulgaris patients. Conclusion The hope level of psoriasis vulgaris patients is positively related with quality of life,so effective and targeted measures should be taken to improve their hope level in order to improve quality of life of psoria-sis vulgaris patients.
5."Procurement"-"management"separation new model for the procurement system in public hospitals

Procurement constitutes a cornerstone of daily operations in public hospitals,involving medical equipment,medical materials,pharmaceuticals,infrastructure projects,office supplies,and service-oriented projects.The responsibility for procurement used to rest on various functional departments overseeing business management,a situation that often led to a lack of transparency and standardization due to decisions made by a single department or a few key cadres.To standardize procurement practices,the national policy has introduced a"procurement"and"management"separation model.In public hospitals,pro-curement includes two main aspects:"procurement"entails the actual execution of purchasing activities,including market re-search,price negotiation,tender document formulation,and contract signing;and"management"involves the preliminary re-search,budgeting and project initiation,installation and commissioning,inventory acceptance,maintenance quality control,and usage management of procured items.The separation of"procurement"and"management"is an important part of the procure-ment management unit in the modern hospital administration.This process-based division ensures the functional distinction be-tween procurement and management,fostering interdepartmental collaboration and mutual oversight,thereby mitigating procure-ment integrity while safeguarding procurement quality.
6.Effect of behavioral intervention on scratching behavior in patients with senile pruritus

Objective:To explore the impact of behavioral intervention on scratching behavior of patients with senile pruritus.Methods:One hundred cases of senile pruritus were randomly divided into observation group ( n=50) and control group ( n=50). All subjects received oral antihistamine.The observation group received behavioral intervention based on the trans-theoretical model, and the control group received routine health education.Visual analogue scale (VAS), Pittsburgh sleep quality index (PSQI) and dermatology life quality index (DLQI) were used to compare the itching symptoms, sleep quality and quality of life of senile pruritus patients in the two groups. Results:After 6 months intervention, the change of scratching behavior in the observation group((1.96±1.54), (8.83±2.05), (10.46±2.24)) was significantly better than those in the control group((4.68±1.48), (12.89±2.02), (14.62±2.12)), and the differences were statistically significant (all P<0.01). Conclusion:Health intervention based on trans-theoretical model can effectively change scratching behavior, relieve itching symptoms, improve sleep quality and the quality of life of senile pruritus patients.
