1.Clinical value of percutaneous microwave ablation combined with synchronous transarterial chemoembolization for the treatment of gastroenteropancreatic neuroendocrine neoplasms with liver metastases

Objective To investigate the safety,effectiveness and prognosis of percutaneous microwave ablation (MWA) combined with synchronous transarterial chemoembolization (TACE) to treat of gastroenteropancreatic neuroendocrine neoplasms (GEP-NENs) with liver metastases (LMs).Methods This retrospective study included 19 cases of GEP-NENs with LMs patients who received percutaneous MWA combined with synchronous TACE treatment from 2013 to 2016.The mRECIST standard was selected to assess the curative effect.SPSS 21.0 software was applied in the statistical analysis of overall survival (OS),progression-free survival (PFS) and factors related to prognosis.Results All patients were capable of curative effect evaluation,including 1 case of complete remission (CR),3 cases of partial remission (PR),7 cases of progressive disease (PD) and 8 cases of stable disease (SD) respectively accounting for 5 %,16 %,37 %,42 %,which exhibited 21% of response rate (RR) and 63% disease control rate (DCR).In the present study,the median OS and median PFS was respectively 25 months and 34 months,and the one-year survival and three-year survival was respectively 95% and 84%.Serum CA199,the WHO classification of LMs and the tumor burden of LMs were the major risk factors of prognosis through single factor analysis of survival,which showed that G3 of the WHO classification of LMs predicted a poor OS (P<0.05) and tumor burden of LMs was negatively related to PFS (P<0.05).It was obviously observed that serum CgA was decreased by the therapy of percutaneous MWA with synchronous TACEfor GEP-NENs (P<0.05).Conclusions Percutaneous MWA combined with synchronous TACE is a safe and effective method to treat GEP-NENs with LMs.
2.Investigation and analysis of clinical skill training course

Objective To improve the clinical skill training courses in order to meet the new challenges and different requirements. Methods Totally 327 clinical physicians of different levels in 36 departments were enrolled and 286 physicians were randomly surveyed with the questionnaire , including subjects' general characteristics, participation rate, training courses and training model. Results Among the trainees, most of them were attending physician and resident physician, and their participation rate was 88.5%(77/87) and 90.0% (172/191) respectively. The most popular and well acknowledged training courses were emergency treatment course such as AHA basic/advanced life support course. Training model like ‘group class, one topic, one hour’ was well accepted by 84.1%(275/324) physicians and course time of 16:30-17:30 was preferred by 65.5% (214/327) physicians. Conclusions A series of most popular and significant training courses are developed. More important-ly, physicians' real need and new expectation to the training course are well recognized, which is im-portant to plan the further training program and courses.
3.Characteristics of iris vessels exposure and its relevant factors in normal full-term neonates

Objective To observe the rate of iris vessels exposure and analyze its relevant factors in normal full-term neonates.Methods A retrospective study.1855 normal full term neonates,including 947 boys and 908 girls,were enrolled.The mean gestational age (GA) was (38.84±1.10) weeks and mean birth weight (BW) was (3 396.52±402.08) g.There were 1235 neonates from normal term vaginal delivery,402 cases of cesarean delivery and 218 cases of forceps delivery.All neonates were examined with hand-held portable slit lamp biomicroscopy within 1 to 3 days after birth by two trained ophthalmologist respectively.Iris vessels exposure was defined as radial red blood vessels along iris fibers.Infants were divided into iris vessels exposure group and iris vessels unexposed group according to the findings of slit lamp biomicroscopy.78 infants with iris vessels exposure were followed up for 42 days after birth till the iris vessels can't be seen under microscope.The differences between the two groups were compared for gender,mode of delivery (MOD),GA,BW and body length (BL).Multiple logistic regressions were used to determine the factors related to iris vessels exposure.Results There were 298 neonates with iris vessels exposure among 1855 neonates and the rate was 16.1%.1557 neonates (83.9%) had unexposed iris vessels.There were no different in gender (x2=0.551) and MOD (x2=3.036) between iris vessels exposure group and unexposed group (P>0.05),while the differences in GA (x2=47.216),BW (t=4.603) and BL (t=3.936) between the two groups were statistically significant (P=0.000).Multiple logistic regression analysis revealed that only GA (β=-0.291,odds ratio=0.747,95% confidence interval:0.656-0.85 1,P=0.000) was correlated to iris vessels exposure significantly.The iris vessels couldn't be seen in 77 of 78 infants with iris vessels exposure when followed up to 42 days.Conclusions The iris vessels exposure in normal full-term neonates is frequently observed.There is a significant inverse correlation between GA and iris vessels exposure.
4.Expression of cytidine triphosphate synthetase 1 in pancreatic cancer cell lines and its clinical significance

Objective To investigate the cytidine triphosphate synthetase 1 (CTPS1) expression in pancreatic cancer cell lines and its impaction on prognosis.Methods The GEO profiles database was searched to collect clinical data of 39 patients of pancreatic ductal adenocarcinoma.Kaplan-Meier curve was used to analyze the relationship between the patients' prognosis and the expression of CTPS1.qPCR and Western blot were applied to detect the expression of CTPS1 in 4 pancreatic cancer cell lines.Results The mRNA expression of CTPS1 in Sw1990,BxPC-3,Panc1,and MIA-Capa2 was 1.00) ± 0.20,2.92±0.95,4.29±0.14,and 2.26±0.33 (t=-16.19,-11.45,-8.09,-13.12,P<0.05) when compared to the normal pancreatic ductal epithelial cell line HPDE of 7.70 ± 0.72.The relative protein expression of CTPS1 in pancreatic cancer cell lines Sw1990,BxPC-3,Panc1,and MIA-Capa2 was 0.40 ± 0.06,0.20±0.09,0.68±0.11,and0.48±0.06 (t=-8.97,-10.5,-4.39,-7.88,P<0.05) when compared to HPDE of 1.09 ±0.12.The overall survival (OS) in patients with higher CTPS1 expression than adjacent tissue was 28 months (95% CI:9.2-46.8 months),and that with lower CTPS1 expression was 13 months (95% CI:8.2-17.8 month),(x2 =4.02,P<0.05).Conclusions CTPS1 in the pancreatic cancer cell lines are of low expression,and the level is positively related to patients' survival time.

With the development of science and technology,medical simulation has been applied extensively.The application of simulation technique in teaching hospitals has become a new choice in modern medical education.As the most basic and important clinical ability,airway management has become a popular training program in China's teaching hospitals.This paper summarizes the current situation of the simulation training of airway management in teaching hospitals at home and abroad,concludesthe existing problems in the simulation teaching of airway management of teaching hospitals in China,such as narrow range of training objects,lack of professional teaching staff,unscientific curriculum design and outdated training model,etc.Then puts forward the countermeasures of expanding the training coverage,establishing the professional teaching team,designing the curriculum design of science,and introducing advanced simulation equipment to improve the quality of simulated teaching.

The SARS-CoV-2 has been spread to 26 countries around the world since its outbreak. By February 16, 2020, more than 68 000 people had been diagnosed with COVID-19. Researchers from all over the world have carried out timely studies on this public health emergency and produced a number of scientific publications. This review aims to re-analyze and summarize the current research findings in a timely manner to guide scholars in relevant fields to further SARS-CoV-2 research and assist healthcare professionals in their work and decision-making. The SARS-CoV-2 related terms were selected in both English and Chinese and were searched in several major databases, including Pubmed, Web of Science, CNKI, Wanfang, and VIP databases. The reference list of each search result was screened for relevance, which was further supplemented to the search results. The included studies were categorized by topics with key characteristics extracted, re-analyzed, and summarized. A total of 301 articles were finally included with 136 in Chinese and 165 in English. The number of publications has rapidly increased since mid-January, 2020, and a peak day was 6th February on which 50 articles were published. The top three countries publishing articles were China, the United States and the United Kingdom. The and its specialty journals have published the most articles, with contribution also from journals such as ( ), ( ), and . All articles were categorized into epidemiology, clinical diagnosis and treatment, basic research, pregnant women and children, mental health, epidemic prevention & control, and others. The literatures related to SARS-CoV-2 are emerging rapidly. It is necessary to sort out and summarize the research topic in time, which has a good reference value for staff in different positions. At the same time, it is necessary to strengthen the judgment of the quality of literatures.

7.Study on the mechanism of Yifei xuanfei jiangzhuo formula against vascular dementia

OBJECTIVE To investigate the mechanism of Yifei xuanfei jiangzhuo formula (YFXF) against vascular dementia (VD). METHODS The differentially expressed genes of YFXF (YDEGs) were obtained by network pharmacology. High-risk genes were screened from YDEGs by using the nomogram model. The optimal machine learning models in generalized linear, support vector machine, extreme gradient boosting and random forest models were screened based on high-risk genes. VD model rats were established by bilateral common carotid artery occlusion, and were randomly divided into model group and YFXF group (12.18 g/kg, by the total amount of crude drugs), and sham operation group was established additionally, with 6 rats in each group. The effects of YFXF on behavior (using escape latency and times of crossing platform as indexes), histopathologic changes of cerebral cortex, and the expression of proteins related to the secreted phosphoprotein 1 (SPP1)/phosphoinositide 3-kinase (PI3K)/protein kinase B (aka Akt) signaling pathway and the mRNA expression of SPP1 in cerebral cortex of VD rats were evaluated. RESULTS A total of 6 YDEGs were obtained, among which SPP1, CCL2, HMOX1 and HSPB1 may be high-risk genes of VD. The generalized linear model based on high-risk genes had the highest prediction accuracy (area under the curve of 0.954). Compared with the model group, YFXF could significantly shorten the escape latency of VD rats, significantly increase the times of crossing platform (P<0.05); improve the pathological damage of cerebral cortex, such as neuronal shrinkage and neuronal necrosis; significantly reduce the expressions of SPP1 protein and mRNA (P<0.05), while significantly increase the phosphorylation levels of PI3K and Akt (P<0.05). CONCLUSIONS VD high-risk genes SPP1, CCL2, HMOX1 and HSPB1 may be the important targets of YFXF. YFXF may play an anti-VD role by down-regulating the protein and mRNA expressions of SPP1 and activating PI3K/Akt signaling pathway.
