1.Intramuscular Lipoma of the Cheek: Report of Two Cases

Two cases of intramuscular lipoma occurring in the cheek of a 39-year-old and of a 55-year-old Japanese males are presented. These were excised by the intraoral approach. Histopathologically, these lesions were composed of the adipose cell-like tumor cells infiltrating in the striated muscles. There has been no evidence of tumor recurrence after operation.
2.An Investigation on Evaluation Structure of the Patient for Ingestion Instruction in Community Pharmacies

Objective: Many studies on patient satisfaction are conducted in community pharmacies. In contrast, the present study assess the community pharmacy function and to the best of our knowledge, appears to be the first to evaluate the professional functions of pharmacists.
Methods: In September 2010, in 1 week, we conducted a survey that focused on pharmacists’ professional abilities. I subjected the responses to factor analysis and covariance structure analysis.
Results: We obtained 2,506 effective responses (appropriately completed and returned questionnaires) of the 4,633 questionnaires originally distributed. Data of seven dimensions were obtained for factor analysis. In all, the various information provision services rendered by a pharmacist were not significant. According to the covariance structure analysis, “safety” (a pharmaceutical management item) and “responsiveness” (a complimentary element) were significant factors. Differences in the degree of influence were confirmed for each parameter in the analysis, depending on the parameter. In addition, the information provision services were significant during the acute period.
Conclusion: Because the information provision services were not significant, the influence of “asymmetric information” warrant further analysis according to the specialty. I believe that it is necessary to examine asymmetric information in greater detail in the future. In addition, I think that an interventional study that is based on these results is also necessary.
3.Empirical Study on Influencing Factors for Evaluation of Medical Service and Intention to Revisit of Patient Perspective in Community Pharmacies

Objective: As a “family pharmacy and pharmacist,” high-quality medical service is demanded. Many patient satisfaction investigations are performed to evaluate community pharmacies, but it has been impossible until now to do so using an improvement index of the medical quality. Therefore, I investigated the influence and evaluation structure of two evaluation concepts regarding quality of medical service and patient satisfaction and intention to use again, which becomes the important key as a “family” pharmacy.
Methods: I used anonymous patient survey data gathered for the purpose of duties improvement in July 2009. I subjected the responses to factor and covariance structure analyses.
Results: Dates for four dimensions were obtained for factor analysis. As a whole, covariance structure analysis showed that, in terms of privacy and only for service quality, entertainment primarily influenced satisfaction. The ingestion instruction had a big effect on both sides. General satisfaction was also substantially influenced by service quality. The difference in influence degree was confirmed at each parameter.
Conclusion: It was found that not only satisfaction but also quality of service were equally demanded in community pharmacies. It is necessary to increase these because most are influenced by ingestion instruction as a factor in service quality. However, future investigation is necessary to clarify the detailed ingestion instruction contents.
4.Benign Fibrous Histiocytoma of the Upper Lip

A rare case of benign fibrous histiocytoma arising in the upper lip in a 76-year-old female is presented. A well-defined tumor measuring 1 cm existed in the submucosal area of the right side of the upper lip. With the patient under local anesthesia, the tumor was excised. Microscopically, the excised tumor was diagnosed as a benign fibrous histiocytoma. There has been no evidence of tumor recurrence after the operation.

6.THE EFFECT OF A SHORT-TERM WEIGHT-LOSS PROGRAM IN OBESE MEN WITH SLEEP DISORDERED BREATHING

A number of studies have shown that sleep disordered breathing (SDB) has a strong relation with obesity. The purpose of this study was to examine the effect of a short-term weight-loss program in obese men with SDB. In our 14-week weight-loss program, forty-one obese men (mean±SD, age ; 49.6±10.8 yr, body mass index ; 27.9±2.5 kg/m2) were assigned to 2 subgroups : diet only (DO ; n=19) and diet plus aerobic exercise (DE ; n=22). 2%, 3%, and 4% oxygen desaturation index (ODI) were measured by pulse oximetry before and after the weight-loss program. Weight and %fat significantly (p<0.05) decreased in the total subject pool consisting of DO plus DE (weight ; -8.4±3.5 kg, %fat ; -7.5±3.5%). 2%, 3%, and 4% ODI significantly decreased by -3.46±5.01 event/hour, -2.37±3.57 event/hour, and -1.99±2.84 event/hour, respectively. Significant correlations were found between 2%, 3%, and 4% ODI at baseline and changes of 2%, 3%, and 4% ODI during the weight-loss program (2% ODI ; r=0.48, 3% ODI ; r=0.51, 4% ODI ; r=0.67). Weight loss and %fat loss did not differ significantly between DO and DE (DO : -7.6±3.2 kg, -6.8±3.2%, DE : -9.2±3.7 kg, -8.1±3.7%). The increase in maximal oxygen uptake was slightly larger for DE (4.7±4.6 ml/kg/min) compared to DO (2.5±3.3 ml/kg/min), but there was no significant interaction. Changes of 2%, 3%, and 4% ODI did not differ significantly between groups. These results suggest that for obese men with SDB, the weight-loss program is an effective method, leading to improvement in SDB, although the combination of aerobic exercise to diet may not produce additional effects to SDB, compared with the diet only.
8.Study on Evaluation of Efficiencies for Drug information by Video and Paper Media in Community Pharmacies

Objective: While pharmacies focus on interpersonal work, patients need to process a lot of information. Therefore, it is important for the pharmacy to provide information according to the situation. This study was conducted for the purpose of providing a video and paper media, and clarifying that it is useful to provide information in various mediums about the method of administering the powdered medicine, which requires an understanding of the operation.Method: Conducted a survey for those who brought prescriptions for infants from March 2016 to about a year and a half. We compared and verified the understanding of the medication method for infants between the paper medium group and the moving image medium group.Results: Differences in the background between the two media use groups were clarified, and path analysis confirmed differences in the factors that affect the reliable administration of each medium to infants. It was revealed that the evaluation of moving image media was higher than the comparison between the groups in terms of usability. Although there was no difference in the ability to use the medium at his own pace, the moving images were significantly higher in understanding the motion.Discussion: There is a limit to the understanding of motions with a single explanation at the pharmacy counter. It was confirmed in this study that information provision using a medium that can be used depending on the situation of the patient is useful as a complementary role in medication instruction. Providing information while appropriately combining information provision in various media is thought to reduce the burden on patients and contribute to improved adherence.
9.Results of a Survey on the Present Status of Undergraduate Clinical Training and Plans for Its Improvement.

We used questionnaires to study the present status of undergraduate clinical training at medical schools in Japan in February 1996. Completed questionnaires were returned by 81%(65) of 80 medical schools and approximately 54%(1, 328 clinical departments) of the schools. The results were as follows. Courses for early clinical exposure in the 1st or 2nd year were provided at 83% of the 65 schools; clinical clerkships in the 5th and 6th years were provided at 28%. Specific behavioral objectives for clinical training were clearly shown to students and teaching staff at 75% of schools. Clinical procedures that medical students were permitted to perform were listed and announced to students and teaching staffs at 66% of schools. Patients were informed and gave consent for clinical training of students at 77% of schools. Essential knowledge and skills of students were assessed before the start of clinical training at 40% of schools, and summative assessment was made at the end of the training at 72%. Training of clinical teaching staff for faculty development was conducted at 51% of schools. Eightynine percent of schools reported a shortage of clinical teaching staff. Similar results were obtained in the survey of clinical departments of university hospitals: most departments complained of a shortage of teaching staff, of students not being active, and of students not being competent to enter clinical training courses. To improve clinical training, the introduction of clinical clerkships and cooperation with community facilities outside universities were the main issues.
10.A Survey on Undergraduate Clinical Training with Special Reference to Clinical Procedures Performed by Medical Students on Patients.

A questionnaire survey on clinical procedures performed by medical students on patients during undergraduate clinical training was conducted in february 1996. Responses were received from 1328 clinical departments of university cospitals at 80 medical schools. Basic clinical procedures that medical students were permitted to perform on patients were recommended by a committee of the Ministry of Health and Welfare. These procedures are divided into three categories: level 1; procedures that medical students are permitted to perform under the supervision of an instructor; level 2; procedures medical students are permitted to perform with supervision under certain conditions; and level 3; procedures for which medical students are generally limited to assisting instructors or to attending and observing patients. The status of performance of the procedures was investigated. Of level-1 procedures (36 procedures), 8 were performed by medical students at more than 80 % of university hospitals, 19 were performed at from 50% to 70%, 9 were performed at less than 50%. Of level-2 procedures (15 procedures), 8 were performed at from 55% to 79% of hospitals and 7 were performed at less than 50%. For level-3 procedures (15 procedures), medical students were permitted to assist and observe 4 procedures at from 82% to 86% of hospitals, 11 at from 50% to 79%, and 1 at40%. In addition, students were permitted to perform 13 level-3 procedures at from 10% to 44% of hospitals and to perform 3 at from 6% to 9%. In many clinical departments, other kinds of procedures specific to the departments were adopted. Teaching media, such as standardized patients' computer-assisted instruction models, and animal materials, were used, and facilities in the community cooperated in training. Respondents wrote many suggestions and opinions about the difficulties and concerns with the legality of students' performing clinical procedures, patients' consent or agreement, minimal essentials of clinical competence of students, the shortage of instructors, and the training and guidelines for instructors.
