1.Covering two soft-tissue defects with combined two island flaps on lower leg

Objective To investigate the surgical procedure of covering two soft tissue defects on a lower leg simultaneously.Methods 3 cases with two soft tissue defects on a lower leg were treated with two combined fascia pedisled island flaps.The reversed fascia pediclod island flap supplied by perforating branches of anterior tibial artery was combined with that by perforating branches of posterior libial artery,the reversed fascia pedicled island (lap of superior medial malleolus was combined with that uf the post lower leg,and the reversed fascia pedicled island flap of superior medial malleolus was combined with the reversed fascia pedicled island flap supplied by perforating branches of anterior tibial artery.Results All flaps survived and the surfaces of the wounds healed well.Conclusion It is a safe,simple and practical method to cover two soft tissue defects on the lower leg with two fascia pedicled island flaps supplied by perforating branches of main arteries of the leg.

Both basic and clinical studies have confirmed that inflammatory response plays an important role in the occurrence and development of atherosclerosis (AS) as well as its caused clinical events.The aggregation phenomena of T lymphocytes,macrophages and dendritic cells (DCs) can be observed in intima and atherosclerotic plaques in the event of atherosclerosis.As the most important antigen-presenting cells activating T lymphocytes,DCs have the important functions of determining T lymphocyte activation,apoptosis,and aggregation.This article reviews the correlation between DCs and atherosclerotic lesions.
3.Material science characteristics and clinical application of bone plate

OBJECTIVE: To introduce material science characteristics and clinical application of bone plate. METHODS: The first author retrieved PubMed Database (http://www.ncbi.nlm.nih.gov/), China National Knowledge Infrastructure (http://www.cnki.net/) and Wanfang Database (http://www.wanfangdata.com.cn) for articles addressing material science characteristics and clinical application of bone plate published from 2000 to 2009. The key words included "bone plate, material science characteristics, biocompatibility, clinical application". Testing articles with reliable argument and evidence as well as close topic were included, and simultaneously, non-original articles were excluded. Ultimately, 25 articles were included. We arranged and analyzed material science characteristics, clinical application, advantages and disadvantages of bone plate. RESULTS: From the material science characteristics, the elastic modulus of bone plate should be close to skeleton so as to reduce stress occlusion effects. In clinical application, we should pad attention to the bonding of biological fixation and mechanical fixation to improve plate structure, resulting in decrease in contacting area of plate and skeleton to diminish or prevent osteoporosis in local regions. CONCLUSION: Presently, bone plate materials mainly contain stainless steel, cobalt alloy and titanium alloy in clinic with their advantages and disadvantages. Among them, medical titanium alloy is an ideal in vivo implant. In clinical application, less invasive stabilization plate provides a powerful technique security for early recovery of motor function following fracture, and shows a spacious application prospect.
4.Optimization of Ultrasonic Extraction of Total Saponins from Marsdenia tenacissima by Box-Behnken Re-sponse Surface Method and Its Purification Technology

OBJECTIVE:To optimize the ultrasonic extraction and purification technology of total saponins from Marsdenia te-nacissima,and to prepare purified total saponins from M. tenacissima. METHODS:Using the extraction rate of total saponins from M. tenacissima as index,ethanol volume fraction,solvent-solid ratio and extraction temperature as factors,ultrasonic extraction technology of total saponins from M. tenacissima was optimized by single factor test and Box-Behnken response surface design. Us-ing the content of total saponins in elution,the effects of ethanol elution volume fraction on purification technology of total sapo-nins by D101 type macroporous adsorption resin was investigated. RESULTS:The optimal ultrasonic extraction technology was as follows as ethanol volume fraction of 85%,solvent-solid ratio of 18.5∶1 (ml/g),extraction temperature of 80 ℃. In verification test,the average extraction rate of total saponins from M. tenacissima was 4.80%(RSD=1.06%,n=3);it was close to predicted value 4.89%,and the deviation was 1.84%. The optimal purification technology was as follows as 45% ethanol as eluant,90%ethanol for purification. Verification test showed that the average content of total saponins from M. tenacissima was 62.45%(RSD=0.88%,n=3). CONCLUSIONS:The optimized technology can be used for the extraction and purification of total saponins from M. tenacissima. The technology is stable and reliable.
5.Research Progress on Chemical Constituents, Pharmacological Effects and In-vivo Metabolism of Gynostemma Pentaphyllum

Gynostemma pentaphyllum belonged to the Cucurbitaceae family.It was also called theQi-Ye-Dan,licorice stem,Gong-Luo Guo-Di,and etc.It contained saponin,flavonoids,polysaccharides,terpenoids and other chemical compositions.Its pharmacological effects included cholesterol-lowering activity,antitumor activity,hypoglycemic activity,anti-aging and boost immunity.This paper reviewed the research progress on chemical constituents,pharmacological effects and in vivometabolism of gynostemma pentaphyllum,in order to provide a theoretical basis for its further research and development.

7.Influence of Infusion Drop Speed on the Efficacy of Tiopronin for Injection in the Treatment of Abnormal Liver Function

Objective:To explore the influence of infusion drop speed on the efficacy of tiopronin for injection combined with sol-vents in the treatment of abnormal liver function. Methods:Totally 74 patients were randomly divided into the observation group and the control group with 37 ones in each. The infusion drop speed was 60-65 drops per minute in the observation group, and that was 30-35 drops per minute in the control group, and the infusion time was controlled in 1h in the observation group and 2h in the control group. The clinical effect and the improvement of liver function were compared between the two groups after a treatment course of 4 weeks. Results:The total effective rate of the observation group(83. 78%) was obviously higher than that in the control group(56. 76%) (P<0. 05) after the treatment, and ALT, AST and TBIL of liver function indices were improved obviously except for ALB in the two group, and the difference was statistically significant between the two groups(P<0. 05). Conclusion:Tiopronin for injection can effectively improve the clinical effect by strictly controlling the time and drop speed of infusion.
8.Ultrastructural investigation in esophageal mucosa of non-erosive gastroesophageal reflux disease

Objective To explore the changes of ultrastructural characteristics including interstitial space dilation(ISD) and the number of desmosome in non-erosive gastroesophageal reflux disease(NERD)with abnormal and normal oesophageal acid exposure.Methods Nineteen patients with(abnormal) oesophageal acid exposure,20 with normal oesophageal acid exposure,10 with reflux(esophagitis)(RE) and 10 asymptomatic controls,classified on the basis of pH-metry,underwent(endoscopy.) Three biopsies were taken from the normal mucosa for light microscopy and transmission(electron microscopy) evaluation.ISD was measured and the number of desmosome in the intercellular spaces was calculated by using a digital scanner from photomicrographs of the specimens.The above two markers were compared with each other among the above groups.Results There was no significant difference in the mean score of the interstitial space between NERD and RE patients [(2.21?0.68)?m versus(2.39?0.42)?m,P0.05)].The mean score of the interstitial space of the control was(0.63(?0.21))?m,much lower than those of NERD and RE patients(P0.01).There was a significant(difference) in ISD between NERD with abnormal oesophageal acid exposure and normal oesophageal acid(exposure) [(2.45?0.67)?m versus(1.91?0.67)?m,P0.05].The mean numbers of desmosome in the intercellular space of the NERD and RE patients were(0.141?0.043) entries/?m~2 and(0.124(?0.044)) entries/?m~2,respectively,significantly lower than those of the controls [(0.221?0.031)(entries/?m~2].) But no significant differences were observed between the RE and the NERD patients(P0.05).Conclusions The interstitial space of esophageal mucosa in the NERD patients is dilated as in the RE.In NERD patients,the dilation of interstitial space is associated with abnormal acid exposure.The numbers of desmosome may also be a marker of esophageal mucosal injury of NERD.
9.Lunasin alleviates the sports articular cartilage injury of rats

Objective To exploer the function and underlying mechanism of Lunasin on sports articular cartilage injury.Methods Wistar rats were randomly divided into control group and model group;after injection molding for 3 times, the model rats were feed for 28 days.Then the model rats were divided into sham model group, 0.01, 0.05 and 0.1 mmol/L Lunasin treatment group respectively.After treatment, ELISA was used to analyze the production of IL-1β,IL-6,TNF-α,MMP-6 and MMP-8.SOD activity and iNOS were evaluated by their ELISA kit.Western blot was used to detect the expression of NRF2, Keap1, LC-3Ⅱ, Bax, Beclin1, p-AMPK, AMPK.Results Compared with control, the dose of IL-1β,IL-6,TNF-α,MMP-6 and MMP-8 in serum of model rats were significantly increased (P<0.05), however, after treatment with Lunasin for 1 month, these inflammatory factors were obviously reduced then that of model rats (P<0.05);Furthermore Lunasin treatment obvi-ously increased SOD activity,up-regulated NRF2 expression and down-regulated the generation of nitric oxide synthase (iNOS) (P<0.05);Additionally, Lunasin can raise the expression of autophagy-related protein(beclin1 and LC-3Ⅱ), reduce the expression of apoptosis protein (Bax) in damaged articular cartilage.ConclusionsLunasin benefits the repair of damaged joints by reducing the production of inflammatory mediators, activating of oxidative stress system and autophagy pathway.
10.Effect of α-phenyl-N-tert-butylinitrone on Expression of Nerve Growth Factor in Spinal Cord Injured Rats

Objective To explore the effect of α-phenyl-N-tert-butylinitrone (PBN) on expression of nerve growth factor (NGF) in serum and spinal cord tissue in rats after spinal cord injury (SCI). Methods 174 female Sprague-Dawley rats were randomly assigned to following groups: normal control group (n=54), normal saline control group (NS group, n=60, intrathecally injected normal saline 15 μl), and PBN group (n=60, intrathecally injected PBN, 3 mg, 15 μl). The model was established with New York University blow device (150 kDyne, 1 s dwell time). PBN was intrathecally injected into the damaged areas 30 min after operation, then once a day for 7 days. The Basso-Beattie-Bresnahan (BBB) Locomotor Rating Scale was used to assess the rats 3 days and 1 day before, and 1 day, 5 days, 10 days, 15 days, 20 days,25 days, 30 days and 35 days after SCI. NGF in the injured spinal cord tissue and serum was measured 1 h, 12 h, 24 h, 48 h, 3 days, 7 days,14 days and 21 days after SCI. Results NGF increased in serum but not in spinal cord. The ratio of NGF/total protein in serum rose and peaked 48 h after SCI, and the ratio was higher in NS group (0.92%±0.02%) than in PBN group (0.77%±0.05%) (P=0.021). BBB scores increased from the 9th day, and PBN group improved better than NS group (P<0.01). Conclusion PBN could reduce the expression of NGF in the SCI rats, and promote the recovery of neurol function.
