1.A kV cone-beam CT based analysis of the setup errors and the corresponding impact on the dose distribution of intensity modulated radiotherapy for thoracic esophageal carcinoma

Objective To measure the setup errors in thoracic esophageal carcinoma treated with intensity modulated radiotherapy (IMRT) using kilo-voltage cone-beam CT (kV-CBCT),and to evaluate the impact of the setup errors on the dose distributions in tumor target volume and the peripheral normal tissues.Methods Twenty-one patients with thoracic esophageal carcinoma undergoing image guided IMRT (IG-IMRT) were included in this study. Using kV-CBCT,173 setup errors of these patients were acquired before treatment.By shifting the isocenters,these setup errors were simulated in the 3-dimentional treatment planning system and the corresponding impact of the dose distribution in tumor target volume and peripheral organs were evaluated.Results The translational deviations of lateral,longitudinal and vertical directions were ( 2.73 ± 1.85 ),( 3.19 ± 2.71 ),and ( 2.35 ± 1.71 ) mm,respectively.The dose of gross tumor volume (GTV) was not impacted significantly by the setup errors. However,comparing with the standard plan without setup errors,the setup errors in the simulated plan reduced the dose of 95% planning target volume (PTV) by 3.38 Gy.The dose to PTV Dmin and PTV Dmean were also reduced by 9.83 Gy and 0.65 Gy respectively. The correction of setup errors improved the conformity index (CI) and the homogeneity index (HI) for PTV.The C1 and HI for the standard plan were 0.74 ± 0.10 and 1.07 ±0.02, respectively. The C1 and H1 for the simulated plan were 0.69 ± 0.08 and 1.13 ± 0.07,respectively.Statistically significance was observed in these differences ( t =3.43 and -3.91 respectively,P < 0.05 ). No statistical significance was observed in the dose differences in lungs,spinal cord and heart between the two plans ( P > 0.05).The mean maximum dose of the spinal cord was (42.20 ± 4.97 ) Gy in the simulation plan,which was (41.37 ± 2.75 ) Gy in the standard plan.For some patients,the maximum dose of the spinal cord exceeded the tolerance level of 45 Gy in the simulation plan,and one case even reached 52.8 Gy.Conclusions Using the image guidance of kV-CBCT,the setup errors can be reduced significantly for patients with esophageal carcinoma receiving IG-IMRT.The correction of setup errors can increase the treatment precision and enhance the dose in PTV.No significant dose changes were observed in the lung,spinal cord and heart as a result of setup error correctio.
2.Content Determination of Trigonelline in Radix Mirabilis by RP-HPLC

Objective To determine the content of trigonelline in Radix Mirabilis.Methods A RP -HPLC method was established.The chromatographic c olumn was Inertsil NH 2 Columns(250?4.6mm,5?m.).The mobile phase was acetonitrile -water(80∶20).The flow rate was 0.8mL?min -1 and the detection wavelength was at 265nm,and the column temperature was 30℃.Results The calibration curve was linear in t he range of 0.1872~0.9360?g .The aver-age recovery for trigonelline was 99.6%and RSD was 1.76%(n=6).Conclusion The method is simple and effec-tive and can be used to determine the c ontent of trigonelline in Radix Mirabilis.
3.Agrobacterium-mediated transformation of LJAMP2 gene into 'Red Sun' kiwifruit and its molecular identification.

Bacterial canker caused by Pseudomonas syringae pv. Actinidiae is one of the most important diseases of kiwifruit (Actinidia chinensis) and leads to considerable yield losses. In order to obtain transgenic plants with resistance for 'Red Sun' kiwifruit to canker disease, a non-specific lipid transfer protein-like antimicrobial protein gene (LJAMP2) from motherwort (Leonurus japonicus) was introduced into 'Red Sun' kiwifruit through Agrobacterium-mediated transformation. After two days of co-cultivation with A. tumefaciens strain LBA4404 harboring 35S:LJAMP2, the transformed explants were transferred to the selection medium containing 25 mg/L kanamycin+3.0 mg/L BA+1.0 mg/L NAA. The regeneration efficiency of kanamycin-resistant shoots reached to 85%. All (100%) of kanamycin-resistant shoots rooted on half-strength MS medium supplemented with 0.8 mg/L IBA and a total of 40 regenerated plantlets were obtained. PCR and histochemical GUS activity analysis show that 23 of 40 lines (57.50%) were positive, suggesting that the LJAMP2 gene was integrated into the genome of 'Red Sun' kiwifruit. Taken together, we established an efficient genetic transformation method for 'Red Sun' kiwifruit using A. tumefaciens and the transformation frequency reached 5.11%. This protocol will be useful for the genetic breeding of 'Red Sun' kiwifruit for improvement of disease resistance.

4.Application of total body irradiation and real-time in vivo dosimetry with semiconductor dosimeter in hematogenous stem cell transplantation

Objective To investigate the safety and clinical outcome of total body irradiation (TBI) and the real-time in vivo dosimetry with semiconductor dosimeter in hematogenous stem cell transplantation (HSCT). Methods Fifty-seven patients requiring HSCT were treated with TBI. The TBI was given with the semi-sitting or standing position or lateralcumbent posture, using 6 MV X-ray beams and opposed parallel fields technique (two or four fields, AP/PA fields) in a single fraction or multiple fractions. The real-time in vivo dosimetry was performed with six diodes positioned on the surface of patients to adjust the dose homo-geneity of the midplane using the different thickness lead sheets. Results Mild to moderate nausea, vomi-ting and swollen parotid occurred in 41 patients after TBI, which were relieved after allopathy therapy. No radiation-induced interstitial pneumonia was observed. All patients fulfilled the HSCT. The homogeneity of relative dose (normalize to umbilicus dose) in the different positions accorded with the requirement of the prescription dose. Conclusions The opposed parallel radiation, with the semi-sitting or standing positions or lateralcumbent posture, combined with the real-time in vivo dosimetry with semiconductor dosimeter is an effective and safe technique for TBI.
5.Role of preoperative neutrophil to lymphocyte ratio, platelet to lymphocyte ratio, prognostic nutritional index in the prognosis of patients with hepatitis B virus-related hepatocellular carcinoma after radical resection

Objective To investigate the role of preoperative peripheral blood neutrophil to lymphocyte ratio (NLR),platelet to lymphocyte ratio (PLR),prognostic nutritional index (PNI) in the prognosis of patients with hepatitis B virus (HBV)-related hepatocellular carcinoma (HCC) after radical operation.Methods This is a retrospective study,involving 426 surgically resected hepatitis B related hepatocellular carcinoma cases in a single center from 2003 to 2012.Results Kaplan-Meier analysis showed patients in NLR ≤ 1.62 group achieve higher rate of recurrence-free and overall survival than that in the NLR > 1.62 group,the difference was statistically significant (P < 0.005);Also PNI > 49.42 group showed higher rate of overall survival significantly than PNI≤49.42 group (P < 0.005).The results of Cox regression multivariate analysis further suggested that both NLR > 1.62 (HR 1.74,P =0.007) and PNI ≤49.42 (HR 0.70,P =0.021) were independent risk factors for overall survival,NLR > 1.62 (HR 1.45,P =0.03) was also an independent risk factor for recurrence-free survival.Conclusion The preoperative NLR and PNI may be independent risk factors for prognosis of patients with HBV-related HCC after radical operation.
6.Effect of methods of invigorating spleen and kidney on the pregnancy outcome of artificial insemination in infertile men with asthenospermia

Objective To explore the effect of invigorating splcen and kidney methods on the pregnancy outcome of the intrauterine insemination (IUI) in infertile men with asthenospermia.Methods a total of 78 male infertility patients with spleen and kidney deficiency type were collected,all cases were randomly allocated into three groups,group A using methods of invigorating spleen and kidney,group B using Antioxidant therapy,and group C as the blank group.There are 26 cases in each group.One course of treatment lasts for 12 weeks.Before and after treatment,the changes of sperm quality index were analyzed by computer assisted sperm analysis (CASA) and sperm morphology.The pregnaucy outcome after IUI was followed up.Results After the treatment,sperm motility and sperm forward movement (PR) difference before and after treatment in group A with was statistically significant (P < 0.05).The differences between patients with semen volume,sperm concentration,sperm morphology and sperm motility parameters before and after treatment in three groups were not statistically significant (P > 0.05).Clinical pregnancy rate and cumulative pregnancy rate and the live birth rate in group A were higher than that of group B and group C,but the difference was not statistically significant (P > 0.05).Conclusion Invigorating spleen and kidney method can improve the sperm motility and forward movement speed of the type of patients for the treatment of male infertility patients with asthenospermia,and it is helpful to improve the pregnancy outcome of the IUI.
7.The efficacy of standardized allergen subcutaneous specific immunotherapy in children with monosensitized versus polysensitized allergic asthma

Objective :
To investigate the efficacy of standardized allergen subcutaneous immunotherapy (SCIT) in
children with asthma sensitized to single dust mite allergens versus multiple allergens and to assess the safety of SCIT.
Methods :
62 children with confirmed allergic asthma who received standardized allergen SCIT were retro⁃
spectively analyzed and divided into the monosensitized group (dust mite results≥ + + + ) and the polysensitized group (dust mite results ≥ + + + combined with other positive allergens) according to the results of skin pricktest , we observed the changes of pulmonary function , medication score and visual analog scale (VAS) scores , children asthma control test (C - ACT) scores , asthma control questionnaire (ACQ) scores before and after treatment in both groupsand compared the efficacy of the two groups. The incidence of local and systemic adverse effects was recorded during treatment in all children to assess the safety of SCIT.
Results :
Standardized allergen SCIT treatmentimproved lung function parameters , medication scores and VAS scores , C ⁃ACT scores , ACQ scores in both the monosensitized and polysensitized groups , with statistically significant differences before and after treatment (P < 0. 05) . In comparison between the two groups , lung function parameters [forced expiratory flow at 50% vital capacity(FEF50% ) , maximum midexpiratory flow(MMEF)] , medication scores , C ⁃ACT scores and ACQ scores
improved significantly in the monosensitized group compared with the polysensitized group after treatment ( P <0. 001) . 62 patients received a total of 2 606 injections during the treatment of SCIT , 6 children had a total of 10 local adverse reactions and 3 children had 3 mild to moderate systemic adverse reactions , with an incidence of 0. 38% for local adverse reactions and 0. 12% for systemic adverse reactions.
Conclusion
The children with asthma in both the monosensitized group and polysensitized group achieved significant and safe clinical outcomes under standardized allergen SCIT. The children in the monosensitized group had more obvious clinical effects than the polysensitized group under standardized allergen SCIT.
