1.Efficacy of Jet ventilation with Manujet Ⅲ for removal of airway foreign body in children

Objective To evaluate the efficacy of manually controlled jet ventilation with Manujet Ⅲ apparatus (VBM Co, Germany) for removal of airway foreign body in children. Methods One hundred and twenty ASA Ⅰ or Ⅱ children aged 10 months-12 yr, weighing 8-35 kg undergoing removal of airway foreign body were randomly divided into 3 groups (n=40 each) : group Ⅰ spontaneous respiration (SR) ; group Ⅱ intermittentpositive pressure ventilation through the short side tube of the rigid bronchoscope (IPPV) and group Ⅲ manually controlled jet ventilation with Manujet Ⅲ (JV). Anesthesia was induced with methylprednisolone 2 mg/kg, propofol 2 mg/kg and γ-OH 70 mg/kg in group SR (Ⅰ) and with methylprednisolone 2 mg/kg, propofol 4-5 mg/kg, fentanyl 1.5 μg/kg and succinylcholine 2 mg/kg in group IPPV and JV (Ⅱ,Ⅲ). The bronchescope insertion condition, the occurrence of hypoxia during and after operation, body movement during operation, the rate of successful removal of the foreign body, the duration of operation, anesthesia emergence time and perioperative side effects were recorded. Resulls The rate of good bronchescope insertion condition and successful removal of airway foreign body were significantly higher, the rate of hypoxia during and after operation, body movement, breath holding and restlessness were significantly lower and duration of operation and anesthesia emergence time were significantly shorter in group IPPV and JV than in group SR (P<0.05). The rate of intraoperative hypoxia was significantly lower in group JV than in group IPPV (P<0.05). Conclusion Jet ventilation with Manujet Ⅲ can reduce the occurrence of hypoxia during removal of airway foreign body in children and does not interfere with the operation.

Human cytomegalovirus(HCMV)infection is quite prevalent in population. HCMV triggers important disorders in pregnant women,immune-compromised individuals and organ-transplant patients. For dec-ades,more and more scholars believe that NK cells are important immune cells against HCMV. The activity of NK cells largely depends on the balance between the signals transducted by inhibitory receptors and activatory re-ceptors,therefore the study of NK receptors is of great importance. It may be helpful to the basic research and clinical treatment of HCMV infection. Here,this paper reviews the changes and the molecular mechanism of NK receptors in the control of HCMV infection.

4.Effects of Glucagon Peptide-1 on Blood Pressure Variability in Diabetic Hypertensive Patients

Objective To investigate the role of glucagon peptide-1(GLP-1) in blood pressure variability (BPV) in diabetic hypertensive patients. Methods A total of 120 diabetic hypertensive patients were included in this study. GLP-1 was used for the treatment. The values of body mass index (BMI), fasting blood glucose, glycosylated hemoglobin, creatine and 24-hour ambulatory blood pressure (BP) monitoring were detected before and after treatment with GLP-1 for six months. The clinical indices were compared before and after treatment. Results The values of fasting blood glucose (mmol/L:7.12± 0.64 vs 9.19±2.78), glycosylated hemoglobin (%:7.00±0.14 vs 8.28±1.32) and BMI (kg/m2:19.30±3.24 vs 24.5±4.53 ) were de-creased to the normal level after GLP-1 treatment for six months (P<0.05). There were significant decrease in the 24-h sys-tolic BP (mmHg:135.02 ± 16.57 vs 139.52 ± 15.60), daytime systolic BP (mmHg:132.5 ± 14.6 vs 136.44 ± 14.24), 24-h systolic BP variability (mmHg:12.20 ± 1.44 vs 12.73 ± 1.66), daytime systolic BP variability (mmHg:11.11 ± 1.48 vs 11.74 ± 1.52 ), and daytime diastolic BP variability (mmHg:7.03±1.42 vs 7.43±1.45) after GLP-1 treatment for six months (P<0.05). Conclu-sion GLP-1 can decrease BPV in diabetic hypertensive patients.

Objective: To investigate the cellular mechanism of renal proximal tubular epithelial cell(PTEC) injury induced by aristolochic acid Ⅰ (AA Ⅰ) and aristololactam Ⅰ (AL Ⅰ). Methods:Human PTEC cell line HK 2 was used as the subject. Cell apoptosis was evaluated by using FACS. Fibronectin (FN) and TGF ?1 levels were assayed in the supernatant from cultured HK 2 cells by ELISA. In the blocking study, anti TGF ?1 neutralizing antibody was used as an antagonist. The changes of FN level and apoptosis were compared. Results: After stimulation by 2.5 mg/L AA Ⅰ, HK 2 cells secreted TGF ?1 at hour 12 and FN at hour 36. HK 2 cell apoptosis was detected at hour 48. Anti TGF ?1 neutralizing antibody (5 mg/L) could suppress AA Ⅰ induced apoptosis by 63.7% ( P

8.Short hairpin RNA-mediated silencing of APRIL gene inhibits proliferation and metastasis of human colon carcinoma SW480 cells in vitro

Objective To study the influence of short hairpin RNA-mediated inhibition of APRIL gene on proliferation,invasion and metastasis of human colon carcinoma SW480 ceils in vitro. Methods After treatment with shRNA, the expression level of APRIL protein in human colon carcinoma SW480 cells was detected by western blotting. And cell adhesion, cell migration and cell invasion of SW480 cells transfected with sh637 were analyzed respectively as compared with non-transfected SW480 cells and mocktransfectant. Results The expression of APRIL protein in SW480 transfected with sh637 were significantly lower than mock-plasmid groups and untransfected ones (F=42.15, P=0.00). Cell proliferation was markedly inhibited, compared with untransfected groups and negative control ones (F=24.76, P<0.05).And the average absorption of cell adhesion in transfected groups, mock-plasmid and non-transfected ones were 0.343, 0.409, 0.412, respectively. Cell adhesion decreased 39% compared with untransfected groups. Similarly, there was significant difference for cell migration (F=65.53, P<0.01) between transfected groups (The average ceil number in exposed area to migrate was 47.89±13.16) and nontransfected (98.78±23.26) as well as mock-plasmid ones (108.01±39.11). Then, in view of the average absorption of cell invasion between transfected groups(0.58±0.10) and non-transfected (0.94±0.23) as well as mock-plasmid ones(1.11±0.21), a prominently difference for cell invasion was also found between them (F=5.771, P<0.05). Conclusion The results suggest APRIL has a proliferation-inducing effect and probably be involved in cell invasion and cell metastasis of colon carcinoma and it could enhance capacity of invasion and metastasis of SW480 cells.
9.Surgical management of yon HippeI-Lindau disease(report of 4 cases)

Objective To discuss the surgical management of von Hippel Lindau(VHL) syn drome.Methods The clinical data of four VHL patients who were clinically diagnosed from March1999 to October 2006 were reviewed.The first patient was a 56 year old man with the chief complaintof hypodynamia and low serum glucose as 2.37 mmol/L.CT scan showed three masses in the the leftkidney.He had a history of cerebral haemangioblastoma ten years before.The second case was a 57 year old woman with the chief complaint of lumbago in the left side.CT scan showed masses in theleft kidney,adrenal gland and panerease.The third case was a 39 year old woman with the chief com plaint of an accident mass in the left adrenal gland.She had the history of cerebellar hemangioblastoma9 years before,spinal hemangioblastoma resection 7 years before.The fourth case was a 41-year oldwoman,she was found brain tumors and cured by gamma radiation abalation.Bilateral renal masseswere found by B ultrasound one month later,CT scan showed four masses in the both kidneys,leftadrenal gland and pancreas.Results All 4 cases underwent surgical approach.The first case under went radical nephrectomy which pathological report was PEComa of kidney.The blood glucose wasnormal one week later.The second case underwent resection of the left adrenal gland,kidney,pancre atic body and tail and spleen.Pathological report was clear cell carcinoma,islet cell tumor and adrenal cyst.Three months later she was found spinal hemangioblastoma and refused treatment.The thirdcase underwent adrenalectomy in the left side and pathologieal report was adrenal pheoehromocytoma.There was not tumor reeurrenee during 2 years' follow up.The nephrectomy and adrenalectomy wasperformed for the last ease whose pathological report was clear celt carcinoma and pheochromoeytoma.Three weeks later,tumor enueleating of the right kidney was undertaken; the result was clear cellcancer.During the follow up for one year there was no relapse of tumor.Conclusions For VHL ac companied with multiple organ tumors,surgery resection is the proper approaeh when tumors of centralnervous system is large.Different approaches could be taken to deal with multiple tumors of VHL such aswatchful waiting,nephron sparing surgery.
10.Approach to the patient with typical Bartter′s syndrome and metabolic syndrome:diagnosis and treatment

Bartter′s syndrome(BS) is a rare renal tubular diseases and an autosomal recessive hereditary disease. The characteristic findings are hypokalemia, metabolic alkalosis, and raised serum renin and aldosterone levels. Combination of metabolic syndrome with Bartter′s syndrome has not been reported so far. Here in, we report a case in order to call attention to the diagnosis and treatment of typical Bartter′s syndrome combined with metabolic syndrome.
