1.Influence of continue nursing for the self -efficacy and compliance of patients with coronary heart disease

Objective To investigate and observe the influence degree of continue nursing for the self -effi-cacy and compliance of patients with coronary heart disease.Methods 90 patients with coronary heart disease were selected as the research subjects,and 90 cases were divided into control group(conventional nursing of coronary heart disease group) 45 cases and observation group(continue nursing group) 45 cases by the method of random number table.Then,the self -efficacy,treatment compliance and satisfaction degree for the nursing of the two groups before the nursing and at first,second and fourth week after the nursing were compared.Results The self -efficacy and treatment compliance of the two groups before the nursing had no significant differences (all P >0.05),while the very high rates of the observation group's self -efficacy at first,second and fourth week after the nursing were 40.00%, 57.78% and 77.78% respectively,the higher rates of treatment compliance were 64.44%,84.44% and 97.78%respectively,the total satisfaction rates for the nursing were 97.78%,100.00%,100.00% respectively,which were all higher than those of the control group(all P <0.05),the differences were significant.Conclusion The influence of continue nursing for the self -efficacy and compliance of patients with coronary heart disease are more active,and it is more definitely to the patients,so the nursing mode is more suitable to patients with coronary heart disease.
2.The effect of intrauterine packing with gauze for hemostasis of uterine atony during operation

Objective To investigate hemostatic effect of uterine tamponade in dealing with postpartum hem -orrhage due to uterine inertia during caesarean section .Methods 72 patients treated with conservative treatment due to bleeding uterine contraction weakness during cesarean section were chosen ,who were dealed with sterile gauze close packing of uterine bleeding ( uterine packing group ) .43 patients underwent conservative therapy of uterine contraction weakness resulted in postpartum hemorrhage were selected to carry out B -Lynch suture hemostasis ( B-Lynch suture group).Bleeding volume,operation time,bleeding efficiency of two groups were calculated .Results The uterine packing group had shorter operation time ,less bleeding,immediate hemostasis rate.The B-Lynch suture group had a tad longer operation time , bleeding more , immediate hemostasis rate low , low efficiency .Immediate hemostasis rate (95%VS 82%)between two groups had statistically significant (χ2 =4.02,P<0.05),and no significant difference in the operation time(t=0.73,P>0.05).Conclusion Uterine packing for postpartum hemorrhage due to uterine inertia during caesarean section has simple operation ,rapid,hemostatic effect,postoperative body without foreign body removal.

5.Operation opportunity of laparoscopic cholecystectomy for senile patients with acute cholecystitis combined with underlying disease

Objective To explore the operation opportunity of cholecystectomy for senile patients with acute cholecystitis combined with underlying disease.Methods 82 elderly patients with acute cholecystitis complicated with underlying diseases underwent cholecystectomy were selected,they were divided into two groups according to the different operation time:the early operation group and the delayed operation group,41 cases in each group.The patients in early operation group were given operation in 72h,while the delayed operation group was given operation more than 72h after attack.The operation time,intraoperative blood loss,intraoperative tissue adhesions,intraoperative exudation,hospitalization time,cost of hospitalization and complications of the two groups were observed and com-pared.Results The operation time of the two groups were (45.32 ±1 2.23)min,(50.89 ±1 3.34)min,there was no significant difference (P >0.05),the intraoperative tissue adhesions of the delayed operation group was more than the early operation group,and the intraoperative hemorrhage and exudation in the early operation group were significantly higher than those of the delayed operation group (P <0.05),and the hospital stay and cost of hospitalization were sig-nificantly lower than those of the delayed operation group (P <0.05).1 case in the early operation group occurred pulmonary infection,3 cases with incision infection,the incidence of complications was 9.76%;1 case in the delayed operation group underwent laparotomy operation,2 cases in the delayed operation group had pulmonary infection,inci-sion infection in 4 cases,the incidence rate of complication was 1 4.63%,there was no significant difference between the two groups (P >0.05).Conclusion The optimal operation timing of acute cholecystitis combined with basic dis-eases in the elderly patients underwent cholecystectomy is within 72h.For patients who can not be immediately given operation due to various reasons,delayed cholecystectomy is safe and feasible.

AIM:To bring forward a method of determining aflatoxin G_2、G_1、B_2、B_1 in six kinds of traditional Chinese drugs by HPLC.METHODS:After being extracted by 70% methanol,purified by immunoaffinity column,aflatoxins were analysed by HPLC with fluorescence detection.RESULTS:Aflatoxin G_2、B_2 showed a good linear relationship at a range of 1.5-60pg,and Aflatoxin G_1、B_1 at a range of 5-200 pg,r>0.999 9.The recovery was between 60%-120%.CONCLUSION:The method is simple,accurate and can be used to determine aflatoxin G_2、G_1、B_2、B_1 in Naoliqing Pill,Renshen Yangrong Pill,Rensen Jiapi Pill,Sanqi Tablet,Jinshuibao Capsule and Bailine Capsule.

8.A Study on the Practicability and Influencing Factors of the Frailty Assessment in Elderly Patients with Coronary Atherosclerotic Heart Disease

Objective:To investigate the correlation and influence factors of three weak assessment scales in the evaluation of debilitating condition of elderly patients with coronary heart disease (CHD).Methods:120 cases with CHD in our hospital were chosen,the clinical material were collected.The Fried weak score,clinical weak score and EFS were assessed.The correlation of three weak assessment scales were analyzed.Results:Fried score,CFS score and EFS score determined 6 cases,8 cases and 14 cases patients with weak respectively,the incidence rate were 5.0%,6.7% and 11.7%,which had no significant difference(P>0.05).The linear correlation analysis indicated the Fried score,CFS score and EFS score had positive correlation(P<0.05),which had consistency with CHD patients.The CFS score and EFS score had no significant correlation (P>0.05).Cox regression analysis showed that the cultural level and grade of cardiac function,living conditions and sleep disorders were influencing factors of Fried scores(P<0.05).Conclusion:Three weak assessment had consistence and different clinical value for the evaluation of weakness in CHD patients,the cultural level and grade of cardiac function,living conditions and sleep disorders were influencing factors of weakness in CHD patients.
9.The prevalence and risk factors of kidney disease in type 2 diabetic patients in rural Shanghai

Objective To identify the prevalence and etiology of kidney disease and the related risk factors in type 2 diabetic patients in rural Shanghai.Methods A cross-sectional study in type 2 diabetic patients was conducted in a community of Shanghai.Questionnaire,clinical examination and laboratory tests were completed to collect the information about sociodemographic and healthcare characteristics.Results A total of 1421 eligible patients with complete information were screened from 1487 type 2 diabetic patients between November 2008 and March 2009.Of them,40.75% were men,59.25% were women,aged 37-86 (61.33 ± 9.65 ) years old,with diabetic duration of 0.25-43.92 (7.85 ± 6.34) years.Among them,43.42% had diabetic retinopathy,21.18% had neuropathy; 69.95% met the screening definition for hypertension,76.07% for hyperlipidemia,15.55% for hyperuricemia and 23.65% for cardiovascular disease.The control rates of fasting blood glucose,glycosylated hemoglobin,blood pressure and serum cholesterol were 57.71%,33.99%,14.22% and 2.46%,respectively.The prevalence of kidney disease,diabetic nephropathy and non-diabetic renal disease was 41.31%,18.51% and 13.44%,respectively; and 9.36% were diagnosed as renal insufficiency of unknown reasons.Age,diabetic duration,hyperuricemia,diabetic retinopathy and poor control of blood pressure were independently associated with kidney disease;age and poor control of blood pressure were independently associated with diabetic nephropathy; age and hyperuricemia were independent risk factors of renal insufficiency in patients with diabetic nephropathy.Conclusions Although the diabetic duration of these subjects is relatively short,the prevalence of complications including diabetic nephropathy is high.The high prevalence of non-diabetic renal disease shows the importance of further screening and diagnoses for prevention.Strict control of blood glucose,blood pressure,serum cholesterol and serum uric acid are key points of cutting down the prevalence of diabetic nephropathy and chronic kidney disease.
10.Early intervention for severe stenosis of non-infarct related artery in patients with acute ST-segment elevation myocardial infarction and multi-vessel disease

Objective To explore the clinical feasibility and safety of early intervention for severe stenosis of non-infarct related artery(non-IRA)in patients with acute ST-segment elevation myocardial infarction(STEMI) and multi-vessel disease(MVD)after successful primary percutaneous coronary intervention(PCI)for infarct-asso-ciated artery(IRA). Methods From May 1st,2011 to December 30th,2016,165 patients with STEMI and MVD were enrolled in our study. After the completion of primary PCI in IRA ,75 patients still in the hospital agreed to undergo a second staged PCI in severe stenosis of non-infarct arteries. We analyzed the in-hospital adverse events ,the length of hospital stay and clinical outcomes during the follow-up in the study population. Results There was no significant difference in the incidence of adverse events between the two groups during hos-pitalization. However,compared to patients treated with the IRA-only PCI,those treated with early intervention for severe stenosis of non-IRA was associated with greater benefits for clinical outcomes(including rehospitalization for heart failure,rehospitalization for ACS,recurrent angina pectoris,necessity for reintervention)during the follow-up except for the all-cause mortality. Conclusion Early intervention for severe stenosis of non-IRA is a feasible and safe procedure in patients with acute STEMI and MVD after successful primary PCI.
