1.Analysis of coagulation related parameters between patients with advanced schistosomiasis cirrhosis and hepatitis B cirrhosis

Objective To analyze the differences of coagulation parameters between patients with advanced schistosomiasis cirrhosis and hepatitis B cirrhosis under different degrees of liver function,so as to provide the evidence for further guidance of judging clinical condition and prognosis. Methods Sixty?three patients with advanced schistosomiasis cirrhosis and eighty pa?tients with hepatitis B induced cirrhosis hospitalized in Jingzhou Central Hospital from January 2014 to June 2016 were served as an advanced schistosomiasis cirrhosis group and a hepatitis B cirrhosis group,respectively,and ninety?six gastropathy pa?tients excluded from other diseases that might affect coagulation in the same period were served as a control group. The levels of PT,INR,Fib,TT,APTT,and PLT of the patients among the above 3 groups and the cirrhosis patients with different Child?Pugh classes were detected and compared. Results The differences of the levels of PT,INR,Fib,TT,APTT,and PLT among the 3 groups were statistically significant(F=84.512,81.672,37.612,104.475,52.497,102.233;all P<0.05). The further analysis showed that PT,INR,TT,and APTT of both the advanced schistosomiasis cirrhosis and hepatitis B induced cirrhosis groups were longer than those of the control group,and the PLT levels of both the former 2 groups were lower than that of the con?trol group(all P<0.05). Compared with the advanced schistosomiasis cirrhosis group,PT,INR,TT,and APTT of the hepati?tis B induced cirrhosis group were longer,and the levels of Fib and PLT were lower(all P<0.05). Among those with the liver function of Grade A,the reduced degree of PLT in the patients with hepatitis B induced cirrhosis was more serious,while for those with the liver function of Grade B and C,the difference between the patients with advanced schistosomiasis cirrhosis and hepatitis B induced cirrhosis was not statistically significant(both P > 0.05). Under all the grades of liver function,TT and APTT of the hepatitis B induced cirrhosis group was longer than those of the advanced schistosomiasis cirrhosis group ,and the Fib level of the former was lower than that of the latter. Among those with the liver function of Grade A and B ,PT and INR of the hepatitis B induced cirrhosis group were longer than those of the advanced schistosomiasis cirrhosis group;while for those with the liver function of Grade C,the difference between the two groups was not statistically significant(all P > 0.05). Conclu?sions There exist differences of the damage degrees of coagulation function between the patients with advanced schistosomiasis cirrhosis and hepatitis B cirrhosis. When the damage degree of the liver function is lighter,the coagulation function in the pa?tients with hepatitis B cirrhosis decreases more significantly;when the damage degree becomes more severe,APPT in the pa?tients with hepatitis B cirrhosis extends more significantly,while the differences of PLT and PT between them are little.
2.In vivo biodegradation of poly-DL-lactic acid ureteral stent in canine and pathological change in its ureter

Objective To observe the in vivo degradation of poly-DL-lactic acid ureteral stent in canine and the pathological change in canine ureter.Methods Poly-DL-lactic acid ureteral stent was inserted into 19 canine ureters,respectively.The stents were taken out every two weeks with their degradation observed with electronic microscopy and their molecular weight change detected by gel permeation chromatography.Pathological change in ureteral tissue specimens stained with hematoxylin and eosin was observed by light microscopy.Results The molecular weight of poly-DL-lactic acid ureteral stent was decreased to 8 033 seven weeks after degradation from 68 900 before degradation,with a degradation rate of 88.3%.Electronic microscopy showed that the number of pores in stent was gradually increased and their diameter became larger.The stent was degraded.The main pathological change observed in canine ureter tissue was non-specific inflammation.Conclusion Poly-DL-lactic acid ureteral stent is good in degradation property and tissue compatibility in vivo.
3.Autologous tissue reconstruction of tarsal plate combined with temporal flap in repairing full-thickness lower eyelid defect

Objective:To evaluate the clinical effect of combination of autologous tissue reconstruction of tarsal plate with temporal flap on repair of full-thickness lower eyelid defect.Methods:Eleven patients (11 eyes) underwent hard palate mucosa or ear cartilage combined the emporal flap with the orbicularis oculi muscle to repair full-thickness defect ofpalpebra inferior.Of the 11 patients,6 had more than 75% eyelid defect area,and 5 had more than 50% eyelid defect area.Results:All 11 eyes closed completely,with no entropion or ectropion,and returned to normal basically.Postoperative follow-up was performed for 6 months to 5 years,3 years and 4 months on average.The function and form of eyelid remained stable.Infection,leakage or contracture was not found on reconstruction tarsus.Conclusion:Reconstruction of eyelid with autogenous hard palate mucosa or ear cartilage combined the emporal flap with the orbicularis oculi muscle is a simple,convenient and effective method.
4.Effect of trehalose on survival rate for fat cells after cryopreservation

Objective:To explore effects of trehalose as a cryopreserve agent on survival rate of fatty tissue after cryopreservation.Methods:The liposuction was used on the abdomen of adult female.After centrifugation and purification,adipose was randomized into the following three groups,the trehalose group,the fetal bovine serum (FBS)+ 10%DMSO group and the physiological saline group.The specimens were cryopreserved at-196 ℃ for 3 months and then the HE staining,glucose transfer method and CK method were used to detect the cell survival rate in each group.Results:The activity of adipose in the trehalose group and FBS+10%DMSO group adipose was higher than that in the physiological saline group (P<0.05);while there was no significant difference between the trehalose group and FBS+10%DMSO group (P>0.05).Conclusion:As cryoprotectant,trehalose could keep fat cell viability,and adipose tissue can be used for clinical transplantation after 3 months' freezing.
5.Allogeneic peripheral blood stem cell transplantation combined with bone marrow transplantation for malignant hematologic diseases

Objective To observe curative effect and clinical outcome in 30 recipients undergoing allogcneic peripheral blood stem cell transplantation (PBSCT) combined with bone marrow transplantation (BMT). Methods 30 patients with a median age of 32.6 years underwent allo-HSCT, of which 11 patients with AML, 14 patients with ALL, and 5 patients with CML They all have a HLA-identical sibling. PBSCswere mobilized with G-CSF. Three hundreds milliliter bone marrow blood was transplanted to the patients on the day that the PBSC was transplanted. Amended Bu/Cy was used as the conditioning regimen. MTXcombined with CsA and MMF was used as GVHD prophylaxis. Results A median number of mononuclear cells of (5.13±2.6)x10~8/kg recipient's weight was collccted from peripheral blood, and (1.3±0.6)x10~8/kgrecipient' s weight from bone marrow blood. Engraftment of neutrophils and platelets was achieved at a median of (12.1±3.25) days and (14±5.33) clays respectively. Ⅰ - Ⅱ acute GVHD occurred in 40.0 % cases,Ⅲ - Ⅳ acute GVHD occurred in 3.3 % cases, and chronic GVHD developed in 43.3 % cases. Severe cGVHD developed in 3.3% cases. The 2 years disease free survival rate (DFS) by the day of transplantation was 72.0 %. Conclusion PBSCT combined with BMT was effective to cure leukemia. The results also suggested that PBSC recipients had an lower incidence of aGVHD and cGVHD as compared with previous reports.
6.Clinical features of CD34-positive and CD34-negative adult patients with acute T-lymphoblastic leukemia

Objective To analyze the clinical features and prognosis of CD34-positive and CD34-negative adult patients with acute T-lymphoblastic leukemia (T-ALL),and to explore the value of CD34 expression for prognosis of patients with T-ALL.Methods 75 adult patients diagnosed with T-ALL from January 2012 to July 2015 in the Department of Hematology,the First Affiliated Hospital of Zhengzhou University,were analyzed retrospectively.According to the expression of CD34,the patients were divided into CD34-positive group and CD34-negative group,and then the clinical characteristics and prognosis of both groups were analyzed.Results In 75 patients,CD34-positive group had 24 (32.0 %) patients and CD34-negative group had 51 (68.0 %) patients.Between the two groups,there was no significant difference in these factors,such as sex,age,infiltration of liver,spleen and lymph nodes,thrombocytopenia,high white blood cell count,abnormal karyotype,complete remission within 4 weeks and central nervous system leukemia (CNSL).The proportions of patients with hemoglobin (Hb) < 90 g/L and expression of myeloid lineage marker were higher in the CD34-positive group than those in the CD34-negative group (x2 =5.888,P=0.015;x2 =10.758,P =0.001,respectively).There were only 18 patients treated with hematopoietic stem cell transplantation (HSCT),57 patients were not.In patients without HSCT,the median survival time in the CD34-positive group and CD34-negative group was significant different (5 months vs.32 months,x2 =9.172,P =0.002).Conclusions CD34 expression in adult patients with T-ALL appears to be associated with Hb < 90 g/L and the expression of myeloid lineage markers.For the patients without HSCT,CD34 is likely negatively related with the prognosis.
7.Research on type selection index of electronic chromoendoscopy system.

The technology of electronic chromoendoscopy consists of narrow band imaging (NBI) and fuji intelligent chromo endoscopy (FICE). The two skills help distinguish between normal mucous membrane and focal zone and raise the detecting rate between the abnormal proliferation and early cancer. Therefore, the exploring research for type selection index of the two technologies and systems will be beneficial to the choice of the highest cost-effective endoscopy system and to the avoidance of wasting resources. By comparing all the indexes, and with purpose of clinical usage, NBI system has advantage over the FICE system. But with purpose for scientific research, the FICE system will be the better choice.

8.Associations of human intestinal Dorea and glucose intolerance in people with obesity

Objective:To evaluate the role of Dorea in glucose intolerance of people with obesity. Methods:This study recruited 113 young individuals with obesity and varying degree of glucose tolerance [body mass index(BMI)≥30 kg/m 2] and 105 controls, comparing the metabolic phenotypes and Dorea abundance. Correlation analysis and ROC analysis were performed to assess the association between Dorea and clinical parameters and its predictive role in predicting glucose intolerance. Results:(1) Metabolic parameters were higher in obesity group than the control group. There was no difference in body weight, BMI and WHR among subgroups classified by glucose tolerance in people with obesity. (2) The abundance of Dorea, Dorea formicigenerans were higher in obese individuals, however showing a downward trend in accordance with glucose intolerance. The abundance was inversely associated with OGTT-2 h plasma glucose and HbA 1C, while positively associated with HOMA-β. Logistic regression demonstrated that Dorea formicigenerans was an independent protective factor after adjusting confounders such as age and gender in the prevention of glucose intolerance. (3) ROC analysis exhibited that the AUC values of Dorea formicigenerans was 0.73 in the total population. Conclusion:Dorea and Dorea formicigenerans exert protective effect on glucose metabolism in obese subjects. The abundance of Dorea and Dorea formicigenerans can be used as predictors of glucose intolerance risk in obese subjects, which facilitate the early screening and monitoring.
