Although the prediction of a delayed spinal cord injury after a low voltage electrical accident is difficult, we present a young paraplegic man who had delayed spinal cord injury after a low voltage electrical accident while working. Because the passage of an electric current is variable, the tissues far distant from the point of entry may be damaged, including the spinal cord. Low voltage itself is not a safe.

7.The Usefulness of Revised Version of the Korean Spinal Cord Independence Measure (KSCIM-R).

OBJECTIVE: The aim of this study was to investigate the suitability of the Revised version of the Korean Spinal Cord Independence Measure (KSCIM-R), which is a functional evaluation tool for spinal cord injury. METHOD: Thirty patients with spinal cord injury were enrolled. The KSCIM was modified to as KSCIM-R. Inter- rater reliability and relative sensitivity of KSCIM-R were evaluated by two raters, biweekly. RESULTS: The Kappa coefficient of the various individual tasks in the KSCIM-R ranged between 0.71 and 0.95 (mean: 0.82), which was higher than the previous version, KSCIM. High correlations were found between the total KSCIM-R scores of the paired raters (r=0.95, p<0.01), and between KSCIM-R scores and total FIM scores (r=0.90, p<0.01). The KSCIM-R detected all the functional changes detected by FIM total scoring, however the FIM missed changes in 16 (16%) of 102 sequential test batteries, which was detected by KSCIM-R total scoring. CONCLUSION: These results demonstrated that the KSCIM-R is reliable and more sensitive than the FIM and KSCIM in reflecting the functional changes of SCI patients.

10.Desire for Rehabilitation Services of Patients with Spinal Cord Injury Admitted in Post-acute Rehabilitation Facilities.

OBJECTIVE: To determine whether the rehabilitation desire of patients with spinal cord injury (SCI) in post-acute rehabilitation hospitals be suitable to objective parameters, and to figure out the discharge plan after post-acute rehabilitation hospitals. METHOD: 98 patients with SCI in post-acute rehabilitation hospitals were included. In order to identify rehabilitation desire, interviews were conducted among patients or their family members in a direct line. Discharge plans of patients after post-acute rehabilitation hospitals were surveyed. All participants completed demographic measures, Korean Modified Barthel Index (K-MBI) and International Classification of Functioning, Disability and Health (ICF) core-set for patients with neurological problems in post-acute rehabilitation facilities to determine the influences on rehabilitation desire. RESULTS: Most (80.6%) of patients desired more intensive rehabilitation services and 47 patients (48.0%) purposed to get physical therapy focused on motor recovery. Only 15 patients (15.3%) planned to discharge to home. Patients who planned to discharge to home had higher K-MBI score and lower numbers of decreased body functioning, activities and participation in ICF core-set than patients who would admit to another rehabilitation hospitals after discharge. CONCLUSION: Appropriate and intensive educations after spinal cord injury on the recovery process and goal of rehabilitation are required to supply adequate rehabilitation services and to avoid unnecessary admission to rehabilitation hospitals. And the rehabilitation program focusing on functional recovery should be emphasized in patients with SCI.
