Objective To observe the long-term smokers with primary hypertension patients characteristics and changes in the circadian rhythm of blood by non-invasive 24-hour ambulatory blood pressure monitoring (ABPM),so as to long-term smokers with primary hypertension the prevention and treatment of patients provide a theoretical reference.Methods A total of 200 cases with essential hypertension from February to October 2012 were divided into 2 groups,group A of 100 patients with longterm smoking (smoked ＞ 10 years the daily smoking 30-40 sticks) with primary hypertension crowd;group B of primary hypertension in 100 patients with non-smoking.All patients underwent 24-hour ABPM the patient's consent,were disabled antihypertensive drugs 1 week,its non-invasive 24-hour ABPM,two groups were compared for 24 h average systolic blood pressure (24 h SBP),24-hour average diastolic pressure (24 h DBP),diurnal mean systolic blood pressure (dSBP),mean nighttime systolic blood pressure (nSBP),daytime average diastolic blood pressure (dDBP),night mean diastolic blood pressure (nDBP)and nighttime systolic blood pressure decreased percentage.Results 24 h SBP,24 h DBP,dSBP,nSBP,dDBP,nDBP in group A were higher than those in group B [(139.60 ± 11.69) mmHg (1 mmHg =0.133kPa) vs.(133.90 ± 12.73) mmHg,(82.03 ± 7.44) mmHg vs.(72.58 ± 8.97) mmHg,(152.50 ± 9.79)mmHg vs.(141.30 ±8.92) mmHg,(84.97 ±7.65) mmHg vs.(76.24 ±8.06) mmHg,(138.20 ±8.73)mmHg vs.(127.00 ±9.74) mmHg,(78.52 ± 10.49) mmHg vs.(68.45 ± 11.01) mmHg,and there were statistically significant differences (P ＜ 0.05); hypertension percentage at night systolic blood pressure in group B was lower than that in group A [(6.23 ± 1.79)％ vs.(10.14 ±2.46)％],there was significant difference (P ＜ 0.05).Conclusions Long-term smoking can make the average blood pressure of essential hypertension crowd 24 h,daytime blood pressure and elevated blood pressure at night,and the long-term smoking population in essential hypertension circadian adjustment feature to affect larger,so as soon as possible to promote smoking cessation and long-acting antihypertensive drugs,thereby reducing the incidence of heart,brain,kidney and other target organ damage.

Objective To investigate the relationship between helicobacter pylori(HP)eradication and curative effect of gastroe-sophageal reflux disease(GERD) .Methods Two hundred and fifty four patients with GERD were prospectively recruited for endo-scope and acid reflux evaluation .According to the results of gastroscope ,the eligible patients were divided into non-erosive reflux disease (NERD) and erosive esophagitis (EE) group ,and each divided into HP positive and HP negative group ,the HP positive group were randomly divided into the anti HP treatment group and not .These patients were assigned to rabeprazole triple therapy (the anti HP treatment group) or rabeprazole(20 mg ,twice per day) single(other groups) for 10 d ,then rabeprazole (20mg ,twice per day) was given to all patients until two months .Then all patients underwent acid reflux evaluation ,EE group underwent endos-copy ,and all patients underwent a 13 C urea breath test 4 weeks after cessation of rabeprazole to determine HP status .Results One hundred and seventy nine GERD patients were included in the study(NERD group 119 cases ,including anti HP treatment group 47 cases ,not anti HP treatment group 32 cases ,HP negative group 40 cases ;EE group 60 cases ,including anti HP treatment group 24 cases ,not anti HP treatment group 15 cases ,HP negative group 21 cases) .HP was eradicated in 76 .6% of the NERD anti HP treatment group and in 75 .0% of the EE anti HP treatment group .Overall ,there is no difference between the anti HP treatment group and other groups on these aspects ,including symptom、esophagitis and acid reflux improvement .Conclusion There are no significant correlation between HP eradication and GERD .

OBJECTIVE:To observe the clinical efficacy and safety of lactulose combined with Bacillus subtilis bifarous via-ble bacteria in the treatment of functional constipation(FC). METHODS:In a randomized double blind controlled study,60 FC pa-tients were randomly assigned into treatment group(28 cases)and control group(32 cases). Both groups received lactulose orally. Treatment group additionally receive B. subtilis bifarous viable bacteria,and control group additionally received placebo continually. 2 weeks later,both groups stopped taking lactulose but continued to take adjunctive drug till 4th week. The defecate frequencies, stool property,constipation symptoms,quality of life and ADR were compared between 2 groups before treatrnent and 2nd,4th week cofter treatment. RESULTS:In the second week ,the defecate frequencies,stool property and constipation symptoms of both groups were significantly improved,with statistical significance (P<0.05). Compared with the control group in the fourth week, these constipation symptoms of treatment group were improved more significantly,with statistical significance (P<0.05). In the fourth week,aspects of quality of life in both groups were significantly improved,with statistical significance(P<0.05);physical discomfort scores,satisfaction scores and total scores of treatment group were improved significantly,compared with the control group,there was statistical significance (P<0.05). There were no severe ADR in both groups. CONCLUSIONS:Lactulose com-bined with B. subtilis bifarous viable bacteria can effectively relieve FC,improve abdominal symptoms and slow the recurrence of constipation,which is safe and reliable.

Objective To study the chemical constituents and structures of the glycosaminogly-cans isolated from the leftover bits of the Chlamys farreri and Argopecten irradisus. Methods The qualitative and quantitative analyses of monosaccharides obtained by alcoholyses with HCl-methylalcohol from the samples were done by gas chromatography with standard contrast and inner standard methods. Mixed with KBr and pressed into pellet, the samples were analyzed by infrared spectrometer scanning from 4000-500 cm-1. Results The contents of monosaccharides from the samples were rhamnose 0. 75%, xylose 0. 63%, fucose 0. 67%, mannose 0. 97%, glucose 1. 11% and galactose 1. 59%, respectively. The infrared spectrum showed that the samples had typical infrared spectra of glycosaminoglycan. Conclusions The glycosaminoglycan isolated from the leftover bits of the Chlamys farreri contains neutral monosaccharides. Compared with standard glycosaminoglycans, the infrared spectra of the two samples are similar to that of hyaluronic acid.

Objective To analyze the clinical treatment effect of non-gland-derived mass on the neck,and discuss the distribution types and the best treatment options.Methods A total of 76 cases with non-gland-derived mass on the neck were analyzed retrospectively,and the treatment effect was analyzed.Results In the 76 cases,25 cases were benign tumors,19 cases were malignant tumors,32 cases were non-rumors.Surgical treatment was preferred to tumors.According to the nature of the lesions,non-tumors mass shoud choose surgical treatment or medication.Conclusion Non-gland-derived mass on the neck with multiple presentations,it is necessary to choose personal therapies according to the characteristics of masses.

Objective To observe influence of retinoic acid on the lung damage and expression of Connexin26(Cx26) of neonatal rats suffered from hyperoxia.Methods Twenty-four 3-day SD rats were randomly divided into three groups:retinoic acid+ hyperoxia,hyperoxia,air group;rats in the group of hyperoxia were suffered 7 days hyperoxia (the level of oxygen was above 950 mL/L),air group was instead of air.All rats were weighed,value of radical alveolar counts(RAC) and microstructure of the lungs were observed by HE staining under light and electron microscope respectively;and the expression of Cx26 in the lungs was determined by immunohistochemistry.Results Compared with air group,weight,value of RAC were both decreased obviously in hyperoxia,retinoic acid+hyperoxia groups[(13.53?1.27),(13.38?1.29),(17.37?0.89)g,F=30.19 P=0;11.15?1.33,12.49?1.47,13.94?0.98,F=9.59 P=0),while the expression of Cx26 increased [(13.68?1.28)%,(17.82?1.72)%,(19.69?1.77)%,F=29.36 P=0].Conclusion Retinoic acid has a protective effection from damaged lung of neonatal rats suffered from hyperoxia,and it may be related to the alteration of expression of Cx26 protein in the lungs.

Objective To evaluate the diagnostic value of Epworth sleepiness scale(ESS) combined with portable sleep monitor (PM) in patients with obstructive sleep apnea syndrome (OSAHS).Methods The questionnaire of ESS was used to evaluate 156 patients with snoring,and then line PM check,monitoring apnea hypoventilation index (AHI),oxygen reduction index (ODI),the lowest oxygen saturation (LSpO2),average blood oxygen saturation (MSpO2).Results One hundred and twenty-four patients were diagnosed to be OSAHS and 32 patients be simple snore by PM in 156 patients.A total of 124 cases,including mild 42 cases,moderate 47 cases,severe 35 cases.There were significant difference in ESS score,AHI,ODI,LSpO2 and MSpO2 between OSAHS in various degrees and patients with simple snore [simple snore:(5.8 ± 2.9) scores,(3.2 ± 1.3) times/h,(3.3 ± 2.6) times,0.911 ± 0.121,0.973 ± 0.132;mild OSAHS:(9.7 ± 3.5) scores,(12.8 ± 23.3) times/h,(14.2 ± 5.7) times,0.875 ± 0.083,0.902 ± 0.095 ; moderate OSAHS:(12.9 ±4.8) scores,(27.6 ± 4.5) times/h,(25.6 ± 6.1) times,0.816 ± 0.069,0.851 ± 0.071 ;severe:(16.1 ± 5.2)scores,(43.6 ± 7.6) times/h,(38.6 ± 7.8) times,0.701 ± 0.061,0.795 ± 0.063] (P ＜ 0.05).With the severity of OSAHS increased,ESS score,AHI,ODI increased gradually,LSpO2,MSpO2 gradually declined,there were statistically significant differences (P ＜ 0.05).ESS score was positively correlated with AHI and ODI (r =0.42,0.51,P ＜ 0.05),and negatively correlated with LSpO2 and MSpO2 (r =-3.70,-4.50,P ＜ 0.05).Conclusions There is a good relativity between ESS score and the monitoring index of PM.Combination of ESS with PM could be used as a primary diagnostic method in patients with OSAHS,especially in primary-care hospitals.
