1.The effects of changes in adhesion molecules on inflammatory response to cardiopulmonary bypass

Objective: To explore the characteristics of cell adhesion molecules (sICAM-1, sP-selectin) changes on inflammatory response during cardiopulmonary bypass, and observe the effect of intervention therapy with ligustrazine on changes of cell adhesion molecules. Methods: 32 cases of congenital heart disease were divided at random into ligustrazine group (20 cases) and control group (12 cases). Ligustrazine (5mg/kg) were added to extracorporeal circulation machine in ligustrazine group. In each group, arterial blood samples were taken at the following time-points after induction of anesthesia, 15 minutes after the start of CPB, and 2 hours, 6 hours, 12 hours, 24 hours after CPB. sICAM-1, sP-selection was measured by double antibodies sandwich enzyme-linked immunosorbent assay (ELISA). Results: Plasma levels of both adhesion molecules in both groups at baseline were in normal ranges. After CPB, levels of adhesion molecules were significantly increased in both groups. 2.Expression and significance of CD44v6 in lung squamous cell carcinoma tissues before and after neoadjuvant radiotherapy

Objective To investigate the effect and clinical significance of neoadjuvant radiotherapy for the expression of CD44v6 in lung squamous cell carcinoma tissues. Methods Fifty cases lung squamous cell carcinoma patients confirmed by aspiration biopsy from May 2013 to January 2015 were collected in Yangquan Coal Mine Group Genernal Hospital,including 20 cases of patients with stageⅢA were treated with neoadjuvant radiotherapy before surgery,and then performed surgery after neoadjuvant therapy. The expression of CD44v6 was detected by immunohistochemistry. The correlation between CD44v6 and clinicopathological features was analyzed by chi?square test. Results Immunohistochemical staining of CD44v6 was performed in tumor tissue of puncture biopsy, the positive rate expression of CD44v6 was 72%( 36/50 ) , it was associated with lymphatic metastasis(χ2 =3. 964, P=0. 046 ) and advanced TNM stage (Ⅲ+Ⅳstage ) (χ2 =4. 276, P=0. 039 ) . The positive expression of CD44v6 protein in tumor tissue was significantly decreased in 20 patients with neoadjuvant radiotherapy compared with before radiotherapy(7. 23±1. 45 vs. 11. 42±1. 31,t=2. 524,P=0. 025). Conclusion Positive expression of CD44v6 in human lung squamous cell carcinoma is related to the malignant clinicopathological features. Neoadjuvant radiotherapy before operation may improve prognosis via down?regulating CD44v6 expression.
3.CT and MRI analysis of central nervous system Rosai-Dorfman disease

Objective To study the CT and MRI imaging features of central nervous system Rosai-Dorfman disease and to enhance knowledge and differential diagnostic ability for central nervous system Rosai-Dorfman disease.Methods The CT and MRI imaging appearances in 4 cases of pathologically proven Rosai-Dorfman disease were retrospectively evaluated and the literature of central nervous system Rosai-Dorfman disease were reviewed.Results Two cases had cranial CT scans,4 cases had cranial MRI scans,On CT scans,cerebral edema was demonstrated in one case and the other case was normaL MRI scans showed the lesions were solitary in saddle area in 3 cases,and multiple in anterior cranial fossa in 1 case.The lesions exhibited iso- to hypointensity on both T1WI and T2WI images.Following intravenous injection of contrast medium,ring-like enhancement was seen in 2 cases and homogeneous enhancement in 1 case.Nodular enhancement was seen in the case of multiple lesions in the anterior cranial fosse.All lesions were dural-based.Conclusions In patients with fever,headache,elevation of the erythrocyte sedimentation rate (ESR) and a polyclonal increase in .γ-globulins,the possibility of central nervous system Rosai-Dorfman disease should be considered when single or multiple dural-based mass lesions,especially in sellar region,were identified by CT and MRI.

5.A modified model of orthotopic small intestinal transplantation in rats

Objective To establish a simple and stable model of orthotopic small intestinal transplantation(OIT) in rats.Methods The harvested en bloc segmental intestine consisted of donor abdominal aorta with superior mesenteric artery and portal vein.After in situ infusion with lactated Ringer′s solution,the graft was stored in 4℃ lactated Ringer′s solution.The segmental intestine transplantation was performed by end-to-side anastomosis of donor abdominal aorta to recipient abdominal aorta.The donor portal vein was anastomosed to the recipient left renal vein by "cuff anastomosis".The graft was anastomosed orthotopically by continuous(sutures).Results Sixteen intestine transplantations were,performed the average time for the arterial and(venous) anastomosis was 25?5 min and 4?1 min,respectively.Among the 16 recipient rats,13 survived more than 5 days.The average survival time was 10.35?2.84 days;the longest survival time was 21 days.Conclusions Graft harvesting,the technique of vascular and intestinal anastomosis and the(maintenance) of adequate blood volume are the key points for success of the operation.This successfully(established) model can serve as an excellent animal model for basic research of small intestinal trasplantation.
6.Prognostic value of pretreatment serum hemoglobin level in early-stage extranodal nasal-type NK/T-cell lymphoma

Objective To investigate the effect of pretreatment serum hemoglobin (Hb) level on the prognosis of early-stage extranodal nasal-type NK/T-cell lymphoma.Methods A retrospective analysis was performed on the clinical data of 175 patients with stage Ⅰ or Ⅱ extranodal nasal-type NK/T-cell lymphoma who were admitted to The Tumor Hospital Affiliated to Shanxi Medical University from 2000 to 2015.The inclusion criteria included Ann Arbor Ⅰ/Ⅱ stage, the primary tumor located in the upper aerodigestive tract, without other malignant diseases, and complete clinical information and follow-up data.Of the 175 patients, 67 received chemotherapy alone, 8 received radiotherapy alone,100 received radiotherapy and chemotherapyed.The survival rate was calculated using the Kaplan-Meier method.The log-rank test was used for univariate prognostic analysis.The Cox regression model was used for multivariate prognostic analysis.Results The univariate analysis showed that pretreatment serum Hb level (≥120 g/L), lactate dehydrogenase (LDH) level (normal), Eastern Cooperative Oncology Group (ECOG) score (0-1), Ann Arbor stage (IE), and radiotherapy were associated with significantly improved progression-free survival (PFS) and overall survival (OS)(P=0.000-0.046).The multivariate analysis showed that pretreatment serum Hb level, LDH level, ECOG score, and Ann Arbor stage were independent prognostic factors for PFS and OS (P=0.000-0.040).Conclusion Patients with a high pretreatment serum Hb level (≥120 g/L) have a better prognosis than those with a low pretreatment serum Hb level (<120 g/L).
7.Efficacy analysis of whole brain confomal intensity-modulated radiation therapy combined with synchronous dosage for treatment of brain metastases

Objective To discuss dosimetric advantage,compliance,efficacy and prognostic factors of whole brain conformal intensity-modulated radiotherapy ( IMRT ) combined with synchronous dosage for treatment of brain metastases.Methods Forty-one patients with metastatic tumor to the brain were confirmed by computed tomography (CT) or magnetic resonance imaging (MRI).They received whole brain irradiation and simultaneous integrated boost to bulky metastases by IMRT.Gross tumor volume(GTV) DT 4994-6990 cGy/ 22-30 fractions/4.4-6.0 weeks,whole brain DT 3990-5000 cGy/22-30 fractions/4.4-6.0 weeks.Results The median follow-up time was 6.4 months.The overall response rate,the median survival time and the overall survival at 1 year were 65.8 %,8 months,24.4 %.The brain-stem,spinalcord,lens,opticnerves were in the limited dose.In 21 patients,radioactive brain edema was happened,the rest did not appear early and late radiation reaction.Conclusion Whole brain irradiation using simultaneous integrated boost to bulky lesion by IMRT show an improvement in dose distribution and significant effect to patients with brain metastases.The therapy is well-tolerated and effective.
8.Clinical study of microbial pathogens and drug susceptibility in appendical intracavity with appendicitis

Objective To investigate the pathogen and drug susceptibility in appendical intracavity with appendicitis, and guide clinically the selection of reasonable antibiotics. Methods Germicultures and drug susceptibility tests were conducted for the secretions from appendical intracavity of the 71 patients with appendicitis. Result (1) Totally 9 species (63 cases) of bacterium were isolated from clinical samples, positive rate of isolation was 88.7%. The first one was escherichia coli (49 cases). (2) The positive rates of germiculture were no differences in patients with different types of appendicitis. Age and BMI were associated with the results of germiculture. Gender, temperature, neutrophil, fecalith were not associated with the positive germiculture. (3) The curative effect of the third and the fourth generation cephalosporins and carbapenems were well. The curative effect of the first generation cephalosporins, quinolones and penicillins were not well. Conclusion (1)In all samples,escherichia coli are the dominant species of bacterium. The distribution of isolation is no difference in patients with different types of appendicitis. (2) The third generation cephalosporins are commonly used in clinical treatment with the obvious anti-bacteria effects, The fourth generation cephalosporins and carbapenems can act as the optimal drug for serious infection. Quinolones is not the optimal antibiotics. Penicillins antibiotic is not proposed to be used.
9.Change of reference gene glyceraldehyde-3-phosphate dehydrogenase expression during 3T3-L1 adipocyte differentiation

Objective To observe the change of glyceraldehyde-3-phosphate dehydrogenase (GAPDH) gene expression during 3T3-L1 adipocyte differentiation as well as other reference gene expressions.Methods The mRNA expressions of several common reference genes were detected by real time-PCR on day 0,1,3,5,and 7 of 3T3-L1 adipocyte differentiation.Western blot was used to confirm the protein expressions of three common reference genes.Results (1) GAPDH and transferrin receptor(TFRC) mRNA expressions were significantly increased during adipocyte differentiation.GAPDH mRNA level was increased by 5.7,7.6,22.0,and 24.5 folds on day 1,3,5,and 7 after induction of adipocyte differentiation,but no apparent changes of β-actin,α-tubulin,peptidylprolyl isomerase A (PIPA),and 18S mRNA expressions were detected.The expression changes of key transcript factors for adipocyte differentiation such as PPARγ2,C/EBPα,and C/EBPβ were under-estimated by real time-PCR if GAPDH was chosen as the reference gene.Western blotting results showed that the GAPDH protein level increased gradually during adipocyte differentiation,especially on day 5 and 7 after adipocyte differentiation.There were no obvious changes of β-actin and α-tubulin protein expressions.(2) Berberine significantly inhibited mRNA and protein expressions of GAPDH in the process of adipocyte differentiation.GAPDH mRNA levels were reduced by 68.1% and 66.3% on day 5 and 7 after induction of adipocyte differentiation,but with no significant change in other reference genes.Conclusion It is not suitable for GAPDH to be used as an endogenous reference gene during 3T3-L1 adipocyte differentiation.
10.Thirty Cases of Front-orbital Fibrous Dysplasia: Intraoperative Optic Canal Localization with Three-bits Method and Outcomes Evaluation

Objective To investigate the indications of optic canal decompression in the patients with front-orbital fibrous dysplasia and the methods of intraoperative optic canal localization and decompression. Methods We collected 30 cases of fibrous dysplasia. All patients had sufficient images assessment. Patients with symptoms underwent surgery, including front-orbital cranioplasty and optic canal decompression. The frontotemporal epidural approaches were used. If there was a proptosis, the approach was extended with the removal of superior orbital ridge. Six patients undertook intraoperative CT and MRI fusion navigation, assisting in confirming the trunk, orbital and cranial orifice of optic nerve. During the operation, the optic canals were decompressed by three-bits method, to confirm the position of optic nerve. Results There were 30 cases of optic canal decompression and one case of vision loss. The visual acuity and vision field of the remaining patients improved to varying degrees. The proptosis disappeared or alleviated after the operation. Thirteen cases were reconstructed with normal internal plate, five cases with titanium plate, nine cases without reconstruction, and two cases were paved with proliferative broken bone on the orbital top; one case recurred with exophthalmos again after five years, but the visual acuity did not decline. Conclusion For the patients with front-orbital fibrous dysplasia, active surgical treatment should be taken, optic canal decompression should be chosen for diminution of vision, craniofacial anaplasty and orbital decompression should be performed in patients with facial deformity. The epidural approach is a good option to locate the optic nerve from the orbital orifice or cranial orifice. Combined with the three-bits method, we can achieve safe and meticulous optic nerve decompression.
