1.The influence of hepatic ischemia-reperfusion injury to the hepatic cell cycle G2/M-related gene Gadd45a and Egr-1 expression

ObjectiveTo explore the possible mechanism that Gadd45a and Egr-1together participate in the DNA damage repair,by analyzing the influence of hepatic ischemia-reperfusion injury (IRI) on the rnRNA and protein expression of hepatic cell cycle-related genes.Method Methods Forty-two SD rats were divided into normal control group,ischemia 30 min group and reperfusion 30 min,1h,2 h,3 h and 24 h groups.The hepatic specimens of rats at the corresponding time points after success of modeling were obtained. Chip experiment was used to screen out the key genes Gadd45a,Egr-1and GAPDH for target genes.We used Northern blot and RT-PCR to confirm the chip's results. The expression of related proteins was detected by using immunofluorescence and immunohistochemistry.Results Northern blot detection results showed that the RT-PCR products and the target genes for testing were concordant.The expression spectral chip signal and RT-PCR detection revealed that the expression of target genes had a 4-fold increase at early reperfusion period (1h) (up-regulation of Egr-1,Gadd45a and HSP70-1a by 9.39,8.28 and10.8 respectively.At the later stage of reperfusion (24 h),the expression of Egr-1,Gadd45a and Hspala was reduced to the normal levels, and the chip screening results were consistent with the RT-PCR test results.Fluorescence immunohistochemistry demonstrated that in the reperfusion1h and 2 h groups,the expression of Egr-1in nuclei of liver cells was positive. Immunohistochemistry indicated that the Gadd45 expression in the nucleus and cytoplasm was gradually increased with increasing reperfusion time.ConclusionGadd45a and Egr-1are the key genes of the nuclear factor (NF)-kB regulatory pathways,and involved in the regulation of cell cycle arrest at G2/M phase and repair of damaged DNA in rat hepatic IRI.
2.Efficacy of differential antihypertensive treatment strategies at early stage in the elderly with acute cerebral hemorrhage

Objective To observe the the therapcutic efficacy of differential antihypertensive treatments on the elderly acute cerebral hemorrhage with different amounts of bleeding. Methods 86 elderly patients with acute cerebral hemorrhage combined by hypertension were randomly divided into two groups:active antihypertensive group and regular antihypertensive group (n=43 for each group),among which 62 cases aged (68.9±5.3) years were in hemorrhage of small amount (volume <30 ml),24 cases aged (70.8 ± 5.7) years in large amount hemorrhage (volume ≥30 ml).The patients in active antihypertcnsive group received intravenous antihypertensive drugs within 1 hour of treatment in the following 7 d to reach systolic pressure 140-160 mm Hg.In contrast,in the control group the patients' systolic pressure reached 180-200 mm Hg at early stage.The hematoma volume was measured by CT before treatment and at 3 d and 7 d after treatment.NIH stroke scale (NIHSS)was used before treatment and 7 d and 28 d after treatment Results In the patients with small amount of bleeding,NIHSS scores and the total efficacy rate at 28 d after treatment and hematoma size at 3 d after treatment were better in active antihypertensive group than in regular antihypertensive group [(4.8±2.7) scores,86.67% aud (2.4±0.6) ml vs.(7.6±3.9) scores,53.13% and (8.1±3.1) ml,all P<0.01]. However,for the patients with large amount of bleeding,there were no statistical differences in the above indexes between the two differential strategies (all P>0.05).Conclusions Active antihypertensive therapy may inhibit early hematoma volume,improve prognosis and enhance clinical efficacy for the patients with small bleeding,but no clinical significance for the patients with large amount of bleeding.
3.Lymph node metastasis and the extent of resection for proximal advanced gastric cancer

This article analyzes the characteristics of lymph node metastasis in proximal advanced gastric cancer,in order to guide the extent of stomach resection (proximal or total gastrectomy) and the resection of spleen or splenic preservation. The metastasis of lymph node in proximal advanced gastric cancer has certain characteristics, which can provide a significant guidance for lymphadenectomy. Total gastrectomy plus splenectomy is a feasible radical surgery for proximal advanced gastric cancer, because of the requirement of lymphadenectomy.
4.Clinical Efficacy of BCG-polysaccharide Nucleic Acid Cominbed with Isotretinoin in the Treatment of Pa-tients with Plane Warts

Objective:To study the clinical efficacy and safety of BCG-polysaccharide nucleic acid combined with isotretinoin soft capsules in the treatment of plane warts. Methods:Seventy patients with plane warts were randomly divided into the observation group and the control group(n=35). The control group were treated with isotretinoin soft capsules, limp 10mg, bid, 4 weeks to 10mg, qd. The observation group were given BCG-polysaccharide nucleic acid 1ml, im, every otherday, on the basis of the contrl group. After 8 weeks of treatment, T cell populations and immunoglobulin levels were detected, and the efficacy and adverse reactions were recorded. Results:After the treatment, T cell population level, IgG and IgA of the two groups were significantly improved(P<0. 01), and the improvement in the observation group was significantly better than that in the control group (P<0. 05). The onset time of the observa-tion group was shorter than that of the control group ( P<0. 01). The total effective rate of the observation group was higher than, that of the control group(P<0. 05). The occurrence of adverse reactions in the two groups were no significant(P>0. 05). Conclusion:The clinical efficacy of BCG-polysaccharide nucleic acid combined with isotretinoin soft capsules in the treatment of plane warts is sig-nificant with promising security.
5.Diagnosis and management of Hashimoto's thyroiditis and coexisting disease

Objective To investigate the diagnosis and surgical management of Hashimoto's thyroiditis and coexisting disease. Methods The clinical data of 54 patients with Hashimoto's thyroiditis were analyzed retrospectively. Results 25 cases were diagnosed as Hashimoto's disease by clinical physical examination, FNAB and immune antibody assay. Diagnostic treatment with medicine was adopted firstly. Of them 24 cured, one was found coexisting thyroid cancer, and then a radical operation was performed. The other 29 cases had been misdiagnosed as surgical diseases because of atypical features and undergoing thyroidectomy; the diagnosis of Hashimoto's disease had been confirmed by pathology, and some coexistence had been found(hyperthyroidism in 3, thyroid carcinoma in 4, malignant lymphoma in 2, thyroid adenoma in 5). Conclusions Attention should be paid to the atypical Hashimoto's disease. FNAB, immune antibody assay, diagnostic treatment, operative exploration and biopsy would be helpful to make the sound diagnoses of Hashimoto thyroiditis and the disease in coexistence, which ought to be operated on corresopondingly.
7.Analysis of the Utilization of Essential Medicines in Outpatient Department of a Women & Children’s Hospi-tal during 2012-2013

OBJECTIVE:To know the utilization of essential medicines in outpatient department of Sichuan Shuangliu Maternal and Child-care Hospital,and provide reference for the rational drug use. METHODS:The types,names,formulations,consump-tion sum and utilization of essential medicines in the outpatient department of the hospital from 2012 to 2013 were statistically ana-lyzed and ranked. The affordability of top 10 drugs in the list of DDDs was evaluated. RESULTS:Compared with 2012,the con-sumption sum proportion of essential medicines in the outpatient department of the hospital in 2013 was increased by 5.80%. The top 10 essential medicines in the list of consumption sum were mainly anti-microbial drugs,anti-anemia drugs and Chinese patent medicines. The relative daily wages of Kangfuxin solution and Amoxillin and clavulanate potassium tablets were respectively 2.12 d and 2.05 d,with poor affordability,and in 2012,they ranked the 5th and 8th in the list of DDDs;the relative daily wages of Xiao-er changweikang granule and Xingpi yanger granule were respectively 2.28 d and 2.14 d,with poor affordability,and in 2013, they ranked the 8th and 10th in the list of DDDs. CONCLUSIONS:The utilization of essential medicines in the hospital is basical-ly good,with a trend of increasing. However,the similar essential medicines with lower prices should be considered when essential medicines are used in the clinic.
8.Effect of Indomethacin suppositories on albumin concentration and insulin resistance of post-operation patients with serious intra-abdominal infection

Objective:To evaluate the effect of Indomethacin suppositories on albumin concentration and insulin resistance of post-operation patients with serious intra-abdominal infection.Methods:38 post-operation patients with serious intra-abdominal infection were divided into two groups randomly.One as treatment group(n=19)and the other as control group(n=19).Albumin,blood glucose,fasting insulin concentration were measured At 72 hours after the operation and 144 hours.Insulin resistance index was calculated using the homeostasis model assessment(HOMA).Finally,the results of two groups were analyzed.Results:The albumin concentration of Indomethacin group at 72 hours was significantly higher than that after 72 hours,Insulin resistance of 144 hours was lower than the control group(P0.05).Conclusion:Postoperative abdominal infection using indomethacin suppository can significantly reduce insulin resistance,and significantly reduced the decomposition of albumin.

10.G3BP: a promising target for cancer therapy.

G3BP (Ras-GTPase-activating protein SH3 domain binding protein), a protein which binds to RasGAP SH3 domain, belongs to RNA-binding protein family, implicating in the downstream of Ras signaling. G3BP harbors the activities of endoribonuclease and DNA helicase, and can induce stress granules formation. G3BP plays a general role in the signal pathways of cell proliferation, differentiation, apoptosis and RNA metabolism. It has been shown to be over-expressed in a number of human malignancies and has a close relationship with tumor invasion and metastasis. Given that it has been implicated in several pathways that are known to be involved in cancer biology, G3BP may provide a new target for cancer therapy.
