1.Determination of Urine Level of Levofloxacin by Second-order Derivative Spectrometry

OBJECTIVE: To determine the urine level of levofloxacin by second-order derivative spectrophotometry.METHODS: The concentration of levofloxacin in human urine was determined at a wavelength of 350~ 360nm using a second-order derivative spectrum peak valley amplitude(P).RESULTS: The standard curve was linear in the concentration range of 2~ 18? g? mL-1(r=0.999 6).The average recovery was 100.2%(RSD=0.66%).The intra-day and inter-day RSD were 0.09% and 1.82%,respectively.CONCLUSION: The method is simple,stable,precise and reproducible,and in which blank urine sampling,the extraction and isolation were not needed,thus it can be used for the monitoring of urine drug level.
2.Correlated study between chronic obstructive pulmonary disease classification and serum albumin level

Objective To explore serum albumin level of the patients with chronic obstructive pulmonary disease(COPD) and disease severity grading of the relationship between the early,better,a reasonable nutritional support in patients with COPD.Methods 120 COPD patients were randomly divided into four levels according to the disease severity,Ⅰ level as the control group,Ⅱ,Ⅲ,Ⅳ grade for the experimental group,collectting fasting serum albumin,micro-nutrition assessment form ( MNA),body mass index ( BMI ),serum albumin,three nutrition indicators were assessed in patients with COPD,the control group data were compared with the three experimental groups.Results Ⅰ level of COPD were well-nourished patients,mild malnutrition and hypoalbuminemia were 34,6,1 respectively.Ⅱ level were 8,9,2,Ⅲ level were 18,12,4,Ⅳ level were 7,13,6.Malnutrition rates were 17.1%,57.9%,47.1% and 73.1%,COPD associated with serum albumin level index was 0.251 (P < 0.05 ) ;COPD patients with grade Ⅰ and Ⅱ,Ⅲ,Ⅳ level of serum white protein-related indices were 0.273,0.331,0.324 ( all P < 0.05 ).Conclusion COPD severity grading and the decline in serum albumin were correlated,the serum albumin level decreased more significantly.
3.Biofilm and Genes in Staphylococcus

OBJECTIVE To determine the association between phenotype and genotype of biofilm in Staphylococcus.METHODS All 156 clinical isolates of coagulase negative Staphylococcus(CNS) and 90 S.aureus(SAU) strains isolating from clinical specimens from Mar 2005 to Jul 2008,as well as 12 CNS strains from nasal swabs of healthy carriers from hospital staff,were collected.Congo red technique was performed to examine slime production.The presence of the intercellular adhesion genes(icaA and icaD) was determined by the polymerase chain reaction method.The phenotype and genotype of biofilm were analyzed.RESULTS Thirty-two out of 168 CNS isolates were biofilm producers(19.0%) while 32 out of 90 SAU isolates were producers(35.6%).All strains positive for icaA positive were also positive for icaD.The 16 strains(10 CNS isolates and 6 SAU isolates) were icaA and icaD positive but do not produced slime.CONCLUSIONS For SAU,the percentage of slime-production strains(35.6%) is higher than that found in CNS(19.0%).The characteristic of biofilm-negative variants having the ica loci might contribute to the dissemination.For clinical isolates of S.epidermidis,ica locus may not be a significant marker discriminating between invasive and contaminating isolates.
4.Urodynamic analysis for the causes leading to dissatisfying result after operation therapy for BPH

Objective To discuss the causes leading to dissatisfying result after operation therapy for BPH.Methods 42 patients who had not acquired satisfying result after operation therapy for BPH were underwent the urodynamic examination.Results 42 patients were classified into two groups according to their different symptoms:difficuhy to void,32 case8,including 8 cases with low contractifity of detrusor,and 24 cases with Madder outflow obstruction(BOO);incontinence,10 eases,including 6 cases of urgency incontinence and 4 cases of stress incontinence.Condusion We can see that the causes leading to dissatisfying resuh after operation for BPH are mainly the dysfunction of detmsor and BOO,and the result of urodynamic examine can guide us the causes and the next therapy.
5.Therapeutic Effect of "SHU JI FANG" in Treating Lumbar discogenic Low Back Pain

Objective To explore the curative effect of the traditional Chinese medicine"SHU JI FANG"in treating lumbar discogenic low back Pain(DLBP).Methods 156 cases were divided into three groups by random with 52 cases in each group.A treatment group was treated by traditional Chinese medicine"SHU JI FANG".A control group 1 was treated by Bextra;A control group 2 was treated bv Chinese traditional medicine Zuogui pill.Results The difiererlce of cure rate and the excellent rate between the treatment group and the control group I did not show significance by X2 check,with P>0.05;while the difference between the treatment Group and the Control group 2 showed significance by X2 check with P<0.05.Conclusion I"SHU JI FANG"is effective in treating discogenic low back pain.
6.A study on function of knee and quality of life in patients with knee osteoarthritis

Objective: To investigate the effect of function of knee on quality of life (QOL) in patients with knee osteoarthritis (OA). Method: Forty patients with knee OA were recruited. Their function, pain and range of motion (ROM) of knee were assessed with Lequesne index, Lysholm knee score (LKSS), visual analogue scale (VAS) and protractor measure respectively. QOL was evaluated with arthritis impact measurement scale (AIMS). Result: The scores of AIMS, Lequesne and VAS in female were significantly higher than that in male. And the LKSS in female was significantly lower than that in male (P<0.05). There was a positive correlation between AIMS score and Lequesne index, VAS score and limit of ROM, but a negative correlation between AIMS and LKSS (P<0.05). Conclusion: The function of knee and QOL in female patients with OA were worse than that in male patients. Pain, limited ROM and weaken function in knee could effect QOL in OA patients to some extent. Health education and clinical treatment should be provided to improve the function of knee and QOL in OA patients.

8.Effect of couple-centered mental intervention on marital quality and subjective happiness in young-and middle-aged breast cancer patients undergoing modified radical mastectomy

Objective To study the influence of couple-centered mental intervention on marital quality and subjective happiness among young-and middle-aged breast cancer patients undergoing modified radical mastectomy.Methods A total of 100 postoperative young-and middle-aged breast cancer patients were divided into observation group (n=50) and control group (n=50) using random digit number table according to the order of hospitalization.The patients in the observation group were given the couple-based mental intervention,and the control were given the routine mental intervention only.The marital quality and subjective happiness between the 2 groups were evaluated and compared by Olson marital quality questionnaire and Memorial University of Newfoundland scale of happiness (MUNSH) respectively before intervention and 6 months after intervention.Result After intervention,both the score of marital quality and subjective happiness in the observation group were significantly higher than those in the control group (P< 0.05).Conclusion The enforcement of couple-centered mental intervention among the young-and middle-aged breast cancer patients undergoing modified radical mastectomy can significantly improve the marital quality and subjective happiness.
9.Evaluation of distraction apparatus for correcting flexion contractures deformities of the knee joint

[Objective]To study the safety,clinical effect and therapeutic characteristics of distraction apparatus for correcting flexion contracture deformities of the knee joint by evaluating the clinical outcomes and investigating the results of biomechanic test.[Method]Forty-nine patients (included 52 knees) were treated by the standardized distraction apparatus and the comprehensive assessment standard was adopted to evaluate the clinical outcomes.Five patients were examined by three-dimensional CT of the knee joints pre-operatively and post-operatively to observe the changes in the knee structure.To observe the effect of distraction on the popliteal artery and the limb nerves,the popliteal artery diameter and blood flow speed were examined in 15 patients and the regio cruris electromyogram and nerve conduction velocity were examined in 6 patients before and after treatment.Three different means of distraction and measurement were underwent in 3 patients so as to observe the changes in the flexion curve and force of posterior soft tissues of the knee.[Result]All patients (52 knees) regained congruous structure and better function,the final excellent and good result rate was 65%.Three-dimensional CT examination of 5 patients showed that the postoperative knee joint structures were superior to the preoperative ones and there weren't damage of articular surface.The statistic results indicated that the popliteal arteries weren't obviously disturbed by distraction,whereas the limb nerves were probably disturbed temporarily.Three methods of distraction and measurement in 3 patients showed different outcomes,indicating that the effect of distraction on soft tissues was influenced by the distraction method,the ages of patients and the extents of deformities of the limb.[Conclusion]The distraction speed of therapeutic method is safe,reasonable and effective in correcting flexion contracture of the knee joint;The distraction force on posterior soft tissues of the knee is influenced by various elements,thus,the distraction speed should be individualized.Low-load and steady stretch are advantageous to soft tissue regeneration and safer.
10.Antithrombosis through activating endothelial target for acetylcholine and its molecular mechanism

AIM To examine antithrombotic effects of arecoline on the arterial thrombosis induced by carrageenin in mice through modulating the functions of endothelium and determine its mechanisms from hemostatic system, the platelet aggregative functions and the bioactive factors released by vascular endothelial cells. METHODS Kappa carrageenin was given ip in mice and mice were fed at the temperature of 20 to 21 degrees and at the humidity of 30 percent to 50 percent. RESULTS On the foregoing models of thrombosis, arecoline could antagonize the formation of thrombosis through activating the endothelial target for acetylcholine in a dose dependent manner and its antithrombotic potency was 250 to 500 times greater than aspirin; while under the same conditions, pilocarpine could not antagonize the formation of thrombosis. The levels of TT, PT, KPTT and MAR had no prominent changes compaired with control groups. The levels of t-PA became higher greatly than normal and the levels of PAI 1 became lower greatly than normal 2 hours after intravenous injection of arecoline in rats. Arecoline could decrease the higher plasma levels of thromboxane A2 and increase the lower plasma levels of prostacyclin in a dose dependent manner in the mice tail thrombosis induced by carrageenin. CONCLUSION The antithrombotic effects of arecoline are associated with activating the endothelial target for acetylcholine closely, but are not associated with muscarinic receptors,and not relevant to hemostatic systems or functions of platelet aggregation directly.
