1.Treatment of Thoracic-lumbar Vertebra Tuberculosis by Focus Eliminate Through Anterior Approach in First Intention and Fixation by Bone Transplantation and Interfixation of Z-plate

Objective To discuss the significance and effect of tuberculosis of thoracic-lumbar vertebra treated by interfixation with Z-plate in anterior approach. Methods Summarize was made in 32 cases of patient with tuberculosis of thoracic-lumbar vertebra from January 2000 to June 2004 ,All were treated by focus eliminate through anterior approach in first intention, autobody bone transplantation inter vertebra and interfixation with Z-plate in anterior approach. Results Followed up for a average of 25 months. 32 cases with tuberculosis of vertebra were cure, whole transplantation bones were bone fusion ,the fusion time was a average of 5 months. rectification angle of back protruding was 17.50,no surgery complication of hemothorax,pheumothorax, aggravation of paraplegia, leak of cerebrospinal fluid,looseness of interfixation and rupture . Conclusion Treatment of tuberculosis of thoracic-lumbar vertebra by focus eliminate through anterior approach in first intention, fixation by bone transplantation and interfixation of Z-plate have importance significance and marked effect.
2.Effect of Damp-heat Factor on the Expression of Aquaporin 3,4 Gene in Gastric Mucosa of Rats

Objective To establish the animal model of damp-heat syndrome(DHS) and explore the expression of aquaporin(AQP) 3,4 gene in gastric mucosa.Methods Thirty SD rats were randomized into 3 groups and observed for 20 days.Group A(normal group,NG) was fed with routine method.Group B(Pi deficiency syndrome group,PDS) was fed alternatively with routine method and aqua of cassia angustifolia Vahl.Group C(DHS) was fed with fat,honey liquid and treated with damp-heat environment.The expression of AQP 3,4 gene in the 3 groups were determined by fluorescence quantitative polymerase chain reaction(FQ-PCR).Results The symptoms and signs in Group C were simiar to the clinical DHS.Expression of AQP3 gene in DHS was statistically significant higher than that in PDS and NG(P
3.Three-dimensional reconstruction and significance of arterial cast on chronic venous insufficiency of low extremity

Objective To evaluate the three-dimensional reconstruction and significance of arterial cast on chronic venous insufficiency of low extremity. Methods A set of specimen on chronic venous insufficiency of left lower extremity amputation residues was infused with epoxy resin-lead oxide through the femoral artery,and then casted. The CT data of the cast specimen was used to reconstruct 3D modal of ar-tery on chronic venous insufficiency of left lower extremity,using the volume rendering(VR) and maximum intensity projection(MIP). Re-sults The cast specimen and 3D reconstruction of chronic venous insufficiency of left lower extremity amputation residues could clearly dis-play distribution and run of lower extremity arteries,blood supply of lesion and local region embolism of small artery ( arteriole) . Conclusion The 3D modal of artery on chronic venous insufficiency of left lower extremity should provide morphological basis of exact etiology and perfect pathogenesis,also has a certain guiding significance of clinical diagnosis and treatment.
4.Analysis of ambulatory EEG in 120 patients with epilepsy or atypical epilepsy

ObjectiveTo explore the clinical diagnostic value of ambulatory EEG (AEEG) in epilepsy and atypical epilepsy.MethodsThe monitoring of 24-hour AEEG were performed in 67 epileptic and 53 atypical epileptic.Results65.7% of epileptic cases presented abnormality, and 34.0% of atypical epilepsy. The 85.5% of epileptic form discharges were found in the stage of sleep, especially in the Ⅰor Ⅱstage of NREM. The main epileptic form discharges was sharp waves.ConclusionThe monitoring of 24-hour AEEG may play important role in the diagnosis of epilepsy or atypical epilepsy, but can do little in the classification and detecting the focus of epilepsy.
5.The expression of molecular chaperone HSP90 and IL-6 in patients with systemic lupus erythematosus.

To explore the expression and clinical significance of molecular chaperone heat shock protein 90 (HSP90) in peripheral blood mononuclear cells (PBMC) and plasma level of interleukin-6 (IL-6) in patients with systemic lupus erythematosus (SLE), HSP90 was detected in PBMC by Western blot assay and the plasma level of IL-6 was measured by ELISA in 38 SLE patients and 20 normal controls. The correlation analysis was performed between the SLE disease activity index (SLE-DAI) and the expression of HSP90 and IL-6. The results showed that there was increased expression of HSP90 in the SLE patients. The active SLE group exhibited higher HSP90 levels (0.82+/-0.10) than the inactive SLE group (0.54+/-0.09) (P<0.01). The expression of HSP90 in normal control group (0.37+/-0.11) showed significant statistical difference as compared to both the inactive and active SLE groups (P<0.01, P<0.01, respectively). The plasma level of IL-6 exhibited a significant increase in both the inactive and active SLE groups (28.99+/-1.74 pg/mL, 44.58+/-9.15 pg/mL, respectively) compared with normal control group (P<0.01, P<0.01, respectively). The expression of HSP90 and IL-6 in SLE patients showed significant positive correlation with SLEDAI scoring (r=0.80, P<0.01: r= 0.74, P<0.01, respectively). In addition, there was a positive correlation between the level of IL-6 and HSP90 in SLE patients (r=0.86, P<0.01). The increased expression of molecular chaperone HSP90 and IL-6 may play an important role in the pathogenesis of SLE by regulating autoimmunity.

AIM: To investigate the changes of vascular endothelial growth factor (VEGF) in the pulmonary circulation of rats with different hypoxia and hypercapnia duration. METHODS: Forty SD rats were randomly divided into normol control group (N), exposed to hypoxia hypercapnia for 2 weeks group (T), for 4 weeks group (F), for 8 weeks group (E). The levels of VEGF were measured and the ultrastructure of pulmonary arterioles was observed by electron microscopy. RESULTS: Mean pulmonary arterial pressure (mPAP), weight ratio of RV to LV+S, the levels of VEGF in serum and lung tissue, the expression of VEGF and VEGF mRNA in group T, F, E were significantly higher than that in group N. With the prolong duration, base of endothelial cell was narrowed, proliferation of smooth muscle cells and collagenous fibers of pulmonary arterioles in rats were increased gradually. CONCLUSIONS: Hypoxia and hypercapnia increase the expression of VEGF mRNA and synthesis of VEGF. VEGF may play an important role in the pathogenesis of hypoxia pulmonary hypertension and reconstruction of pulmonary artery.
8.Pathological Features, Secretion and Helicobacter Pylori Infectionin Gastric Mucosa of Chronic Superficial Gastritis withSpleen-Stomach and Damp-Heat Syndrome

[Objective] To investigate the pathological features, secretion of interleukin-8 (IL - 8), tumor necrosis factor alpha (TNF-?) and secretory immunoglobulin A (sIgA) and helicobacter pylori (HP) infection in gastric mucosa (GM) of chronic superficial gastritis with spleen-stomach and damp-heat syndrome (SSDHS) and to explore their correlations. [Methods] Fifty-five cases of chronic superficial gastritis included 35 cases of SSDHS (Group A) and 20 cases of spleen-deficiency syndrome (Group B). Fifteen asymptomatic volunteers served as normal control group (Group C). Pathological changes, HP infection rate, levels of IL - 8, TNF -? and sIgA in GM were observed. [Results] HP infection rate was higher and inflammatory change was more obvious in Group A than those in Group B and Group C (P 0.05). [Conclusion] Gastric mucosa of chronic superficial gastritis with SSDHS presents inflammatory changes, which is related with the increase of IL - 8 and TNF -? secretion and the decrease of sIgA, and HP infection may be one of the factors inducing and enhancing the pathological changes.
9.Relationship Between Syndrome Patterns of Traditional Chinese Medicine and Apoptosis- Related Oncogene mRNA Expressions in Precancerous Lesions of Gastric Cancer

[Objective] To study the relationship between syndrome patterns of traditional Chinese medicine and bcl- 2 oncogene, p53 suppression gene mRNA expressions in precancerous lesions of gastric cancer (PIGC). [Methods] Forty PLGC cases were endoscopically and pathologically confinned, including 24 cases of moderate dysplasia of gastric mucosa, 9 cases of severe dysplasia, 7 cases of incomplete colonic intestinal metaplasia. Of the 40 cases, 10 were complicated with Qi stagnation, 12 with stomach-heat, and 18 with blood stasis, mRNA expression of bcl- 2 oncogene and p53 suppression gene were detected in situ by molecular hybridization method. [Results] The mRNA overexpression of bcl- 2 oncogene and p53 suppression gene were found in PLGC, and the expression was gradually increased with the progress of lesions. In the complicated cases, the mRNA expression of bcl- 2 oncogene and p53 suppression gene were the least in the cases with Qi stagnation and less in the cases with stomach- heat than in the cases with blood stasis. [Conclusion] Abnormal mRNA expression of bcl- 2 oncogene and p53 suppression gene were found in PLGC cases and related with different complicated cases, indicating the specificity of different syndrome patterns.
10.Investigations into non governmental hospitals in Guangdong

Objective To find out about the current state of development and existing problems in non governmental hospitals and to explore their development direction by collecting quantitative and qualitative data. Methods Investigations in the form of questionnaires were made into all the non governmental hospitals province wide and a descriptive analysis was made of the data collected. Results Non governmental hospitals in the province witnessed fast development, and yet they were generally small scaled, lacked actual strength, and were unevenly distributed, claiming only 3% of the market share; they paid special attention to the unique features of specialties and employed flexible operational modes, and yet they were still confronted with such problems as insufficient support policies from the government and personnel instability. Conclusion There is still a long way to go before the monopoly of state run hospitals is broken, diversified modes of hospital running are developed, and fair and orderly competition is formed so as to enhance the vitality of medical institutions. Administrative departments of health should fully realize the role and social functions of non governmental hospitals and actively lead their development in a healthy direction with sound policies.
