Objective To compare the current trcmment of intertrochanteric fracture fixation commonly used in the two systems:proximal femoral nail(PFN-A)and the dynamic hip screw(DHS)of clinical efficacy and evaluate the treatment of the two intertroehanteric fracture of the gifted bad.Methods The clinical use on closed reduction and traction,respectively PFN-A with DHS and the treatment of intertrochanteric fracture patients with 106 cases and 115 cases,compared with its ease of operation,and the effect of surgery,postoperative complications.Results The clinical material demonstrated that in after PFN-A treatment thighbone thick pmsperom bone fracture technique,technique,the complication is short in DHS.Conclusion PFN-A well-designed,with simple anti-rotation indeed,surgical trauma,less bleeding,fewer complications,etc.,is the treatment of intertrochanteric fracture fixation one of the ideal.
3.The clinical significance of nitric oxide and endothelin-1 in exhaled breath condensate and serum of patients with acute lung injury/acute respiratory distress syndrome

Objective To observe the level of the nitric oxide (NO) and endothelin-1 (ET-1) in the exhaled breath condensate(EBC)and serum of the patients with ALI /ARDS, and investigate its clinical significance. Methods The study group included 52 mechanical ventilation patients with ALI/ARDS in ICU , which were divided into the survival and death group, while 30 healthy volunteers were recruited as healthy control. EBC samples of the healthy control and the study group on the 1st day and 5st day were collected by EcoScreen condenser with the synchronous collection of the venous blood. The concentrations of NO and ET-1 in the EBC and serum were measured by EIA. Results The levels of NO and ET-1 in EBC and serum of the patients with ALI /ARDS were all significantly higher than those of the healthy control. After treatment , the levels of NO and ET-1 in EBC and serum of the patients all decreased significantly compared with before treatment. After treatment , The levels of NO in EBC and serum of the survival group were significantly lower than those of the death group. After treatment , the levels of ET-1 in serum of the survival group was significantly lower than that of the death group. Conclusions Detecting the levels of NO and ET-1 in the EBC and serum can reflect oxidative stress , inflammatory reaction and endothelial injury in lung of patients with ALI/ARDS.

5.Mammographic and MRI Appearances of Primary Non-Hodgkin's Lymphoma of Breast

Objective To discuss the features of MRI and molybdenum target X-ray in mammary primary non-Hodgkin's lymphoma.Methods 3 cases of mammary primary non-Hodgkin's lymphoma were studied.The features of MRI and molybdenum target X-ray werecollected and analyzed(all 3 cases had molybdenum target X-ray in MLO site and CC site,one had MRI image).Results All 3 caseslesions were in single side,one case had two lesions in one side.The diameter of all the lesions were more than 3cm.The lesion presented as iso-or slight hyperdensity shadow,and related to breast tissue and density was hemogeneous.The tumor was irregular but no bristle andcalcification.There was no skin and nipple involved in superficial of mammary gland.In T_1WI showed even low signal.In T_2WI showedeven little high or equal signal.Intensity was hemogeneous and obvious.The curve of time-signal intensity was platform pattern.Conclusion The features of MRI and molybdenum target X-ray in mammary primary non-Hodgkin's lymphoma are characteristic and helpful for deciding correct therapy plan.
6.Three-dimensional imaging reconstruction of the ulnar nerve functional fascicular groups

Objective To reconstruct on the basis of histological study the computerized 3D ima ges of the fascicular groups of ulnar nerve,which can be used to guide the microsurg ical repair of injuries of brachial plexus and ulnar nerve and to provide morpholog ical evidence for the functional rec overy of intrinsic muscles of hand after root avulsion of brachial plexus.Methods The successive AchE-staining histo logical cross-sections were made.T he photos of the sections were transformed into digital information and sto red in the computer.Different functional fas-cicular groups were distinguished a nd marked on each section.The topographic database was analyzed by image pro-cessing software,with the support o f OpenGL three dimensional graphic l ibrary.3D images of the fascicular g roups of ulnar nerve were reconstructed usin g the contour-based surface constru ction method in the program written w ith VC language.Results The 3D images of the individual external surface of the reconstructive fa scicular groups were displayed and the human-computer in teraction was realized by performin g the arbitrary rotation and the rand om cross-section.By selecting the int erested object,the spatial structu re of any fascicular group could be di splayed in the same space simultaneously.Compare d with the result of microanatomy,th e 3D shape could truly reflect the str ucture,adjacent relationship and arrangem ent of fascicular groups in the ulnar nerve trunk.Conclusions Since the re-constructive 3D images can truly reflect the arrangement and distribution of the functional fascicular grou ps,they will have positive effects on understanding the internal structure of ulnar nerve,designing operative procedur e and adjusting operative method for surgeons.[
7.Microanatomic study on arrangement and distribution of the fascicular groups in ulnar nerve trunk to the intrinsic muscles

Objective To observe the arrangement and distribution of the fascicular groups in the ulnar nerve trunk to the intrinsic muscles so as to provide anatomical evidence for selective repair in the clinic.Methods 12 cadaveric upper extremities recently embalmed with formalin were obtained. The ulnar nerve and its branches were exposed. Microanatomy and longitudinal contradissection of each functional fascicular group were performed by tracing their terminal branches, and the arrangement and distribution were observed and recorded. The fascicular groups to the intrinsic muscles of hand in the nerve trunk were observed to see how they crossed and mixed with other fascicular groups. The diameter of the nerve trunk and the fascicles to the intrinsic muscles were measured, and the area and the distribution range of the fascicles calculated. Results The shape and the diameter of the ulnar nerve trunk varied at different positions. The fibers of functional fascicular groups had the definite differentiation at distal while interfused and diffused at proximal. The fascicular groups to the intrinsic muscles of hand were the largest and their fibers relatively concentrated (the distribution range occupied 40% ~ 80% of the ulnar nerve), and had the definite localization at the forearm and distal to the 3/8 segment of upper arm. Conclusion Since the fascicles to the intrinsic muscles of the hand are the most important in the ulnar nerve, selective repair by using epineural and perineural techniques at the forearm and upper arm will reduce the loss of nerve fibers and will contribute to the functional recovery.
8. Application of SOS/umu in screening cytoprotectors of plants against mitomycin C-induced genotoxic damage

Objective: To establish a system and method for screening plant extract against mitomycin C-induced genotoxic damage. Methods: Salmonella typhimurium TA1535/pSK1002 and acute toxicity experiment of mice were used, and the effects of the extracts from ten plants, Chrysanthemis Flos, Allii Bulbus, Zingiberis Rhizoma Recens, Ginkgo Folium, Ginseng Radix et Rhizoma, Vitis Viniferae Semen, Gemmae Camelliae Sinensis Folium, Ganoderma, Acanthopanacis Senticosi Radix et Rhizoma seu Caulis, and Sojae Semen, and the extract combinations on mitomycin C-induced genotoxic damage were observed by SOS/umu test. Results: Significantly protective effects of five extracts, including the extracts from Allii Bulbus, Vitis Vindferae Semen, Gemmae Camelliae Sinensis Folium, Acanthopanacis Senticosi Radix et Rhizoma seu Caulis, and Sojae Semen against mitomycin C-induced genotoxicity were observed. Acanthopanacis Senticosi Radix et Rhizoma seu Caulis extract (1.5 g/L) could inhibit the mitomycin C-induced genotoxicity (67.12%). Combinations of any two extracts showed higher antimutagenic capacity than any single one. Among all the combinations, Acanthopanacis Senticosi Radix et Rhizoma seu Caulis-Gemmae Camelliae Sinensis Folium and Acanthopanacis Senticosi Radix et Rhizoma seu Caulis-Vitis Viniferae Semen showed the highest activity, and the inhibition rate of the former against the mitomycin C-induced genotoxicity was 83.2%. In vivo tests showed that Acanthopanacis Senticosi Radix et Rhizoma seu Caulis- Gemmae Camelliae Sinensis Folium could significantly decrease the micronucleus rate and sperm abnormality rate of mice induced by mitomycin C and also increase the thymus indexes. Conclusion: Based on the results, it is clearly proved that the SOS/umu is not only a useful and convenient way to evaluate the antimutagenic ability of plant extracts, but also could be used as a kind of rapid screening model for cytoprotector with high throughput screening of candidate extracts or compounds.
9.Effects of 137 Cs γ-rays on proliferation, differentiation and mineralization of osteoblastic cells in vitro

Objective To evaluate the effect of gamma irradiation on the proliferation,differentiation,and mineralization of murine osteoblastic cells,and to investigate the related molecular mechanism.Methods Osteoblastic cells were irradiated by different doses (0,0.5,1.0,2.0,5.0 Gy)of 137Cs γ-rays.Cell morphology was observed with a microscopy,cell viability was analyzed by MTT assay,and ALP activity was analyzed by the methods of enzyme histochemistry and PNPP.Meanwhile,gene expressions of ALP,osteocalcin (OC),collagen Ⅰ,osteoprotegerin (OPG) and receptor activator of nuclear factor-kB ligand (RANKL) were measured by semi-quantified RT-PCR.Results Cell viability decreased with the radiation doses over 1.0 Gy ( t =6.197 - 18.677,P < 0.05 ).After radiation with a dose over 2.0 Gy,the cell number and the junctions of cell protrusions decreased,the cells had low refractivity and the activity and mineralization ability of ALP were also inhibited ( t =2.790 -2l.374,P <0.05).In addition,the expressions of ALP and OC mRNA were down-regulated significantly (t =3.563 -16.508,P < 0.05) when the radiation dose was higher than 0.5 Gy,and the expressions of OPG,OPG/RANKL mRNA were down-regulated ( t =12.942,4.954,P < 0.05 ) at 5 Gy.But the expressions of collagen Ⅰ and RANKL mRNA were not affected by irradiation.Conclusions The osteoblastic cells were significantly influenced by γ-irradiation,including morphological changes,inhibition of cell proliferation,differentiation and mineralization ability. Meanwhile,mRNA expressions of ALP and OC were downregulated.OPG/RANKL may be a main pathway of osteoblastic cell damage under high dose radiation.
10.An Analysis on Nosocomial Infection after Surgical Operation

0.05);the main infection site was lower respiratory tract(61.97%),followed by surgical site infection(SSI)(23.94%).CONCLUSIONS In order to reduce postoperation nosocomial infection rate,we should take following measures:Improve pulmonary function,remove tracheal intubation as early as possible,shorten lasting time of surgery and days before operation,keep aseptic technique,strengthen nursing,enhance immunity,and use antimicrobial agents reasonably.
