1.Role of transforming growth factor beta 1 in the formation of restenosis following vascular injury

Transforming growth factor ?1(TGF-?1) is a growth and differentiation factor with highly multi-effect and multi-function,it widely regulates many physiological and pathological processes,such as body's growth,development,inflammation,repair and immune.The vascular restenosis is attributable to a local inflammation after vascular injury,which results in the production of cytokines.TGF-?1 in the local level can stimulate the proliferation and migration of smooth muscle cells,and promote the phenotypic transition and migration of adventitial cells,as well as production and deposition of local extracellular matrix protein,thus promoting vascular restenosis.This article attempts to investigate the role of TGF-?1 in restenosis after pathological vascular injury by the biological characteristics of TGF-?1,pathology of vascular restenosis,relationship between TGF-?1 and vascular restenosis,as well as the mechanism of TGF-?1 on restenosis after vascular injury.
2.Predictive Value of HER2/neu Amplification and Lymphovascular Invasion for Axillary Lymph Node Metastasis in Early Breast Cancer Patients

Objective To study the predictive value of HER2/neu amplification and lymphovascular invasion for axillary lymph node metastasis in T1 breast cancer patients(mass<2 cm).Methods Reviewed the medical record of 206 T1 breast cancer patients who were the presence or absence of lymph node metastasis with IHC and FISH,analyzed the association be-tween ALNM and various clinicopathological predictive factors such as age,tumor size (T1a,T1b,T1c),multiplicity,the his-tologic grade,the nuclear grade,the presence of lymphovascular invasion (LVI),the estrogen and progesterone receptor sta-tus,an HER2/neu expression,the Ki-67 labeling index and the bcl-2 expression,and discussed the correlation of the various factors with axillary lymph node metastasis of T1 breast cancer.Results One hundred and thirty-nine were the node nega-tive group (T1N0)and the remaining 67 cases were allotted to the node positive group (T1N1-3),age (χ2 = 6.484,P =0.011),LVI (χ2 =72.813,P <0.001),histologic grade (χ2 =74.813,P =0.019),HER2/neu (χ2 =74.813,P <0.005),Ki-67 (χ2 =6.255,P =0.012)and bcl-2 (χ2 =4.977,P =0.026)were the statistically significant predictive factors related to node metastasis.The factors such as tumor size (χ2 =1.544,P =0.254),surgical method (χ2 =2.414,P =2.414),and ucle-ar grading HG (χ2 =2.017,P =0.159),estrogen receptors ER (χ2 =0.140,P =0.709),progesterone receptor PgR (χ2 =2.199,P =2.199),have no significant statistical correlation with early breast cancer lymph node metastasis.Conclusion HER2/neu overexpression and LVI were related to the increased incidence of ALNM in T1 breast cancer patients,LVI was the most predictive factor of ALNM.
3.Effect of peroxisome proliferators-activated receptor gamma agonist on phenotypic transition of adventitia fibroblasts induced by transforming growth factor beta 1 in vitro

BACKGROUND: Previous studies demonstrated that adventitia fibroblasts exhibit important role in the hyperplasia of newly born endomembrane after blood vessel injury. However, the mechanism regarding phenotypic transition of adventitia fibroblasts in vitro remains unclear. OBJECTIVE: To observe effect of peroxisome proliferators-activated receptor ?agonist (PPAR ?) on phenotypic transition of adventitia fibroblasts induced by transforming growth factor ?1 (TGF-?1) in vitro, as well as the expression of collagen protein Ⅰ. DESIGN, TIME AND SETTING: A randomized grouping control experiment was performed at the Tissue Engineering Laboratory of Ninth Affiliated Hospital of Shanghai Jiao Tong Medical College between July 2007 and October 2008. MATERIALS: The aorta thoracica's adventitial fibroblasts were cultured from Sprague Dawley rats, with 2-month-old, weighing 120 g. The TGF-?1 was supplied by American GenWay Biotech Company. The rosiglitazone power was purchased from Beijing Comens Chemical Co., Ltd. METHODS: The experiment was divided into 3 groups: In the TGF-?1 group, fibroblasts was induced with TGF-?1 with different concentrations (3, 5, 10, 15 ?g/L) for 48 hours; in the rosiglitazone group, fibroblasts was induced with rosiglitazone (5, 10, 30, 50 ?mol/L) for 45 minutes followed by co-culture with 10 ?g/L TGF-?1 for 48 hours. There was no intervention measure in the control group. MAIN OUTCOME MEASURES: The phenotypic transition of fibroblasts was measured by ?-smooth muscle actin, the expression of smooth muscle ? 2-actin, as well as collagen type Ⅰ was determined by protein immunoblotting. RESULTS: Compared with the control group, 5 ?g/L TGF-?1 could significantly stimulate the changes of phenotypic transition of fibroblasts as well as the expression of smooth muscle ?2-actin and collagen type Ⅰ, which reach a peak level when treated with 10 ?g/L TGF-?1 (P 0.05). Methods Rat NASH models were established and the rats were divided randomly into five groups: the normal group, model group, high-dosage group and low-dosage groups of the treatment, and Dongbao Gantai positive drug group. Histopathological changes of the liver were observed. The levels of MDA, superoxide dismutase (SOD), and NO (nitric oxide) in the liver homogenate were determined. Expressions of cyclooxygenase-2 (cox-2) and inducible nitric oxide synthase (iNOS) in the liver were determined with immunohistochemical technique. Results In model group, extensive adipose degeneration and inflammatory cell infiltration were found in the liver. Compared with those in normal group, SOD activities in model group were obviously decreased (P<0.01), the contents of MDA and NO were obviously increased (P<0.01), and the expressions of cox-2 and iNOS proteins were obviously increased (P<0.01). Compared with those in model group, the activities of SOD in treatment groups were obviously increased (P<0.01), the contents of MDA and NO were obviously decreased (P<0.01), and the expressions of COX-2 and iNOS proteins were obviously decreased (P<0.01). The effect of high-dosage group was more obvious than that of Dongbao Gantai group (P<0.05). Conclusion Sagittara Sagittifolia L. Xiao-Zhi Pills can effectively prevent and treat nonalcoholic steatohepatitis in rats, and its mechanism may be associated with ameliorating hepatocellular steatosis, removing the free radicals and enhancing the anti-oxidation and anti-inflammation capability.
6.Effect of polymorphism of PAI-1 promotor region gene and its plasma level on patients with acute pulmonary thromboembolism

Objective To investigate the effect of polymorphism of plasminogen activator inhibitor-1(PAI-1) promotor region gene,plasma tissue plasminogen activator(t-PA)and PAI-1 on patients with acute pulmonary thromboembolism(APTE).Method Fifty-two patients with APTE were divided into two groups according to the presence or absence of traditional enviromnent risk facters,and there were26 patients in each gnup,and auother 57 healthy indiriduals as controls were analyzed.The genotypos of subjects were determined for the 4G/5G polymorphism of PAI-1 gene using polymerase chain reaction based restriction fragment length polymorphism analysis.Plasma PAI-1 and tPA were measured by ELISA.Results(1)The ratio of 4G/4G genotype in group without traditional environment risk factors was much higher than that of the other two groups.(2)Plasma t-PA decreased and plasma PAI-1 elevated significantly in group without traditional environment risk factors compared with that of the other two groups.(3)Except the 5G homozygous,plasma PAI-1 level in group without traditional environgment risk factors was significantly higher than the other two groups.There existed correlation between 4G allele and plasma PAI-1.Conclusions 4G/5G polymorphism of PAI-1 gene is associated with AFFE.4G/4G genotype increases the risk of APTE for individuals without traditional risk factors.There are hypercoagulation and hypofibrinolysis in APTE patients without traditional risk factors.
7.Effect of Jianpi Xiaozhang Tablets on Biliation in Rats

Objective:To observe the effect of Jianpi Xiaozhang tablets on the bile secretion in rats. Methods: Totally 60 rats were divided into 6 groups randomly. Jianpi Xiaozhang tablet suspension(high, moderate, low dosages), Shudantong suspension, Qinggan Lidan capsule suspension and 0. 5% carboxymethyl cellulose with the same volume was respectively given with intragastric ga-vage once daily for continuous 5 days. The bile was collected to detect total bile acids ( TBA) , total cholesterol and total bilirubin. Re-sults:Jianpi Xiaozhang tablets could notably increase TBA and total bilirubin in the bile (P<0. 01), and decrease the cholesterol content significantly (P<0. 01). Conclusion:Jianpi Xiaozhang tablets can promote the bile secretion.
8.Estrogen receptor negative primary breast carcinoma:correlation of human epidermal growth factor receptor type 2 overexpression with mammography and clinic features

Objective To explore the correlation of human epidermal growth factor receptor type 2 overexpression (HER-2+)and mammographic and clinical features in breast cancers with estrogen receptor negative expression (ER-).Methods 190 cases of ER-breast cancer were included in this study.There were 78 cases with HER-2+,and 112 cases with HER-2-.The pathological and mammographic features were compared between the two groups.Results Compared with the HER-2- group,the HER-2+ group was more in older women (P<0.001)and more likely to have a lymphonodus metastasis (P=0.011).For mammographic manifesta-tions,pure calcifications were more detected in the HER-2+ group (P=0.000),and pure masses were more detected in the HER-2-group (P<0.001).The HER-2+ group was more likely showed as a lobulated mass (P<0.001),and with spiculated margin (P=0.000). However,the HER-2- group was more likely showed as a round mass (P=0.014),and with smooth margin (P=0.000).The cal-cifications of HER-2+ group were more likely showed as malignancy calcifications (P=0.000).Conclusion HER-2 overexpression breast cancer with ER- has some mammographic and clinical features,which is helpful for individual diagnosis and treatment.
9.Cigu Xiaozhi pills's influence on lipid peroxidation and TNF-alpha expression in liver tissues of rats with nonalcoholic steatohepatitis.

OBJECTIVETo investigate the effect of Cigu Xiaozhi pills on expression of tumor necrosis factor alpha (TNF-alpha) in rat with nonalcoholic steatoheptatitis (NASH).
METHODFifty male SD rats were divided randomly into five groups: the normal group (n = 10), the model group (n = 10), the high dosage group (n = 10) and the little dosage group of the treatment (n = 10), Dongbao Gantai group (n = 10). Rats in normal control group were fed with standard diet. Rats in other four groups were established models of nonalcoholic steatohepatitis and were treated simultaneously with traditional Chinese medicine and positive medicine. Histopathological changes in the liver were observed. The serum ALT, AST, TG, TC and hepatic MDA, SOD, GPX were detected histologically. The expression of TNF-alpha in the liver was determined using the immunohistochemical technique and RT-PCR.
RESULTIn model group, extensive adipose degeneration and inflammatory cell infiltration were found in the liver. In treatment groups steatohepatitis were markedly alleviated. Compared with model group, TG, TC, ALT, AST of the serum and MDA of liver tissue were reduced significantly, and SOD, GPX were increased significantly in treatment groups and positive control group (P < 0.05). TNF-alpha was not expressed almost in normal rat liver and was expressed highly in model rat liver. Compared with model group, the TNF-alpha mRNA and protein expression were significantly lower in liver of treatment groups and positive control group (P < 0.05). And the effects of high dosage group surpass than those of Dongbao Gantai group (P < 0.05).
CONCLUSIONTNF-alpha plays an important role in NASH pathogenesis. Cigu Xiaozhi pills can effectively treat experimental nonalcoholic steatohepatitis in rats, and its mechanism may be associated with ameliorating hepatocellular steatosis, removing the free radicals and enhancing the capability of anti-oxidation and anti-inflammatory.

10.Clinical Analysis of Surgical Trauma between Complete Video-assisted Thoracoscopy and Traditional Open Surgery for Lung Cancer

Objective To compare the traumatic indicators of complete video-assisted thoracoscopic surgery (c-VATS) with traditional open surgery (TOS) for lung cancer patients, and further explore the minimal invasiveness of c-VATS. Methods From January 2010 to February 2013, 45 cases with complete video-assisted thoracoscopic surgery (c-VATS gurop) and 42 cases with traditional open surgery (TOS group) were compared in operation time, intraoperative blood loss, duration of chest drainage, the perioperative cytokine and immune cells (CRP,WBC,N,L),pain score and shoulder function score. Results There was no significant difference between groups in sex, age, pathological type, stage,operating time and the perioperative cytokine and immune cells (CRP, WBC, N, L) ( >0.05);Compared with TOS group, c-VATS group intraoperative blood loss was significantly reduced [(167.4±68.5) mL vs (288.6±84.0) mL, =0.000];Drainage time of TOS group was significantly longer than that of c-VATS group [(4.64±2.10) days vs (3.20±1.20) days, =0.000];The postoperative pain measurements in c-VATS group were less than that in TOS group, but the difference between groups was not significant on postoperative 1 day ( =0.542) and significant on postoperative 3 day,7 day,30 day ( = 0.034, 0.000, 0.000) . Patients activities of daily living scores at postoperative 7 day and 30 day in c-VATS group were significantly better than those in TOS group, with statistically significant differences ( =0.000,0.000) . Conclusion Whether the short-term effect or the long-term results,the VATS lung resection is better than the traditional open surgery. c-VATS resection of lung cancer is more invasive.
