Objective To observe the relationship between serum bilirubin levels of elderly heart failure patients and type, clinical symptom and prognosis of the heart failure. To explore the pathophysiological role of heme oxygenase 1/CO bilirubin system in the pathogenesis of the heart failure. Methods Serum total bilirubin and direct bilirubin and indrect bilirubin levels of 57 inpatients admitted to our depatment because of heart failure from April 1998 to June 2000 were examined in the second day, first and second week after admission, respectively. Results (1) Compared with the recovery stage of the heart failure, the serum total bilirubin levels during the heart failure attack increased by 52 9% and 78 9%, respectively than the first and second week after the attack(26 1 ?mol/L vs 17 4 ?mol/L and 14 6 ?mol/L, P

Objective:To explore the influence of neuregulin- 1 (NRG-1)intervention on pacing threshold of low voltage area in rats after myocardial infarction. Methods:Myocardial infarction model of rat was established via an-terior descending coronary artery ligation. The 34 rats with myocardial infarction were randomly and averagely di-vided into two groups:NRG-1 group (received NRG-1peritoneal injection)and control group (received peritoneal injection of normal saline of the same volume). After two weeks, pacing thresholds were tested in low voltage area related with infarct in rats after myocardial infarction, and Cx43 (an integral membrane protein of the connexin family)expression was measured in low voltage area. Results:After drug intervention two weeks, compared with control group, there was significant reduction in pacing threshold [(1.466±0.503)V vs.(0.7167±0.194)V,P =0.002]in NRG-1 group;there was significant rise in Cx43 expression [(0.30±0.15)vs.(0.95±0.20),P <0.001] in low voltage area in NRG-1 group.Conclusion:Neuregulin-1 significantly reduces pacing threshold of low voltage area in rats after myocardial infarction, the mechanism may be related to increase Cx43 expression and improve e-lectrical conductivity of myocardial cells.

OBJECTIVETo compare the clinical effects between closed reduction and internal fixation (CRIF) and total hip arthroplasty (THA) for displaced femoral neck fracture.
METHODSIn this prospective randomized study, 285 patients aged above 65 years with hip fractures (Garden III or IV) were included from January 2001 to December 2005. The cases were randomly allocated to either the CRIF group or THA group. Patients with pathological fractures (bone tumors or metabolic bone disease), preoperative avascular necrosis of the femoral head, osteoarthritis, rheumatoid arthritis, hemiplegia, long-term bed rest and complications affecting hip functions were excluded.
RESULTSDuring the 5-year follow-up, CRIF group had significantly higher rates of complication in hip joint, general complication and reoperation than THA group (38.3% vs. 12.7%, P<0.01; 45.3% vs. 21.7%, P<0.01; 33.6% vs. 10.2%, P<0.05 respectively). There was no difference in mortality between the two groups. Postoperative function of the hip joint in THA group recovered favorably with higher Harris scores.
CONCLUSIONFor displaced fractures of the femoral neck in elderly patients, THA can achieve a lower rate of complication and reoperation, as well as better postoperative recovery of hip joint function compared with CRIF.

Objective To improve the diagnostic and therapeutic ability of Brucellosis by analyzing the epidemic and clinical characteristics. Methods A retrospective analysis was done on the data of Brucellosis patients treated in our hospital from 2007 to 2016.Results Since the first case was diagnosed in 2012,27 patients [19 male and 8 female, mean age (44.4 ± 16.9) years] were confirmed by clinical manifestations and positive bacterial cultures results. The annual number of cases from 2012 to 2016 was 1,1,6,4 and 15.Among them,10 cases (37.0%) had a history of close contact with goat,7 cases (25.9%)with raw mutton,1 case (3.7%) with raw beef and 1 case (3.7%) with suspicious laboratory contamination while 8 cases (29.6%) had no evident risk factors for Brucellosis. The common clinical manifestation included fever (81.5%),lumbago/joint pain (55.6%),fatigue (33.3%) and hyperhidrosis (22.2%).The white blood cell count was normal among 20 cases (74.1%) while 6 cases (22.2%) with leukopenia.Mild to moderate anemia in 20 cases (74.1%) and decreased platelet number in 4 case (14.8%).The percentage of elevated alanine aminotransferase, aspartate aminotransferase and lactate dehydrogenase was 32.0%,48.0% and 100%,respectively.Decreased albumin level was found in 23 cases (92.0%).The percentage of elevated erythrocyte sedimentation rate (ESR),C-reactive protein and serum ferritin 75.0%,82.3% and 77.8%,respectively,while 12 cases (85.7%) with procalcitonin level below 0.5 ng/L.According to follow-up for at least half year, all the cases were cured by active medical management. Conclusions The number of Brucellosis cases is rapidly increasing in our hospital. It's of great significance to know the epidemic and clinical characteristics of Brucellosis.

Objective:
The aim of this study was to describe the distribution of neck-shoulder symptoms among middle school students, and to explore its influence factors.
Methods:
A cross-sectional survey was conducted on 24 middle schools in Shenyang, Zhengzhou and Shenzhen by purposive sampling method. In each middle school, 3 to 4 classes were selected from each grade all the students in the selected class would be recruited to the survey to investigate the demographic characteristics, neck-shoulder symptoms, physical exercise time, academic stress, screen behavior, sedentary behavior and other information by questionnaire. A total of 10 566 questionnaires were issued and 10 270 valid questionnaires were withdrawn. The prevalence of neck-shoulder symptoms among students were compared by different characteristics. Logistic regression models were applied to examine influencing factors associated with neck-shoulder symptoms.
Results:
The prevalence of neck-shoulder symptoms among middle school students was 19.2% (1 968/10 270), while it was 22.6% (1 137/5 039) among girls and 15.9% (831/5 231) among boys; the difference showed statistical significance (

With the spread of novel coronavirus pneumonia (NCP) in December 2019, the management and rehabilitation of elderly patients with hip fractures and protection of medical staff face new challenges, and need to be adjusted appropriately under this very circumstances. Hip fractures in the elderly account for more than half of osteoporotic fractures. Expert group formulate this consensus so as to make better decision against this epidemic and protect patients' families and medical staff. This consensus elaborates not only epidemic condition of NCP, but also general principles of medical admission, treatment and protection for both medical staff and patients, in order to provide some reference and promote the standardization of clinical diagnosis and treatment of elderly patients with hip fractures under the condition of NCP.
