1.Acute kidney injury in newborns and chronic kidney disease

Once umbilical cord ligation,neonatal kidney will be responsible for maintaining the whole-body homeostasis and the excretory requirements of the newborn. Meanwhile,there is a significant physiologic changes that occurs during the transition from intrauterine to extrauterine life. Neonatal renal reserve is less than adults,for the structure and function of the kidney still undergo a maturation process. Under the normal condi-tion,the adaptive changes of the renal function,with neonatal growth and development,can keep a delicate bal-ance of the internal environment. If they are exposed to a variety of external stressors,their immature kidney is especially vulnerable,which will not only cause fluid,electrolyte and acid-base imbalances and the adverse prog-nosis,but effect kidney structure and function further mature and increase the long-term risk of chronic kidney disease as well.
2.Keyhole operation for treatment of the intracranial aneurysm with the assistant of neuroendoscope

Objective To discuss the operation effect and the role of neuroendoscope which was used to assist the keyhole approach in treatment of intracranial aneurysms Methods The keyhole surgery with the assistant of neuroendoscope was used to treat 14 cases of aneurysm After initial exposure of pterional or superciliary arch keyhole approach with the aid of microscope,the rigis endoscpe was introduced to confirm regional anatomy,including the aneurismal neck and adjacent structure After clipping,introduced again to inspect the positions of the clip and nearby structures. Results After operation,all the 14 cases were recovered without any severe complication. Conclusions There was a sufficient exposure in the visualization of the anatomy of the aneurysm and surrouding structures,the injuries of operation were relieved,the time in operation and hospital can be shortern,as well as reliable operation effect.
3.The expression and clinical significance of interleukin-2 and interleukin-6 and tumor necrosis factor-α in the serum samples of patients with brucellosis

Objective To explore the change and value of interleukin (IL)-2,IL-6 and tumor necrosis factor (TNF)-α in the serum samples of patients with brucellosis.Methods The levels of serum IL-2,IL-6 and TNF-α were measured using the enzyme linked immunosorbent assay (ELISA) method in 155 patients with brucellosis in different clinical stages (including 69 cases in acute,34 cases in subacute and 52 cases in chronic periods) and 50 healthy controls.IL-2 was used to represent the helper T cell (Th)-type 1 cytokines,IL-6 represent Th2 type cytokines,and the Th1/Th2 ratio in different clinical stages was compared.Results The expression level of serum IL-2 in patients with brucellosis was different (ng/L:acute:10.15 ± 2.01;subacute:9.53 ± 1.68;chronic:6.76± 1.31;control:47.25 ± 5.68),the differences were statistically significant (F =74.921,P < 0.05).The expression levels of serum IL-2 in acute,subacute and chronic periods of brucellosis patients were significantly lower than that of healthy controls (all P < 0.05).The expression levels of serum IL-2 in acute and subacute periods of brucellosis patients were higher than that of chronic period patients (all P < 0.05).The expression of IL-6 and TNF-α in serum of patients in different clinical stages were different (ng/L:acute:615.22± 341.07,802.55 ± 479.53;subacute:478.45 ± 105.33,680.21 ± 366.95;chronic:306.37 ± 96.12,455.36 ± 176.27;control:121.45 ± 30.16,87.51 ± 24.03),the differences were statistically significant (F =57.692,63.210,all P < 0.05).The levels of serum IL-6 and TNF-o were significantly higher in patients with brucellosis in acute,subacute and chronic clinical stages than that of healthy controls (all P < 0.05).Serum IL-6 and TNF-α levels in acute period of brucellosis patients were higher than those of subacute and chronic periods patients (all P < 0.05);the levels of serum IL-6 and TNF-α in subacute period of brucellosis patients were also significantly higher than that of chronic period patients (all P < 0.05).The Th1/Th2 ratio was different (acute:0.02 ± 0.00;subacute:0.02 ± 0.00;chronic:0.23 ± 0.02;control:0.41 ± 0.06) in clinical patients with brucellosis,the differences were statistically significant (F =17.843,P < 0.05),the value of Th1/Th2 in acute and subacute period patients were lower than that of chronic period (all P < 0.05).Conclusion IL-2,IL-6 and TNF-α may play an important role in the pathogenesis of brucellosis,and their dynamic changes can reflect the progression and outcome of brucellosis.
4.Dynamic changes in the immune function of children with mycoplasma pneumoniae pneumonia on different disease stages

ObjectiveTo investigate the changes and functions of T lymphocyte subsets,immune globulin and complement in children with mycoplasma pneumoniae pneumonia(MPP) on different disease stages.MethodsThe levels of T Iymphocyte subsets of CD3,CD4,CD8 and immunoglobulin ( IgG,IgA IgM),and complement ( C3,C4 ) in the peripheral blood were detected on acute and recovery stages in 28 children with MPP by flow cytometry and immune nephelometry.Twenty-five healthy children were recruited as control group.ResultsAmong these subjects of MPP children on acute stage,the levels of CD3,CD4,CD8,and CD4/CD8 in the peripheral blood were (58.71 ± 11.63)%,(32.36 ± 8.06)%,(28.19±6.23 ) % and 1.15 ± 0.41 respectively,and on recovery stage,the levels of CD3,CD4,CD8,and CD4/CD8 were (61.29 ±10.17)%,(34.14 ±7.22)%,(26.47 ±6.01)%,and 1.29 ±0.37 respectively.Both on acute stage and on recovery stage of MPP children,the levels of CD4,CD4/CD8 were significantly lower than those in control group [ (39.53 ± 6.16 ) %,1.83 ± 0.49 ],and CD8 was significantly higher compared to thecontrol group( 1.83 ± 0.49 ),P<0.01.CD3 were lower than that in control group [ (63.03 ± 12.32) % ] on acute stage (P<0.01 ),and no significant difference on recovery stage (P>0.05).During the acute stage of MPP,IgG [ ( 14.50 ±3.86) g/L] and IgM [ ( 1.67 ±0.56) g/L] were obviously higher than those in control group [ (7.92 ± 2.62 ) g/L,( 1.06 ± 0.32 ) g/L,P<0.01 ],and C3 [ ( 0.83 ± 0.42 ) g/L ] were obviously lower compared to the control group [ ( 1.37 ± 0.33 ) g/L,P<0.05].There were no significant differences of IgA and C4 between MPP and control groups ( P>0.05 ).ConclusionChildren with MPP had celhilar immune and humoral immune disorders.Through the detection of T lymphocyte subsets,immunoglobulin and complement,it will be helpful to judge the effectiveness of clinical treatment,which provides a theoretical basis for the clinical application of immune regulators.
5.Early effects of delayed ligature of the umbilical cord on the blood volume in premature infants

Objective To evaluate the effect of delayed ligature of the umbilical cord on the blood volume in premature infants in early time. Methods From Oct 2007 to Jun 2010,194 premature infants with vaginal birth in department of genecology and obstetrics were randomly divided into routine ligature of the umbilical cord group (100 cases) and delayed ligature of the umbilical cord group (94 cases), comparision were made on ligature time, blood routine examination,blood pressure,oxyhemoglobin saturation and anemia incidence in 24 h,48 h,and 72 h after birth,and hemobilirubin on 1 d,2 d,3 d,and 7 d after birth. Results There were significant differences between the two groups in delayed ligature time [(100. 14 + 10. 57) s vs (22. 40 +5.02) s,P <0. 05)]. The hemoglobin and blood pressure in delayed ligature umbilical group were increased significantly(P < 0. 05). There was 3 cases (3. 19%) of anemia in delayed ligature of the umbilical cord group, and 11 cases (11.00%) in routine ligature of the umbilical cord group, which showed significant difference (P < 0. 05). And the hemobilirubin on 7 d after birth showed no significant difference between the two groups(P > 0. 05). Conclusion Delayed ligature of umbilical cord has a significant influence on hemoglobin and blood pressure of premature infant. It can increase the blood volume, and reduce the incidence of anemia in early time.
6.Animal Host of Schistosoma japonicum and Transmission of Schistosomiasis in Poyang Lake Region

The article clarified, according to the recent research results which have been done in Poyang Lake region of Jiangxi province, bovines are the most important animal hosts of Schistosoma japonicum and infected bovines are main infectious sources of Schistosomiasis in Poyang Lake region. Spring (March to June) is the " cross transmission stage" between definitive host and intermediate host, that is, spring is not only the susceptible season of infection for livestock and human, but also is the susceptible season of infection for snails. In flood season (July to August), the transmission of Schistosomiasis between livestock and snails belong to " low level phase" . Autumn (September to Octber) is the second seasonal peak of cercariae and is the susceptible season of infection for livestock and human. Winter(November to Feburary in next year) is the non- susceptible season of infection. Most of the susceptible zones of infection for livestock, human and snails are the marshlands near the endemic villages.
7.Expression and significance of Toil-like receptors 2, 4 in brucellosis patients

Objective This study was designed to compare the mRNA expression of Toll-like receptors 2 (TLR2) and Toll-like receptors 4 (TLR4) in monocytes between brucellosis cases and healthy controls,to explore the role and value of TLR2 and TLR4 in the pathogenesis of brucellosis.Methods From March to June in 2015,a total of 155 brucellosis cases at different clinical stages were diagnosed in Ulanqab Center for Endemic Disease Prevention and Control and 50 healthy controls were selected as research subjects.Subjective clinical symptoms of all patients were observed.Serological detection of all cases was done using serum tube agglutination test (SAT).The mRNA expression of TLR2 and TLR4 in peripheral blood cell was detected by real-time PCR technique.Results Fever,hyperhidrosis,weak,bone ache and hepatosplenomegaly were found in patients of acute (69 people),subacute (34 people) and chronic (52 people) periods.The incidences of fever and hyperhidrosis in acute and subacute periods were higher than that in chronic period,and the differences were statistically significant (x2 =58.427,26.190,all P < 0.01).The incidence of bone ache in chronic period was higher than that in acute period,and the difference was statistically significant (x2 =9.264,P < 0.01).In the agglutination titer of 1:800 and higher,the positive rate of chronic period was lower than those in acute and subacute periods,and the differences were statistically significant (x2 =14.302,8.682,all P < 0.01).The mRNA expression levels of TLR2 and TLR4 were different among different clinical periods of brucellosis patients,and the differences were statistically significant (F =17.502,24.931,all P < 0.01).The expression levels of TLR2 mRNA (8.67 ± 2.39) and TLR4 mRNA (12.38 ± 3.87) in acute period of brucellosis patients were higher than those of subacute period (5.21 ± 1.76,7.62 ± 2.21),chronic period (1.25 ± 0.47,1.72 ± 0.55) and healthy control (1.17 ± 0.23,1.43 ± 0.62),and the differences were statistically significant (all P < 0.01).The expression levels of TLR2 and TLR4 mRNA in subacute period of brucellosis patients were higher than those of chronic period and healthy controls,and the differences were statistically significant (all P < 0.01).The expression levels of TLR2 and TLR4 mRNA were not significantly different (all P > 0.05) between chronic period and healthy controls.Conclusion TLR2 and TLR4 may be the specific recognizing receptors participated in the immune response in brucellosis and associated with the occurrence and development of brucellosis.
8.Clinical application of endoscopic thyroidectomy

Objective To study the feasibility and effect of scarless endoscopic thyroidectomy(SET) and minimally invasive video-assisted surgery of the thyroid(MIVA) endoscopic technique. Methods SET: Incisions were made on the anterior part of the breast and mareolata,blunt dissection of the subcataneous planes of the neck and chest were administered .MIVA: Incisions were made 3cm above the thymus notch and the operation was video assisted in the thyroid adenoma extripation and subtotal thyroidectomy. The thyroid nodules were extirpated or subtotal thyroidectomy was performed. Results All 10 cases of the SET and 12 cases of the MIVA were successful performed and without complications. Conclusions For thyroid surgery,SET is a good cosmetic operation,MIVA is a minimal trauma and effective operation.
9.Effect of biodegradable flow-diverting device on abdominal aorta and normal branches in rabbits

Objective To evaluate the safety of biodegradable flow-diverting devices (FDs) for treatment of intracranial aneurysms in rabbits.Methods Biodegradable polyglycolic acid FDs (PGA-FDs) were implanted into the abdominal aorta in 12 New Zealand rabbits,covering the ostium of a lumbar artery.Animals underwent angiography at 3,6,and 12 weeks after device implantation;and 3 animals were sacrificed at 3 and 6 weeks for pathology by HE staining,Veigert resorcinol (VG) staining,and CD34,CD68 and α smooth muscle actin (αSMA) immunohistochemistry.The diameter of abdomen arota and the patency of side branches covered by the devices were assessed.All animals were sacrificed after angiography for pathology at 12 weeks.Results No side branch occlusions were noted,either immediately after implantation or at follow-ups.Abdominal aorta neointimal hyperplasia was minimal,and there were no differences immediately after device implantation,or at 3,6,and 12 weeks after device implantation (abdominal aorta diameter:[3.35±0.24] mm,[3.33 ±0.12] mm,[3.23±0.16] mm,[3.12±0.10] mm and [3.30±0.11] mm).Pathology results showed the neointima covered the strips of PGA-FD,and minial inflammation was abserved around the strips.The PGA strips were obsorbed completely at 12 weeks follow up with no apparent inflammation.The neointima predominantly comprised of smooth muscle cells and collagen fibers.Conclusion The PGA-FD is safe for abdominal aorta and normal side branches at short-term follow-up.
10.Biodegradable flow-diverting stent for treatment of saccular wide-neck aneurysm in rabbits

Objective To evaluate the feasibility of biodegradable flow-diverting stent for treatment of intracranial wide-neck aneurysm.Methods Twenty adult New Zealand rabbits were used to induce wide-neck aneurysm models by elastase,and then,these models were divided into 3 groups:in group one (n=7),biodegradable polyglycolic acid flow-diverting stents (PGA-FDs) were implanted into parent arteries and covered the necks of aneurysms;in group 2 (n=7),the bare metal FDs replaced PGA-FDs;the observation period of animals in group one and group 2 was 3 months;in group 3 (n=6),the PGA-FDs were implanted into parent arteries with 6 weeks of observation period.The status of aneurysm occlusion and patency of side branches covered by the stent were assessed,and all animals were sacrificed:HE staining was used to observe the thrombosis in the aneurysms,Elastic fiber staining was used to observe the neointimal thickness of parent arteries,and immuohistochemical staining was employed to detect the expressions of CD34,CD68 and alpha-actin (αSMA) in the parent arteries.Results Complete aneurysm occlusion rate in groups one,2 and 3 was 83.3% (5/6),0% (0/6) and 66.7% (4/6),respectively.The neointimal thickness in parent artery of groups one,2 and 3 was (111±69) μm,(70±48) μm and (190±52) μm,respectively;that in group 3 was significantly higher than that in group one and 2 (P<0.05).The neointima comprised predominantly smooth muscle cells and collagen fibers.Some CD34 positive vascular endothelial cells could be seen in the neointima of parent arteries of groups one and 2.Conclusion The PGA-FD is an effective device to occlude aneurysms without inducing obvious neointimal thickness and side branch stenosis in the 3-month follow-up.
