1.Changes of phosphorylated ERK 1/2 and c-fos in the medial prefrontal cortex of post-traumatic stress disorder rats

ObjectiveTo observe the changes of phosphorylated extracellular signal-regulated kinase 1/2 (pERK1/2) and c-fos in the medial prefrontal cortex (mPFC) of post-traumatic stress disorder (PTSD) rats. Methods Male Wistar rats were randomly divided into control group and PTSD model group. The model group rats were exposed to the single-prolonged stress (SPS) to set up the rat PTSD model. The expression of pERK1/2 was detected using immunohistochemistry and Western blotting, and the expression of c-fos mRNA was detected using reverse transcription-polymerase chain reaction (RT-PCR). Results The result of immunohistochemistry analysis showed that the number of pERK1/2-positive cells of control group and model group were 10.4±2.07 and 48.8±10.08 respectively (P<0.01), and integral optical density were 24.955±3.691 and 110.810±10.643 respectively (P<0.01). And Western blotting showed that relative expression quantities of pERK/β-actin were 0.510±0.052 and 1.109±0.106 respectively (P<0.01). The quantities of c-fos mRNA relative expression of control group and model group were 0.267±0.067 and 1.049±0.131 (P<0.01). Conlusion The levels of pERK1/2 and c-fos increase significantly in mPFC of PTSD model rats. The ERK signal transduction pathway in mPFC might play an important role in the pathogenesis of PTSD.
2.Clinical analysis of cardiac arrest in 5 cases during liver transplantation

Objective To analysis the causes of cardic arrest during liver transplantation, and investigate the preventive measures. Methods Clinical records about 5 patients with cardiac arrest in liver transplant were studied retrospectively. Results Occurrence of cardiac arrest was 2.1 % (5/240). All the events happened at 3-6 min after graft reperfusion, and arrest lasted 3-8 min. Two patients died of resuscitation failure. After graft reperfusion, CVP and MPAP were much high in 5 patients. All of them had hypothermia and hypocalcemia. Three of them also had acidosis and two complicated with hyperkalemia. In one case, the difference between PaCO_2 and P_ETCO_2 was 47 mm Hg. Conclusion Many factors, such as disturbances of electrolyte and acid-base equilibrium or hypothermia, seemed to have some contribution to cardiac arrest occurrence after reperfusion. Various methods should be used to counter it.
3.Secondary structure and physico-chemical property of fusion proteins: Analysis with bioinformatics network resource

AIM: To analyze secondary structure and physico-chemical property of the fusion protein with bioinformatics network resource, and explore the expression of a secretory anti-osteoblastic carcinoma single-chain bi-functional antibody gene. METHODS: ①The single-chain variable fragment (ScFv) antibody gene and interleukin-2 (IL-2) gene were subcloned into corresponding restriction sites of retrovirus expression vector PLxSN. Mediated by liposome, the recombinant plasmid pL(ScFv-IL-2)SN was packaged with PA317 and selected in G418 to obtain the positive clones, which were able to produce stable retrovirus, and then osteosarcoma (OSC) cells were infected by the recombinant retrovirus, terming OSC/ScFv-IL-2. The virus titer was detected by using NIH3T3.②The transfected OSC9901 cells by ScFv-IL-2 gene were identified by polymerase chain reaction (PCR), reversed transcription-PCR and Western blotting. After the fusion protein was constructed, DNAssist and ANTHEPROT V5 softwares were used to analyze the amino acid sequence, the secondary structure, and the physico-chemical property of fusion protein. RESULTS: ①After the restriction enzyme and PCR identification, the pL(ScFv-IL-2)SN as a fusion protein expression vector, was constructed successfully, and high titer C26 cells were obtained; the expression of recombinant protein was confirmed by Western blotting.②On the fusion genes, the DNA sequence was analyzed with DNAssist nucleic acid sequence analysis software, and their secondary structure and physico-chemical property were analyzed with ANTHEPROT V5. CONCLUSION: The property of fusion protein can be analyzed and forecasted by means of bioinformatics network resources, and the approach may provide evidences for investigating single-chain bi-functional antibody gene.
4.Advances in cerebral autosomal dominant arteriopathy with subcortical infarcts and leukoencephalopathy

Cerebral autosomal dominant arteriopathy with subcortical infarcts and leukoencephalopathy (CADASIL) is the most common hereditary cerebral small vessel disease.NOTCH3 missense mutation causes its coded cysteine occurring odd change and then affects the conformation and function of protein of NOTCH3.The abnormal NOTCH3 protein has vascular smooth muscle toxicity and finally deposits in the cerebral small blood vessels and causes the disease.Usually,CADASIL can be suspected by its typical clinical manifestations and neuroimaging findings.Its diagnosis needs genetic testing or skin biopsy to find the outer granular osmiophilic deposits of small vascular smooth muscle cells or immunohistochemical NOTCH3-ECD staining positive.For nearly two decades,the studies on genetics,pathogenesis,clinical manifestations,and diagnostic techniques of CADASIL have made great progress,however,many important questions have not been fully clarified and have new discoveries,such as the NOTCH3 gene mutation pattern and loci,and the relationship between gene phenotype and clinical phenotype,optimization of diagnosis process,depth study of pathogenic mechanism,exploration of new discoveries,new therapeutic targets and concepts.This article reviews the genetic characteristics,pathogenesis,and clinical diagnosis and treatment technology of CADASIL.
5.Clinical application of ultrasound-guided femoral nerve and lateral femoral cutaneous nerve block for patients undergoing hip fracture surgery

Objective To observe the application of ultrasound-guided femoral nerve block (FNB) and lateral femoral cutaneous nerve block (LFCNB) for patients undergoing hip fracture surgery.Methods 60 patients scheduled for hip fracture surgery undergoing LMA general anesthesia were randomly divided into 3 groups,20 cases in each group.Before transfer patients from bed to operating table,A group received dezocine 5mg iv,B group received fascia iliaca compartment block(FICB),C group received FNB combined with LFCNB.40mL of 0.375% ropivacaine was injected guiding by ultrasound in B group and C group.The time of sufficient sensory block and awake,the dosage of propofol and remifentanil,MAP and HR at pre-block (T1),20min after block (T2),transfer bed (T3),LMA insert (T4),skin incision(T5),LMA remove(T6) and sober(T7) were recorded.Pain was assessed using visual analogue scale(VAS) pre-and post block.The incidence of using vasoactive drugs,agitation,pain and adverse reaction were also recorded.Results The time of sufficient sensory block and awake,the dosage of propofol and remifentanil in A,B and C groups were as following:A group (not measured),(20.3 ± 1.3) min,(835 ± 6.7) mg,(1 285 ± 18) μg;B group (i2.2 ±2.7)min,(13.3 ± 1.4)min,(610 ±9.9)mg,(835 ± 15) μg;C group (9.7 ± 2.4)min,(12.8 ± 1.5) min,(555 ± 6.5) mg,(785 ± 16) μg.The time of awake,the dosage of propofol and remifentanil in B group and C group were significantly lower than those in A group(F =2.62,2.41,2.45,all P < 0.05).The time of sufficient sensory block in C group was lower than that in B group (p < 0.05).The MAP and HR at T2,T3,T5 and T7 in A,B and C groups were:A group (115 ± 4) mmHg,(90 ± 8) beats/min,(135 ± 6) mmHg,(98 ± 8) beats/min,(104 ±6) mmHg,(87 ± 4) beats/min,(120 ± 5) mmHg,(88 ± 8) beats/min;B group (102 ± 3) mmHg,(81 ± 6) beats/min,(112 ± 5)mmHg,(82 ± 8)beats/min,(89 ±6) mmHg,(72 ± 3) beats/min,(100 ±6)mmHg,(76 ± 8) beats/min;Cgroup (100 ± 3) mmHg,(80 x 6) beats/min,(109 ± 6) mmHg,(83 ± 5) beats/min,(86 ± 5) mmHg,(70 ± 3) beats/min,(99 ± 5) mmHg,(75 ± 5) beats/min.The levels of MAP and HR in B group and C group were significantly lower thanthose in A group(F =2.25,2.85,2.87,2.91,all P < 0.05).The VAS scores at T2,T3,and T7in A,B and C groupswere:A group (3.9 ± 0.7) points,(8.2 ± 0.3) points,(6.0 ± 0.8) points;B group (2.3 ± 0.4) points,(4.1 ±0.4) points,(2.2 ± 0.7) points;C group (2.1 ± 0.5) points,(2.4 ± 0.4) points,(1.2 ± 0.4) points.The VAS scoresin B group and C group were significantly lower than those in A group (2.36,2.82,2.88,all P < 0.05).The VASscores at transfer bed and sober in C group were significantly lower than those in B group (F =2.32,2.38,all P <0.05).The incidence of using ephedrine/atropine,urapidil/esmolol,PONV,agitation,pain and incision pain in A,Band C groups were:A group 30%,30%,25%,25%,40%;B group 10%,10%,0%,0%,10%;C group 10%,5%,0%,0%,0%.The number of patients who required vasoactive drugs and adverse reaction in B group and C group were significantly lower than those in A group(x2 =7.58,8.81,9.11,9.11,8.89,all P <0.05).The incidence of incision pain at sober in C group was lower than that in B group(x2 =9.21,P < 0.05).Conclusion The ultrasound -guided FNB and LFCNB can obviously shorten the onset time,reduce the dosage of general anaesthetic and maintain the stability of henodynamics during the perioperative period.It has effective analgesia during transfer of patients from bed to operating table and sober.
6.Preventive Measures of Security in PACS

Objective To investigate the methods and the value of computer virus prevention in picture archiving and communication system(PACS).Methods By establishing a safe and sound management system which employed both hardware and software preventive measures,and strengthening security awareness in personnel,virus infection in PACS was prevented.Results A marked improvement compared to previous network environment was achieved and the virus infection rate reduced by 80%,that were ensuring secure and stable operations of PACS served accurately and efficiently for hospitals and departments.Conclusion Security measures discussed above are effective in preventing computer virus in local area network systems.
7.Protective effect of ?-scretase substrate-base peptide on cellular model of Alzheimer's disease

Objective To study the effect of ?-scretase substrate-base peptide(BACEsp) on cellular model of Alzheimer's disease(AD).Methods The cellular model of AD established with neuroblastoma cell SK-N-SH induced by amyloid precursor protein(APP) was infected with the supernatants including recombinant retrovirus pLXSN-BACEsp.MTT assay was used to test cell viability and immunocytochemistry was used to observe APP expresion in cells before and after BACEsp treatment.Results Cell viability and APP expression in the cells that had been first infected by recombinant retrovirus pLXSN-BACEsp were apparently higher than those only transfected by APP and last infected by recombinant retrovirus pLXSN-BACEsp.Conclusion BACEsp has an obviously protective effect on APP-induced neuron toxicity of cellular model of AD.
8.Clinical observation and analysis of aldosterone escape in patients with non-diabetic nephropathy by RASI-therapy

Objective To investigate the incidence and influencing factors of aldosterone escape in patients with non-diabetic nephropathy by RASI-therapy. Methods A total of 104 patients with non-diabetic nephropathy were treated with ARB or combination therapy of ACEI and ARB in a mean follow-up period of 12 months. Aldosterone escape was determined according to the change of plasma aldosterone concentration before and after treatment during 6-month and 12-month ACEI/ARB treatment, while the influencing factors of aldosterone escape in patients with non-diabetic nephropathy was also analyzed after therapy with RASI . Results In 12 months, the incidence of aldosterone escape was significantly higher than that in 6 months (26.92% vs. 14.42%, P = 0.007). After 12-month treatment, the difference was statistically significant in incidence of aldosterone escape among different stages of CKD (P = 0.027). Compared with 6-month incidence of aldosterone escape in the losartan group, 12-month incidence increased evidently (P = 0.020). The Ald level was positively correlated with urinary protein excretion and the Scr level (r = 0.431, P = 0.003 and r = 0.336, P = 0.009, respectively), and negetively correlated with levels of the eGFR (r = -0.275, P = 0.006). Univariate Logistic regression demonstrated that risk factors of aldosterone escape included pre-treatment values of the urinary protein excretion (OR = 3.671, P = 0.028) and the eGFR (OR = 0.972, P = 0.019). Multivariate Logistic model revealed pre-treatment values of the eGFR was positively associated with aldosterone escape (OR = 0.970, P = 0.012). Conclusion The incidence of the aldosterone escape increases along with the time of treatment. Renal function has correlated with aldosterone escape and pre-treatment value of the eGFR is an independent risk factor of aldosterone escape.
9.Sentinel lymph node detection of rabbit tongue by indirect CT lymphography combined with nanocarbon tracer.

OBJECTIVE:
To investigate the value of indirect CT lymphography (CT-LG) combined with nanocarbon tracer suspension staining in the sentinel lymph node (SLN) detection of rabbit tongue (cancer).
METHOD:
Eight rabbits were divided into two groups, i. e. normal control group (6 rabbits) and VX2 tongue carcinoma group (2 rabbits). Pre-contrast CT images were taken for all rabbits. The mixture of iohexol and nanocarbon tracer suspension (iohexol 0.25 ml and nanocarbon tracer 0.25 ml, respectively, volume ratio = 1:1) was used as contrast agent of indirect CT-LG. Indirect CT-LG was performed as the contrast agent was injection into right ventrolateral submucosa of the tongues. CT images were acquired at 1, 5, 10 and 15 min after injection.
RESULT:
There were no cervical lymph nodes clearly identified in any group on the pre-contrast CT images. After the contrast agent injection, indirect CT-LG showed an ipsilateral SLN with rapid enhancement which was the only enhanced SLN, located lateral to the larynx-trachea region in all rabbits of VX2 control group and carcinoma group. The CT attenuation values of SLN were (58.80 ± 5.45) HU, (134.80 ± 28.92) HU, (91.00 ± 13.08) HU and (63.80 ± 4.32) HU at 1, 5, 10 and 15 minutes after indirect CT-LG. After the cervical anatomy guided by CT scan, all of the right deep SLN showed a distinct enrichment of black dye highly consistent with the locations on the indirect CT-LG images.
CONCLUSION
It is more accurate for sentinel lymph node detection by indirect CT lymphography combined with nanocarbon tracer staining.

10.The influence of B、T lymphocytes decreased in Apo E gene knockout mice

Objective To study the influence of Apo E gene knockout on the lymphocytes . Methods Cells from the peripheral blood, spleen and bone marrow of Apo E knockout and wildtype mice were stained with kinds of antibodies , and analyzed by flow cytometry .Results Compared with wildtype mice , significant differences were found in B and T lymphoctes in the peripheral blood and spleen , but there was no significant difference in pre B cells , T lymphocytes in the thymus and long term hematopoietic stem cell in the Apo E knockout mice .Conclusion Numbers of B lymphocytes decreased in the peripheral blood and spleen , but there was no significant difference in B , T lymphocytes development , and numbers of long term hematopoietic stem cell in Apo E gene knockout mice .
