Chinese Journal of Primary Medicine and Pharmacy 2014;(15):2296-2298
Objective To compare plasma concentrations of biomarkers of endothelial dysfunction between patients with primary aldosteronism (PA) and essential hypertension (EH),and to determine whether elevated levels of these biomarkers could predict development of early organ damage .Methods 56 PA patients ( the observation group) and 56 EH patients(the control group) matched for age, sex, blood pressure and duration of hypertension were included in this study .The plasma levels of biomarkers reflecting endothelial dysfunction [ von Willebrand factor (vWF),soluble intercellular adhesion molecule 1(sICAM-1),oxidized low density lipoprotein(ox-LDL)]were detec-ted and compared between PA and EH patients .The left ventricular mass index ( LVMI) determined by echocardio-graphy,24-hour urinary protein quantitative determination and urinary albumin excretion rate ( UAER) were analyzed to evaluate early organ damage .Results ( 1 ) In the observation group , the plasma markers of endothelial damage vWF (133.4 ±54.9)%,sICAM-1 (412.1 ±75.2)μg/L and ox-LDL (13.7 ±9.0) U/L levels were higher than those in the control group(t=4.642,3.955,2.843,all P<0.05),and the observation group had a higher 24 h urina-ry protein(t=2.042,P<0.05) and LVMI(t=7.235,P<0.05).(2)Correlation analysis showed that LVMI was positively correlated with PAC,ox-LDL,vWF (r=0.447,0.385,0.393,P=0.020,0.046,0.029),and was nega-tively correlated with admission blood K +(r=-0.294,P<0.05).UAER was positively correlated with PAC,vWF and sICAM-1 (r=0.622,0.389,0.499,all P<0.05),and was negatively correlated with admission blood K +(r=-0.314,P<0.05).Conclusion Patients with PA have severer endothelial dysfunction reflected by multiple bio-markers and earlier organ damage than patients with EH ,and plasma aldosterone concentration and multiple endotheli-al dysfunction biomarkers could independently predict early organ damage .

Objective:
To investigate the prevalence and influencing factors of chronic diseases among cured leprosy patients in Huai'an City, so as to provide insights into chronic diseases control among cured leprosy patients.
Methods:
All cured leprosy patients in Huai'an City in 2021 were enrolled from the National Leprosy Management Information System in China. The demographic data, family status, leprosy diagnosis and treatment, health status and development of chronic diseases were collected using face-to-face questionnaire surveys and physical examinations, and the factors affecting the development of chronic diseases were identified among cured leprosy patients using a multivariable logistic regression model.
Results:
Totally 708 cured leprosy patients were included, with a male to female ratio of 2.75︰1, and a mean age of (71.84±10.09) years. The overall prevalence of disability was 48.45%. The prevalence of chronic diseases was 31.78% among participants, and the prevalence rates of hypertension, diabetes, coronary heart disease, stroke, dyslipidemia, tumor, chronic obstructive pulmonary diseases were 25.14%, 4.10%, 3.67%, 3.67%, 2.26%, 2.26% and 0.85%, respectively. There were 154 participants with a type of chronic diseases (21.75%), 61 with two types of chronic diseases (8.62%) and 10 with 3 and more types of chronic diseases (1.41%). Multivariable logistic regression analysis identified complete loss of labor ability (OR=1.484, 95%CI: 1.015-2.171), labor income as the major economic source (OR=0.621, 95%CI: 0.407-0.948), being cared of (OR=1.832, 95%CI: 1.203-2.788) and disability (OR=1.715, 95%CI: 1.127-2.608) as factors affecting the development of chronic diseases among cured leprosy patients.
Conclusions
There is a high risk of chronic diseases among cured leprosy patients with loss of labor ability, with government subsidy and others' funding as the major economic source, requiring to be cared of and with disability in Huai'an City.

Chinese Journal of Physical Medicine and Rehabilitation 2016;38(7):504-508
Objective To compare the effectiveness of botulinum toxin type A ( BTXA) and ethyl alcohol ( EA) in treating lower extremity spasticity after stroke. Methods This was a randomized, case-control study. A to-tal of 92 eligible stroke survivors completed the study. They were randomly divided into a BTXA group of 48 and an EA group of 44 according to a random number table. The gastrocnemius, soleus and posterior tibial muscles of the af-fected limb were chosen as injection sites. The BTXA group was injected with 50 to 200 IU of BTXA ( at 50 U/ml) at one to four sites in each muscle, with a total injection dose of less than 600 U. The EA group was injected with less than 10 ml of 50% EA (0.1 to 0.5 ml at each site). Before and 2, 4 and 12 weeks after the injection, both groups were evaluated using the modified Ashworth scale (MAS), a 3 m timed up and go test (TUG), a timed 10 meter walk ( 10m-WT) and each was asked to assess their pain level using a visual analogue scale ( VAS) . Any adverse re-actions were also observed. Results Two weeks after the injection, the average MAS score of both groups had im-proved significantly compared to that before the injection. The average improvement in the BTXA group was signifi-cantly less than in the EA group. No significant differences were found in other measurements. After four weeks the average MAS score of the BTXA group was still significantly different from that before injection or from 2 weeks previ-ously, but the EA group now showed no significant difference from before the injection. The average TUG, 10m-WT and VAS scores of both groups had improved significantly compared to those of the earlier time points. Twelve weeks after the injection, the average MAS, TUG, 10m-WT and VAS scores of the BTXA were still significantly improved compared to before the injection, but in the EA group only the average score VAS reading was significantly improved. There were then significant differences between the two groups in all of the measurements. Conclusions Both BTXA and EA can relieve muscle spasticity. Both take effect within 2 weeks, but the former has fewer side effects than the latter and a longer duration of therapeutic effect.

This paper is to explore changes of intestinal mucosal barrier, intestinal flora, and bacterial translocation in rats with severe acute pancreatitis (SAP). Twenty four male SD rats were randomly divided into the control group (n = 10) and the experimental group (n = 14). The model of severe acute pancreatitis of rats was induced by the method of injecting adversely 5% sodium taurocholate into the common biliary-pancreatic duct. All of the rats were killed after 24 hours and the level of the serum amylase and the plasma endotoxin was determined after that. The pathological changes of pancreas and small intestine were observed through hematoxylin-eosin staining (HE staining) and the abdominal viscera bacterial translocation rates were tested. With the method of real-time polymerase chain reaction (RT-PCR) the quantity of the intestinal flora was analyzed. In the control group, the level of Escherichia coli, Lactobacillus and Bifidobacterium were 2.08 ± 1.29, 11.04 ± 7.55 and 12.21 ± 4.95, respectively. On the contrast, the level of Escherichia coli in the cecum contents was much higher (9.72 ± 3.58, P < 0.01), while the Lactobacillus number was decreased significantly (0.67 ± 0.34, P < 0.01), and the Bifidobacterium number was also decreased (4.59 ± 3.42, P < 0.05) in the experimental group, so the ratio of Bifidobacterium/Escherichia coli was reversed. Besides, in the experimental group, the plasma endotoxin positive rates and the bacterial translocation rates were much higher (P < 0.01 or P < 0.05) and the pathology scores of pancreas and small intestines were also significantly higher (P < 0.01) than those in the control group. These results indicated that in severe acute pancreatitis rats, the intestinal mucosal barrier was severely damaged and the dysbacteriosis occurs in the intestinal canal. And these might relate to the occurrence and development of multiple organ infection.
