1.Analysis of the clinical diagnosis and treatment of neonatal severe disease complicated with hyponatremia

Objective To explore the best scheme of clinical diagnosis and treatment of neonatal severe disease complicated with hyponatremia.Methods 120 severe disease neonatal with hyponatremia were randomly divided into observation group and control group,60 cases in each group.The observation group was given special care and treatment for the cause,the control group was given conventional treatment in intensive care and conventional sodium.The incidence rate of severe disease and the treatment effect of hyponatremia were compared.Results After treatment,the total incidence rate of severe disease of the observation group was 8.3%,that was 23.3% in the control group,there was significant difference between two groups (x2 =3.858,P < 0.05).In observation group,the markedly effective rate of hyponatremia was 83.3%,that was 63.3% in the control group,there was significant difference between two groups (x2 =10.231,P < 0.05).Conclusion Special intensive care and aimed at pathogeny cure can obviously reduce the probability of hyponatremia complicated with severe disease,and it can significantly improve the treatment effect of hyponatremia,it has fairly well clinical value.
2.Analysis on Medical Insurance Impact on Chinese Rural Elders’ Health Demand

The data is taken from Chinese Longitudinal Healthy Longevity Survey(CLHLS)between 2005 and 2008 to study health insurance’s impact on Chinese rural elders’ health demand. It found that health insurance significantly increased Chinese rural elders’ total health expenditure by 19%; however, it could neither significantly lengthen life expectancy nor decrease the risk of death. Chinese rural elders’ excessive demand induced by health insurance is probably inefficient. Therefore, to insure Chinese rural elders’ diseases could be treated, the government should improve medical security and control unreasonable increases in health service price.
3.Expression of HIF-1α and VEGF in synovium of knee joint in adjuvant-induce-arthritis rat

Objective:To investigate the expression of HIF-1 and VEGF and the correlation with pathological score of synovium in adjuvant-induce-arthritis rat knee joint.Methods: Models of adjuvant arthritis(AA)in rats were made successfully, the arthral pathological score was calculated, production of HIF-1α and VEGF protein was assayed by histoimmunchemical staining at different stage after CFA inoculation.Results:The HIF-1α and VEGF concentration in experimental group were higher than that in control group, and there was significant difference (P<0.01).And correlation between them was significantly(P<0.01), similar with pathological score of synovium(P<0.01).Conclusion:HIF-1α and VEGF play important roles in the development and progression of synovium of knee joint in AA rat.The interaction of them promote development and progression of neonatal vascular in synovium.

Within three years ,86 cases of tumors in the pineal region were treated by X- Knife radiosurgical technique,using the standard axial serial CT scan for stereotactic localization,the target were localized by the BRW coordinate system.Radionics RSA- 3 X- Knife treatment planning system were used to make plan,Philips SL- 75- 14 Linac was used to produce X- ray.After treatment, follow- up ranged 6~ 42 months (meanly 24 months),most of the patients showed improvements within 1` 6 months. The results of this report proved that the X- Knife treatment for tumors in the pineal region might be an effective,economical and reliable method.
7.Randomized controlled trial ofYinggencaoformula on psoriasis vulgaris with blood-heat TCM syndrome

ObjectiveTo evaluate the clinical curative effect and security by performing the clinical study ofYinggencao formula in treatment of psoriasis vulgaris with blood-heat TCM syndrome.MethodsA total of 75 patients, diagnosed with Psoriasis Vulgaris Blood-heat RCM syndrome, were randomizedly divided into the treatmeat group with 39 patients and the control group with 36. The treatment group tookYinggencao formula twice daily, while the control group tookQingdai capsules three times daily. All the patients were treated 12 weeks. PASI scores were used as the main outcome and to estimate the curative effect rates.ResultsThe PASI scores of patients in the treatment group (6.97 ± 2.02vs. 16.88 ± 2.91;t=14.380,P=0.009) and the control group (13.14 ± 3.18vs. 17.49 ± 2.32;t=7.780,P=0.013) after treatment showed significantly lower than the scores before. The PASI scores showed significant difference between the two groups after the treatment (P=0.027). The total effective rate of treatment group was significantly higher than the control group (76.9%vs. 61.1%;χ2=5.120, P<0.05).Conclusions TheYinggencao formula therapy showed better effect thanQingdai capsules therapy in treatment of psoriasis vulgaris with blood-heat TCM syndrome.
8.Effects of antisense oligodeoxynucleotide on expression of heparanase mRNA and invasion of pancreatic carcinoma in hypoxia

Objective To study the regulation of heparanase expression by hypoxia and its correlation to the invasiveness of tumor cells. Methods BxPC-3 cells were cultured in hypoxia in vitro and the heparanase mRNA and protein expression were detected by reverse transcriptional polymerase reaction chains (RT-PCR) and western blot respectively. Matrigel invasion assay was used to observe the invasive abilities of tumor cells in hypoxia and in the status of heparanase was inhibited by antisense oligodeoxynucleotide (AS-ODN) targeting to the heparanase gene promoter. Results In normoxia, there was a relatively low level of heparanase mRNA and protein expression in cultured BxPC-3 cells. In hypoxia, heparanase expression, mRNA and protein which expressed consistently, were inhibited slightly at 3h and upregulated significantly at 6h, 12h, 24h and 48h. When the heparanase expression was inhibited by AS-ODN, the heparanase mRNA and protein maintained low in hypoxia, however, the nonsense oligodeoxynucleotide (NS-ODN) did not block upregulation of heparanase expression. In matrigel assay, after 48h incubation, number of BxPC-3 cells that penetrated the Matrigel-coated filter of transwell chamber was increased 96.2% in hypoxia (P<0.01), the Hpa AS-ODN (400 nmol/L) inhibited the invasive cells by 37.2% (P<0.05). Conclusions BxPC-3 cells invasion ability is enhanced by hypoxia through upregulation of heparanase mRNA and protein expression in BxPC-3 pancreatic cancer cell lines.
9.Cranioplasty of skull vault defect and skull base reconstruction: diagnosis and management

Objective To discuss and analyze the diagnosis,management and surgical techniques for cranioplasty of skull vault defects and skull base reconstruction in order to raise the therapeutic effect. Methods The clinical data of 169 cases of skull vault and skull base defects treated with cranioplasty of skull vault defect and skull base reconstruction were retrospectively analyzed.Results Overlay tech- nique or inlay technique with imported titanium alloy mesh was used in 160 cases,and homologous bone was used in 9 cases.The surgical time ranged from 3 months to 8 years after injury.Eight cases presented with complications such as hematoma,subcutaneous effusion,infection and epilepsy postoperatively,but no operative death occurred.Conclusion For patients with skull vault defect with the diameter≥3 cm the best operative time is 3 months after injury,and for patients with intracranial and extracranial communica- ting tumors,skull base reconstruction can be performed when tumors are removed.Much attention should be paid to perioperative management and surgical skills.
10.Closed reduction and internal fixation versus total hip arthroplasty for displaced femoral neck fracture.

OBJECTIVETo compare the clinical effects between closed reduction and internal fixation (CRIF) and total hip arthroplasty (THA) for displaced femoral neck fracture.
METHODSIn this prospective randomized study, 285 patients aged above 65 years with hip fractures (Garden III or IV) were included from January 2001 to December 2005. The cases were randomly allocated to either the CRIF group or THA group. Patients with pathological fractures (bone tumors or metabolic bone disease), preoperative avascular necrosis of the femoral head, osteoarthritis, rheumatoid arthritis, hemiplegia, long-term bed rest and complications affecting hip functions were excluded.
RESULTSDuring the 5-year follow-up, CRIF group had significantly higher rates of complication in hip joint, general complication and reoperation than THA group (38.3% vs. 12.7%, P<0.01; 45.3% vs. 21.7%, P<0.01; 33.6% vs. 10.2%, P<0.05 respectively). There was no difference in mortality between the two groups. Postoperative function of the hip joint in THA group recovered favorably with higher Harris scores.
CONCLUSIONFor displaced fractures of the femoral neck in elderly patients, THA can achieve a lower rate of complication and reoperation, as well as better postoperative recovery of hip joint function compared with CRIF.
