1.Counting the use of specialized palliative care services

The primary aim of this study was to compare the ratios of specialized palliative care use to all cancer death using 2 methods: 1) total number of patients who received either of specialized palliative care services (unadjusted), and 2) number of patients after adjustment of potentially duplicated counts (adjusted). The research team obtained patient list from all specialized palliative care services, and counted the number of the patients who received any specialized palliative care services. The ratio of adjusted value to unadjusted value was 0.59, and had large region differences.Unadjusted values had, although overestimated, essentially similar trends in changes by year and differences in the regions. In conclusion, total number of patients who received either of specialized palliative care services could be simple and feasible indicator to roughly determine the activity of specialized palliative care services, but exact number of the patients who received specialized palliative care services should be determined on the basis of the patient lists without duplicated counts.
2.Changes experienced by physicians and nurses after a region-based palliative care intervention trial: the OPTIM-study

The primary aim of this study was to collect the views of physicians and nurses in the regions where community-based palliative care intervention trial, the OPTIM-study, was performed. A content analysis of free comments of the questionnaire survey was conducted. Questionnaires were mailed to 1,763 physicians and 3,156 nurses after the intervention, and 706 and 2,236 responses were obtained, respectively. A content analysis identified 327 free comments from physicians and 737 from nurses. As favorable effects, the categories [Multidisciplinary teams and community networks were established] [Home-based care is widespread] [Medical knowledge and skills are acquired just as those involved in palliative care] emerged. The main effects of the community palliative care program included the establishment of multidisciplinary teams and community networks, development of home-based care, and increasing knowledge, skills, and awareness about palliative care.
3.EFFECTS OF WEIGHT RELEASE ON CARDIOPULMONARY FUNCTIONS DURING TREADMILL WALKING

There are various methods for gait exercise in rehabilitation medicine. These methods are useful for patients to control weight bearing and gait pattern. But they have good points and bad points. The purpose of this study was to determine the effects of weight release on cardiopulmonary function during walking, and to examine the potential of this weight-release method in rehabilitation medicine. Subjects were 15 males who had no cardiopulmonary disease or past illness. They wore a respiratory mask connected with metabolic analyzers and a lifting jacket, and they performed three walking tasks on a treadmill at 4.8 km/hour speed for five minutes. During walking, they were lifted up for the weight release employing 3 different traction forces, by a lifting system (Pneu-Weight, Pneumex, Inc.). The three conditions for traction were adjusted for their body weight percentage : 0% (full weight bearing ; FWB), 25% and 50% body-weight release. Each trial was measured with a metabolic analyzing system and electromyography. There were no significant differences among the three conditions in respiratory parameters such as oxygen uptake and minute ventilation. However, systolic blood pressure significantly decreased in the FWB condition. There were various sorts of changes in the integral electromyogram. For example, gastrocnemius increased and decreased, bicepsbrachii increased without weight-releasing, and increased more with weight-release. During exercise, oxygen uptake corresponded to the amount of muscle activity, and ventilation and heart rate were increased by oxygen requirement increases. These results indicate that weight-release saved muscle activity, and gait patterns were changed to less thrust force. The amount of total muscle activity as a postural adjustment and thrust was unchanged in this gait, but muscles used were altered. In rehabilitation medicine, we should give much thought to these changes. This weight-release walking method is useful, because the load can be adjusted according to the breathing circulation constant.
4.Requests for cancer treatment and palliative care: suggestions from 821 advanced cancer patients and bereaved families

It is important to collect patients' and their families' opinions to provide good palliative care. This study aims to analyze the contents of free description provided by the survey for cancer patients and bereaved families, which was performed before the intervention of The Outreach Palliative Care Trial of Integrated Regional Model (OPTIM) study. Requests for and good points of cancer treatment and palliative care were collected and classified. 1,493 advanced cancer patients and 1,658 bereaved families in four areas received the questionnaire, and 271 patients and 550 families filled in the free description. Cancer patients and bereaved families had demands for improved communication with medical staff, improved quality of pain relief, financial support of treatment, more educational activities on palliative care, and improved cooperation within and outside hospitals. 5.Baastrup's Disease Is Associated with Recurrent of Sciatica after Posterior Lumbar Spinal Decompressions Utilizing Floating Spinous Process Procedures.

STUDY DESIGN: Retrospective case-control study. PURPOSE: To determine whether kissing spine is a risk factor for recurrence of sciatica after lumbar posterior decompression using a spinous process floating approach. OVERVIEW OF LITERATURE: Kissing spine is defined by apposition and sclerotic change of the facing spinous processes as shown in X-ray images, and is often accompanied by marked disc degeneration and decrement of disc height. If kissing spine significantly contributes to weight bearing and the stability of the lumbar spine, trauma to the spinous process might induce a breakdown of lumbar spine stability after posterior decompression surgery in cases of kissing spine. METHODS: The present study included 161 patients who had undergone posterior decompression surgery for lumbar canal stenosis using a spinous process floating approaches. We defined recurrence of sciatica as that resolved after initial surgery and then recurred. Kissing spine was defined as sclerotic change and the apposition of the spinous process in a plain radiogram. Preoperative foraminal stenosis was determined by the decrease of perineural fat intensity detected by parasagittal T1-weighted magnetic resonance imaging. Preoperative percentage slip, segmental range of motion, and segmental scoliosis were analyzed in preoperative radiographs. Univariate analysis followed by stepwise logistic regression analysis determined factors independently associated with recurrence of sciatica. RESULTS: Stepwise logistic regression revealed kissing spine (p=0.024; odds ratio, 3.80) and foraminal stenosis (p<0.01; odds ratio, 17.89) as independent risk factors for the recurrence of sciatica after posterior lumbar spinal decompression with spinous process floating procedures for lumbar spinal canal stenosis. CONCLUSIONS: When a patient shows kissing spine and concomitant subclinical foraminal stenosis at the affected level, we should sufficiently discuss the selection of an appropriate surgical procedure.
