2.Effective comparison between nonoperative and operative treatment in calcaneal fractures

[Objective]To evaluate the effect of nonoperative and operative treatment in calcaneal fractures.[Method]Operative effect of 38 cases of calcaneal fracture of all kinds(42 foots)were analyzed.Accordind Essex-Loprestis classification,6 foots suffered type I tongue fracture,28 foots suffered compressed fracture(type Ⅱ),8 foots suffered serious comminuted fracture(type Ⅲ).All patients underwent open reduction interal fixation by Buttress plate(AO calcaneal fracture plate or simulated AO calcaneal fracture plate made domestically) from the extended "L" lateral approach.Postoperatively,no patient needed plaster fixation.All patients started non-weight-bearing fuctional exercise at 2-3days,gradual weight-bearing walking at 6-8 weeks.The result of operated group was compared with nonoperative treatment group.The follow-up period was from 10 months to 36 months,with an average of 17monthes.[Result]Assessed the effect by Maryland foot score,the base line included every foot.Total excellent and good rate in operative group was 93%,it was 100% in type I fracture group,96% in type Ⅱ fracture group,75% in type Ⅲ fracture group.The result of nonoperative treatment group was:total excellent and good rate was 67%,it was 92% in type I fracture group,77% in type Ⅱ fracture group,there was not excellent and good patients in type Ⅲ fracture group.[Conclusion]It is important to regain the flatness of subtalar joint and calcaneocuboid joint,maintain the normal shape of foot.The authors recommend that type Ⅱ and type Ⅲ fractures of calcaneus according Essex-Loprestis classification were treated with open reduction and interal fixation by Buttress plate.By selecting the appropriate approach,proper reducting and postoperative treating rightly,it is an ideal measure to treat calcaneal fracture by selection of treatment method acciding to patient's individual status and fracture type.
3.Clinical significance of continual monitoring multiple serum biochemical factors after traumatic fracture

[Objective] To investigate the clinical significance of changes in serum biochemical factors in trauma patients,and to improve the initial determination of the injury degree and rescue ratio.[Method]From April 2002 to December 2004,48 trauma patients were divided into two groups of serious and slight injury by ISS score.The author monitored the concentration of serum GLU,CR,BUN,LDH,CK,CKMB,LDL,GPT,GOT,IgG,IfA,IgM,C3,C4,CRP and the WBC,PLT,BT(body temperature)at the first day and after 3,7,14 days,recorded the therapy method and results.The difference between the two groups was compared by statistic method.[Result]In 48 patients,There were 12 in slight injury group,36 in serious injury group.The concentration of GLU,LDH,CK,CKMB,CRP in serious group was more than that in slight group,but the concentration of LDL,IgG,IgA,IgM,C3,C4 was lower(P0.05).There were 6 patients suffered MODS,2 cases suffered ARDS,3 patients suffered upper digestive tract bleeding,2 cases combined with infection,1 case suffered DIC,and I death in serious group;there was 1 MODS,1 infection and no death in slight group.[Conclusion]The combined use of the AIS-ISS90 score and serum biochemical factors is advantageous to the determination of injury degree and prognosis.

5.Experiment Research of Alteration of Blood Lipids,Glucose and Blood Goagulation Factors in Hyperlipoidemia

Objective To study the changes and correlations of the blood lipids, glucose and blood coagulation factors in the animal models and patients with lipodystrophia. Methods The models of hyperlipoidemia were set up in 30 rabbits, and 40 cases of the hyperlipoidemia and 40 cases of the healthy subjects randomly were enrolled in this study. Their blood lipids, glucose and coagulation factors were detected. Results The serum levels of TC,TG,LDL-C and Glu in the rabbit models and patients with hyperlipoidemia notably increased compared with controls(P0 05). HDL-C level increased in the rabbit models(P0 05). Conclusions The change of the blood lipids level in the rabbit models was different from that of the patients with hyperlipoidemia. But both may company the abnormality of carbohydrate metabolism, and hyperlipoidemia may affect the endogenous blood coagulation system to cause the blood to present the high coagulation.
6.Interfixation of complicate intertrochanteric hip fracture

[Objective]To discuss the effective treatment of complicate intertrochanteric hip fracture.[Method]Thirty-five cases with complicate intertrochanteric hip fracture.The class of fracture,surgery method,operating time,complicatico,union rate of fracture were amounted statisti cally.[Result]All the 35 cases were surgical treated,l 4 cases with PFN(priemeral femur nail),21 cases with DHS(dynamic hip screw)with mean time of operation 1.8 h(1.5~3 h),mean amount of blood loss during operation 300 ml(150~500 ml),mean time in bed 2.4 monthes (1.2~3 monthes),mean following-up time 12 months(6~16months).Thirty-one cases got union,3 cases union time delayed,1 case nounion,union rate was 88.5%.Four cases suffered hip iversion( 11.4%),the screw in the neck removed in 6 cases(8.5%),5 cases feeled pain in hip( 14.2% ).[Conclusion]The complicate intertrochateric hip fracture is a kind of fracture which can not be treated easily.Using PFN and DHS can not control the stability effectively.Additional fixation of the greater trochantar fracture is necessary.Added out-fixation and avoid early bearing are useful.
7.Multi-incision with multiple low-profile plates for fixation of Rüedi-Allg(o)wer type Ⅱ and Ⅲ pilon fracture

Objective To assess the outcome of fixation of Rüedi-Allg(o)wer type Ⅱ and Ⅲ pilon fracture using multiple low-profile plates (MLPP) via multiple incisions.Methods A retrospective study was made on 21 cases of pilon fracture immobilized using MLPP via multiple incisions between July 2007 and March 2012.There were 7 cases of Rüedi-Allgower type Ⅱ and 14 type Ⅲ.Fractures were reduced and fixed based on X-ray films and CT scans for fracture line and effect area.Quality of reduction was evaluated by Burwell-Charnley radiological score.Ankle and hindfoot functions were evaluated by American Orthopedic Foot & Ankle Society ankle-hindfoot scale.Fracture types,healing rate,and wound complication incidence were recorded.Results All fractures were healed,with the excellence rate of 95% (20/21).According to AOFAS score,the results were rated as excellent in 11 cases,good in 6,fair in 4,with the excellence rate of 81%.None suffered from wound infection,except for wound dehiscence in 1 case and traumatic arthritis in 5.Conclusion Treatment of pilon fractures by MLPP via multiple incisions can reduce wound complications,promote reduction and stabilization of fracture,and satisfy the demand of early movement and late weight bearing of the ankle.
8.Primary reamed intramedullary nailing for femoral shaft fractures associated with thoracic injuries

Objective To investigate complications and mortality of primary reamed intramedullary nailing for femoral shaft fractures in patients a ssociated with thoracic injuries. Methods A retrospective a nalysis of trauma data was performed and the case selection based on the followi ng criteria: (1) patient age of 17-65 years, (2) patients with multiple injury ( ISS≥16) as well as complicated thoracic trauma (AIS≥2), (3) hospitalization stay ≥48 hours, (4)no serious pre-existing medical diseases in history, (5) as sociated femoral shaft fractures treated with reamed intramedullary nailing, ext ernal fixation, plate, cast, traction and unreamed intramedullary nailing. The s elected patients were divided into two groups according to the interval from inj ury to operation, ie, those within 24 hours as Group A and beyond 24 hours as Gr oup B. Results During the study period, 96 patients met the criteria for the investigation. There were 57 patients in the Group A and 39 pa tients in the Group B. 9.Prognosis and management of recurrent primary clear cell carcinoma of liver

Objective To evaluate the prognosis and management of recurrent primary clear cell carcinoma of liver (PCCCL).Methods 214 patients with PCCCL treated by curative resection from January 1996 to March 2006 were retrospectively studied.Tumour recurrences were classified into early (≤1 year) and late (>1 year) recurrences.Results Of 99 patients who developed recurrences,28 developed early recurrence while 71 developed late recurrence.The patients with recurrences were treated with re-resection (n=33),percutaneous ethanol injection (PEI,n=7),radiofrequency ablation (RFA,n=10),transcatheter arterial chemoembolization (TACE,n =27),systemic chemotherapy (n=1),Chinese medicine (n=1),and conservative management (n=20).The re-resection rate was higher in the late than in the early recurrence group (P=0.04).In this study,reresection,PEI,and RFA were considered as curative therapies.There was no significant difference in the overall survival (OS) for patients who received these different curative therapeutic procedures (P=0.68).The 1,3-,and 5-year OS of patients with recurrences who were treated with curative treatment were comparable to those patients who did not develop recurrences (100%,86.0%,63.5% vs 85.2%,72.2%,64.3%,P=0.71).The 1-,3-,and 5-year OS of patients who received TACE for recurrences were 100%,66.7%,and 44.4% respectively.The results were poorer than patients who received curative treatment for recurrences (P=0.03),but were better than those who received conservative management after recurrences (80.0 %,25.0 %,and 10.0 %,P< 0.01).Conclusions Reresection,PEI and RFA are optimal curative methods for recurrent PCCCL.TACE plays an important role in the management of patients with recurrent PCCCL who cannot be treated with curative methods.
10.Effect of team-based rehabilitation education on daily life ability of elderly patients having undergone operations on hip fractures

Objective To evaluate the effect of team-based rehabilitation education on daily life ability of old patients having undergone operations on hip fractures.Methods Sixty elderly reconvalescents from operations on hip fractures were equally assigned into the intervention and control groups according to admission sequence:The intervention group received rehabilitation education by community nurses at months 1,3 and 6 and the control group were managed with convalesce at home.The two groups were compared in terms of daily life ability at the three time points. Result The daily life ability of the intervention group was better than that of the control group at all the three time points(P<0.05). Conclusion The team-based rehabilitation education is effective for the enhancement of daily life ability of old patients having undergone operations on hip fractures.
