1.Research progress of diagnosis and treatment in primary breast diffuse large B-cell lymphoma

Primary breast lymphoma is a rare form of extranodal lymphoma with distinct characteristics.It comprises diverse histologic subtypes with diffuse large B-cell lymphoma (DLBCL) as the most common one.In this review,the primary breast DLBCL was focused and the definition,staging,the role of systemic chemotherapy,local adjuvant radiotherapy and rituximab in treatment of primary breast DLBCL were described.
2.Role of radiotherapy in lymphoma treatment and its updation

Lymphoma is a group of malignancies that affect lymphatic system of the body,and often originates from the abnormal cloned proliferation of B,T or NK cell lineage.It contains mainly two categories:Hodgkin lymphoma (HL) and non-Hodgkin lymphoma (NHL).The treatment options consist of chemotherapy,radiotherapy,immunotherapy and radioimmunotherapy,et al.It has been demonstrated that lymphomas are typically radiosensitive tumors and undergo apoptosis readily after radiation therapy(RT).RT produces very high local complete response rate for all lymphoma subtypes.This review will mainly focus on the role of RT in the treatment of aggressive lymphoma,indolent lymphoma,extranodal lymphoma and HL,as well as the application of modem RT techniques.
3.Exclusion of an outbreak of human brucellosis

Objective To prevent misdiagnoses of brucellosis and misjudgment of brucellosis outbreak through differential diagnosis between influenza and brucellosis of three new cases who were in the same work unit and underwent the longest latent period.Methods Contact history of the three new cases was investigated epidemiologically and physical examination and blood test were taken.Brucellosis antibodies of the cases and abortion cow were tested by serum tube agglutination test (SAT),and pathogen was isolated.The tonsilla and pharyngeal secretion substance of cases were collected for detection of influenza virus RNA by real-time PCR.Results The cases had contact history with brucellosis abortion cattle and/or the animal contaminated environment,and had a fever,fatigue and other symptoms.Total blood cell(WBC) was normal and proportion of lymphocyte was increased.The antibody titer in SAT test was 1:200(+++),but pathogen isolation was negative for all three cases.Brucellosis antibody of abortion cow was positive and antibody titer was 1:800 (++).The test of Bv RNA of influenza virus was positive in the three cases.Conclusions The three cases are brucellosis latent infection combined with influenza and we have excluded a misjudgment of pestilence of human brucellosis outbreak.
4.Comparison of the treament to adhesion ileus between laparoscopic surgery and traditional surgery

Objective To compare the treament to adhesion ileus between laparoscopic surgery and traditional surgery.Methods 59 cases of patients with adhesion ileus were selected.32 patients were given laparoscopic adhesion ileus loose solution method.And the other 32 patients were given traditional surgery.Results The operation time(t =12.135,P =0.002),peristalsis recovery time (t =9.053,P =0.001),postoperative infection rate (x2 =8.346,P =0.004),obstruction again (x2 =4.863,P =0.026) and hospital stays (t =4.724,P =0.028) in laparoscopic surgery group was significantly better.The differences between both groups were statistically significant (P <0.05).Conclusion Compared with the traditional open surgery,laparoscopic adhesion ileus loose solution with surgical trauma is small.Operation time and the postoperative recovery time is short,It can be used as the main treatment of adhesion ileus.
5.The Controversy of Surrogacy and its Ethical Analysis under the Perspective of Population and Social Vicissitude

With the aggravation of population aging and the change of social convention,especially after the introduction of two-children policy,some families wish to have two children can not be achieved due to various reasons.Therefore,they use assisted reproductive technology to achieve fertility aspirations.Surrogate is a controversial topic in the assisted reproductive technology,the contradictions between the normal reproductive rights of the infertile woman and the women who have lost their children but passed the age of childbearing and the bad ethical consequences and rights disputes of surrogacy.How to avoid the bad ethical consequences and meet the demand of reasonable surrogate is the major issue worthy to be discussed.
6.Advances of the Research on Mechanisms of Breast Cancer Escaping from Host Immune Surveillance

Objective To review the mechanisms of breast cancer escaping from host immune surveillance. Methods The current literatures on the mechanisms of breast cancer cells escaping from host immune surveillance were reviewed in the following aspects: alterations of MHC-Ⅰmolecule phenotype, deficiency of costimulatory molecules, apoptosis of T-lymphocytes induced by breast cancer cells presenting Fas ligand (FasL) and TNF-related apoptosis-inducing ligand (TRAIL) and host immune tolerance induced by tumor cells. Results Loss of classical antigen-presenting human leukocyte antigen (HLA) class-Ⅰmolecules, expression of non-classical HLA class-ⅠmoleculesHLA-G, loss of costimulatory molecule B7, apoptosis of T-lymphocytes induced by tumor cells presenting FasL and TRAIL and antigen-inducing host immune tolerance were related to breast cancer escaping from immune surveillance. Conclusion Breast cancer cells escape from host immune surveillance by altering MHC-Ⅰmolecules, lacking of costimulatory molecules, inducing of apoptosis of T cells and host immune tolerance. But the factors resulting in breast cancer escaping from host immune surveillance are not yet clear and should be further studied.
7.A Study on the Relationship of Time Management Disposition and Coping Style, Personality Characteristics of College Students

Objective: To study the relation between college students' time management disposition and their coping styles and personality characteristics.Methods: 282 college students were investigated with Time Management Disposition Scale,The Simplified Coping Style Questionnaire and Y-G Personality Test.Results: ①Time management disposition of college students was obviously correlated with their coping style.It was positively correlated with positive coping style in totality and each dimension,and was negatively correlated with negative coping style in totality and sense of time control,and sense of time efficiency.②Some factors of time management disposition had significant correlations with some traits of Y-G Personality Test.Conclusion: Time manage disposition was obviously correlated with coping styles and personality characteristics.
8.Diagnosis and treatment of 7 cases of lacerated fractures of the inferior patella in children

Objective To analyze the diagnosis and treatment of lacerated fractures of the inferior patella in children. Methods Totally 7 children with lacerated fractures of the inferior patella have been treated by surgery since 1989. Their ages ranged from 8 to 15 years old, and symptom duration from 1 hour to 6 days. 3 of them were fixed by Kirschner wire and tension belt, and 4 sutured with absorbable suture line in the shape of “8”. The evaluation of treatment effects was done according to the relevant knee joint scoring system. Results Postoperative follow ups ranged from 1 to 4 years. The radiograms after operation showed that the fractures united completely. The joint functions of all patients were excellent. Conclusion In the absence of MR (magnetic resonance), the lacerated fractures of the inferior patella can be diagnosed by analyzing the case history, symptoms, physical signs and the results of patella position measurements, before definite confirmation by surgical check-up.
9.Value of clinical manifestations in predication of prognosis of patients with diffuse axonal injury

ObjectiveTo investigate the relationship of clinical manifestations with prognosis of patients with diffuse axonal injury (DAI) and select the indices that can accurately evaluate the prognosis so as to create a predictive model of outcome of DAI patients.MethodsClinical data of 98 DAI patients were retrospectively analyzed to identify the relationship between the indices and outcomes through Cochran-Mantel-Haenszel x2 test or Student-Newman-Keuls one way variance test via SAS 8.1.Then,the indices was analyzed through multivariant cumulative stepwise way so as to establish the Logistic regression model. Results (1)The indices including Glasgow Coma Score (GCS) on admission,combined with other type of cerebral injuries,unstable vital signs,abnormal pupil size and reactivity,decerebrate rigidity,imaging with characteristic DAI signs,paralysis of the limbs and length of coma were correlated with the outcome,while the indices including gender,age,injury mechanism,pathologic signs and decorticate rigidity were insignificantly associated with the outcome.(2) The logistic regression model showed that GCS on admission,paralysis degree of the limbs,abnormal pupil size and reactivity,and combined with cerebral injuries were the optimum predictors of outcome of the DAI patients.ConclusionsThe logistic regression model in the study is a reliable tool in predicting the prognosis of DAI patients.The patients wit higher GCS on admission may have better outcome.The patients with light degree of paralysis,normal pupil size and reaction,and combined with none cerebral injuries may have sound prognosis.
