1.The synthetic molecules YK51 and YK73 attenuate replication of dengue virus serotype 2

Dengue virus infection has been posing alarming economic and social burden on
affected nations. It is estimated that 50-100 million dengue infections occur annually with
over 2.5 billion people at risk for endemic transmission. In the effort to develop effective
antiviral agents, we previously reported potential antiviral activities from selected array of
natural products and compounds against dengue virus serotype 2 (DV2). In this study, we
report the synthesis of two efficacious novel compounds, YK51 and YK73, and their activities
against DV2 replication. Both compounds were chemically synthesised from nicotinic acid
using a modified method for the synthesis of dihydropyridine. The products were tested with
cell-based assays against DV2 followed by a serine protease assay. As a result, both YK51 and
YK73 exhibited intriguing antiviral properties with EC50 of 3.2 and 2.4 µM, respectively. In
addition, YK51 and YK73 were found to attenuate the synthesis of intracellular viral RNA and
protect the switching of non-classic mechanism of protein translation. These compounds
demonstrated inhibitory properties toward the activity of DV2 serine protease in a dose
dependent manner. These findings demonstrate that both YK51 and YK73 serve as DV2 serine
protease inhibitors that abrogate viral RNA synthesis and translation. Further investigation
on these compounds to corroborate its therapeutic properties towards dengue is warranted.
2.Biorational control programme for the German cockroach (Blattaria: Blattellidae) in selected urban communities.

This study assessed the effectiveness of a biorational control approach using 2% hydramethylnon gel bait on German cockroaches, Blattella germanica (L.) in some residential and hospital buildings in South Western Iran. In total, three buildings consisting of 150 apartment units and 101 hospital units were monitored weekly via sticky trap for German cockroach infestations over a period of eight months. These infested units were randomly subjected to intervention and control treatments. Pamphlets and posters were provided and lectures were given to support the educational programmes as a tactic of the biorational system. Survey on cockroach index for intervention units showed 67-94% recovery to achieve clean level of infestation for intervention units of the residential buildings and 83% for the hospital. Mean percentage reductions for treatment groups throughout the 15-week treatment period were 76.8% for the residential buildings and 88.1% for the hospital, showing significant differences compared to the control groups. Linear regression of infestation rates were recorded weekly after treatment and their negative slope for treatment groups substantiated significant reductions for interventions. The results of this study showed that biorational control method, using gel bait, educational programmes and sanitation, is an effective way to manage German cockroach infestation.
3.Knowledge on Bone Banking among Participants in an Orthopaedic Conference: A Preliminary Survey

Despite increasing use of bone graft in Malaysia, there was
still lack of data to quantify knowledge level on bone
banking among orthopaedic community who are involved in
transplantation related work. Therefore, a survey on
awareness in tissue banking specifically bone banking, usage
and choice of bone grafts was conducted. From 80
respondents, 82.5% were aware about tissue banking
however only 12.5% knew of the existence of tissue banks in
Malaysia. Femoral head was the bone allograft most often
used as a substitute to autograft. Only 34.8% respondents
preferred irradiated bone grafts whilst 46.9% preferred nonirradiated,
indicating the need to educate the importance of
radiation for sterilising tissues. Exhibition was the most
preferred medium for awareness programme to disseminate
information about bone banking in the orthopaedic
community. The professional awareness is necessary to
increase the knowledge on the use of bone graft, hence to
increase bone transplantation for musculoskeletal surgeries
in the country.
4.Rubella outbreak amongst residential students in a military vocational school of Malaysia.

An outbreak of rubella occurred amongst 303 newly recruited residential Form IV students in a military vocational training school in Malaysia. Of the 303 Form IV students, 77 gave a history of acute illness. Rubella specific IgM was detected in the sera of 46.5% (141/303) whereas rubella specific IgG was detected in 100% of all Form IV students. Sixty five students with no clinical history of acute illness during the outbreak period had detectable rubella IgM in their sera and rubella specific IgM was detected in the sera of all symptomatic students except one. Maculopapular rash was the commonest presenting clinical feature among students with acute rubella infection in this outbreak (97.4%) followed by fever (88.2%). The duration of rash ranged from one to nine days with a mean of 4.6 days. Of the 65 students that had both fever and rash, 56 (85.2%) students had maculopapular skin eruption on the same day as the date of onset of fever, six (9.2%) developed the rash a day after the onset of fever and three (4.6%) had the rash after two days of fever. The duration of fever ranged from one to eight days with a mean of 3.5 days. The duration of conjunctivitis ranged from one to four days with a mean of 2.3 days, and all those who developed conjunctivitis had mild eye-discharge without photophobia. The duration of arthralgia ranged from one to three days with a mean of 2.1 days. The commonest type of joints affected was knee joints (66.7%, 12/18), followed by elbow and shoulder joints (27.8%, 5/18) and wrist joints (5.6%, 1/18). A good clinical history of the temporal relationship between the occurrence of rash and fever during the outbreak could easily differentiate rubella illness from that of measles.

5.Apoptosis changes and SA-beta galactosidase expression in stress-induced premature senescence (SIPS) model of human skin fibroblasts.

Stress-induced premature senescence (SIPS) model is in vitro model of cellular aging. In this study, apoptosis was evaluated in SIPS model and in replicative senescent fibroblasts. We also compared the activity of senescence-associated beta-galactosidase (SA-beta gal) as a biomarker of cellular aging. Our results suggested that SIPS model and senescent fibroblasts might share similar mechanism of aging and apoptosis pathway.
6.The Effectiveness of Gentamicin against Acanthamoeba Cysts in Vitro

Acanthamoeba is a free-living protozoa which causes serious ocular problem. Acanthamoeba keratitis
is becoming more prevalent amongst contact lens wearers and it can cause loss of vision and blindness
if not treated properly. The objective of this research is to determine the effectiveness of gentamicin
against six Acanthamoeba spp. isolates, of which three were clinical isolates (HS 6, HKL 95, HTH 73)
and three environmental isolates (SMAL 7, SMAL 8, TTT 9). Cyst suspension from the chosen isolates
were exposed to gentamicin. After 48 hours of incubation at temperature of 30°C and 37ºC, each mixture
was filtered and filtration membrane was put onto non-nutrient agar laid with Escherichia coli. The agar
plates were incubated for three days at 30oC and 37oC and the plates were examined daily until day 14
to look for the presence of Acanthamoeba trophozoites under inverted microscope. The presence of
trophozoites indicated the ineffectiveness of gentamicin. Gentamicin was found to be effective against
Acanthamoeba cysts from all the test strains at both incubation temperatures. The minimum cysticidal
concentration (MCC) mean value of gentamicin was 0.193 mg/mL at 30oC and 0.229 mg/mL at 37oC.
So, we concluded that gentamicin has cysticidal potential towards Acanthamoeba.
7.Sensitivity of Acanthamoeba Cyst to AntimicrobialAgents

The objective of this research is to study the sensitivity of six Acanthamoeba spp. isolates, of which three were from the clinical isolates (HKL 95, HTH 40 and HS 6) and the remaining three from environmental isolates (TTT 9, TL 3 and SMAL 8) to antimicrobial agents. Methods: The antimicrobial agents chosen for this purpose were polyhexamethylene biguanide and chlorhexidine. Cyst suspensions from the chosen isolates were exposed to PHMB and chlorhexidine respectively. After 48 hours incubation time at 30°c,each mixture was filtered and filtration membrane was put onto non-nutrient agar laid with Escherichia coli. The agar plates were incubated for three days at 30°c and examined daily until day 14 to detect the presence of Acanthamoeba trophozoites under the inverted microscope. The presence of thropozoites indicated the ineffectiveness of the antimicrobial agents. Results: Both of the antimicrobial agents tested were found to be effective against Acanthamoeba cysts from all the test starins. Polyhexamethylene biguanide gave minimum cysticidal concentration (MCC) mean value of 2.848mg/mL while chlorhexidine showed MCC mean value at a concentration of 3.988mg/mL. Conclusion: It can be concluded that the Acanthamoeba were sensitive to polyhexamethylene biguanide and chlorhexidine.
8.Lowering Dietary Glycaemic Index through NutritionEducation among Malaysian Women with a History of Gestational Diabetes Mellitus

Introduction: Gestational diabetes mellitus (GDM) increases risks for type 2
diabetes and cardiovascular diseases. Low glycaemic index (GI) diets improve
cardio-metabolic outcomes in insulin-resistant individuals. We examined the
feasibility of lowering GI through GI-based-education among Asian post-GDM
women. Methods: A 3-month investigation was carried out on 60 Malaysian
women with a mean age of 31.0±4.5 years and a history of GDM. Subjects were
randomised into two groups: LGIE and CHDR. The CHDR group received
conventional healthy dietary recommendations only. The LGIE group received
GI based-education in addition to conventional healthy dietary recommendations.
At baseline and after 3-months, dietary intake of energy and macronutrient
intakes including GI diet and glycaemic load was assessed using 3-day food
records. Diabetes-Diet and GI-concept scores and physical activity levels were
assessed using a questionnaire. Adherence to dietary instructions was measured
at the end of 3 months. Results: At the end of 3 months, the LGIE group had
significant reductions in energy intake (241.7±522.4Kcal, P=0.037, ES=0.463), total
carbohydrate (48.7±83.5g, P=0.010, ES=0.583), GI (3.9±7.1, P=0.017, ES=0.549) and
GL (39.0±55.3, P=0.003, ES=0.705) and significant increases in protein (3.7±5.4g,
0.003, ES=0.685) and diet fibre (4.6±7.3g, P=0.06). The CHDR group had a significant
reduction in fat only (5.7±9.4g, P=0.006, ES=0.606). There was a 30% increase in
GI-concept scores in the LGIE group (p< 0.001). Changes in GI-concept scores
correlated significantly to the reduction in dietary GI (r = -0.642, P=0.045). Dietary
adherence was comparable in both groups. Conclusion: GI-education improves
GI-concept knowledge and helps lower dietary glycaemic index among women
with a history of GDM.
9.Prevalence Of Non-Steroidal Anti-Inflammatory Drugs (NSAIDS) Use In Malaysian Adults And Associated Factors: A Population-Based Survey

Chronic use of non-steroidal anti-inflammatory drugs (NSAIDs) is associated with adverse effects. However, NSAIDs are among the most popular pain killers and easily available over the counter. This study aimed to determine the prevalence of NSAIDs use in Malaysian adults and among those with chronic diseases such as arthritis, kidney disease, hypertension, heart disease and asthma. It also examined the factors associated with NSAIDs use. Data from the National Health Morbidity Survey (NHMS) 2011, a nation-wide survey was analysed. A total of 18231 respondents aged 18 years and above responded to this module. Multivariate logistic regression was used to investigate the association between NSAIDs use and associated factors. The overall prevalence of NSAIDs use among Malaysian adults was 14.2% (95%CI 13.3-15.1). Of the respondents, 4.2% (95%CI 3.8-4.7) took NSAIDs once daily. NSAIDs use was highly associated with those who were ever-told to have arthritis (aOR: 3.03; 95%CI 2.60-3.52) and have difficulty of performing daily activities or work (aOR: 2.06; 95%CI 1.86-2.28). Those who were ever-told to have kidney disease (aOR: 2.36; 95%CI 1.74-3.20), ever-told to have asthma (aOR: 1.36; 95%CI 1.17-1.58), ever-told to have heart disease (aOR: 1.34; 95%CI 1.08-1.65), known hypertension (aOR: 1.22; 95%CI 1.08-1.37) also were associated with NSAIDs use. By socio-demographic profiles, NSAIDs use was positively associated with those who have government benefit scheme or private health insurance (aOR: 1.44; 95%CI 1.31-1.58), higher education level (aOR: 1.35; 95%CI 1.20-1.51), higher household income (aOR: 1.26; 95%CI 1.11-1.44, aOR: 1.12; 95%CI 1.02-1.24), currently working (aOR: 1.25; 95%CI 1.13-1.39) and female (aOR: 1.17; 95%CI 1.07-1.28). NSAIDs use was less likely among those aged 60 years and above (aOR: 0.83; 95% CI 0.72-0.97), Chinese (aOR: 0.41; 95% CI 0.36-0.47) and ‘Others’ ethnicity (aOR: 0.82; 95% CI 0.67-0.99) compared to Malay ethnicity. NSAIDs use is prevalent in Malaysian adults and associated with co-morbidities and higher socio-demographic status, thus appropriate awareness should be promoted and highlighted in the community.

10.Effects of Gelam Honey on Oxidative Stress in Lung Cancer Cells

Gelam honey was reported to exhibit anti-oxidative, anti-tumour and anti-inflammatory properties against many types of cancer. The main aim of the present study was to evaluate the effect of Gelam honey on the oxidative stress level of human lung cancer cells. IC50 of Gelam honey was identified by treating A549 cells with different doses of honey (50-200 mg/ml). The cells were divided into four groups and induced with stress using hydrogen peroxide (H2O2) accordingly: control, H2O2, Gelam honey, H2O2 + Gelam honey. After 24 hrs of treatment, oxidative stress markers such as malondialdehyde (MDA) and protein carbonyl, were determined. Induction of oxidative stress significantly increased the levels of MDA (p<0.05) but had no effect on protein carbonyl levels. Treatment with Gelam honey was found to reduce MDA levels (p<0.05) in A549 cells exposed to H2O2 but had no effect on the level of protein carbonyl. Interestingly, Gelam honey treatment alone had no effects on the levels of MDA and protein carbonyl. In conclusion, Gelam honey reduces lipid peroxidation but not protein oxidation in human lung cancer cells subjected to oxidative stress.
