1.CT features of solid-pseudopapfllary tumors of pancreas: comparison with clinical and pathologi-cal findings

Objective To evaluate the features of solid-pseudopapiHary tumors of pancreas (SPTP),and to compare with the clinical and pathological findings in order to improve the diagnostic accura-cy.Methods The CT appearances were analyzed retrospectively in 5 patients with pathologically proven SPTP.There were 4 women and 1 man,with the mean age of(16.20±7.49)years.Plain CT and contrasted CT were performed in all 5 cases.Results The tumors of 4 caseswerelocatedinthe head and neck of pan-creats and 1 case was in the body and tail of pancreas.Clear tumor margins were seen in all cases.The mean diameter of these tumors was 5.2 cm(3.0-9.0 cm).Two cases were predominantly composed of cystic por-tions,2 cases were mainly composed of solid portions,and 1 case was composed of similar proportions of solid and cystic portions.Inhomogeneous enhancement Was rewealed in contrasted CT scan,but Was lower in both phase than normal pancreas.Tumor border discontinuous ring-shape calcification was seen in 1 case,dilata-tion of the main pancreatic duct Was demonstrated in 1 case and no metastasis were seen.Conclusions Certain CT features may suggest the diagnosis of SPTP.CT combined with the clinical feature is helpful to the diagnosis.
2.Analysis of the Antidiabetic Agents Used in60Hospitals of Southwest China During the Period 2000~2002

OBJECTIVE:To survey and evaluate the present situation and trends of antidiabetic agents used in hospitals of southwest China so as to provide information for the production,sale and consumption units.METHODS:To investigate the antidiabetic agents used in60hospitals of southwest China during the period2000~2002in respect to the sum of money of consumption,Kinds of drugs,DDDs and DDC.RESULTS:In recent3years,the annual increasing rates of consumption of oral antidiabetic agents and insulin injection were14.12%and42.09%,the proportions of costs for the oral antidiabetic agents and insulin injections in total consumption of antidiabetic agents were67.20%and32.78%respectively.CONCLUSION:The use of insulin is increasing quickly and the new dosage has developmental future.The Metformin and Glipizide are the important oral antidiabetic agents.

4.Report on 220 cases of endoscopic septoplasty with submucous resection

Objective To explore the efficiency of endoscopic septoplasty with submucous resection for the treatment of 220 patients with deviated nasal septum and report our experience in this field. Methods From May 2006 to May 2010, 220 patients with deviated nasal septum were treated by endoscopic septoplasty with submucous resection to decompress the stress points, resect the deviated parts and implant the reshaped bone of nasal septum, while retaining the normal structure of nasal septum with minimally invasive technique. Results Satisfactory effects were achieved in all the 220 patients, showing centered septal structure and disappeared symptoms such as nasal obstruction, headache and nasal hemorrhage, but no complications including perforation of nasal septum, depressed nasal bridge and septal chalasia and flaring. Conclusion Rhino-endoscopic septoplasty with submucous resection can substantially retain the original structure of nasal septum and thus is proven to be a desirable operation with multiple advantages including minimal invasiveness, quicker healing and fewer complications.
5.Determination of human tear proteins using orbitrap mass spectrometry

Objective:
To determine human tear proteins using nanoliter liquid chromatography coupled to quadrupole orbitrap mass spectrometry (NanoLC-Q-Orbitrap-MS), and perform a bioinformatics analysis of main proteins.
Methods:
Human tear samples were collected with capillary, transferred to 3 kDa ultrafiltration tubes containing 400 μL of superpure water and centrifuged at 12 000×g for 15 min. Repeated extraction of tear proteins were performed four times, and following digestion with trypsin, the proteins were separated using the Waters NanoAcquity peptide BEH C18 column (1.7 μm, 100 μm×100 mm) and determined using NanoLC-Q-Orbitrap-MS with the mobile phase of 0.1% formic acid aqueous solution and acetonitrile (0.1% formic acid) in the full MS/dd-MS2 mode. The types of proteins were characterized in the Uniprot database using the software Proteome Discoverer version 2.1 and verified using bovine serum albumin. The tear proteins were subjected to gene annotation analysis using the String database.
Results:
A total of (387±160) human tear proteins were yielded, with a relative standard deviation (RSD) of 4.13%, and there were 25 types of proteins with a relative high abundance, including lipocalin 1, lysozyme and lactoferrin. The peptide sequence coverage of bovine serum albumin was (86.08±2.61)%, with a RSD of 3.03%. The 25 major tear proteins were involved in substance transduction among cells, homeostasis process, negative regulation of the endopeptidase activity, detection of chemical stimulants and humoral immune responses, and the 16 proteins had close interactions. Lacritin, lipocalin 1, lactoferrin, lysozyme and zinc-α 2-glycoprotein, which had a relative high abundance, had close biological connections.
Conclusion
NanoLC-Q-Orbitrap-MS is stable, reliable and feasible for detection of multiple proteins in tears.
6.Protective effects of standard dose prednisone combined with glutamine on Graves ophthalmopathy after 131I treatment

Objective To investigate the protective effect of standard dose prednisone(starting dose about 0.4 mg/kg bw) combined with glutamine(2 g/d) for Graves' ophthalmopathy(GO) after 131I treatment.Methods The prospective randomized controlled trial was performed and included 116 consecutive patients with mild-to-moderate GO or no active GO after 131I treatment,but with high risk factors.The experimental group(59 cases) started to take oral prednisone(0.44 ± 0.13)mg/kg at 2 d atter theatment,meanwhile replenishing glutamine,and prednisone was gradually decreased by 5mg per 2 weeks and stopped until 2 months;the control group(57 cases) received the initial dose prednisone(0.43± 0.14)mg/kg without replenishing glutamine,and the rest was same as the experimental group.The GO change and prednisone adverse reactions in 2,4,6 months after treatment were evalua ted in the two groups.Results The baseline characteristics had no significant difference between the two groups.In comparison of the experimental group and control group after 6-month treatment,the CAS score was (2.8 ± 0.8 vs.3.5 ± 0.9),exophthalmos degree was (19.6±8.1)mm vs.(21.7±3.0)mm,eyelid width was (11.3±2.9)mm vs.(13.8±3.1)mm,the improvement degree in the experimental group except for degree was superior to the control group (P<0.01).No new onset GO or deterioation oc curred in the two groups;the experimental group had 42 cases(71.2 %) of GO improvement and 17 cases(28.8 %) of stability,while the control group had 39 cases(68.4%) of GO improvement and 18 cases(31.6%) of stability,and the curative effects had no sttis-tical difference between the two groups(P>0.05).The side effects in the control group were more than those in the experimental group(63.1% vs.30.5%,P<0.05).the body mass increase was more obvious [(3.8±1.8) kg vs.(1.4±1.2)kg,P<0.01],and SBP and DBP in the two groups were slightly increased[(10±4)mm Hg vs.(9±5)mm Hg P>0.05].Conclusion Using the initial dose of oral prednisone(about 0.4 mg/kg) for 2 months and simultaneously replenishing glutamine (2 g/d)can effectively improve mild-moderate GO,and effectively reduce the adverse reactions of GC.However it is needed to extend the follow-up time to assess whether it can truly prevent the deterioration of GO,and to conduct a further study for the role of glutamine.
7.Effect of guizhi plus gegen decoction on ultrastructural changes of intervertebral disc annulus fibrosus cells.

OBJECTIVETo observe the effect of Guizhi plus Gegen Decoction (GGD) on ultrastructural changes of intervertebral disc annulus fibrosus cells.
METHODSRats' intervertebral disc annulus fibrosus cells were isolated and cultured using adherence wall screening method. After annulus fibrosus cells were intervened by GGD, the microstructure and ultrastructural features of untreated annulus fibrosus cells and annulus fibrosus cells treated by GGD containing serum at different concentrations were observed under optical microscope and electron microscope.
RESULTSUnder optical microscope, most annulus fibrosus cells showed irregular polygons and few in star shape with rich superficial ecphyma. The nuclei were oval, large and complete. Under electron microscope, most cells in the blank group were oval after intervened by GGD containing serum at different concentrations. The nucleus was large, deviated, and irregular, the heterochromatin scattered diffusely, partial mitochondria vacuolized, and rough endoplasmic reticulum dilated. In the low dose GGD group, increased mitochondria and condensed density could be seen. The rough endoplasmic reticulum were expanded, lipid drops or glycogen could be occasionally seen. In the middle dose GGD group, increased endoplasmic reticulum expansion and condensed density could be seen. More medium density protein sediment could be seen. Increased mitochondria with condensed density could be seen, showing irregular cystic form with various sizes nucleus. In the high dose GGD group, increased rough endoplasmic reticulum with obvious expansion could be seen. More high density protein sediment could be seen. The nuclei were deviated. More mitochondria could be seen with secretory granules in them.
CONCLUSIONSAfter intervened by GGD containing serum at different concentrations, the ultrastructure of annulus fibrosus cells were manifested as follows: (1) The endoplasmic reticulum increased more in the middle and high dose GGD groups than in the blank group and the low dose GGD group. Greater density protein sediment occurred, especially in the high dose GGD group. (2) GGD played an important role in preventing ultrastructural changes induced by the degeneration of annulus fibrosus cells.

8.The bone marrow morphology characteristics and the clinical significance of six patients with severe fever with thrombocytopenia syndrome

Objective To investigate the bone marrow morphology characteristics in patients with severe fever with thrombocytopenia syndrome (SFTS)and to evaluate the clinical value of bone marrow morphology examination in the diagnosis of SFTS.Methods The bone marrow morphology and clinical data of six newly diagnosed SFTS patients with positive test of novel bunyavirus infection admitted to Taizhou Hospital from July 2011 to November 2014 were retrospectively analyzed.Results The increased ratio of lymphoid and monocytoid histiocytes (0.02 - 0.23 )and hemophagocytic phenomenon were observed in all of the six patients′ bone marrow smears.Two of the six cases were diagnosed with hemophagocytic lymphohistiocytosis (HLH)according to the clinical diagnosis criteria and the remaining four cases were defined as highly suspected HLH.Conclusions The progression of SFTS disease might be closely related with HLH induced by novel bunyavirus infection.Morphology examination of bone marrow smear is beneficial for the early differential diagnosis of SFTS.
9.Study on female sexual dysfunction in type 2 diabetic Chinese women.

OBJECTIVETo investigate the female sexual dysfunction (FSD) in type 2 diabetes patients, by comparing the sexual function between type 2 diabetic women and non-diabetic women with Female Sexual Function Index (FSFI).
METHODS115 type 2 diabetic women and 107 age-matched non-diabetes women were enrolled with similar backgrounds. Their sexual functions were evaluated with FSFI. Metabolic parameters such as body mass index, blood lipid profile, hemoglobin A1C, plasma glucose were also collected.
RESULTSTotal score of FSFI of the type 2 diabetic women were significantly lower than that of the non-diabetic controls (18.27±8.96 vs. 23.02±5.78, P=0.000). Scores of the FSFI domains (desire, arousal, lubrication, orgasm, satisfaction, pain) of the type 2 diabetic group were also lower than those of the control group. According to the FSD criterion (FSFI<25) available in China, the percentage of FSD in the type 2 diabetic group was significantly higher than that of the control group (79.2%vs. 55.0%, P<0.001). These trends seemed more prominent in pre-menopause subgroups. The logistic regression analysis indicated that age and diabetes were independent risk factors of FSD. Body Mass Index (BMI) also had influence in the diabetes group.
CONCLUSIONFindings from this study showed that there are more FDS in Chinese type 2 diabetic women than in their non-diabetic counterparts, especially in pre-menopause participants.

10.Treatment of HBeAg positive chronic hepatitis B by xiaoyao powder combined with interferon-alpha: a clinical observation.

OBJECTIVETo study the efficacy of Xiaoyao Powder (XYP) combined with interferon alpha (IFN-alpha) in treating HBeAg positive chronic hepatitis B (CHB) patients and the effect on their quality of life (QOL).
METHODSTotally 193 patients with HBeAg-positive CHB confirmed by liver biopsy were randomly assigned to 2 groups, Group A (94 cases) and Group B (99 cases). IFN-alpha1b was subcutaneously injected to patients in Group A at the dose of 50 microg, thrice per week. Those in Group B additionally took XYP. The therapeutic course for all was 24 weeks. Clinical efficacy was observed by assessing ALT restoration rate, HBeAg negative rate, HBeAg conversion rate, HBV DNA negative rate, complete response rate, partial response rate, and symptoms integral. The evaluation of QOL was performed by using chronic liver disease questionnaire (CLDQ) score. Adverse reaction occurrence rate was observed in the two groups.
RESULTSBetter effects were obtained in Group A on ALT restoration rate, HBeAg negative rate, HBV DNA negative rate, complete response rate, partial response rate, TCM symptoms integral, the total effective rate of TCM sysmptoms, CLDQ score, and adverse reaction rates, showing statistical difference when compared with Group B (P < 0.05, P < 0.01).
CONCLUSIONXYP could elevate the efficacy of TCM symptoms of HBeAg-positive CHB patients and anti-viral effect, improve their QOL, and reduce adverse reaction of IFN-alpha.
