1.Analysis of epidemic trend of Keshan disease in Chuxiong City of Yunnan Province from 1967 to 2012

Objective To observe the epidemic trends of Keshan disease (KD) from 1967 to 2012 in Chuxiong City,in order to provide a scientific basis for prevention and control of the disease.Methods The data below was collected and analyzed with epidemiological method.KD cases reported through registration and the death case reported were collected in Chuxiong City from 1960 to 2012,also the reported monitoring results of KD and the adult KD screening results of dilated cardiac patients and mountain patients in 2012 were collected,then epidemic trends of KD was analysed comprehensively.Results There were 1 569 cases of KD registered from 1967 to 2012.There were 566 death cases of KD.The total prevalence rate of KD was 7.7% (226/2 953) by monitoring children KD at the diseased areas of KD and the potential KD patients accounted for 97.3% (220/226) from 1990 to 1994.The total prevalence rate of KD was 0.6% (44/7 174) by surveillance at KD diseased areas from 2005-2012 and the potential KD patients accounted for 70.5% (31/44),there were 7 children cases of KD under 15 years who accounted for 15.9% from 2005 to 2012.Totally 313 551 children were given sodium selenium for supplying selenium from 1976 to 2008.Totally 151 cases of KD were found by screening from the patients with dilated cardiac patients since 1988 and they were all chronic KD patients.There were 112 cases of KD in 2012 and there were 6 children cases of KD under 15 years,which accounted for 5.4%.Chuxiong City Keshan disease epidemic presented from subacute Keshan disease to Keshan disease,to latent Keshan disease a slow transition;age from children (<6 years) to children (<15 years) to adult-oriented features.Conclusions The incidence and prevalence rate of KD has reached the basic control standards,but children KD is coexisting with adults KD.The pathogenic factors have not been eliminated,disease monitoring and health promotion should be carried out.
2.The abnormalities of noninvasive urodynamics in early diabetes cystopathy

Objective To study the abnormalities of noninvasive urodynamios in early diabetes eystopathy (DCP) and provide diagnosis evidences in its early stage. Methods According to the disease course (less or more than 1 year), 85 patients with type 2 diabetes mellitus (T2DM) were divided into new lydiagnosed diabetic group and non-newly diagnosed diabetic group. Thirty healthy cases were involved in normal control group. All of them were checked with the technology of noninvasive urodynamics to measure maximal flow rate, average flow rate, the volume leading to first bladder sensation and residual urine volume. Results As to the 32 newly diagnosed diabetic group, maximal flow rate was (18.4±6.9) ml/s, and average flow rate was (10.6 ± 5.3) ml/s, 18 cases were detected to have bladder residual urine, with the average residual urine volume of (13.2 ± 17.3) ml, and the DCP detection rate was 56.2%(18/32). As to the 53 non-newly diagnosed diabetic group, maximal flow rate was (14.7 ± 6.6) ml/s, and average flow rate was (9.5±4.7) ml/s,38 cases were detected to have bladder residual urine, with the average residual urine volume of (19.3 ± 18.4) ml, and the DCP detection rate was 71.7%(38/53). There was no residual urine detected in normal control group. Their maximal flow rate was (25.7 ± 5.9) ml/s, and average flow rate was (18.0 ± 4.9)ml/s. Compared with that in normal control group, maximal flow rate, average flow rote and residual urine volume decreased in both newly diagnosed diabetic group and non-newly diagnosed diabetic group(P< 0.01). As compared with that in newly diagnosed diabetic group, maximal flow rate in non-newly diagnosed diabetic group decreased obviously (P< 0.05). Conclusions The abnormalities of urodynamics may happen in the early stage of diabetes, and maximal flow rate may be as the most sensitive index. With the new technology of noninvasive urodynamies, we DCP can be diagnosed in early stage and evaluated the function of bladder dynamically.
3.Effects of different organic solvents on slow-release recombinant human bone morphogenetic protein-2 microcapsules

BACKGROUND:In literatures, the recombinant human bone morphogenetic protein-2 (rhBMP-2) loaded microcapsules can be fabricated by double emulsion solvent evaporation method with different organic solvents, such as methylene chloride, ethyl acetate or their mixture. But so far it is not determined yet which one is better. OBJECTIVE: To optimize the preparation method of microcapsules encapsulating rhBMP-2 and to compare the effects of different organic solvents on the microcapsules. METHODS:Polylactic acid-polyethylene glycol-polylactic acid copolymer as microcapsules was used to prepare rhBMP-2 loaded microcapsules with double emulsion solvent evaporation method. Four kinds of organic solvents, methylene chloride (group A), mixture of methylene chloride and ethyl acetate (group B), ethyl acetate (group C) and acetyl acetone (group D) were chosen as oil phases to compare their effects on microcapsule's morphology, diameter, and encapsulation efficiency. Passage 3 bone marrow mesenchymal stem cels from rats were co-cultured with prepared microcapsules for 14 days, and then alkaline phosphatase activity was detected. RESULTS AND CONCLUSION:Compared with the other organic solvents, dichloromethane could cause microcapsules with the smaler and more uniform shape (4-10 microns) and the highest encapsulation efficiency; the microcapsules prepared by mixture of methylene chloride and ethyl acetate had relatively wide size distribution and moderate encapsulation efficiency; the microcapsules prepared by acetylacetone were difficult to form and keep the bioactivity of rhBMP-2. After cultured with rat bone marrow mesenchymal stem cels for 14 days, the alkaline phosphatase activity in groups A, B and C was significantly higher than that in group D and there was no significant difference between group A and group B; the alkaline phosphatase activity in groups A and B was significantly higher than that in group C (P< 0.05). The results show the rhBMP-2-loaded microcapsules prepared by methylene chloride as organic solvent have good shape, high encapsulation efficiency, and good biological activity.
4.Expression of Neuritin and its clinical significance in gastric cancer

Objective To research the expression of Neuritin in gastric cancer .and the relationship between the expression of Neuritin with the occurrence and development of gastric cancer .Methods Collected 58 surgical specimens of gastric cancer from 2010 to 2013 in the first affiliated hospital of Shihezi University ,immunohistochemistry was used to examine the expression of Neu-ritin in gastric cancer and normal tissues near the cancer of the stomach .Results Immunohistochemical staining showed that Neu-ritin was moderately or highly expressed in 96 .55% (56/58) of gastric cancer ,and Neuritin was moderately or highly expressed in 94 .83% (55/58) of normal tissues near the cancer .There was no statistically significantly difference between two groups (P>0 .05);Neuritin was highly expressed in 82 .76% (48/58) of gastric cancer ,and Neuritin was highly expressed in 15 .52% (9/58) of normal tissues near the cancer ,differences between the two groups had statistically significantly (P<0 .05) .Expression of Neuritin was not correlated with gender ,age ,TNM stage ,infiltration depth ,lymph node metastasis ,differentiated degree ,the pathologic type or distant metastasis(P> 0 .05) .Conclusion The Neuritin expression level between gastric cancer and normal tissues near the cancer exist differences .There is overexpression of neuritin in gastric cancer .
5.The application of noninvasive urodynamics in early detection of diabetic cystopathy

Objective To evaluate the early detection of diabetic cystopathy(DCP)with the technology of noninvasive urodynamics.Methods 70 patients with type 2 diabetes mellitus(DM)and 30 normal control subjects were checked with the technology of noninvasive urodynamics.Based on their disease course of less or mole than 5 years.the DM patients were divided into two groups.Maximal flow rate,average flow rate,the volume leading to first bladder sensation and residual urine volume were measured by using noninvasive urodynamic technology.Results Among the 70 DM patients,34 were detected to have bladder residual urine,so the DCP detection rate was 48.6%.In the patients with DCP,the average residual urine volume Was 7-139 ml(30.1±27.1)ml,while there was no residual urine in the normal control group.As compared with the normal control group,maximal flow rate and average flow rate were decreased in all the patients with DM and those with DCP(P<0.01).After follow up of the disease,the patients with a course of more than five years of disease control had even lower maximal flow rate and average flow rate.Conclusion Maximal flow rate decrease and bladder residual urine detected with the technology of noninvasive urodynamics may be widely used in early detection and early diagnosis of DCP.
6.Effect of combined treatment with rosiglitazone and body weight control on the patients with impaired glucose tolerance

After treatment for 3 months in the patients with impaired glucose tolerance (IGT), plasma glucose, insulin and homeostasis model assessment for insulin resistance (HOMA-IR) were decreased both in rosiglitazone therapy group and in the group treated with rosiglitazone combined with body weight control.Those parameters in the latter group were decreased more significantly than those in the former.Rosiglitazone ameliorates the insulin resistance in patients with IGT, which is further improved when combined with body weight control.
7.Prevalence survey of metabolic syndrome and its components in the elderly at Tanggu Community in Tianjin

Objective To investigate the prevalence of metabolic syndrome (MS) and its components among the elderly at Tanggu community in Tainjin. Methods The elderly inhabitants were drawn out randomly from one community at Tanggu in Tianjin to examine the MS-related indexes. There were 623 samples with complete data aged (67.4±6.0) years, including 261 males with mean age of (68.3±6.1) years and 362 females with mean age of (66.9±6.0) years. Diagnosis of MS was done by criteria of Chinese Medical Association Diabetes Branch in 2004. Results The standardized prevalence of MS for the elderly in the community was 18.5% with the highest risk group aged 60~69 years. Standardized prevalences of hypertension, high triglycerides, high blood sugar, overweight, obesity, low HDL-C were 49.5%, 38.8%, 25.3%, 42.0%, 4.2%, 3.0% in the elderly. Conclusions The prevalence of MS in the elderly is lower in Tanggu community in Tianjin than in Shanghai, Foshan in Guangdong, Qingdao in Shandong and Beijing city. The condition in female is more severe than in male, the highest prevalence occurs in hypertension, and overweight, high triglycerides and high blood sugar follow it, but few of them are obesity, low HDL-C is fewer too.
8.Biomechanical studies of single segmental spinal interbody fusion with bisegment fixation for thoracolumbar burst fracture type B

BACKGROUND: Thoracolumbar burst fracture Dennis type B does not have significant damage in the intervertebral discs of the inferior vertebral body. To reduce fusion segment and remain normal intercalated disc, single discectomy (damaged vertebral body and supervisor vertebral body) is proposed, but because of the damage to the vertebral body, implanted screw is easy to loose following excising partial vertebral body, even cannot be fixed. However, it is still unknown whether implanted screw in the inferior vertebral body of the damaged vertebral body was stabilized using two segment discectomy with fixation.OBJECTIVE: To analyze the feasibility of single segmental spinal interbody fusion with bisegment fixation for thoracolumbar burst fracture type B.DESIGN, TIME AND SETTING: The randomized controlled in vitro study was conducted at the Laboratory of Biomechanics, Southern Medical University from June 2007 to June 2008.MATERIALS: A total of 20 fresh freezing 7-9 months pig samples (T_(13)?L_3 segment) were used, comprising 10 integral samples and 10 L_1 type B thoracolumbar burst fracture samples prepared by pre-injury and weight dropping technique.METHODS: Pig fresh thoracolumbar specimens from T_(13)?L_3 were collected to create models of type B thoracolumbar burst fracture. There were 4 groups in this study. Ten of them were selected as intact group (n=10) (fresh pig T_(13)?L_3 segment). T_1 vertebral endplate pre-injury and weight dropping technique and incremental trauma approach were used. Denis' type B burst fracture was produced, and ten of them were selected as unstable group (n=10). Firstly, unstable group was decompressed by discectomy and semivertebraectomy in upper half of the vertebral body, single level was fused with iliac and U-FRONT anterior thoracolumbar system were placed between T_(14) and L_2, as single discectomy with fixation group (n=10). Then lower disc of injury vertebra discectomy and vertebraectomy, fused with iliac U-FRONT anterior thoracolumbar system were placed between T_(14) and L_2, as two segment discectomy with fixation group (n=10). The bone graft was longer 1 mm than the bone graft region.MAIN OUTCOME MEASURES: The flexion, extension, right/left lateral bending, and right/left axial rotation range of motion (ROM) of T_(14)?L_2 were measured in each group on the spinal three-motional test machine at 10 N穖.RESULTS: The flexion, extension, right/left lateral bending, and right/left axial rotation were not stable in the unstable group.ROM was significantly increased in the unstable group compared with the intact group (P< 0.01). The primary stability was significantly elevated in the single discectomy with fixation and two segment discectomy with fixation groups. The flexion, extension, right/left lateral bending, and right/left axial rotation ROM were significantly reduced in the single discectomy with fixation and two segment discectomy with fixation groups compared with the unstable group (P < 0.05). The flexion, extension, right/left lateral bending ROM was significantly decreased in the two groups compared with the intact group, but axial rotation ROM was significantly increased (P< 0.05). Axial rotation ROM was smaller in the single discectomy with fixation group compared with the two segment discectomy with fixation group (P < 0.05).CONCLUSION: Single segmental spinal interbody fusion with bisegment fixation for thoracolumbar burst fracture type B had a good immediate stability in flexion, extension, lateral bending motion. Compared with traditional partial corpectomy L_1 between the caudal and cranial endplate of the adjacent vertebrae with bisegmental fixation, it had a better immediate stability in axial rotation.

Objective:To study the reliability and validity of schizophrenia quality of life scale (SQLS)in China.Method:Eighty-eight patients with schizophrenia were assessed with the SQLS.Results:The Cronbach's alpha were in 0.58-0.93.The split-half reliability coefficient were in 0.91-0.94.The test-retest reliability coefficient were in 0.57-0.76.The content validity and construct validity reached psychometrically acceptable level.Conclusion:The SQLS is a simple and reliable scale.
10.Effects of orthotopic ileal neobladder on upper urinary tract function

Objective To explore bladder carcinoma eystectomy and orthotopic ileal neobladder postoperative,the impact of new bladder on upper urinary function.Methods Twenty-eight patients with muscle invasive bladder urothelial carcinoma undergoing cystectomy W-shaped orthotopic ileal neobladder in Department of Urology of the Nuclear Industry 215 Hospital of Shaanxi Province(Mar.2006-Jun.2010)were investigated.All patients were followed for over 2 years.Assessment items included creatinine determination,urinary B-Ultrasound determination of the amount of residual urine volume and hydronephrotic cystography.Results Four weeks after the operations,all patients were removed bladder catheter angiography and found no obvious contrast extravasation.After 3 months mild hydronephrosis was found in 8 cases (28.6%),including 2 cases (7.1%) before surgery associated with hydronephrosis,the difference being not statistically significant(x2=0.49,P > 0.05).The mild hydronephrosis was found in 5 cases (17.9%) 2 years postoperation,whithout statistically significant difference compared with preoperation (x2 =0.22,P > 0.05).Preoperative serum creatinine was (72.92 ± 14.58) mmol/L,while 3 months after surgery serum creatinine was (83.42 ± 15.18) mmol/L (t =-6.43,P < 0.05).Preoperative and postoperative serum creatinine was within the normal range.Two years after surgery serum creatinine was (82.50 ± 14.39) mmol/L,with significant difference compared with that of preoperation (t =-4.67,P < 0.05),but were in the normal range,no clinical significance,the postoperative 3 months bladder capacity (160 ± 23) mL,while 2 years later bladder capacity residual urine volume of (58.7 ± 9.7) mL and (430 ± 21) mL,residual urine volume (61.3 ± 37.1) mL(t =-0.37,P> 0.05).Conclusion Orthotopie ileal bladder ideal substitute for postoperative cystectomy with less impact on the upper urinary tract function.
