1.Different Effects of Triptergium.reglii On T and B cell Function

The suppressive effects of T. reglii on mouse cellular and humoral immune responses were studied. Lymphocyte proliferation effect induced with ConA,and PFC and OHS reactions with TD Ag(TNP-SRBC)were effectively suppressed by the drug. But the similar effects on LPS and TI Ag (TNP-SRBC) have not been found. The results shown that target cells of T. reglii effects may be T lymphocytes and the drug may not have a direct effect on B cells. The drug also shown a suppressive effect on IL-2 production by mouse spleen cells. The phenomenon suggests that the suppressive effects of T. reglii on humoral immune response may be indirectly mediated through suppressing helper T cells.
2.The determination of immune functions of mouse experimental pyelonephritis infected with c. albicans

A pyelonephritis model of mouse is made by c. albicans infection. The determination of im-mune functions in model show that, both proliferation response of T cell to ConA and IL-2 activ-iting of spleenocytes of model animal to inductionwith ConA increased at 30 days after injected.The proliferation response of B cell to stimulation with LPS inhanced from 15 through 30 days,primary Ab response of model animal to SRBC stimulation inhanced between 15-30 days afterimmunization. DTH response to stimulation with c. albicans protein Ag raised since 15 days andup to peak at 30 days. Agglutination titer of serum raised up 15 days, and remained at higherlevel untill 30 days.
3.Evaluation of Validity of Lipoprotien(a) as a Diagnostic Test for Coronary Heart Disease and the Stegnotic Degree of Coronary Artheroscle-

Objective To evaluate the validity of lipoprotien(a) [Lp(a)] concentration as diagnostic test for coronary heart disease (CHD) and stegnotic degree of coronary artherosclerosis. Methods 416 cases of patients who were assessed with coronary angiography were divided into four groups according to the stegnotic degree of coronary artherosclerosis. There were no stenosis and lipid plaque group, single branch group, double branch group, three branch and suffusive stenosis group. TC, HDL - C, LDL - C, ApoAl, ApoB, Lp(a) were examined in early morning fasting for 12 hours after the next admission day. Other risk factors were also recorded, such as hypertension. Results When Lp(a) concentration was used alone to diagnose CHD, the best cutoff value was 15 mg/dl. The crude agreement was 58.2% , the sensitivity was 57.5% , and the specificity was 59.3% . Along with the stegnotic degree of coronary artherosclerosis heavier, the levels of the crude agreement and the sensitivity had a indreasing trend,and levels of the false negative rate had a deoreasing trend. Conclusion The validity of Lp(a) concentration as diagnostic test for CHD is not satisfactory.
4.Effects of hydroxycamptothecin on TGFb1, a-SMA and collagen I expression in rat hepatic satellite cells.

To investigate the molecular mechanism of hydroxycamptothecin (HCPT)-mediated anti-hepatic fibrosis by evaluting its effects on expression of tumor growth factor-beta 1 (TGFb1), alpha-smooth muscle actin (a-SMA) and collagen I (Col I) in hepatic satellite cells (HSCs). Cultured HSCs were treated with different concentrations of HCPT: low-dose group, 0.25 mg/L; middle-dose group, 0.5 mg/L; high-dose group, 0.75 mg/L; and control group, 0 mg/L. Cell proliferation was assessed by the MTT assay. The mRNA expressions of TGFb1, a-SMA and Col I were determined by reverse transcription-polymerase chain reaction. The protein expressions of TGFb1 and a-SMA were detected by Western blot. The content of Col I in the cultured HSCs' supernatant was measured by enzyme-linked immunosorbent assay. The MTT absorbance values of the low-dose group (0.631+/-0.074), middle-dose group (0.469+/- 0.012) and high-dose group (0.204+/- 0.001) were significantly lower than that of the control group (0.793+/-0.098; F = 82.86, P less than 0.01). Compared with the control group, the HCPT-treated groups showed significantly down-regulated gene expressions of TGFb1 (control: 0.716+/-0.064 vs. low: 0.611+/-0.040, middle: 0.510+/-0.014, high: 0.403+/-0.026), a-SMA (control: 0.696+/-0.075 vs. low: 0.579+/-0.037, middle: 0.470+/-0.024, high: 0.299+/-0.017), and Col I (control: 1.019+/-0.056 vs. low: 0.835+/-0.022, middle: 0.696+/-0.055, high: 0.322+/-0.104) (all, P less than 0.01). Meanwhile, HCPT-treated HSCs showed significantly reduced protein expressions of TGFb1 (control: 0.872+/-0.053 vs. low: 0.654+/-0.047, middle: 0.545+/-0.042, high: 0.436+/-0.039) and a-SMA (control: 0.858+/-0.050 vs. low: 0.620+/-0.045, middle: 0.525+/-0.042, high: 0.434+/-0.052) (all, P less than 0.01). The Col I levels secreted by HSCs were significantly lower in the HCPT-treated groups (low: 168.367+/-16.453 ng/ml; middle: 141.284+/-11.731 ng/ml; high: 132.910+/-10.048 ng/ml) than in the control group (188.733 +/-18.299 ng/ml) (all, P less than 0.01). The mechanism of HCPT-mediated anti-hepatic fibrosis may involve down-regulation of TGFb1 expression to inhibit HSC proliferation and activation, as well as reduction of Col I synthesis and secretion.

5.Relationship between intermediate hyperglycemia and acute cerebrovascular disease

Objective To study the relationship between intermediate hyperglycemia and acutece rebrovascular disease. Methods 365 consecutive patients with acute cerebrovascular disease were enrolled. The fasting, non-fasting plasma glucose levels and lipid levels were measured. The non-diabetes patients undergone oral glucose tolerance test (OGTT) 1 week after the onset of the disease. All were classified as having normal glucose tolerance (NGT), intermediate hyperglycemia [IFG(impaired fasting glucose), IGT(impaired glucose tolerance), and co-existence of IFG and IGT] and diabetes mellitus (DM). The rates of the accepted risk factors for atherosclerosis (AS), the severity of the stroke and AS on B scan were compared among the groups. Results The rates of IFG, IGT, and co-existence of IFG and IGT were 1.37%, 24.66%, and 3.01%, respectively. The rate of hypertension was higher in DM subjects (P= 0.007) than in NGT subjects, while the rates in NGT subjects and in DM subjects compared with that in intermediate hyperglycemia subjects were not of statistical significance (P>0.05). And there were no statistical significances in the rates of smoking, drinking and dyslipidemia. Meanwhile no statistical significances were found in the severity of the stroke and the signs of AS detected by duplex scanning (P>0.05). Conclusion Intermediate hyperglycemia is common in the patients with acute cerebrovascular disease. R is important in the pathogenesis of cerebrovascular disease when combined with the other risk factors for AS.
6.Correlation between impaired glucose tolerance and carotid atberomatous plaque in patients with acute cerebral infarction

Objective To investigate the relationship between impaired glucose tolerance(IGT)and carotid atheromatous plague in patients with acute cerebral infarction and analyze the risk factors for plaque formation.Methods The 326 patients hospitalized in our department for acute cerebral infarction were divided into diabetes mellitus(DM)group,IGT group and normal glucose tolerance (NGT)group.The clinical features,biochemical indices and results of Doppler ultrasound examination of the carotid artery were compared between the 3 groups.Results The body mass index(BMI),total cholesterol(TC),and low-density lipoprotein cholesterol(LDL-C)of IGT group were markedly higher than those in NGT group(P<0.05).Both the IGT and DM group showed significantly increased carotid intimal-medial thickness,plaque detection rate and the incidence of carotid scleratheroma in comparison with the NGT group(P<0.05).Logistic regressive analysis identified age,TC,LDL-C and 2-h postprandial serum glucose as the independent risk factors for carotid scleratheroma.Conclusion In patients with acute cerebral infarction,those having impaired glucose often show obvious carotid scleratheroma with a severity similar to that in the DM patients.
7.Study on the kinesis of cellular immunity in adults vaccinated with recombinant hepatitis B vaccine.

OBJECTIVETo evaluate the kinesis of cellular immunity in adults who were vaccinated with yeast recombinant hepatitis B(rHB) vaccine and the correlation between cellular and humoral immune responses induced by the vaccine.
METHODSEight adults were vaccinated with rHB vaccine according to 0, 1,2 month schedule. The peripheral blood mononuclear cells(PBMCs) were collected at the 3, 8, 21, 34 and 65 days after the first dose. The high purity of CD4+ and CD8+ T cells obtained by sorting from PBMCs were restimulated with recombinant hepatitis B surface antigens (rHBsAg) or peptides. The spot forming cell (SFC) of IFN-gamma, IL-2 and IL-4 of CD4+ and CD8+ T cells were detected by enzyme-linked immunospot (ELISPOT).
RESULTSThe characteristics of IFN-gamma, IL-2 and IL-4 of CD4+ and CD8+ T cells appeared different after immunization with rHB vaccine. IFN-gamma of CD8+ and CD4+ T cells could be detected early with stable SFC, while the IL-2 and IL-4 of CD4+ T cells appeared late but increased after the second and third dose of vaccination. The positive rate of IL-4 of CD4+ T cells were significantly correlated with the positive rate of anti-HBs, while the SFCs of IL-4 and IL-2 of CD4+ T cells were also significantly related to the titers of anti-FIBs.
CONCLUSIONIFN-gamma could be detected early after rHB vaccination in adults, and the positive rates of IL-4 and IL-2 were correlated with that of anti-HBs.

8.Cellular immune responses of recombinant hepatitis B(rHB)vaccine and HBsAg derived from Hansenular polymorpha cells

Objective To study the kinetics of immune response in mice and human immunized with rHB vaccine or rHBsAg derived from yeast cells(Hansenula polymorpha).Methods With different doses,the level of IFN-γ secreted by spleen mononuclear cells(MNC)including CD8+T cells by MACs of mice were detected by enzyme-linked immunospot(ELISPOT)methods after stimulation in vitro with HBsAg MHC class Ⅰ peptide S28-39,respectively.At serial time points.the immunized mice were detected for IFN-γ by ELISPOT as above and for the lymphocytotoxicity test(CTL)by specific lysis assay.The levels of IFN-γ,IL-2,IL-5 and anti-HBs in mice induced by rHB vaccine were detected after single or three doses.Four adults were vaccinated with rHB vaccine according to 0,1 and 2 month schedule.The peripheral blood mononuclear cells(PBMCs)were collected at the 3,8,21,34 and 65 days after the first dose.The CD8+T cells with high purity obtained by sorting from PBMcs were stimulated with rHBsAg or HBsAg peptides.The SFC of IFN-γ,IL-2 and IL-4 of CD4+ and CD8+ T cells were determined by ELISPOT.Results The cytokine of IFN-γ became detectable on day 7 and its peak value appeared on day 14 by ELISPOT.The CTL was detected on day 7 and the maximum lysis of CTL appeared on day 28.The cellular immune response of IFN-γ of MNCs were significantly correlated with the doses vaccinated from 1 μg to 8 μg(Υpositive rates=0.951,Ppositive rates=0.049<0.05;rSFC=0.996,PSFC=0.000<0.05).IFN-γSFC of CD8+T cells were significantly associated with the doses from 1 μg to 4 μg(Υ=0.999,P=0.025<0.05).The HBsAg specific cellular immune and humoral responses of mice immunized with three doses were significantly higher than that with a single dose(P<0.05).The characteristics of IFN-γ,IL-2 and IL-4 of CD4+ and CD8+ T cells were variable between individuals immunized with the same rHB vaccine.The level of IL-2 and IL-4 of responders were significantly related to the titer of anti-HBs.Conclusion Data from this study showed the kinesis of cellular immunity in mice and adults vaccinated with rHBsAg or rHB vaccine respectively.and the characteristics of cellular immune response in adults induced by the vaccine.Our data provided the basis of standardizing the analysis of cellular immune response to rHB vaccine.
9.Survival analysis on liver failure patients treated with an artificial liver support system.

OBJECTIVETo evaluate the efficacy of artificial liver support system (ALSS) in the treatment of liver failure patients.
METHODSThis is a prospective, multi-center, controlled, large sample clinic trial. 518 patients with liver failure from 5 hospitals were studied and followed. All the patients received similar pharmacological manipulation according to one and the same protocol but were divided into an ALSS treatment group and a control group without ALSS treatment. The ALSS treatment procedures included plasma exchange, molecular adsorbent recirculating system (MARS), plasma exchange plus hemofiltration and other combined nonbioartificial methods. The analysis of survival time was computed using the Kaplain-Maier method, and comparison among groups was done using Log-Rank, Breslow and/or the Tarone-Ware test.
RESULTSSurvival time of acute liver failure patients was prolonged from 4.0+/-0.2 days to 8.0+/-0.4 days (P=0.004). ALSS was shown to be two times more effective. ALSS increased the survival time of acute on chronic (A on C) liver failure patients from 27.0+/-1.6 days to 39.0+/-4.0 days (P less than 0.01). In addition, it increased the survival time of the patients in the middle and end stage of subacute liver failure and A on C liver failure, but had no significant effects on early stage patients. The survival time of middle stage patients was 38.0+/-17.5 days in the control group vs 66.0+/-18.6 days in the ALSS group (P less than 0.05). The survival time of end stage patients of the control group and the ALSS group was 18.0+/-4.0 days vs 26.0+/-2.5 days (P less than 0.01).
CONCLUSIONSMulti ALSS treatment is more effective than the standard medicinal liver care treatment. Multi-ALSS treatment could increase survival time of patients suffering from acute liver failure or A on C liver failure, especially in their middle and end stages. It is important and necessary to treat these patients with ALSS.

10. Current trends of breast reconstruction after mastectomy in China: a cross-sectional study

Objective:
To investigate the current trends of breast reconstruction(BR) after mastectomy in China.
Methods:
A list of hospitals with more than 200 cases of breast cancer surgery per year nationwide was obtained, and 110 institutions were selected according to the geographical distribution. The research was conducted in the form of a questionnaire survey, and 92.3% (169/183) of the questions were single-choice questions. Information such as demographics of surgeons and hospitals, number of mastectomy and BR, type and timing of BR was included in the survey. Survey formal notification letter was issued by the China Anti-Cancer Association Breast Cancer Committee and Chinese College of Surgeons, Committee of Mammary Surgeons. Questionnaires were sent to the respondents of each center by email. The survey time range was from January 1, 2017 to December 31, 2017. All data were completely collected before September 7, 2018.
Results:
A total of 110 units participated in the survey. In total, 87.3% (96/110) of the hospitals have conducted BR surgery. The BR after mastectomy was 10.7% (6 534/61 099), among this, implant BR accounted for 65.7%(4 296/6 534), autologous BR accounted for 20.1% (1 312/6 534), and autologous combined implant BR accounted for 14.2% (927/6 534). Immediate reconstruction accounted for 67.6% (4 417/6 534) of BR, while delayed BR accounted for 32.4% (2 097/6 534). In 2017, 77.8% (35/45) of the plastic surgery departments cooperated with general surgery departments. General BR could be conducted after mastectomy accounted for 83.6% (92/110).
