5.Treatment of Colles fracture with manipulative closed reduction and U-shaped gypsum functional fixation.

OBJECTIVETo investigate the effect of U-shaped gypsum functional fixation after closed manipulative reduction for treatment of Colles' fractures.
METHODSFrom January 2011 to April 2014,47 cases of Colles fracture were treated by closed reduction and U-shaped gypsum functional fixation including 12 males and 35 females with an average age of 54.8 years old ranging from 8 to 72 years old. The time from injury to treatment was 40 min to 3 d. The patients were closed fractures without neurovascular injury. After manual reduction and U-shaped gypsum functional fixation, the thumb of injury hand were traction by contralateral hand, and other fingers of injury hand were taken flexion to exercise grip function. According to the situation of fracture healing, plaster was removed at 4 to 8 weeks' fixation, the wrist joint functional exercise was strengthened after plaster removed.
RESULTSAll patients were followed up from 6 to 12 months with an average of 6.4 months, the fracture healing time was 4 to 8 weeks. According to the Cooney wrist function scoring: the result was excellent in 44 cases, good in 2 cases, 1 case.
CONCLUSIONU-shape gypsum functional fixation for treatment of Colles fracture limits the activity of thumb and extensor tendon, can reduce shortening of radial by traction of the injury thumb with the contralateral hand; it is good for blood circulation of hand and wrist, and swelling.

6.Application of case base study combined with evidence-based medicine in internal medicine practice

There are some disadvantages in internal medicine practice teaching now. To find a new teaching model, and implement a teaching reform is the key to improve teaching quality. We try to apply the case introduction to the teaching method of evidence based practice in the department of internal medicine . The new teaching model which combined case base study with evidence-based medicine in internal medicine practice can help put forward questions and guide students to analyze questions by the case. It also helps find evidence and solve questions by evidence-based method. The organic combination of the two, not only can stimulate students' learning enthusiasm and initiative, de-velop the correct clinical thinking, but also can effectively promote students' self-learning ability to update their knowledge. It is worth widely developing in internal medicine practice.
7.TLC-scanning Determination of Geniposide in Compound Zhizi Granules(Gardenia Jasminoides Ellis, Radix Scutellariae, etc.)

8.The Characteristics of the Species Distribution and Detection of Vancomycin Resistance Genotypes in Enterococci

Objective To study the characteritics of the species distribution, vancomycin resistance phenotypes, and genotypes in Enterococci in Shenzhen Second People,s Hospital. Methods Screening vancomycin-resistant Enterococci (VRE) with agar screen, and detecting the phenotypes and genotypes of vancomycin resistance or intermediate enterococci with E test and multiple PCR-restriction fragment length polymorphism. Results Among 310 Enterococcus spp strains isolated f rom clinical samples, 235 strains (75.8 %) were Enterococcus faecalis, 71 strains (22.9 %)were E. f aecium, 3 strains (1.0%) were Enterococcus gallinarum, and 1 strains (0.3%) was Enterococcus casseliflavus. 7 strains were vancomycin-resistant or intermediate to Enterococci, vanA for 2 strains were detected, vanC1 for 3 strains were detected, vanC2/3 for 1 strains was detected, 1 strains was unknown. Their phenotypes confirmed to the genotypes. Conclusion 2 strains vanA were found. It is important for clinical laboratory to detect VRE with right methods to instruct rational application of antibiotics clinically and prevent VRE prevalence.

Objective To evaluate the diagnostic value of rapid ELISA method in detecting D‐dimer combined with cardiac tro‐ponin I (cTnI) for early exclusion of acute myocardial infarction (AMI) .Methods The samples from 178 consecutive patients with chest pain and suspected AMI were tested for cTnI and D‐dimer on admission;then cTnI was also tested at 6 h after admission . AMI was definitely diagnosed by echocardiography combined with angiography .The cTnI cut‐off for diagnosing AMI was 0 .11μg/L (accounting for 10% of total CV lowest level) .Results AMI was diagnosed in 40 cases(22 .4% ) .The negative prediction value (NPV) on admission was 87 .9% for single cTnI and increased to 98 .4% at 6 h after admission .The cut‐off for single D‐dimer on admission was 200 ng/mL FEU and its NPV was 87 .9% .No false negative was observed when both cTnI and D‐dimer were nega‐tive on admission .Conclusion The combination detection of cTnI and D‐dimer has the potential to be used as an early exclusion di‐agnosis of suspected AMI in the patients with chest pain at arriving in the emergency department ,thus shorten the patient′s hospi‐tal duration and save costs .
