1.Expressions of light chain immunoglobulins kappa and lambda in multiple primary malignant tumors in senile patients

Objective To investigate the abnormal expressions of light chain immunoglobulins kappa (Ig?) and lambda ( Ig?) in multiple primary malignant tumors (MPMT) in elderly patients. Methods The expressions of Ig? and Ig? in specimens obtained from multiple malignant tumors in 21 elderly patients were investigated. Immunohistochemical staining was performed with mouse antihuman Ig? and Ig? monoclonal antibodies. Among 21 patients with multiple primary malignant tumors, the first tumor which occurred in the digestive tract was detected in 20 patients. In 17 patients first and second tumors were found to occur in the digestive tract. In 21 patients with MPMT the relationship between the expressions of light chain immunogloblin and survival time was analyzed. Results Positive staining of Ig? and/or Ig? in specimens from the first and/or second tumor tissues was found in 16 patients(76 2%). 2.Preparation of gastric adenocarcinoma related cDNA microarray

Objectives To prepare the gastric adenocarcinoma related cDNA microarray and to screen differentially expressed genes between human gastric adenocarcinoma and normal gastric mucosa. Methods A set of cDNA microarray slides containing a duplicate set of 576 cDNA spots. These selected clones have been verified in sequence. The PCR products of human genes were spotted onto a chemical-material-coated glass plates in array. DNAs were fixed on the glass plate after series of treatment. Both RNAs from the gastric adenocarcinoma and normal gastric mucosa were reversely transcribed to the cDNAs with the incorporation of fluorescent dUTP to prepare the hybridization probes. The mixed probes were hybridized to the cDNA microarray. After high-stringent washing, the cDNA microarray was scanned for fluorescent signals to display differences between two tissues. Results Gastric adenocarcinoma related cDNA microarray slides contained 111 differentially expressed genes of gastric adenocarcinoma, 37 known genes related to gastric carcinogenesis (such as p21ras, p53, etc.) and control genes. The results of 10 couples of hybridizations to the gastric adenocarcinoma related cDNA microarray slides with Cy3-labeled cDNA probe from normal gastric tissue and Cy5-labeled cDNA probe from gastric adenocarcinoma showed analyzed spots uniformity background and clear signal. Among the 576 target genes, the expression level of 48 genes differed between gastric adenocarcinoma and normal mucosa in 5 or more samples (eight genes differed in all samples). There were 39 down-regulated and 9 up-regulated genes in gastric adenocarcinoma. Conclusion Gastric adenocarcinoma related cDNA microarray slides were successfully developed. The results of the experiments showed that gastric adenocarcinoma related cDNA microarray slides are reliable, sensitive and reproducible.

4.Induction of tolerance to knee allografts in rabbits receiving FK506 and antilymphocyte serum

Objective To assessed the ability of FK506 and antilymphocyte serum(ALS) to induce mixed chimerism and tolerance for knee composite tissue allograft in rabbits without chronic immunosuppression. Methods Male Flap-eared rabbits were used as donors,and female New Zealand white rabbits were used as recipients. Vascularized heterotopic knee allotransplantation was performed. Treatment consisted of ALS(1 ml/kg) only,FK506(0.5 mg/kg) only and a combination of FK506 and ALS which were administered 24h before transplantation. ALS was administered intraperitoneally every day in the first three days. FK506 was administered gastric intubation every day for fifteen days. Survival times of knee allografts were observed. Donor specific hematopoietic chimerism and histology sections were tested. Results Survial time of knee allografts with ALS single was(17.4?1.8)d,and(23.8?1.5)d with FK506 single,(40.4?2.9)d with FK506 and ALS,contrasting with(13.6?0.8)d of control. Chimerism rate show a stabilization of 3 weeks above 10% after protocol discontinue in groups with FK506 and ALS. Conclusions Short-term administration of FK506 and ALS in this ways prolong survival time of vascularized knee allograft in rabbits and could maintain mixed chimerism and tolerance for 3 weeks.
5.Preventing strategies of bladder cancer recurrence after surgery for upper tract urothelial carcinoma

Objective To investigate the methods for preventing bladder cancer recurrence after surgical treatment for upper tract urothelial carcinoma.Methods Of the 156 patients who underwent total nephroureterectomy for clinically localized transitional cell carcinoma of the upper urinary trace,139 were followed up and their data were retrospectively analyzed.Of the 139 cases,78 had pyelic carcinoma and 61 had ureteral carcinoma.The tumor cell grading showed G_1 in 19 cases,G_2 in 88 and G_3 in 32.The tumor staging showed T_a-T_1 in 38 cases,T_2 in 80 and T_3-T_4 in 21.The tumor diameter ranged from 0.8 to 6.0 cm.Results Radical nephroureterectomy and resection of partial bladder wall around the ureteral orifice(1.5-2.0 cm) were performed in all the 139 patients;of whom 55(39.6%) experienced subsequent bladder cancer recurrence during a mean follow-up of 52 months(range,1-10 years).The bladder cancer recurrence rate was 18.5%(5/27) in patients whose ureter had been ligated before the kidney was dissociated;and the recurrence rate was 27.5%(14/51) in patients whose ureter had not been firstly ligated.The recurrence rates in those who had prophylactic intravesical instillation immediately after surgery and in those who had the instillation 3 weeks after surgery were 32.3%(10/31) and 34.9%(30/86),respectively.The recurrence rate was 20.0%(4/20) in those who had intravesical instillation on the surgical day and 3 weeks after surgery sequentially,which was significantly lower than that(39.3%,26/66) in those who had the instillation simply 3 weeks after surgery(P

Objective To investigate the effect ot ionizing radiation (IR) on the expressions of HMGB1 in the radiation-sensitive and radiation-resistant human cervical cancer cells and to analysis the role of HMGB1 in the regulation of radiosensitivity.Methods Human cervical cancer cells HeLa and its radioresistant strain HeLaR cells were irradiated with different doses of X-rays.The cells were collected at different time points after irradiation.The expressions of protein and mRNA of HMGB1 were detected by Western blot and real-time quantitative PCR.Results At the protein level,the expression of HMGB1 in HeLaR cells was significantly reduced at 6-36 h after 2,5 and 10 Gy X-ray irradiation (t =3.574-9.754,P <0.05),and then it was recovered to the control level at 48 h after IR.On the contrary,the expression of HMGB1 in HeLa cells was significantly increased at 6,12,48 h after 2 Gy IR (t =3.945-4.864,P<0.05),at 6,36,48hafter5 GyIR (t=-2.875-3.295,P<0.05),and at 36,48 h after 10 Gy IR (t =-4.480,-4.517,P < 0.05).At mRNA level,the trend of HMGB1 expression alteration was consistent with that of protein expression.Conclusions The changes of HMGB1 expression can be differently induced by X-rays in the human cervical cancer radiation-sensitivity cells and radiation-resistant cells.HMGB1 may be involved in the radioresistance of human cervical cancer.
7.The clinical analysis of 142 cases of minimally invasive transthoracic device closure of ventricular septal defect

Objective To summarize the minimally invasive transthoracic ventricular septal defect (VSD) occlusion operation experience in the clinical application. Methods 142 VSD patients (admitted from August, 2013 to October, 2014) were chosen. All patients were treated with minimally invasive transthoracic device closure under the monitoring of transoesophageal echocardiogram. Results 136 cases received successful closure, and 6 cases still need CPB conventional extracorporeal circulation operation. After follow-up for 3 ~ 12 months for 102 patients, 16 cases remain residual shunt. Conclusion The short-term therapeutic effect of minimally invasive transthoracic device closure of VSD is satisfying. However , the long-term follow-up remains to be studied.
8.Urinary telomerase activity in the diagnosis and recurrence screening of bladder carcinoma

Objective To investigate the significance of urinary telomerase activity (TA) in the diagnosis and recurrence screening of bladder carcinoma. Methods TA in urine of 46 cases of superficial bladder carcinoma and 46 normal subjects was estimated by the method of PCR ELISA, and it was monitored continuously in 20 cases after transurethral resection of tumors. Results TA in urine of patients with bladder carcinoma increased significantly as compared with the controls.It dropped to normal range after tumor removal, but rose again with tumor recurrence.The specificity and sensitivity of TA detection were 93.5% and 47.8% respectively. TA was positively related to the grade of tumors whereas there was no association between the preoperative value of TA and early recurrence. Conclusions The examination of TA in urine is helpful for the diagnosis of bladder carcinoma and it is related to the differentiation degree of tumors.It may become an important supplement to cystoscopy in the screening of tumor recurrence.
9.Evaluation for application of remote electrocardiogram recorder in community residents

Objective: To evaluate the value of clinical application of Heart of the City type TJH-03A remote electrocardiogram recorder (remote ECG). Methods: A total of 1 014 community residents from Wuhan received ECG examination by Heart of the City type TJH-03A remote ECG recorder. Clinical and ECG data of all subjects were collected and analyzed. Results: There were 359 (35.40%) cases with normal ECG and 655 (64.60%) cases with abnormal ECG, after remote ECG examination . There were 581 cases had a variety of arrhythmia in 655 cases with abnormal ECG, so there were 828 cases times with abnormal ECG, in which there were (1)320 cases times (38.65%) with repolarization abnormality,(2)98 cases times(11.84%) with sinus bradycardia, (3)98 cases times (11.84%)with myocardial ischemia,(4)78 cases times (9.42%)with conduction block,(5)56 cases times(6.76%) with ventricular hypertrophy and high voltage,(6)43 cases times(5.19%) with atrial premature beats(7)37 cases times (4.47%)with atrial fibrillation,(8)35 cases times (4.23%)with sinus tachycardia,(9)24 cases times (2.90%)with ventricular premature beats, (10)19 cases times (2.29%)with suspected myocardial infarction according to ECG abnormality rate order. In addition, there were 5 cases times with QT interval prolongation, and abnormal ECG with abnormality rate ≤3 cases times: atrial tachycardia, atrial flutter, supraventricular tachycardia, ventricular tachycardia, Brugada wave, dextrocardia. Conclusion: Heart of the City type TJH-03A remote electrocardiogram recorder is convenient to use, it can perform remote electrocardiogram record, and find a variety of arrhythmias. It’s important for ECG diagnosis, especially for diagnosis of arrhythmias.
10.Analysis of risk factors of death of critical patients treated in emergency department

Objective To study the mortality and risk factors of death of critical patients treated in emergency department for initial stabilization and life support. Method The clinical data of 1240 critical patients from January 2005 to December 2006 were retrospectively analyzed. The patients were divided into death group and survival group. The differences of demographics, symptoms, physical signs and laboratory findings of patients between two groups were analyzed by using univariate and multivariate logistic regression analysis, sex, age, visiting time after attack, the history of chronic diseases, temperature, respiratory rate, heart rate, mean arterial pressure, respiratory dysfunction, circulatory dysfunction, hepatic dysfunction, gastrointestinal dysfunction, renal dysfunction, coagulation disorders, acid base and electrolyte disturbances, lencocyte count,platelet count, Glasgow coma scale (GCS) score and acute physiology and chronic health evaluation Ⅱ (APACHE Ⅱ ). Results There were higher mortality and morbidities of patients with diseases of respiratory, digestive, circulatory and nervous systems. The mortality of patients with the history of chronic diseases was higher (P < 0.01) ,and there were more patients with chronic obstructive pulmonary disease(COPD), chronic cardiac insufficiency, diabetes mellitus or cirrhosis of liver in death group (P < 0.05). The mortality of patients with 3 dysfunctional organs was 32.81%, and the mortality of lity of those with five dysfunctional organs was 76.67% . Logistic regression analysis indicated that male gender, age between 46 and 65, respiratory dysfunction, circulatory dysfunction, gastrointestinal dysfunction, hepatic dysfunction, low Glasgow coma scale (GCS) score and high APACHE II score were risk factors of the death of critical patients. Conclusions The mortality of patients with the history of critical diseases is higher. The more dysfunctional organs, the higher mortality is. Age between 46 and 65, male gender, and dysfunction of lung, circulation, gastrointestinal tract,and liver,and low CCS score and high APACHE II score are risk factors of the death of emergency and critical disease.
