1.Effect of Yinxing Mihuan Oral Solution on Neovascularization and Vascular Remodeling in Chemical Photothrombosis-induced Focal Cerebral Ischemia Mouse Model

ObjectiveTo investigate the effects and underlying mechanism of Yinxing Mihuan oral solution (YM) on neovascularization and vascular remodeling in chemical photothrombosis-induced focal cerebral ischemia model in mice. MethodFifty SPF C57BL/6J mice were randomly divided into sham group, model group, ginaton group (12.5 mg·kg-1), and low- (YM-L, 412 mg·kg-1) and high-dose (YM-H, 824 mg·kg-1) YM groups, with 10 mice in each group. The focal cerebral ischemia model was established by chemical photothrombosis method. Drugs in each group were administered by gavage for 14 consecutive days after operation. The modified neurological severity score (mNSS) and measurement of forelimb grasping were used to evaluate the neurologic impairment of mice. The vascular density of infarct border-zone (IBZ) was measured by fluorescein labelled dextran (FITC-dextran) method. The morphology of IBZ was evaluated and observed by hematoxylin-eosin (HE) staining. The expression of proteins related to neovascularization and vascular remodeling in brain tissues, including vascular endothelial growth factor (VEGF), platelet and endothelial cell adhesion molecule 1 (CD31), hypoxia-inducible factor-1α (HIF-1α), von Willebrand factor (vWF), and angiogenin (ANG), was detected by Western blot. ResultCompared with the sham group, the model group showed manifest neurological deficits (P<0.01), weakened forelimb grasping (P<0.01), increased vascular density of IBZ (P<0.01), and obvious pathological changes, such as neuronal necrosis and gliocyte proliferation. After treatment for 14 days, compared with the model group, the YM-H group showed improved neurological deficits (P<0.01), and the YM-L group and the YM-H group showed strengthened forelimb grasping (P<0.01). Moreover, the YM-L group displayed increased vascular density of IBZ (P<0.05), reduced pathological damage, and up-regulated protein expression of CD31, ANG, HIF-1α, and vWF (P<0.05, P<0.01). ConclusionYM could improve motor function and pathological morphological impairment in chemical photothrombosis-induced focal cerebral ischemia mouse model, and the underlying mechanism might be related to the promotion of neovascularization and vascular remodeling in IBZ.
2. Research progress of sacral neuromodulation procedure in the treatment of chronic constipation and fecal incontinence

Sacral neuromodulation (SNM) procedure has become a new therapy to treat chronic constipation and fecal incontinence. The surgical procedure is easy and safe. It has a small incision compared with traditional surgery and is mainly used in patients whose traditional treatment was unsuccessful. Chronic constipation is one of the most common digestive symptoms. The quality of life in patients with severe constipation has decreased greatly. Although the incidence of fecal incontinence in China is not as high as that of constipation, it also seriously affects the life of the patients, resulting in a decline in the quality of life. Although the mechanism of SNM is uncertain, with more studies conducted, understanding has become more profound, and the curative effect has been recognized. SNM can improve the symptoms and the quality of life. Many studies have reported SNM treatment. Furthermore, some trials on SNM have been conducted. It is used after colorectal resections to promote symptoms of bowel dysfunction. However, few studies reported regarding SNM for constipation and fecal incontinence in China, and knowledge regarding SNM is limited. In this article, we will mainly discuss SNM in the treatment of chronic constipation and fecal incontinence, and its research progress on the mechanism and method, surgical procedure, effectiveness, complications, postoperative contraindications, and the population who need to pay attention, in order to provide reference for the treatment of SNM in China.
3. Superficial circumflex iliac artery perforator flap for reconstruction of buccal and plate soft tissue defects

Objective:
To investigate the therapeutic effect of free superficial circumflex iliac artery perforator (SCIP)flap for reconstruction of soft tissue defects secondary to resection of retromolar and lateral buccal squamous cell carcinoma.
Methods:
From January 2014 to January 2017, eight patients with retromolar and lateral buccal squamous cell carcinoma received radical resection and reconstructed with SCIP flap immediately. CTA and color Doppler sonography were routinely performed before the surgery. According to the size of the defect in the recipient area, the flap vascularized by the perforator vessel was carefully prepared and transferred to the buccal-pharynx-palate composite defect. The recipient area and donor area were sutured tightly after arteriovenous anastomosis under microscope. The survival and functional recovery of the flap were observed after operation.
Results:
The flap sizes ranged from 5 cm× 6 cm to 7 cm×9 cm.The mean diameter of the superficial circumflex iliac arteries was 0.65 mm. And the mean diameter of the veins was 1.2 mm. The mean arterial pedicle length was 7.0 cm, and the venous pedicle length was 8.0 cm. Eight flaps were all survived. The shape of the buccal-parapharyngeal-palate was good and the mouth opening was normal after operation.
Conclusions
Superficial circumflex iliac artery perforator flap was a good choice for repairing the defect of parapharyngeal squamous cell carcinoma in the posterior molar region.

Allergic rhinitis (AR) is a global health problem that causes major illnesses and disabilities worldwide. Epidemiologic studies have demonstrated that the prevalence of AR has increased progressively over the last few decades in more developed countries and currently affects up to 40% of the population worldwide. Likewise, a rising trend of AR has also been observed over the last 2–3 decades in developing countries including China, with the prevalence of AR varying widely in these countries. A survey of self-reported AR over a 6-year period in the general Chinese adult population reported that the standardized prevalence of adult AR increased from 11.1% in 2005 to 17.6% in 2011. An increasing number of original articles and imporclinical trials on the epidemiology, pathophysiologic mechanisms, diagnosis, management and comorbidities of AR in Chinese subjects have been published in international peer-reviewed journals over the past 2 decades, and substantially added to our understanding of this disease as a global problem. Although guidelines for the diagnosis and treatment of AR in Chinese subjects have also been published, they have not been translated into English and therefore not generally accessible for reference to non-Chinese speaking international medical communities. Moreover, methods for the diagnosis and treatment of AR in China have not been standardized entirely and some patients are still treated according to regional preferences. Thus, the present guidelines have been developed by the Chinese Society of Allergy to be accessible to both national and international medical communities involved in the management of AR patients. These guidelines have been prepared in line with existing international guidelines to provide evidence-based recommendations for the diagnosis and management of AR in China.

5.Application of domestic snare in retrograde percutaneous coronary intervention for coronary chronic total occlusion lesions

Objective To evaluate the safety and efficiency of domestic snare applied during retrograde percutaneous coronary intervention (PCI) for chronic total occlusion (CTO) lesions.Methods A total of 27 patients who underwent retrograde PCI for CTO and used domestic snare during the procedure were enrolled in our study from March 2012 to November 2016.Clinical data, angiographic characteristics and PCI details were collected.Clinical data, characteristics of CTO lesion, effect of the domestic snare and snaring time were retrospectively analyzed.Special complications related to the domestic snare and major adverse cardiovascular events (MACE) were also documented.Results Domestic snare was used in all the procedures, which included facillitating the micro-catheter to pass through the CTO lesions in 6 patients and assisting the RG3/rotational guide-wire externalization in all the 27 patients.Mean snaring time was 3.5±5.4 minutes.Stents were successfully implanted in 26 patients except in 1 patient who failed to receive stent implantation for severe coronary calcification.No complications including coronary dissection, fracture of guide-wire and unreleased snare happened during the procedures and no MACE occurred during hospitalization.Conclusions Domestic snare facilitates retrograde micro-catheter crossing CTO lesions and retrograde guide-wire entering the guiding catheter and externalization.It is a simple, safe and efficient method.
6.Research on DNA Hydroxymethylation Modification in Digestive System Carcinoma

DNA hydroxymethylation modification is an important part of genome epigenetic regulation.The demethylation process from 5-methylcytosine (5-mC) to 5-hydroxymethylcytosine (5-hmC) is catalyzed by Tet protein.Abnormal genomic methylation leads to the occurrence of a variety of tumors.Hydroxymethylation is modified as a kind of demethylation and is inseparable from tumorigenesis.The expression of 5-hmC changes accompanied with the development and progression of digestive system tumors,which may be associated with the TET protein family and IDH mutation.It suggested that DNA hydroxymethylation is involved in the development and progression of digestive system tumors.This paper reviews the relationship between DNA hydroxymethylation and digestive system tumors,and aims to provide a new direction for the study of Hydroxymethylation modification in digestive system tumors.
7. Risk factors and survival analysis for synchronous esophageal carcinoma in patients with hypopharyngeal carcinoma

Objective:
To investigate the risk factors and survival status of hypopharyngeal carcinoma with synchronous second primary carcinoma of the esophagus.
Methods:
One hundred and sixty patients with newly diagnosed hypopharyngeal carcinoma from January 2009 to December 2012 were retrospectively reviewed. The clinical data, tumor-related information and follow-up results were collected and analyzed.
Results:

8.Effects of Electroacupuncture at Weizhong on Expression of Bcl-xL and Bax in Rats with Lumbar Disc Degeneration

Objective To explore the effect of electroacupuncture at Weizhong (BL40) acupoint on the expression of Bcl-xL and Bax in rats with lumbar disc degeneration. Methods Thirty male adult Sprague-Dawley rats were randomly divided into sham-operated group (n= 10), model group (n=10), and electroacupuncture group (n=10). The lumbar disc degeneration model was established by puncturing the annulus fibrosis in the model group and electroacupuncture group. One month after modeling, the electroacupuncture group was electroacupunctured at Weizhong acupoint for four weeks. HE staining was used to observe the pathological changes. Immunohistochemical staining was used to observe the expression of Bax in the lumbar disc tissue. Western blotting was used to detect the expression of Bcl-xL and Bax protein. Results HE staining showed that the degrees of lumbar disc degeneration from light to severe was ranged as: the sham-operated group, the electroacupuncture group and the model group. The expression of Bcl-xL was lower (P<0.05), and the expression of Bax was higher (P<0.01) in the model group than in the sham-operated group, which was opposite (P<0.01) in the electroacupuncture group than in the model group. Conclusion Electroacuponcture at Weizhong may upregulate the expression of Bcl-xL and downregulate the expression of Bax in rats with lumbar disc degeneration.
9.A comparative study of two commonly used preoperative imaging-aided design methods for superficial circumflex iliac artery perfo-rator flap

Objective:To evaluate the accuracy of the application of color Doppler sonography (CDS) and computer tomography angiography (CTA) in preoperative perforator identification and flap design and provide theoretical support for the restoration of oral maxillofacial defect with free superficial circumflex iliac artery perforator flap (SCIAPF). Methods: (1) Preoperative CDS and CTA techniques were performed to map the SCIA perforators of 29 adult patients diagnosed with malignant tumor in the oral maxillofacial head and neck regions. These patients were scheduled for concurrent reconstruction surgery. (2) A diagnostic test was designed to com-pare the CDS and CTA techniques. Results:(1) A total of 18 patients underwent flap preparation. SCIA was not found in one of the pa-tients during surgery, but was observed intra-operatively in the other 17 patients. The average SCIA diameter was 0.69 ± 0.20 mm. (2) The diagnostic test showed a CDS sensitivity of 75.0%, a CDS specificity of 82.4%, and an area under the ROC curve of 0.79. The CTA sensitivity was 75.0%, the specificity was 94.2%, and the area under the ROC curve was 0.85. The diameters measured by CDS and CTA were compared with the diameter measured intra-operatively. Significant differences were observed among the three diame-ters (P<0.05). The average diameter measured by CDS was 0.84 ± 0.14 mm. The average diameter measured by CTA was 1.01 ± 0.19 mm. Conclusion:CDS and CTA are relatively reliable technologies for preoperative detection of perforator vessel. The use of CDS and CTA technology mapping for SCIAPF can provide accurate information about the perforator, including the position of the perforator and the relationship between the peripheral tissues and the caliber of the vessel.
10.Pharmacognostical Study on Castanea Mollissima Blume Shell

This study was aimed to offer a scientific basis for the differentiation and control quality of Castanea mol-lissima Blume shell. The determination was given from the morphological identification, microscopical identification and TLC identification. The results showed that through obtained information such as morphological traits, tissue powder and TLC characteristics, the longitudinal section micrographs of C. mollissima Blume shell and the micro-scopic images of tissue powder had been received. It was concluded that the study provided a reliable reference for the identification of the quality control standards of C. mollissima Blume shell.
