1.The clinicopathologic characters and prognosis of cervical cancer in 831 young women under 35 years old

Background and purpose:Several prior studies have evaluated that the overall incidence of cervical cancer has decreased in the past 30 years, while it seems that the age of mortality is younger, especially young women under 35 years old. The prognosis of cervical cancer in young women still needs to be determined. The purpose of this study was to determine the main clinicopathologic characteristics of cervical cancer in young women aged under 35 years and the influence of age on prognosis. Methods:A retrospective review was performed for 831 patients under 35 years old , 1 737 cases over 35 years old was used as a control group. Clinical and pathologic variables including clinical stage, pathological type, tumor size, lymph node metastases(LNM),depth of invasion(DI) and lymphovascular space involvement (LVSI)were analyzed using Peason chi-squowe test. The 5-year survival and 2-year recurrence rates were analyzed by Kaplan-Meier method.Results:The younger patients were characterized by a higher rate of early stage with 64% vs 37.8% in control group(P=0.000),non-squamous cell carcinoma were 14.90% in the study group and 6.80% in the control group(P=0.000),bulky lesion were 52.00% vs 30.44%(P=0.000), LNM were 33.46% vs 23.93% (P=0.000),deep stroma invasion(DSI) were 51.88% vs 39.94% (P=0.005),There was no significant difference in LVSI for two groups with 37.03% in the young group and 38.26% in the control group (P=0.663).The clinical stage and tumor size of young patients were related with LNM,DI and LVSI(P

Objective To investigate the effects of ramipril on nitric oxide(NO) concentration and endothelial nitric oxide synthase(eNOS) activation in lung of rats with pulmonary arterial hypertension induced by monocrotaline(MCT).Methods Sixty Sprague-Dawley rats were randomly divided into three groups: normal control group,MCT group and ramipril group.Rats in MCT group and ramipril group were subcutaneously injected with 60mg/kg of MCT.Then the rats in ramipril group received ramipril gavage and rats in MCT group received normal saline gavage for 4 weeks,respectively.Rats in control group were subcutaneously injected with normal saline first and then received normal saline gavage for 4 weeks.Right ventricular systolic pressure(RVSP) and right ventricle hypertrophy index(RVHI) were measured.The ratio of arteriol wall thickness/vascular external diameter(WT%) and wall area percentage(WA%) were evaluated.NO concentration in lung was determined.eNOS,P-Ser1177-eNOS and Akt phosphorylation were analyzed by Western blotting.Results The RVSP,RVHI,WT%,WA% were significantly increased,and NO concentration,the level of eNOS,P-Ser1177-eNOS,the state of Akt phosphorylation were significantly decreased in MCT group compared with that in control group(P

4.Effect of Antipsoriatic Drugs(Tripdiolide etc.)on Leukotriene A4Hydrolase Activity

Objective To investigate the effects of some compounds,bestatin,tripdiolide,etc,on leukotriene A4hydrolase activity.Methods LTB4,product of leukotriene A4hydrolase,was determined by reverse phase high performance liquid chromatography(PR-HPLC).Bestatin,an inhibitor of LTA4hydrolase was used as a positive control.Results Tripdiolide could inhibit LTA4hydrolase activity in a dose-depen-dent pattern.The50%inhibitory concentration values(IC 50 )was2.58?10 -5 ?g/mL.The other compounds,an-thralin,cyclosporin A,tretinoin,calcipotriene,clobetasol propionate,methotrexate,and erythromycin,had no effects.Conclusions Tripdiolide possibly has anti-inflammatory effect through down-regulation of leukotriene A4hydrolase in psoriasis.
5.Studies on the changes and clinical significance of peripheral blood T lymphocyte Subsets in 386 Patients with Hand, Foot and Mouth Disease

Objective To detect the changes of peripheral blood T lymphocyte subsets in patients with hand,foot and mouth disease (HFMD) and investigate its clinical significance.Methods Three hundred and eighty-six patients with HFMD and 78 healthy children were detected the peripheral blood T lymphocyte subsets levels using the flow cytometry (FCM).The patients were divided into 325 common patients and 61 critically ill patients according to the severity of disease.Among the 386 patients,264 were in infection stage and 122 were in recovery stage.Compared the differences of peripheral blood T lymphocyte subsets levels in patients and healthy controls,common patients and critically ill patients,infection patients and recovery patients respectively.Results The patients' numbers of CD3 +,CD4 +,CD8 + cells and CD4 +/CD8 + ratio were (47.4 ± 10.6) %,(24.0 ±8.0) %,(23.2 ± 7.4) % and 1.0 ± 0.5 respectively.These parameters were significantly different with those in the control group ((68.4±7.5)%,(38.2 ±6.9)%,(18.8 ±4.6)% and 2.0 ±0.4 respectively;t =3.16,4.62,2.23 and 2.51 respectively,P < 0.05).The common patients' numbers of CD3 +,CD4 +,CD8 + cells and CD4+/CD8 + ratio were (53.0 ±9.8)%,(26.5 ±7.9)%,(20.4±7.5)%and 1.3 ±0.6,which had significant differences compared with those of the critically ill patients ((42.3 ± 10.9) %,(18.7 ± 4.7) %,(24.2 ± 7.4) % and (0.7 ± 0.4) respectively; t =2.25,2.47,1.91 and 2.32 respectively,P < 0.05).The infection patients' CD3 +,CD4 +,cell numbers and CD4 +/CD8 + ratio were (45.3 ± 10.5) %,(21.6 ± 8.4) %and 0.9 ± 0.5.These parameters were significantly different with those of the recovery patients ((54.7 ±9.9)%,(27.5 ± 9.1)% and 1.4 ± 0.6 respectively; t =2.41,1.87 and 2.92 respectively,P < 0.05).Conclusion The cellular immunity of patients with HFMD was impaired.The T lymphocyte function was suppressed,and the level of suppression was positively correlated to severity of the illness.
7.Exploring case management services for people with severe mental disorder and in a stable ;condition in Zhongshan

Objective To examine the impact of case management on hospitalizations of the chronically and severely mentally ill patients in Zhongshan. Methods Patients with severe and chronic mental illness,aged ≥ 15 years and living in pilot area were divided into two groups naturally since the program of case management launched, 65 cases in the group of case management and 112 patients in the group of standard management. Hospitalizations of the two groups before and after case management were compared. Results Data were analysised with MIXED procedure. Length of stay in days per admission (LOS) of both groups decreased with time in years (F=11.02, P=0.001), and the decline in LOS of case management group was greater than that of standard management group (F=9.02, P=0.003). The average admissions of case management group was more than that of standard management group (F=4.98,P=0.03). There was no significant differences in average hospitalization incidents before and after case management in both groups(case management group 5.13%vs. 5.38%, standard management group 7.14%vs. 8.92%, P>0.05). Conclusions Case management was effective in reducing hospitalizations for a group of Chinese with chronic and severe mental illness, and may contribute to the balance on mental health resources between community and hospital.
8.Establishment of TMJ defect models and evaluation on repair effect of CFR-PEEK material artificial joint in rabbits

Objective:To investigate the method of arthroplasty for reconstructing femporomandibular joint (TMJ) after the establishment of TMJ articular defect models in the rabbits,and to clarify the replacement and repair effects of carboh fiber reinforced polyetheretherketone (CFR-PEEK) material TMJ prostheses,and to provide an experimental basis for the clinical application of CFR-PEEK artificial joints to replace TMJ.Methods:Thirteen healthy adult Japanese rabbits were randomly divided into experimental group (n=6),positive control group (n=4) and negative control group (n =3).The right TMJ articular process of the rabbits in experimental group were resected to establish the joint defect models,and then the CFR-PEEK artificial joints were impanted.The right TMJ articular process of the rabbits in positive control group were resected,and the joint defect models were established without the CFR-PEEK artificial joint implantation.The rabbits in negative control group didn't receive any treatment.The body weights of the rabbits in each group were measured in 13 weeks.The effects of fixation and replacement after artificial joint implantation were assessed by CT imaging.Results:The TMJ articular process defect model was successfully implanted with the CFR-PEEK artificial joint.The CT imaging results after threedimensional reconstruction and CT images by contrast in vitro showed that the CFR-PEEK-implanted artificial joint was fixed well,it could be instead of the normal joint function.Compared with positive control group,the weights of rabbits in experimental group were significantly increased (P< 0.05) at 13 weeks;compared with positive control group,the weights of rabbits in experimental group and negative control group were both significantly increased (P>0.05) at 13 weeks,but there was no significant difference between them (P>0.05).The weights of rabbits in experimental group and negative control group kept normal growth;the weights of rabbits in positive control group was increased slowly,and even stopped growing or was negative.Conclusion:The CFR-PEEK artificial joint can be successfully implanted into the TMJ defect model and be used to repair the defect which help to restore chewing function.The CFR-PEEK is expected to be an ideal material for reconstructing TMJ.
9.Carcinoma ex pleomorphic adenoma composed of three malignant components in parotid gland:A case report and literature review

Objective:To investigate the diagnosis and prognosis of one patient with carcinoma ex pleomorphic adenoma (CXPA)composed of three malignant components in parotid gland,and to raise the clinicians'awareness of the disease.Methods:A patient was presented to hospital because of the mass in left parotid gland region for more than 30 years and the accompanied pain lasted for one month.After color ultrasonography,the left parotid gland tumor resection was performed. Results: The operation was successful. The postoperative pathological diagnosis results comfirmed as CXPA,it was composed of three malignant components,including non-specific adenocarcinoma,ductal carcinoma,and myoepithelial carcinoma.Conclusion:CXPA composed of three malignant components at the same time are extremely rare.CXPA is difficult to diagnose and its prognosis is poor.The clinicians are supposed to improve the understanding of CXPA.
10.Investigation on the current status of the allocation of radiological diagnosis and treatment resources in Linyi

Objective To understand the present state of radiological diagnosis and treatment resources allocation and to provide data support for the health administrative department to further optimize the allocation of resources in Linyi. Methods All hospitals which conducted radiological diagnosis and treatment in the city were surveyed. The data were analyzed by using SPSS software. Results There were 305 radiological diagnosis and treatment institutions, with 943 sets of radiological diagnosis and treatment equipment in Linyi. There were 0.89 sets of diagnostic and therapeutic equipment per 10000 people, and the average number of diagnostic and therapeutic equipment per institution in urban areas is 1.48 times that in county areas. There were 2208 radiological diagnosis and treatment staff in the city, and each hospital had 7.24 radiation staff, among which the number ofin tertiary, secondary, primary and unrated hospitals was 89.63, 17.64, 3.37 and 2.77, respectively. Conclusion The allocation of Diagnostic Radiology equipment is out of balance in Linyi, and the large-scale Diagnostic Radiology equipment is less and distributed centrally. The number of diagnostic and therapeutic radiographers per 10000 people is insufficient and unevenly distributed. Radiological diagnosis and treatment resources should be allocated scientifically to promote balanced development among regions and optimize the allocation of resources.
