1.The effection of environmental and health message forecasting service offer to the allergic rhinitis in children.

OBJECTIVE:
To investigate the impact of environment and message information for the treatment efficacy of children with allergic rhinitis (AR).
METHOD:
To choose 100 cases of children diagnosed with AR. The parents of children record daily symptom scores of AR of children everyday. While the doctors send the short message to the parents by using meteorological environment warning forecast technology to tell the parents the risk of AR attacks. We observe the morbidity of AR of the children and the treatment efficacy of children for 1 year. The 100 cases control group children diagnosed AR, their parents do not record daily symptom scores of AR and the doctors do not give the short message. We also observe the morbidity of AR of the control group children and the treatment efficacy of the children for 1 year.
RESULT:
The 100 cases intervention group children with AR, there are 11 cases refused to participate the group. We included a total 89 cases, 74 cases of children did a full year of intervention, 15 cases of children gave up. The control group 100 cases, nine cases refused to participate, 91 cases were included. The average episodes in the intervention group was 4. 67 times, the control group was 8. 12 times,--there were significant differences between the groups statistically. The execution rate on the prescribed clinic date in the intervention group visits was 91. 5%, while in the control. group, was only 67%. The compliance of parents of children to complete course of medication in the intervention group was 95. 6%, while the control group was 74.1%. Both sets of data are statistically significant differences. The children with sinusitis in intervention group was 26.97%, significantly lower than the 64.04% in the control group. The incidence in the intervention group and the control group of secretory otitis media was 8.99% and 6.60%, there was no statistically significant differences.
CONCLUSION
It can significantly increase the degree of attention of parents of children with AR that the parents of children record daily symptom scores of AR of children everyday, while the doctors send the short message to the parents by using meteorological environment warning forecast technology to tell the parents the risk of AR attacks. It can also improve the execution rate on the prescribed clinic date and improve compliance of parents of children to complete course of medication. It can significantly reduce seizure frequency and severity of episodes of AR, thereby improving the quality of life of children with AR, reduce the economic burden on families and society.

2.Meta-analysis of curative effect of operations for duodenal diverticulum

Objective To evaluate the curative effect of different operative methods for duodenal diverticulum. Methods Articles of operations for duodenal diverticulum that were published between January 2000 to August 2006 were retrieved from China Journal Full-text Database, and the symptom remission rate and complication rate were underwent Meta-analysis. All data were analyzed by chi-square test. Results There was significant difference in symptom remission rate between Billroth Ⅱ subtotal gastrectomy, Roux-en-Y gastrojejunostomy and diverticulectomy, diverticulum inverting suture, sphincteroplasty of Oddi (χ~2= 21.269, 4.285, 12.914, 19.266, 3.938, P < 0.05). The difference in complication rate between Billroth Ⅱ subtotal gastrectomy, Roux-en-Y gastrojejunostomy and diverticulectomy, diverticulum inverting suture reached statistical significance (χ~2 =4.164, 9.166, 7.926,4.659, 4.858, P < 0.05). Conclusion For duodenal diverticulitis and the related complications which can not be effectively managed by internal medical treatment and with obvious symptom, Billroth Ⅱ subtotal gastrectomy and Roux-en-Y gastrojejunostumy are safe and effective.
3.Study on hospital-acquired infections caused by invasive yeast in cancer patients

Objective To investigate the clinical distribution and drug-susceptibility characteristics of yeast speeies in tumor patients with invasive yeast infections in order to provide the gist for clinical therapy.Methods The medical records of 70 tumor patients with invasive yeast infections from January 2006 to December 2008 were collected.retrospectively.The relationship between different tumor types and invasive yeast infections was analyzed.The clinical distribution and drug-susceptibility characteristics of 70 isolates of yeast were investigated.Results The sites of invasive yeast infections were abdominal(32 cases),blood(18 cases),biliary(9 cases),central nervous system(6 cases)and chest infection(5 cases).respectively.The most common species causing the above infections was Candida albicans(34 isolates,48.6%).The mortality of invasive yeast infection was 15.7%.The main types of tumor in 70 patients with invasive yeast infections were pancreatic carcinoma(12 cases),stomach carcinoma(11 cases)and hepatocellular carcinoma(7 cases).No relationship was found between tumor types and yeast infections with different species(P=0.804,P>0.05).There were 18 patients with tumor metastasis.Among them.9 patients had yeast bloodstream infections and 6 with yeast abdominal infections.Yeast bloodstream infections were more likely to be found in patients with tumor metastasis(χ~2=5.357,P<0.05).Seventy isolates of yeasts were all sensitive to triazoles.The MIC values of amphotericin B aginst 6 isolates of Candida and 2 Laurent cryptococcal were 2μg/ml.Conclusion Patients with tumor metastasis should be paid more attention for yeast invasive infections.and triazoles kept activity against yeasts tested.
4.Research on mandibular bone mineral density in patients with end-stage renal disease

Objective To investigate the effectiveness of panoramic mandibular index (PMI) in detecting the bone mineral density (BMD) of mandibular bone and whole-body bone in patients with chronic end-stage renal failure. Methods A total of thirty patients with peritoneal dialysis treatment were used as experimental group and 31 healthy adults were used as control group. The panoramic jaw tomography was taken for the measurement of superior PMI (sPMI) and inferior PMI (iPMI) in two groups. The dual-energy X-ray absorptiometry (DXA) was used to detect BMD of lumbar spine bone. Data were compared between two groups. The correlation of sPMI, iPMI and BMD of lumbar spine bone was analyzed. Results All indicators including sPMI(0.262 2 ± 0.026 7 vs 0.284 2 ± 0.025 4, t=3.301) , iPMI (0.314 1 ± 0.028 3 vs 0.334 1 ± 0.027 5, t=2.808) and BMD of lumbar spine bone (0.832 3 ± 0.101 0 vs 0.906 9 ± 0.113 6,t=2.709) were significantly lower in experimental group than those in control group (P<0.01). There was a positive correlation between sPMI and iPMI with BMD of lumbar spine bone (r=0.439 and 0.389, P<0.05). The BMD of lumbar spine bone was significantly lower in female patients than that of male patients in control group (0.849 7±0.114 7 vs 0.968 0±0.076 3,t=3.357). The BMD of lumbar spine bone was also significantly lower in female patients than that of male patients in experimental group (0.775 4±0.068 4 vs 0.882 1±0.099 9,t=3.365). There were no significant differences in values of sPMI and iPMI between male and female patients of two groups. Conclusion The BMD of mandibular bone is lower in patients with chronic end-stage renal failure than that of the normal people. PMI index is a simple and effective method to detect the BMD of mandibular bone, which can reflect the BMD of whole body bone in patients with chronic end-stage renal disease.
5.Fungi Distrubition and Drug-resistance in Tumor Patients with Fungi Infection

90%),but they had low susceptibility rate to itraconazole(70.62%-50.45%).CONCLUSIONS The main species of fungi isolated from clinic is Can albicans which is susceptible to antifungal agents.It is important to give active treatment and choose antifungal agents reasonably for improving the prognosis and reduce drug-resistant strains.
6.Effects of meteorological and environmental factors on subjective symptoms of allergic rhinitis in children.

OBJECTIVE:
To analyze the possible effects of meteorological and environmental factors on the subjective symptoms of allergic rhinitis (AR) in children.
METHOD:
According to the daily subjective symptom records in AR children, the relationship between the subjective symptoms of AR in children and the meteorological environmental factors was analyzed. Mixed model was used to analyze the data.
RESULT:
The temperature and humidity had a negative correlation with symptoms score. Every 1 degrees C of temperature decrease was linked to 0.04 points increase in subjective symptoms scores (P < 0.01). Every 10% e in humidity decrease was linked to 0.04 points increase in subjective symptoms scores (P < 0.05). There was positive relationship between PM2.5, PM10 and symptom scores. Every 10 μg/m3 increase of PM2.5 or PM10 was linked to 0.02 (P = 0.0246) or 0.03 (P = 0.0293) points increase in subjective symptoms scores, indicating that air pollution could aggravate the symptoms of children in AR.
CONCLUSION
It suggested that higher temperature and higher humidity may induce lower sympotem while PM2. 5 and PM10 may induce higher sympotem in AR children.

7.Isolation, Identification and Growth Characteristics of Pseudomonas putida Strain M6 with Malachite Green Decolorization

Six bacterial strains with malachite green decolorization ability were isolated from a sediment of aquaculture pond, and strain M6 was selected by further enrichment culture in nutrition broth with malachite green and decolorization rate comparison. The decolorization rate of strain M6 to malachite green was 97.14% in the conditon of 30?C and 150 r/min, and its morphology was observed by gram stain and electronmicroscopy, its physiological and biochemical characteristic was studied by ATB bacteria identification in-strument for identification of bacteria, and its 16S rDNA sequence was determined following PCR amplifi-cation, the sequence was aligned and the phylogenic tree was instructed with those bacterial strains of high identity with strain M6. In addition, its growth characteristics was also studied. The experimental results showed that strain M6 was gram negative and bacilliform with a flagellum at one end. Its size was 0.45 ?m ?0.84 ?m. Its colony produced on common agar plate appeared as round, light blue, dense, hard to choose; 16S rDNA sequence of strain M6 had high identity of 98%~99% with Pseudomonas sp. located in GenBank and strain M6 had the most close relative relation to Pseudomonas putida OW-16 (Locus number: DQ112328.1). Combined the results of the traditional morphological, physiological, biochemical character-istics and 16S rDNA sequence analysis, strain M6 was identified as Pseudomonas putida (Locus number: EU348741.1). Additionally, its growth curve in the condition of 30?C and 150 r/min was as follows: lag phase was 0~4 h, log phase was 4 h~64 h, stationary phase was 64 h~80 h, decline phase was after 80 h. Its best growth conditions were pH 7 and 30?C,and in the rotational speed of 50 r/min to 250 r/min. Its concen-tration increased with the increase in rotational speed.
8.Random bladder biopsy and hydrodistention with cystoscopy under anesthesia play an important diagnostic and therapeutic role in bladder pain syndrome/interstitial cystitis

Objective To evaluate the diagnostic and therapeutic role in bladder pain syndrome/interstitial cystitis (BPS/IC) of random bladder biopsy and hydrodistention with cystoscopy under anesthesia.Methods A retrospective review of cases in our BPS/IC center was performed from 2005 to 2010. One hundred and nineteen patients were included who are diagnosed as the bladder pain syndrome/interstitial cystitis (BPS/IC).There were 32 male patients,aged 47 to 64 years,and 56 years on average; 87 female cases,aged 23 to 67 years,49 years on average.Patients with bladder pain symptoms underwent a thorough evaluation which include voiding diary,pelvic pain、urgency and frequency questionnaire,urine culture,cytology,acid fast bacilli and upper tract imaging.Cystoscopy and random bladder biopsy had been undertaken with general anesthesia.Before and after hydrodistention with cystoscopy,the patients daily frequency of urination,the maximum urine volume,pain scores,symptom scores,QOL score were observed to understand whether there are significant improvement. Results One hundred and nineteen BPS/IC cases underwent random bladder biopsy and hydrodistention with cystoscopy under anesthesia,and finally 102 cases were confirmed of IC; 17 cases were not IC. Eight patients who were previously diagnosed as BPS/IC were found bladder transitional cell carcinoma as the cause of bladder pain symptoms( including 4 cases carcinoma in situ,1 case Low grade non-invasive bladder cancer,3 cases High grade invasive bladder cancers),and 4 of whom had no hematuria. Mean time from the occurrence of BPS to diagnosis of transitional cell carcinoma was 10.8 months. Three patients previously diagnosed as BPS/IC were found tuberculous cystitis as the cause of symptoms,and one eosinophilic cystitis,three chemical cystitis,two radiation cystitis were also detected.Before hydrodistention with cystoscopy under anesthesia,the daily frequency of urination in 102 cases diagnosed for IC patients was 42.1 ±5.6; the maximum urine volume was 141.0 ± 8.3 ml; pain score 7.6 ± 3.0; O'Leary-Sant questionnaire symptom score was 27.7 ± 4.2; QOL score was 7.6 ± 2.4.After hydrodistention with cystoscopy under anesthesia,the daily frequency of urination in 102 IC patients was 23.3 ± 3.4,and the maximum urine volume was 352.0 ± 1.7 ml ; pain score was 3.3 ± 4.3 ; O'Leary-Sant questionnaire symptom score was 12.5 ± 7.3 ; QOL score was 3.2 ± 5.1. Before and after hydrodistention with cystoscope under anesthesia,all of the daily frequency of urination,the maximum urine volume,pain scores,symptom scores,QOL score in 102 IC patients were significant improved (P < 0.05 ). Conclusions BPS/IC remains a diagnosis of exclusion.Random bladder biopsy and hydrodistention with cystoscopy under anesthesia play an important diagnostic and therapeutic role in BPS/IC.
9.Mutations in a Large Pedigree with Y-STR Genetic Markers

Objective To explore the mutation of Y-STR loci in meiotic allelic transmission in a large pedigree. Methods The oral swabs of 163 male individuals were collected froma L in pedigree. Twenty-two Y-STR genetic markers were typed with AGCUY 24 fluorescent detection kit (AGCUY 24 sys-tem), which also contained 16 Y-STR markers included in Y filerTmmultiple amplification kit (Y filer system). The genotyping results of Y-STR loci were compared between each two males in the pedigree. Results There were 20 and 30 kinds of haplotypes obtained with Y filer and AGCUY 24 systems in 163 male individuals fromthe L in pedigree, respectively. The rates referred to haplotype differences (RRHD) of these two typing systems between male pairs were 0.910 5 and 0.922 7, respectively. The average number of marker differences were 6.582 1 and 9.824 8, respectively. The RRHDincreased along with the incidents of meiosis. Conclusion Y-STR mutation leads to different Y-STR haplotypes among the male members in a paternal pedigree and the rate of difference increases along with the incidents of meiosis.
10.Effect of community-based comprehensive intervention on stroke-prevention-related knowledge, belief and behavior of hypertensive patients

Objective To explore the effect of community comprehensive intervention on stroke-prevention-related knowledge, belief and behavior of hypertensive patients. Methods The community-based comprehensive interventions were done to 77 hypertensive patients, with the measures including archives establishment, hypertension-phased management, regular follow-up and multi-mode health education. The stroke-prevention-related knowledge, belief and behavior of them were assessed by the stroke-preventing knowledge questionnaire (SPKQ), champion′s health belief model scale (CHBMS) and (stroke-prevention behavior questionnaire,SPBQ) before and after the intervention. Results Their knowledge on daily-life and pre-stroke symptoms and part of their behaviors (health duty, physical exercises and stress management after the intervention were superior, as compared to the pre-intervention (P<0.05). There were no significant differences in other stroke-related knowledge and behaviors and health belief in comparison of the pre-and post-intervention (P>0.05). Conclusion The community-based intervention can improve parts of stroke prevention knowledge and health behavior among hypertensive patients. The intervention needs to be modified based on ecological models of health promotion and health belief model.
