1.The effect of procyanidin on periprosthetic osteolysis caused by TCP wear particles in the mouse calvaria and its mechanism.

OBJECTIVE:
To investigate the protective effects of procyanidin on periprosthetic osteolysis caused by tricalcium phosphate (TCP) wear particles in the mouse calvaria and its mechanism.
METHODS:
Forty-eight male ICR mice were randomly divided into sham group, TCP group, and procyanidin (0.2 mg/kg, 1 mg/kg, 5 mg/kg)-treated group (n=12). A periprosthetic osteolysis model in the mouse calvaria was established by implanting 30 mg of TCP wear particles onto the surface of bilateral parietal bones following removal of the periosteum. On the 2 day post-operation, procyanidin (1 mg/kg, 5 mg/kg) was locally injected to the calvaria under the periosteum every other day. After 2 weeks, all the mice were sacrificed to collect the blood samples and the calvaria. Periprosthetic osteolysis and osteoclastogenesis in the mouse calvaria were observed by tartrate resistant acid phosphatase (TRAP) staining and HE staining. mRNA levels of TRAP, capthesin K, c-Fos and NFATc1 in the periprosthestic bone tissue were examined by real-time fluorescence quantitative PCR. Serum contents of total anti-oxidation capacity (T-AOC) and MDA, and superoxide dismutase (SOD) activity were determined by chemical colorimetry. Protein expressions of autophagic biomarkers such as Beclin-1 and LC-3 in periprosthetic bone tissue of the calvaria were examined by Western blot.
RESULTS:
Compared with sham group, periprosthetic osteolysis, osteoclastogenesis, mRNA levels of TRAP, capthesin K, c-Fos and NFATc1, and serum MDA content were increased significantly in the TCP group (P<0.05), whereas serum T-AOC level and SOD activity were decreased. The protein expressions of Beclin-1 and LC-3, and the conversion of LC3-II from LC3-I were both up-regulated markedly in the mouse calvaria of TCP group (P<0.05). Compared with TCP group, osteolysis, osteoclastogenesis, mRNA levels of TRAP, capthesin K, c-Fos and NFATc1 and serum MDA content were decreased obviously in the procyanidine group (P<0.05), serum T-AOC level and SOD activity were increased, the expressions of Beclin-1 and LC-3, and the conversion of LC3-II from LC3-I were down-regulated obviously in the mouse calvaria of procyanidin group (P<0.05).
CONCLUSION
Procyanidin has a protective effect of periprosthetic osteolysis caused by TCP wear particles in the mouse calvaia, its mechanism may be mediated by inhibition of oxidative stress and autophagy.

2.Effects of rosuvastatin in homocysteine induced mouse vascular smooth muscle cell dedifferentiation and endoplasmic reticulum stress and its mechanisms.

OBJECTIVE:
To investigate the effect of rosuvastatin on homocysteine (Hcy) induced mousevascular smooth muscle cells(VSMCs) dedifferentiation and endoplasmic reticulum stress(ERS).
METHODS:
VSMCs were co-cultured with Hcy and different concentration of rosuvastatin (0.1, 1.0 and 10 μmol/L). Cytoskeleton remodeling, VSMCs phenotype markers (smooth muscle actin-α, calponin and osteopontin) and ERS marker mRNAs (Herpud1, XBP1s and GRP78) were detected at predicted time. Tunicamycin was used to induce, respectively 4-phenylbutyrate(4-PBA) inhibition, ERS in VSMCs and cellular migration, proliferation and expression of phenotype proteins were analyzed. Mammalian target of rapamycin(mTOR)-P70S6 kinase (P70S6K) signaling agonist phosphatidic acid and inhibitor rapamycin were used in Rsv treated VSMCs. And then mTOR signaling and ERS associated mRNAs were detected.
RESULTS:
Compared with Hcy group, Hcy+ Rsv group (1.0 and 10 μmol/L) showed enhanced α-SMA and calponin expression (<0.01), suppressed ERS mRNA levels (<0.01) and promoted polarity of cytoskeleton. Compared with Hcy group, Hcy+Rsv group and Hcy+4-PBA group showed suppressed proliferation, migration and enhanced contractile protein expression (<0.01); while tunicamycin could reverse the effect of Rsv on Hcy treated cells. Furthermore, alleviated mTOR-P70S6K phosphorylation and ERS (<0.01)were observed in Hcy+Rsv group and Hcy+rapamycin group, compared with Hcy group; while phosphatidic acid inhibited the effect of Rsv on mTOR signaling activation and ERS mRNA levels (<0.01).
CONCLUSIONS
Rosuvastatin could inhibit Hcy induced VSMCs dedifferentiation suppressing ERS, which might be regulated by mTOR-P70S6K signaling.

3.Exploration of the application of drones in blood emergency support

【Objective】 To explore the application of drones in blood emergency support during the 19th Asian Games Hangzhou in Shaoxing venue, so as to provide reference for future major events. 【Methods】 A retrospective analysis of drone delivery of emergency blood to two designated rescue hospitals in Shaoxing for the Asian Games was conducted. Ten emergency and 30 routine land blood delivery records from two designated hospitals in 2023 were randomly selected to compare with the blood delivery using drones. The blood temperature during the drone flight were monitored, the cold chain control during drone flight was evaluated, and the application of drone flight was reviewed. 【Results】 During the Hangzhou Asian Games, a total of 14 drones were used for blood delivery. The delivery time(min) by drone from the blood station to one of the designated hospitals was 11.83±0.09, and 20.80±3.29 by emergency land delivery and 23.23±3.54 by routine land delivery (P<0.05), meanwhile was 17.50±0.08, 24.50±3.27 and 26.57±3.23 respectively to the other hospital(P<0.05). Compared with land emergency blood delivery and routine blood delivery, blood delivery time by drone was much shorter, demonstrating better timeliness and stability. The temperature monitoring during the drone delivering showed that the suspended red blood cells and platelets were in a range of 3.8-6.8℃ and 21.8-22.8℃, with a temperature difference of 0.5-0.9 ℃ and 0.2-1.0℃ between takeoff and landing respectively, demonstrating that the blood cold chain was under control, and the delivery ran safely and smoothly. 【Conclusion】 During major events, the use of drones for emergency blood delivery ensures the quality of the blood, with good timeliness, feasibility and operability, and is worthy of promotion and application.
4.Preparation and performance characterization of pseudo-ginseng entrapped in crosslinked chitosan/polyacrylic acid/poly(ethylene oxide) nanofibrous membrane.

OBJECTIVETo prepare the cross-linked chitosan/poly(acrylic acid)/poly (ethylene oxide) nanofibrous membrance loaded with pseudo-ginseng and to determine its characteristics.
METHODSPseudo-ginseng entrapped in chitosan, poly (acrylic acid), poly (ethylene oxide) nanofibrous membrane loaded with pseudo-ginseng was prepared by electrospinning and thermal treatment method. The surface morphology of fiber membrane was observed by scanning electron microscopy and the chemical structures were characterized by infrared spectroscopy; the thermal decomposition temperature was analyzed by the thermogravimetric analysis. UV-Vis spectra were used to evaluate the in vitro release properties.
RESULTSThe average diameter of the prepared nanofibrous particles was (181 ± 71) nm. The tensile strength of fiber membrane increased by 35.3% and the decomposition temperature increased from 197℃ to 208℃ after crosslinking. Compared with casting film, the structure of fiber membrane increased the release rate and the overall amount of active components from pseudo-ginseng.
CONCLUSIONThe preparation of chitosan fiber loaded with pseudo-ginseng is simple and the dispersion of pseudo-ginseng is homogeneous. This fibrous pseudo-ginseng exhibited good release performance, providing a new Chinese medicine formulation.

5.Hypothalamic-Pituitary-Adrenal Axis and Epilepsy

Epilepsy is a recurrent, transient seizure disorder of the nervous system that affects the intellectual development, life and work, and psychological health of patients. People with epilepsy worldwide experience great suffering. Stressful stimuli such as infection, mental stress, and sleep deprivation are important triggers of epilepsy, and chronic stressful stimuli can lead to frequent seizures and comorbidities. The hypothalamic-pituitary-adrenal (HPA) axis is the most important system involved in the body’s stress response, and dysfunction thereof is thought to be associated with core epilepsy symptoms and related psychopathology. This article explores the intrinsic relationships of corticotropin-releasing hormone, adrenocorticotropic hormone, and glucocorticoids with epilepsy in order to reveal the role of the HPA axis in the pathogenesis of epilepsy. We hope that this information will yield future possible directions and ideas for fully understanding the pathogenesis of epilepsy and developing antiepileptic drugs.
6. Impact of Herpud1 in the homocysteine-induced phenotypic switching of vascular smooth muscle cells

Objective:
To investigate the impact of homocysteine inducible endoplasmic reticulum(ER) protein with ubiquitin like domain 1 protein (Herpud1) in the homocysteine (Hcy) -induced phenotypic switching of vascular smooth muscle cells (VSMCs).
Methods:
VSMCs were derived from thoracic aortic artery of male Sprague Dawley rats and cultured VSMCs (4-7 passage) were treated with various concentrations of Hcy (0, 100, 500 and 1 000 μmol/L) and applied to immunofluorescence to observe the morphological changes of VSMCs via SM-actin staining. Western blot was used to detect the expression of VSMCs phenotypic markers, including Osteopontin, Calponin and smooth muscle myosin heavy chain (SM-MHC) and the expression of endoplasmic reticulum stress (ERS) related proteins, including C/EBP-homologous protein (CHOP), inositol-requiring kinase 1 (IRE-1) and glucose regulating protein 78 (GRP78) in the absence and presence of non-selective inhibitor of ERS, 4-phenylbutyric acid (4-PBA, 2 mg/ml). The Herpud1 mRNA and protein levels were determined in Hcy-stimulated VSMCs treated with 4-PBA or transfected with specific siRNA targeting Herpud1.
Results:
Compared with the control group, SM-actin staining results showed that the shape of VSMCs treated with different concentrations of Hcy for 24 hours changed from long fusiform into round form, arrangement of myofilament became irregular and the most significant alteration was found in the 500 μmol/L Hcy group. To study the diagnosis and treatment of primary testicular neoplasms. Methods:39 cases of primary testicular neoplasms were analysed retrospectively. Results:There were 4,28 and 7 cases in the groups of 1~4 years (10.3%)、15~50 years (71.8%) and over 50 years (17.9%) respectively. 20 patients were semi noma (51.3 % ), 18 nonseminoma (46.1% ), 1 lymphoma (2.6% ). The cardinal clinical manifestation was painless solid enlargement of the testis. Ultrasonography demonstrated a hypoechoic or heterogeneous echoic mass. CT and MR could provide informations about the retroperitoneal metastasis lymph nodes and malignant changed ab dominal testis. Elevation of AFP and β-HCG noted in 50% and 40% of patients with nonseminomatous germ cell tumor? (NSGCT). Combined therapy, including radical orchiectomy、 retroperitoneal lymph node dissection (RPLND) ,radiation therapy and chemotherapy,were taken. 30 patients have been followed up for 2~5 years,2 of them died of distant metastasis. Conclusions: An intratesticular mass must be considered malignant until proved otherwise. Ultrosonography should be included a conventional examination. CT and MR scan is conducive to clinical staging. Tumor marks is helpful to predicting prognosis and monitoring therapeutic response. The choice of treatment is based upon the pathological type and clinical stage.
8.Population ecology of medical shellfish and the infection rate of Angiostrongylus cantonensis in Longhai City,Fujian Province, China

The aim of the study was to investigate the population ecology of medical shellfish and the infection of An-giostrongylus cantonensis in Longhai ,Fujian Province ,China .Aquatic and terrestrial shellfish were collected in survey points according to different types of breeding grounds .Then ,lung-microscopy method was involved in the detection of the lung tis-sue in Ampullaria gigas .Other shellfishes were mashed to detect the third-stage larvae of Angiostrongylus cantonensis .Hom-ogenization and lung microscopy were compared in the detection of the larvae of A .cantonensis in Achatina snails .Factors re-lated to the environment and influence of shellfish hosts were also included .Results showed that 8 species of molluscans were found ,including Pila gigas ,Bellamya aeruginosa ,Bellamya lithophaga ,Melanoides tuberculata ,Achatina fulica ,Vag-inulus alte ,Philomycus bilineatus ,and Bradybaenasimilaris with 1 673 specimens in 27 survey points from 9 townships .The infectionratewas19.78% inaverage.TheinfectionrateinV.altewas56.63% (47/83);theinfectionratesforA.fulicaand P .gigas were 39 .32% (92/234) and 27 .14% (130/234) ,respectively .The infection rate of each survey point was closely re-lated to the distances from the residents living area .Morever ,A .cantonensis larvae were detected in M .tuberculata .Lung mi-croscopy and homogenization method detection rate was 87 .1%and 100 .0% ,respectively .The difference was statistically sig-nificant .In conclusion ,V .alte ,A . fulica and P .gigas were A . cantonensist infection dominant population . The infection rate was closely related to micro-ecological environment for all kinds of shellfish .M .tuberculata was the new host of A .can-tonensis .Lung microscopy method should not be used in the qualitative screening detection of A . f ulica infected with A .can-tonensist .
9.Clinical efficacy of warm needling therapy on cervical spondylosis of neck type based on the theory of "treatment both for the neck and lumbus".

OBJECTIVETo compare the difference in the short-term and long-term efficacy on cervical spondylosis of neck type between warm needling therapy in the regions of both neck and lumbus and that only in the region of neck.
METHODSEighty-one patients of cervical spondylosis of neck type were randomized into group A (41 cases) and group B (40 cases), in which 2 cases dropped out. Finally, 40 cases in the group A and 39 cases in the group B accomplished the trial. In the group A, the warm needling therapy was applied to the acupoints in the region of neck and the lumbus. Fengchi (GB 20), Tianzhu (BL 10), Neck-Bailao (EX-HN 15), Wangu (GB 12), Tianyou (TE 16) and(including the tender points and code-like masses on palpation) were selected in the region of neck. Dachangshu (BL 25), Qihaishu (BL 24) and Jiaji (EX-B 2) of L5 were selected in the region of lumbus. The warm needling was applied to Fengchi (GB 20), Tianzhu (BL 10), Dachangshu (BL 25). In the group B, the warm needling therapy was applied only to the acupoints in the neck, which were same as the group A. The treatment was given once every two days, three times a week in the two groups. Separately, before treatment, 1 week after treatment, at the end of 2-week treatment and at the end of 1 month follow-up, the score of neck pain questionnaire (NPQ), the score of range of motion (ROM) in the cervical region and the score of the cervical symptoms were recorded. The efficacy at the end of treatment and in the follow-up was evaluated.
RESULTSCompared with those before treatment, the scores at all the observation time points were significantly improved in the two groups after treatment (all<0.05). In the follow-up, NPQ score, ROM score and the score of cervicalsymptoms were different significantly between the two groups (all<0.05). The results in the group A were better than those in the group B. At the end of 2-week treatment, the total effective rate was 92.5% (37/40) in the group A and was 87.2% (34/39) in the group B (>0.05). In the follow-up, the total effective rate was 87.5% (35/40) in the group A, better than 64.1% (25/39) in the group B (<0.05).
CONCLUSIONSThe treatment for both neck and lumbar regions with warm needling therapy and the treatment in the local area all achieve the short-term efficacy on cervical spondylosis of neck type. For the long-term efficacy, the treatment for both neck and lumbar regions achieves the better result as compared with the routine treatment in the region of neck.
10.Changes of etiology and management of male urethral stricture in China: a multicentre evaluation

ObjectiveTo investigate the etiology and management of male urethral stricture at 8 medical centers in China during the period from 2004 to 2009 years,and to investigate whether there were any changes in etiology and management of urethral stricture with time change.MethodsThe database on 3455 male patients with urethral stricture who underwent treatment at 8 medical centers in China between January 2004 and December 2009 were prospectively collected.The databases were analyzed for possible cause of stricture and treatment techniques for urethral stricture,and for the changes in etiology and management with time change.ResultsThere were 3455 operations for urethral stricture during the study period.The main causes of urethral strictures were traumas in 1833 patients (53.05%),among which pelvic fractures were in 1327 (38.41%) and perineal trauma in 506 (14.65%).The second cause was iatrogenic causes in 1181 patients (34.18% ),among which transurethral operations or examinations were in 602 (17.42%),hypospadias surgery in 291 (8.42%) and urethral catheterization in 164 (4.75% ).Less common causes were urethritis in 201 patients (5.82%),lichen sclerosus in 149 (4.31%),undefined in 91 (2.63%).The treatments of urethral strictures were endourological surgery including internal urethrotomy and dilation and open urethroplasty including end-to-end urethroplasty and the substitude urethroplasty etc.The ratios of using various techniques in total number of patients were obviously different by time.The most application technique for treatment of urethral stricture was endourological surgery ( 709 ) during 2004 -2006 and occupied 52.67% in total number of patients.It was gradually decreased during 2007 -2009 (726) and only occupied 34.42% (P <0.01 ).Open urethroplasty gradually increased during 2007 -2009 ( 1243,58.94% ) compared with the first three years (563,41.83% ) (P < 0.01 ). Conclusions During the recent years there was an increase in the incidence of urethral stricture being trauma and iatrogenic causes.The main treatments of urethral strictures were endourological surgery and open urethroplasty.Endourological surgery was significantly decreased in total number of patients,while open urethroplasty were significantly increased during the late three years.
